Abstract

Alumina oxide ceramics were produced by plasma pressure compaction (P2C) sintering process. Two types of pure α-alumina (Al2O3) and a mixture of alumina and titanium diboride (TiB2) powders were used as starting materials. Microstructure and mechanical properties, namely hardness, elastic modulus, and fracture toughness, were analyzed and correlated to the type of the sintered powders and the adopted manufacturing route. The microstructural development and the chemical composition variation induced by the sintering process were assessed by using scanning electron microscopy and x-ray diffraction. Nano-indentation and Chevron notch beam techniques were adopted to estimate the mechanical properties of the sintered ceramics. The conducted analyses show the capability of P2C technique to produce sound alumina ceramics. Pure alumina bulks exhibit a good level of compaction and mechanical properties close to those achievable with conventional sintering processes, such as hot isostatic pressing or spark plasma sintering. No significant alterations in the chemical composition of the ceramics were observed. The addition of the titanium diboride in the alumina powders caused a moderate increase in the grain size lowering the hardness and Young's modulus of the sintered alumina and, at the same time, increased its fracture toughness to the occurrence of toughening mechanisms, like crack bridging and crack deflection. Abstract

Temporal bone fractures are a common lesion of the base of the skull. The diagnosis and management of temporal bone fractures require a multidisciplinary approach. Variable clinical presentations may arise from such fractures, ranging from an asymptomatic course to very serious consequences. The aim of this study was to report our experience with a series of patients with temporal bone fractures and to propose a diagnostic/therapeutic algorithm. This study enrolled 141 patients, 96 (68.1%) males and 45 (31.9%) females, ranging in age from 20 to 60 (average age: 39 +/- 4.1 years), with temporal bone fractures who were referred to Cardarelli Hospital between 2006 and 2018. The present paper presents a classification of temporal bone fractures and typical clinical sequelae and provides an illustration of their prognosis and treatment.

Abstract

The cancer therapy (Hadrontherapy) with accelerated particle beams using a superconducting bending dipole magnet is an innovative method to treat several kinds of tumors minimizing the irradiation of the surrounding tissues and avoiding intercepting vital organs. In order to validate the feasibility of such a magnet, an accurate study of the winding technique is carried out, together with a winding test of a coil prototype.

Abstract

We aimed to evaluate the usefulness of C-reactive protein (CRP) and procalcitonin (PCT) as markers of infection, sepsis and as predictors of antibiotic response after non-emergency major abdominal surgery. We enrolled, from June 2015 to June 2019, all patients who underwent surgery due to abdominal infection (peritoneal abscess, peritonitis) or having sepsis episode after surgical procedures (i.e. hepatectomy, bowel perforation, pancreaticoduodenectomy (PD), segmental resection of the duodenum (SRD) or biliary reconstruction in a Tertiary Care Hospital. Serum CRP (cut-off value < 5 mg/L) and PCT (cut-off value < 0.1mcg/L) were measured in the day when fever was present or within 24 h after abdominal surgery. Both markers were assessed every 48 h to follow-up antibiotic response and disease evolution up to disease resolution. We enrolled a total of 260 patients underwent non-emergency major abdominal surgery and being infected or developing infection after surgical procedure with one or more microbes (55% mixed Gram-negative infection including Klebsiella KPC, 35% Gram-positive infection, 10% with Candida infection), 58% of patients had ICU admission for at least 96 h, 42% of patients had fast track ICU (48 h). In our group of patients, we found that PCT had a trend to increase after surgical procedure; particularly, those undergoing liver surgery had higher PCT than those underwent different abdominal surgery (U Mann–Whitney p < 0.05). CRP rapidly increase after surgery in those developing infection and showed a statistical significant decrease within 48 h in those subject being responsive to antibiotic treatment and having a clinical response within 10 days independently form the pathogens (bacterial or fungal). Further we found that those having CRP higher than 250 mg/L had a reduced percentage of success treatment at 10 days compared to those < 250 mg/mL (U Mann–Whitney p < 0.05). PCT did not show any variation according to treatment response. CRP in our cohort seems to be a useful marker to predict antibiotic response in those undergoing non-emergency abdominal surgery, while PCT seem to be increased in those having major liver surgery, probably due to hepatic production of cytokines. Abstract

