Background: The aim of this study was to analyze the influence of sociodemographic and clinical variables as determinants of mortality and survival in patients with ST-segment elevation acute myocardial infarction in the Eastern Amazon., Design and Methods: This observational, longitudinal, and retrospective study was conducted at the Gaspar Vianna Clinical Hospital Foundation in patients hospitalized from January 2017 to June 2020. Patients were divided into two groups: those who survived (G1) ( n = 646) and those who died (G2) ( n = 37). Sociodemographic and clinical variables associated with mortality and survival in these two groups were analyzed., Results: Patients with STEMI who had the highest risk of death were often the oldest (G1: 61.58 ± 10.74 years; G2: 69.57 ± 9.02 years; t = -4.492; p = 0.001), with Killip III-IV classifications (OR = 0.13; 95% CI = 0.02-0.71; p = 0.03), and with diseases such as heart failure (OR = 0.07; 95% CI = 0.004-1.50; p = 0.168) or renal failure (OR = 0.03; 95% CI = 0.006-0.16; p = 0.0001). In addition, female sex (hazard ratio = 2.073; 95% CI = 1.413-5.170), Killip III-IV classifications (hazard ratio = 4.041; 95% CI = 1.703-18.883) and the presence of heart failure (hazard ratio = 34.102; 95% CI = 4.410-263.684) or renal failure (hazard ratio = 14.278; 95% CI = 3.275-62.248) shortened in-hospital survival., Conclusions: Specific sociodemographic and clinical aspects influenced mortality and survival in patients with acute ST -elevation myocardial infarction., Competing Interests: The author(s) declared no potential conflicts of interest with respect to the research, authorship, and/or publication of this article., (© The Author(s) 2023.)

Environmental stresses disturb the endoplasmic reticulum (ER) protein folding. However, primary metabolic responses induced by ER stress remain unclear. Thus, we investigated the morphophysiological and metabolomic changes under ER stress, induced by dithiothreitol (DTT) and tunicamycin (TM) treatments in sorghum seedlings from 24 to 96 h. The ER stress caused lipid peroxidation and increased the expression of SbBiP1, SbPDI, and SbIRE1. The development impairment was more pronounced in roots than in shoots as distinct metabolomic profiles were observed. DTT decreased root length, lateral roots, and root hair, while TM decreased mainly the root length. At 24 h, under ER stresses, the glutamic acid and o-acetyl-serine were biomarkers in the shoots. While homoserine, pyroglutamic acid, and phosphoric acid were candidates for roots. At the latest time (96 h), kestose and galactinol were key metabolites for shoots under DTT and TM, respectively. In roots, palatinose, trehalose, and alanine were common markers for DTT and TM late exposure. The accumulation of sugars such as arabinose and kestose occurred mainly in roots in the presence of DTT at a later time, which also inhibited glycolysis and the tricarboxylic acid cycle (TCA). Amino acid metabolism was induced, which also contributed TCA components decreasing, such as succinate in shoots and citrate in roots. Plants have developed mechanisms to avoid harmful effects of Na + accumulation, such as the signaling pathway of carrier proteins Na + /H + (NHX) and salt overly sensitive (SOS). Besides, endoplasmic reticulum (ER) could integrate plant cell response. Thus, we aimed to understand the effects of ER homeostasis impairment, and its relationship to salt stress during early stages of the Sorghum bicolor CSF 20 a salt-tolerant variety. Three days old seedlings were challenged with NaCl (0, 50, 75 and 100 mM), dithiothreitol (DTT) at 0, 2.5, 5.0 10.0 mM, and the combined NaCl and DTT treatments. Tunicamycin (TUN) was also used as a second inducer of ER stress in a quantitative PCR, to corroborate with DTT's results. There was no significant change in growth parameters under NaCl treatments. Nevertheless, seedling length, mass and Na + content were decreased as DTT concentration was increased. Under combined NaCl and DTT treatments, shoot length and fresh and dry masses were maintained at control levels. On the other hand, the levels of Na + were decreased, in comparison to NaCl treatment. Genes analyzed by qPCR revealed that NaCl was able to induce all of them, except for SbbZIP60, however it was induced under combined stresses. In conclusion, the results indicated that S. bicolor seedlings of CSF 20 variety were tolerant to salt and sensible to ER stress. Exposure of pregnant women to pesticides in an occupational or incidental manner has been pointed out as a possible cause of health problems in newborns. Cypermethrin and methomyl are insecticides whose active compounds and metabolites can