1. A Top-Down Approach Based on the Circularity Potential to Increase the Use of Reclaimed Asphalt

Abstract

Resource depletion and climate change, amongst others, are increasingly worrying environmental challenges for which the road engineering sector is a major contributor. Globally, viable solutions that comply with the principles of circular economy (CE) are being investigated that can replace conventional asphalt mixtures in a post-fossil fuel society. The use of reclaimed asphalt (RA) is a widely used and well-established method to reduce the environmental and economic impacts of asphalt mixtures while increasing their circularity. However, RA’s market supply and demand have not yet been systematically analyzed and established. Moreover, the actual circularity potential and the opportunity of re-circulating RA in a closed-loop model have not yet been methodically defined. To address this, a three-layered framework to quantify and assess the circularity potential (Ω) of RA has been developed. To give stakeholders and legislative bodies a simple method to assess the opportunities available to them to become “more circular”, a novel equation has been formulated. This takes the form of a three-level indicator that considers: technical aspects, the effect of the RA market, and the legislative restrictions. A case study in Germany was structured and undertaken to develop and verify the proposed approach. The results indicate that the available RA is insufficient to cover the needs of asphalt mixture production; even though RA production is significantly lower than the actual need of asphalt mixtures, it is not utilized in its entirety. An impactful step forward is the alteration of the regulations to support the higher utilization of RA in asphalt mixtures, and subsequently, the increased circular opportunity and potential of RA. Thus, Circularity potential (Ω) is a composite indicator that can support stakeholders, designers, and asset managers during the process of decision-making, to follow more circular operational, design, and asphalt pavement management patterns.

Abstract

The Major Histocompatibility Complex (MHC) is a hyper-polymorphic genomic region, which forms a part of the vertebrate adaptive immune system and is crucial for intra- and extra-cellular pathogen recognition (MHC-I and MHC-IIA/B, respectively). Although recent advancements in high-throughput sequencing methods sparked research on the MHC in non-model species, the evolutionary history of MHC gene structure is still poorly understood in birds. Here, to explore macroevolutionary patterns in the avian MHC architecture, we retrieved contigs with antigen-presenting MHC and MHC-related genes from available genomes based on third-generation sequencing. We identified: 1) an ancestral avian MHC architecture with compact size and tight linkage between MHC-I, MHC-IIA/IIB and MHC-related genes; 2) three major patterns of MHC-IIA/IIB unit organization in different avian lineages; and 3) lineage-specific gene translocation events (e.g., separation of the antigen-processing TAP genes from the MHC-I region in passerines), and 4) the presence of a single MHC-IIA gene copy in most taxa, showing evidence of strong purifying selection (low dN/dS ratio and low number of positively selected sites). Our study reveals long-term macroevolutionary patterns in the avian MHC architecture and provides the first evidence of important transitions in the genomic arrangement of the MHC region over the last 100 million years of bird evolution.

3. The Gap Between Daily Hospital Bed Supply and Demand: Design, Implementation, and Impact of Data-Driven Pre-Noon Discharge Targets

Abstract

Hospitals have sought to increase pre-noon discharges to improve capacity, although evidence is mixed on the impact of these initiatives. Past interventions have not quantified the daily gap between morning bed supply and demand. The authors quantified this gap and applied the pre-noon data to target a pre-noon discharge initiative.The study was conducted at a large hospital and included adult and pediatric medical/surgical wards. The researchers calculated the difference between the average cumulative bed requests and transfers in for each hour of the day in 2018, the year prior to the intervention. In 2019 an intervention on six adult general medical and two surgical wards was implemented. Eight intervention and 14 nonintervention wards were compared to determine the change in average cumulative pre-noon discharges. The change in average hospital length of stay (LOS) and 30-day readmissions was also calculated.The average daily cumulative gap by noon between bed supply and demand across all general care wards was 32.1 beds (per ward average, 1.3 beds). On intervention wards, mean pre-noon discharges increased from 4.7 to 6.7 (p0.0000) compared with the nonintervention wards 14.0 vs. 14.6 (p = 0.19877). On intervention wards, average LOS decreased from 6.9 to 6.4 days (p0.001) and readmission rates were 14.3% vs 13.9% (p = 0.3490).The gap between daily hospital bed supply and demand can be quantified and applied to create pre-noon discharge targets. In an intervention using these targets, researchers observed an increase in morning discharges, a decrease in LOS, and no significant change in readmissions.