In this chapter, authors provide a description of boundary element method (BEM) applications in biomechanics, with a focus on advantages and limitations of BEM versus other numerical methods such as finite element method, finite difference method, and meshless methods. In addition to a general overview, the chapter focus on how the BEM approach can be advantageous in those biomechanical problems involving fracture mechanics and contact modeling. To this aim, its preprocessing flexibility to tackle sharp geometric changes and complex remeshing is highlighted. The comparison among BEM and other numerical approaches proceeds through the evaluation of inherent accuracy, preprocessing and postprocessing efforts, and run times. Bio-CAD models with complex shapes are usually created from medical images acquisition, computer tomography or magnetic resonance scan, with different modeling techniques, which result in different accuracy and usability of the generated tessellated or surface computer-aided design (CAD) geometry. Special attention must be drawn to the mathematical reconstruction of bio-CAD model to facilitate the meshing process in the BEM environment and reduce the geometrical imperfections generated during the CAD to computer-aided engineering translation phase. BEM is best suited to reproduce accurately high surface stress gradients that are generally a modeling issue (e.g., in bone–implant contact simulations). Working with 3D models, the mesh refinement in the neighboring areas where high stress gradients are expected is much facilitated when using BEM, also because it is possible to use discontinuous elements and circumvent the constraint of a continuous mesh. BEM approach is certainly more accurate for linear analysis but, on the other hand, less versatile in some areas like those of highly nonlinear material behavior. A short description of some case studies showing the described advantages of BEM approach is reported.

36. CR6086 Is Highly Effective and Improves Methotrexate Effect in a Mouse Model of Rheumatoid Arthritis

Abstract

Background/Purpose: CR6086, a selective EP4 antagonist, dose-dependently improves disease features in rheumatoid arthritis (RA) models in rodents. Indeed, recent studies highlight the role of the EP4 receptor in modulating autoimmunity and in counteracting bone erosion. Aim of the present study was to test CR6086 as an add-on medication with methotrexate (MTX). Methods: DBA/1 male mice were immunized with bovine type II collagen (BCII) in CFA. On arthritis onset, animals were assigned to the following experimental groups: vehicle, oral CR6086 30 mg/kg/day, MTX 1 or 3 mg/kg/three times a week alone or in combination with daily CR6086 30 mg/kg. Edema measurement and clinical scores were blindly determined daily before drug administration. After 2 weeks of treatment, mice were sacrificed and serum BCII antibodies measured. Paw joints were blindly scored for histological features. Data were analyzed by ANOVA or by Kruskal-Wallis test followed by appropriate post-hoc comparison test. Results: CR6086 strongly and significantly reduced score and edema within the first week of treatment compared to vehicle. MTX 3 mg/kg modestly reduced clinical signs over the second week of treatment, while MTX 1 mg/kg was inactive. CR6086/MTX combined treatments significantly reduced clinical score and edema within the first week of treatment. Figure 1 reports the AUC analysis of the whole treatment. Abstract

At the Max Planck Institute for plasma physics in Greifswald, Germany, the world's largest nuclear fusion experiment of modular stellarator type Wendelstein 7-X has started plasma operation. The hot hydrogen plasma is confined in a plasma vessel by an electromagnetic field generated by 50 non-planar and 20 planar superconducting coils. The superconducting coils are encased in cast stainless steel housings. The coils are bolted onto a central support ring and welded together by so called lateral support elements (LSEs). In this paper, a procedure, based on a global–local finite element method (FEM)–dual boundary element method (DBEM) approach, is developed to simulate the propagation of multiple cracks detected in LSEs and undergoing a fatigue load spectrum. The global stress analysis on the superconducting coils is performed by FEM whereas the sub-modelling approach is adopted to solve the crack propagation in the DBEM environment. The boundary conditions applied on the DBEM submodel are the displacements calculated by the FEM global analysis, in correspondence of the cut surfaces (there are no body forces nor external loads applied on the submodel volume). Two cracks are simultaneously introduced, and a linear elastic fracture mechanics analysis is performed. Results in terms of cracks growth rates and evolving crack shapes are provided, and the residual life of the component is forecast.