cause harm to the health of pregnant women and their offspring. Both contribute to increases in the concentrations of oxidizing agents that damage DNA, causing mutations. Thus, the use of antioxidant substances, such as melatonin, could serve as protection against genetic damage, reducing the problems caused by insecticides. Here, we evaluated the protective effect of exogenous melatonin in adult rats and their offspring exposed to methomyl and cypermethrin during the gestational period. Thirty adult rats were divided into five groups from the first day of pregnancy: NC - negative control, Mth received 0.02 mg/kg/day of methomyl, Cyp received 0.05 mg/kg/day cypermethrin, MthMl received 0.02 mg/kg/day methomyl combined with 10 mg/kg/day melatonin, and CypMl received 0.05 mg/kg/day cypermethrin combined with 10 mg/kg/day melatonin. Blood and liver samples were collected from the dams; blood, liver, and brain samples were collected from the offspring. All tissues were used for comet assay; blood was used for micronucleus test. Our results demonstrate that cypermethrin and methomyl induce significant genetic damage in the tissues of dams and offspring. Treatment with melatonin significantly reduced cypermethrin-induced genetic damage in dams and pups; however, melatonin was not effective against the genetic damage caused by methomyl. Thus, melatonin may be considered a promising agent for the reduction of genotoxic effects induced by cypermethrin but appears to be inefficient against the genotoxic effects caused by methomyl, probably due to differences in the metabolism of each pesticide., (Copyright © 2019 Elsevier B.V. Purpose: To assess the stability and reliability of femtosecond laser-assisted cataract surgery (FLACS) incisions design and dimensions using anterior segment optical coherence tomography (AS-OCT) imaging., Setting: Renato Ambrósio Ophthalmologic Study Center from Hospital Oftalmológico de Brasília, Brasília, Brazil., Design: Prospective nonrandomized controlled case series., Methods: Eyes undergoing FLACS with triplanar main temporal clear corneal incision (CCI) were evaluated at the end of the case. Eyes that required any incision hydration, surgical complications or lacked follow-up were excluded. The AS-OCT was performed after femtosecond delivery; at the end of the case; at 1 day and at 30 days after surgery. Data of pachymetry, endothelial and epithelial gaps, Descemet detachment and CCI architecture were compared., Results: Eleven eyes from 11 patients completed follow-up. Corneal thickness was statistically different between after femtosecond delivery and the end of the case ( P -value <0.05), but without difference compared to 30 days evaluation. There was an increase of Descemet detachments ( P -value <0.05) and endothelial gaps ( P -value =0.0133) at the end of the case compared to post-femtosecond delivery. As for the architecture of the CCI, significant difference was found between the parameters of entry angle and exit angle measured with AS-OCT and the programmed., Conclusion: The AS-OCT was capable of visualizing changes in the cornea at the CCI. Purpose: To compare visual outcomes and performance between bilateral implantation of a diffractive trifocal intraocular lens (IOL) Acrysof ® PanOptix ® TFNT00 and blended implantation of two different near add power bifocal IOLs: Acrysof ® Restor ® SV25T0 in dominant eye and Acrysof ® Restor ® SN6AD1 in the nondominant eye., Methods: This prospective, nonrandomized, consecutive and comparative study assessed 20 patients (40 eyes) who had bilateral cataract surgery performed using the IOLs described. Patients were divided into groups, bilateral trifocal implant and blended implant. Evaluation included measurement of binocular uncorrected and corrected distance visual acuity at 4 m (UDVA, CDVA) and uncorrected intermediate (60 cm) and near (at 40 cm) visual acuity; contrast sensitivity (CS) and visual defocus curve., Results: Postoperative CDVA comparison showed no statistical significance between groups. UDVA was significantly better in the trifocal groups. Under photopic conditions, the trifocal group had better CS in higher frequencies with and without glare. The binocular defocus curve demonstrated a trifocal behavior in both groups, with the bilateral trifocal group exhibiting better performance for intermediate vision., Conclusion: Both lens combinations were able to provide good near, intermediate and distance vision, with the trifocal group showing significantly better performance at intermediate distances and better CS under photopic conditions., Competing Interests: Disclosure The authors report no conflicts of interest in this work.
