A busca pela excelência nos tratamentos torna necessária a ampliação do conhecimento acerca dos materiais e suas características, assim como o efeito da estrutura dental subjacente no resultado final de restaurações. Assim, a presente tese foi dividida em três artigos. O primeiro artigo foi uma revisão sistemática de estudos in vitro que investigou a influência de agentes de cimentação e fatores associados à cor em facetas cerâmicas. Nos 21 artigos incluídos na revisão foram coletados dados referentes ao efeito do agente de cimentação, sistemas cerâmicos, efeito do substrato, envelhecimento e tratamentos de superfície na alteração de cor. Considerando os aspectos de cor dos agentes de cimentação, a translucidez e o valor apresentaram as maiores diferenças de cor visíveis. O efeito da cor do agente de cimentação nas facetas foi afetado pela espessura e opacidade da cerâmica. O segundo artigo foi um estudo in vitro para avaliar a capacidade de mascaramento de diferentes camadas e opacidades em facetas de resina composta direta sobre diferentes cores de substratos. Doze facetas de resina composta foram alocadas de acordo com a cor da resina composta (A1 e A2) e opacidade das camadas (monocamadas - esmalte, corpo ou dentina; bicamadas - esmalte + corpo ou esmalte + dentina; tricamada - esmalte + corpo + dentina) sobre as diferentes cores de substratos (A1, A3, C4 e C4 +). A capacidade de mascaramento e a translucidez foram calculadas com base nas coordenadas de cores CIE L * a * b * avaliadas com um espectrofotômetro. As diferentes camadas de resina composta proporcionaram capacidade de mascaramento para os diferentes substratos testados. Destacou-se a importância do conhecimento das diferenças de cor e opacidade dos materiais, principalmente para substratos dentais mais escuros. E para o terceiro artigo foi realizado um estudo clínico para avaliar a influência da cor do agente de cimentação, cor e espessura da cerâmica e do substrato dental nas propriedades ópticas de facetas cerâmicas. Foram avaliados 16 pacientes que receberam 95 restaurações de dissilicato de lítio. As coordenadas de cor CIE L * a * b * foram medidas com um espectrofotômetro antes e após o preparo dental, cerâmica sobre substrato e cerâmica cimentada. Foi calculada a diferença de cor (ΔE00) para cada variável (cor inicial, cor do substrato, espessura da cerâmica e valor do agente de cimentação). Todas as cores do agente de cimentação influenciaram as propriedades ópticas finais das facetas cerâmicas, as cerâmicas mais espessas apresentaram maior capacidade de mascaramento e o agente de cimentação de alto valor mostrou diferença de cor abaixo do limite de aceitabilidade sobre o substrato mais escuro. Com base nos estudos apresentados, foi possível concluir que a reprodução de dentes naturais é um processo complexo e torna-se importante o correto planejamento e adequada escolha do material restaurador, principalmente em casos de substratos escurecidos. The aesthetic result of restorations, direct or indirect, is influenced by several factors, such as the color and thickness of the restorative material, the luting agent used and the type and shade of the dental substrate, which are performed synergistically in the final result. The search for excellence in dental procedures is necessary to expand knowledge about materials and their characteristics, such as the effect of the underlying dental structure on the final result of restorations. Thus, the present thesis was divided into three articles.The first article was a systematic review of in vitro studies to investigate the influence of luting agents and factors associated with color of ceramic veneers. Data regarding the effect of light-cured luting agent, ceramic systems, effect of background, accelerated aging and surface treatments on color change were collected from 21 articles included in the review. Considering the shade aspects of luting agents, translucency and value showed the greatest visible color differences for CLVs. The effect of luting agent shade on the color of veneers is greatly affected by ceramic thickness and opacity. The second article was an in vitro study to evaluate the masking ability of different layers and opacities in direct composite resin veneers over different substrate shades.Twelve resin composite veneers were allocated according to composite layering shades (A1 and A2) and opacity (Monolayers – enamel, body, or dentin; Bilayers – enamel + body, or enamel + dentin; Trilayer – enamel + body + dentin) on simulated tooth-colored substrates in four different shades (A1, A3, C4 and C4+). The masking ability and translucency were calculated based on CIE L*a*b* color coordinates measured with a spectrophotometer. The resin composite layering technique yielded masking ability to the different tooth-colored substrates tested. The importance of knowing the differences in color and opacity of the materials was highlighted, especially for darker dental substrates. And for the third article, was a clinical study to evaluate the influence of the color of the luting agent, color and thickness of the ceramic and the dental substrate on the optical properties of ceramic veneers. Sixteen patients who received 95 lithium disilicate restorations were evaluated. The color coordinates CIE L * a * b * were measured with a spectrophotometer before and after dental preparation, ceramic over substrate and luted ceramic. The color difference (ΔE00) for each variable (baseline shade, substrate shade, ceramic thickness and luting agent shade value) was used to calculate. All the shades of the luting agent influenced the optical properties of the ceramic veneers, the thicker ceramics showed greater masking ability and the high value luting agent showed a color difference below the acceptability limit on the darker substrate. Based on the studies presented, it was possible to conclude that the reproduction of natural teeth is a complex process and the correct planning and adequate choice of restorative material is important, especially in cases of darkened substrates.