5. A prescriptive optimization approach to identification of minimal barriers for surgical patients

Abstract

Ensuring timely patient discharges is central to managing a hospital’s patient flow; however, discharges are dependent on the coordination of multiple care teams and thus are highly decentralized in nature. Many large hospitals have established capacity management centers to centrally direct and inform flow and support clinical teams across the hospital system, but they often lack transparency into what are the actionable, high-yield barriers to discharge that they need to focus on to be most effective. Moreover, these barriers are patient-specific and context-dependent, i.e., a patient’s clinical-operational context determines what issues must be resolved and with which urgency. In this study, we leverage a machine learning model that predicts which patients are likely to be discharged in the next 24 hours together with a mixed-integer prescriptive optimization model to identify a subset of issues calledminimal barriersthat stand in the way of discharging a patient. Such barriers balance two aims: a high likelihood that the patient will be discharged from the hospital in the next 24 hours if these barriers are resolved; and a high likelihood that these barriers will indeed be resolved. We empirically demonstrate the efficacy of the proposed formulation and solution methodology in identifying a small number of minimal barriers using real data from a large academic medical center.

6. A Novel Solution to Find the Dynamic Response of an Euler–Bernoulli Beam Fitted with Intraspan TMDs under Poisson Type Loading

Abstract

This contribution considers a virtual experiment on the vibrational response of rail and road bridges equipped with smart devices in the form of damping elements to mitigate vibrations. The internal damping of the bridge is considered a discontinuity that contain a dashpot. Exact complex eigenvalues and eigenfunctions are derived from a characteristic equation built as the determinant of a 4 × 4 matrix; this is accomplished through the use of the theory of generalized functions to find the response variables at the positions of the damping elements. To relate this to real world applications, the response of a bridge under Poisson type white noise is evaluated; this is similar to traffic loading that would be seen in a bridge’s service life. The contribution also discusses the importance of smart damping and dampers to sustainability efforts through the reduction of required materials, and it discusses the role played by robust mathematical modelling in the design phase.

Abstract

This contribution concerns recommendations which could be made to Italian regulatory bodies to improve their use of reclaimed asphalt (RA) in the road engineering sector. It is essential for the use of RA to be established as a standard practice since it has been proven that it can serve as an end-of-waste product that complies with the principles of a circular economy within both an open- and a closed-loop approach [1,2]. Several aspects will be covered, starting with an analysis of European nations whose economies can be classed as “more circular” compared to Italy’s. This refers to nations which have a high usage of RA. Furthermore, nations which have clear regulatory guidelines on the use of RA in road construction or, alternatively, nations which have very lax statutory requirements on pavement design allowing best practice to reign could be considered as more circular if these regulations or permissiveness result in a greater uptake in the use of RA in road construction.

Abstract

The reverse logistics (RL) performance of construction and demolition waste (C&DW) differs significantly between countries. Although some countries are successfully circular, other countries are still far behind; with C&DW being largely directed to landfills or being illegally dumped. There is a lack of guidelines on the requirements for transitioning countries towards C&DW diversion, and little attention has been given to the factors that influence the success of C&DW RL and increased circularity. This study aims to develop a framework that helps in structuring and assessing the success of C&DW RL on a regional level and serves as a guide for transitioning developing countries towards implementing such operations. Since the framework should help evaluate where interventions are needed to enhance performance, the exact domains to be used and their priorities should be identified. A systematic litera- ture review was conducted to identify the critical factors that affect the outcome of RL operations. After their identification, the interrelationships between the factors and the dependence and driving power of each are examined through interpretive structural modelling. This analysis provides valuable insights and thus, allows for the establishment of a hierarchy of factors that can serve as a guide for decision-makers. The developed framework can be utilized for enhancing the efficiency of current RL operations as well as for identifying the direction of action and efforts needed to shift developing countries towards the implementation of circular economy in the sector of C&DW.

11. Low-Volume Bowel Preparation Is Associated With Reduced Time to Colonoscopy in Hospitalized Patients: A Propensity-Matched Analysis

Abstract

Delays in inpatient colonoscopy are commonly caused by inadequate bowel preparation and result in increased hospital length of stay (LOS) and healthcare costs. Low-volume bowel preparation (LV-BP; sodium sulfate, potassium sulfate, and magnesium sulfate ) has been shown to improve outpatient bowel preparation quality compared with standard high-volume bowel preparations (HV-BP; polyethylene glycol ). However, its efficacy in hospitalized patients has not been well-studied. We assessed the impact of LV-BP on time to colonoscopy, hospital LOS, and bowel preparation quality among inpatients.We performed a propensity score-matched analysis of adult inpatients undergoing colonoscopy who received either LV-BP or HV-BP before colonoscopy at a quaternary academic medical center. Multivariate regression models with feature selection were developed to assess the association between LV-BP and study outcomes.Among 1,807 inpatients included in this study, 293 and 1,514 patients received LV-BP and HV-BP, respectively. Among the propensity score-matched population, LV-BP was associated with a shorter time to colonoscopy (β: -0.43 [95% confidence interval: -0.56 to -0.30]) while having similar odds of adequate preparation (odds ratio: 1.02 [95% confidence interval: 0.71-1.46]; P = 0.92). LV-BP was also significantly associated with decreased hospital LOS among older patients (age ≥ 75 years), patients with chronic kidney disease, and patients who were hospitalized with gastrointestinal bleeding.LV-BP is associated with decreased time to colonoscopy in hospitalized patients. Older inpatients, inpatients with chronic kidney disease, and inpatients with gastrointestinal bleeding may particularly benefit from LV-BP. Prospective studies are needed to further establish the role of LV-BP for inpatient colonoscopies.