No entanto, a aparência estética final é influenciada por diversos fatores, tais como a cor e a espessura da cerâmica, o agente de cimentação e a cor do substrato dental, os quais atuam sinérgicamente no resultado estético final. Este estudo foi dividido em três artigos envolvendo restaurações totalmente cerâmicas. Inicialmente, um estudo in vitro foi conduzido para avaliar a influência de diferentes cores de resina flow utilizadas como agente de cimentação na cor final de laminados cerâmicos de diferentes opacidades sobre discos que simulavam substratos dentais de alto valor. Posteriormente, foi apresentado um relato de caso clínico no qual o paciente foi tratado com opções restauradoras totalmente cerâmicas. Além disso, um estudo clínico avaliou a influência de fatores relacionados à variação de cor em restaurações cerâmicas, baseado em análise com espectrofotômetro. No estudo in vitro, discos cerâmicos foram combinados à discos de resina flow com diferentes cores (A1, A2, B1, WO - opaco e IL - translúcido). Foram avaliados o parâmetro de translucidez e a alteração de cor (?E*00) de todos os espécimes sobre discos que simulavam substratos dentários (A2 e B1) e calculados através do método CIEDE2000. O caso clínico relatou a confecção de um laminado cerâmico e de uma coroa como opção restauradora para os incisivos centrais superiores que apresentavam restaurações em resina composta insatisfatórias. No estudo clínico, os valores de ?E*00 foram obtidos de onze pacientes que receberam trinta e oito restaurações cerâmicas em dissilicato de lítio. As coordenadas de cor CIEL*a*b* foram avaliadas com espectrofotômetro para restaurações cerâmicas, cor inicial do dente (baseline), após o preparo dental (substrato), e após a cimentação das cerâmicas. A variação de cor e o parâmetro de translucidez foram calculadas através do método CIEDE2000. O estudo in vitro demonstrou que a opacidade da cerâmica associada com as diferentes cores de agente de cimentação afetou o resultado de cor final dos laminados sobre diferentes substratos. O caso clínico demonstrou que as restaurações cerâmicas restabeleceram a condição estética do paciente. O estudo clínico indicou que a reprodução da cor de dentes naturais é um processo complexo, uma vez que todas as condições avaliadas apresentaram diferenças clinicamente visíveis para as cerâmicas cimentadas. A cerâmica de dissilicato de lítio apresentou menor capacidade de mascaramento sobre substratos escurecidos, enquanto as coordenadas L*, a*, e b* foram influenciadas pela cimentação. Ceramic restorations associate the optical properties of natural teeth with adequate long-term clinical outcomes. However, the final esthetic appearance is affected by several factors, such as the color and thickness of the ceramic, the luting agent and the dental substrate, which act synergistically on the final esthetic result. This study was divided into three papers involving all ceramc restotations. At first, an in vitro study was conducted to evaluate the influence of different shades of flowable resin composite used as luting agent on the final shade of porcelain veneers of different opacities over high value simulated dental substrates. Afterward, it was conducted a clinical report in which the patient was treated with two different indications of all ceramic restoration. Further, the clinical study evaluated the influence of color-related factors on the color variation of ceramic restorations based on spectrophotometric analysis. In the in vitro study porcelain discs were paired with flowable resin composite disks of different shades (A1, A2, B1, WO – white opaque or IL – translucent). The translucency parameters and color variation (?E*00) of all specimens were evaluated over the simulated dental substrates (A2 and B1) and calculated by CIEDE2000 methods. The clinical report described the confection of a laminate veneer and a fixed single crown to restore superior central incisors that showed unsatisfactory resin composite restorations. In the clinical study, the ?E*00 values were obtained from eleven patients who received thirty-eight lithium disilicate ceramic restorations. The CIEL*a*b* color coordinates were measured with a spectrophotometer for ceramic restorations, initial tooth shade (baseline), prepared tooth (background), and luted ceramics. Color variation and translucency parameters were calculated by CIEDE2000 method. The in vitro study showed that opacity of the porcelain veneer paired with luting agents of different shades affected the final appearance of the restorations over distinct simulated tooth substrates. The case report showed that the ceramic restorations reestablished the esthetic condition of the patient. The clinical study indicated that natural tooth color reproduction is a complex process since all conditions evaluated presented clinically visible color differences for luted ceramic. The lithium disilicate ceramic showed lower masking ability over darker background, while L*, a*, and b* coordinates were cementation dependent.