Abstract

Generalized linear models unite a wide variety of statistical models in a common theoretical framework. This paper discusses GLMLAB-software that enables such models to be fitted in the popular mathematical package MATLAB. It provides a graphical user interface to the powerful MATLAB computational engine to produce a program that is easy to use but with many features, including offsets, prior weights and user-defined distributions and link functions. MATLAB's graphical capacities are also utilized in providing a number of simple residual diagnostic plots.

Abstract

Ambient modal identification, also known as Operational Modal Analysis (OMA), aims to identify the modal properties of a structure based on vibration data collected when the structure is under its operating conditions, i.e., no initial excitation or known artificial excitation. This procedure for testing and/or monitoring historic buildings, is particularly attractive for civil engineers concerned with the safety of complex historic structures. However, since the external force is not recorded, the identification methods have to be more sophisticated and based on stochastic mechanics. In this context, this contribution will introduce an innovative ambient identification method based on applying the Hilbert Transform, to obtain the analytical representation of the system response in terms of the correlation function. In particular, it is worth stressing that the analytical signal is a complex representation of a time domain signal: the real part is the time domain signal itself, while the imaginary part is its Hilbert transform. A 3DOF numerical example will be presented to show the accuracy of the proposed procedure, and comparisons with data from other methods assess the reliability of the approach.

Abstract

Background: Among people with diabetes, those with kidney disease have exceptionally high rates of cardiovascular (CV) morbidity and mortality and progression of their underlying kidney disease. Finerenone is a novel, nonsteroidal, selective mineralocorticoid receptor antagonist that has shown to reduce albuminuria in type 2 diabetes (T2D) patients with chronic kidney disease (CKD) while revealing only a low risk of hyperkalemia. However, the effect of finerenone on CV and renal outcomes has not yet been investigated in long-term trials. Patients and Methods: The Finerenone in Reducing CV Mortality and Morbidity in Diabetic Kidney Disease (FIGARO-DKD) trial aims to assess the efficacy and safety of finerenone compared to placebo at reducing clinically important CV and renal outcomes in T2D patients with CKD. FIGARO-DKD is a randomized, double-blind, placebo-controlled, parallel-group, event-driven trial running in 47 countries with an expected duration of approximately 6 years. FIGARO-DKD randomized 7,437 patients with an estimated glomerular filtration rate ≥25 mL/min/1.73 m2 and albuminuria (urinary albumin-to-creatinine ratio ≥30 to ≤5,000 mg/g). The study has at least 90% power to detect a 20% reduction in the risk of the primary outcome (overall two-sided significance level α = 0.05), the composite of time to first occurrence of CV death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure. Conclusions: FIGARO-DKD will determine whether an optimally treated cohort of T2D patients with CKD at high risk of CV and renal events will experience cardiorenal benefits with the addition of finerenone to their treatment regimen. Abstract

The dynamics of beams equipped with tuned mass dampers is of considerable interest in engineering applications. Here, the purpose is to introduce a comprehensive framework to address the stochastic response of the system under stationary and non-stationary loads, considering inertia effects along the spring of every tuned mass damper applied to the beam. For this, the key step is to show that a tuned mass damper with spring inertia effects can be reverted to an equivalent external support, whose reaction force on the beam depends only on the deflection of the attachment point. On this basis, a generalized function approach provides closed analytical expressions for frequency and impulse response functions of the system. The expressions can be used for a straightforward calculation of the stochastic response, for any number of tuned mass dampers. Numerical results show that spring inertia effects may play an important role in applications, affecting considerably the system response.

The presentation will examine the relationship between technological developments and sector-specific regulation in the electronic communications sector, using examples from the European Union. The presentation will be in three parts. What influences sector-specific regulation: how a number of diverse factors influence the direction and shape of sector-specific regulation: public policy, economics, law, commercial interests and technological developments. How technology can influence regulation in the communications sector, for example, how regulation focuses on the last mile bottleneck and how the market dynamics and regulatory focus change with the evolution to NGA networks. How regulation can influence technology in the communications sector, for example, how regulation creates incentives and disincentives for new technological developments. The overall aim is to illustrate that one of the major differentiators of regulation in the communications sector (and a huge challenge for the regulators) is that it must cope with the speed of technological development — in a way that is seldom seen in other “utility” sectors that are also subject to sector-specific regulation.

Abstract

Objective To acoustically evaluate the cries of SIDS infants and compare these cry features to a group of healthy term (HT) infants, as well as previously published results for SIDS infants. Methods Pain-induced crying episodes were collected from four infants during the first weeks of life that later died of SIDS. Temporal and spectral features of each crying episode were characterized based on measures of cry duration, cry fundamental frequency ( F 0 ), and cry formant frequencies ( F 1 and F 2 ). Results The SIDS infants were found to produce cries with longer duration compared to HT infants. The cries of SIDS infants also differed from HT infants in regard to the absolute difference in F 2 − F 1 frequency. Conclusions The acoustic features considered in the present study support the contention that the cries of SIDS infants are reflective of atypical respiratory–laryngeal control. Although research of this nature is rare, there is evidence to suggest an acoustic profile of crying that is specific to SIDS.

Abstract

At the end of the 1980s as the British Paediatric Association was working to emerge as a separate entity outside the umbrella of the Royal College of Physicians, the British Association of Perinatal Medicine was continuing the work of trying to establish specialist perinatal care. Throughout that decade the differentiation of obstetric units and neonatal units into those offering complex care and those that offered a less specialist service to a local population had made little progress or indeed no progress in some parts of the UK. At that stage neither the BPA nor the Royal College of Obstetrics and Gynaecology (RCOG) fully recognised the need to establish a coordinated specialist neonatology and high-risk obstetric service across the country. However it was at the end of the 1980s, as therapeutic options began to grow, that there was a growing recognition of a role for specialist neonatology and neonatal centres, even though the vast majority of specialist neonatal care was delivered by consultants who had both paediatric and neonatal patients and who worked outside the specialist neonatal centres. It is probably fair to say it was large outside influences which drove a major change in this situation.

Abstract

Summary Objective The purpose of the present study was to examine the acoustic features of crying demonstrated by infants who experienced apnea of infancy (AOI) and compare these features to a non-AOI group of infants. Based on past physiological descriptions of AOI, three predictions in regard to the influence of AOI on acoustic cry features were proposed: (1) the rate of crying would be significantly faster among infants with AOI, (2) the latency of crying onset would be significantly longer among infants with AOI and (3) the F 0 characterizing an overall episode of crying would be significantly lower among infants with AOI. Patients and methods Pain-induced crying episodes were collected from a group of healthy term infants (HT) and those with AOI. One complete crying episode was obtained from each infant and analyzed acoustically with regard to durational and spectral features of the cry. Results Infants comprising the AOI group were found to demonstrate a significantly longer cry latency and lower F 0 compared to HT infants. Conclusions The acoustic cry features measured for the AOI infants are discussed with regard to past reports of poor arousal and decreased muscle tone. A model of AOI crying is proposed whereby the autonomic nervous system and associated pathways are slower to interpret pain stimulus compared to HT infants.

Abstract

Pregnant women, revealed by good antenatal screening to be without obstetric risk, may prefer to deliver at home rather than in hospital. However, the possibility of unanticipated complications requires for their safety the presence of two health professionals in the home and the existence of an emergency flying squad. As these may not always be available, it is suggested that such deliveries might alternatively be conducted in ‘maternity homes’ close to, but independent and separate from, the maternity hospital. These homes would be staffed by midwives and family doctors. Expectant mothers would occupy their own suite (with or without partner and family). Should complications then arise, obstetric/paediatric skills would be readily available nearby.

It concludes with a summary of points made during a discussion at the 2004 European Forum for Victim Services conference about whether or not services to victims of hate crime should be provided by mainstream victim services or specialist agencies.

Abstract

Government policies and programmes in Canada have been slowly evolving from a focus on institutional care to traditional programmes in the community. Now, a new paradigm of services is emerging which emphasizes the concepts of independent living. This study collected extensive information from government officials and consumer groups across Canada regarding the trends in barrier-free housing, accessible transportation and personal supports from 1980 to the present time. The research documents some of the major issues with government initiatives as well as promising innovations. Although many positive changes were made during the Decade of Disabled Persons (1983-1992), many of these initiatives were eroded with major funding cutbacks in the mid- to late 1990s. This paper concludes with ways to change the present service delivery system to ensure that everyone can live independently in the community.