12. Effect of Pre-treatment with Chlorhexidine on the Retention of Restorations: A Randomized Controlled Trial

This study aimed to evaluate the effect of chlorhexidine (CHX) application on etched dentin on the 6-month retention of restorations placed on non-carious cervical lesions (NCCLs). A randomized controlled split-mouth and triple blind trial was carried out. Patients (n=42) with at least two non-carious cervical lesions were included. NCCLs were randomly assigned to two groups: control (placebo solution) or test group (2% CHX solution for 60 s after acid etching and before the adhesive application). Class V restorations (n=169) were performed with an etch-and-rinse adhesive system and composite resin by 10 trained operators. A calibrated examiner evaluated the restorations at 1 week (baseline) and at 6 months using the FDI criteria. The primary outcome was retention of the restorations. The analysis of factors associated to failure of restorations was carried out by Fisher's exact test (α=0.05). After 6 months of follow-up, 3.4% (CI 95% 1.3-7.3) of the restorations failed. There was no statistically significant difference between control and CHX (p=0.920). Regarding the cavity variables, deeper (p=0.04), wider (p=0.004) and wedge-shaped (p=0.033) cavities failed more. Both treatments provided acceptable clinical performance of the restorations. The use of CHX as a adjuvant in dentin adhesion did not influence the retention of Class V restorations after 6 months of follow-up. O objetivo deste estudo foi avaliar o efeito da aplicação de clorexidina (CRX) em dentina condicionada na retenção de restaurações confeccionadas em lesões cervicais não-cariosas (LCNC) após 6 meses. Ensaio clínico randomizado triplo cego do tipo boca dividida foi conduzido. Pacientes (n=42) com no mínimo duas LCNC foram incluídos. LCNC foram randomizadas em dois grupos: controle (solução placebo) ou grupo teste (aplicação de CRX 2% por 60 s após o condicionamento e antes da aplicação do adesivo). Restaurações Classe V (n=169) foram realizadas com adesivo de condicionamento ácido total e resina composta, por 10 operadores treinados. Um examinador calibrado avaliou as restaurações após 1 semana (base) e 6 meses usando os critérios da FDI. O desfecho primário foi retenção das restaurações. A análise dos fatores associados à falha das restaurações foi conduzida com Teste Exato de Fisher (α=0,05). Após 6 meses de acompanhamento, 3,4% (IC 95% 1,3-7,3) das restaurações falharam. Não houve diferença estatística entre os grupos CRX e controle (p=0,920). Com relação às variáveis das cavidades, cavidades mais profundas (p=0,024), largas (p=0,004) e em formato de cunha (p=0,033) falharam mais. Ambos os tratamentos (CRX e controle) proporcionaram performance clínica aceitável das restaurações. O uso de CRX como coadjuvante na adesão à dentina não influenciou a retenção das restaurações Classe V após 6 meses de acompanhamento.

- Abstract
Objective: This study evaluated the oral health-related quality of life (OHRQoL) of older adults participating or not in Seniors Centers (SC)., Methods: Two independent samples were compared: older adults who participate in SC (n = 124) and older adults who visited Primary Healthcare Centers (PHC) and do not participate in SC (n = 164). The data collected consisted of sociodemographic (sex, age, educational level, marital status, family income) and psychosocial characteristics-Sense of Coherence (SOC), anxiety and depression using HADS, happiness-, and oral clinical evaluation-use and need of dental prosthesis and decayed (D), missing (M), or filled (F) teeth. The resulting OHRQoL was evaluated using the Oral Health Impact Profile (OHIP-14). The Mann-Whitney test was used to assess the associations between the independent variables and the OHIP-14. Poisson regression models were also used in the analyses (α=0.05)., Results: In the PHC, of the 270 individuals invited to participate in the study, 164 (60.7%) were interviewed and clinically examined; while in the SC, of the 166 individuals invited to participate in the study, 124 (74.7%) were interviewed and clinically examined. After adjustments for sociodemographic, psychosocial and clinical factors, we found that the impact on OHRQoL was 2.8 times higher (95%CI 2.0-4.2) for older adults who did not participate in SC., Conclusion: Older adults who participated in SC showed better perception on OHRQoL, independently of sociodemographic, psychosocial and clinical factors.
