12. Dimensions of Occlusoproximal Cavitated Carious Lesions as a Cut-Off Point for Restorative Decision in Primary Teeth.

Objective: To investigate whether the dimensions of cavitated dentin carious lesions on the occlusoproximal surfaces of primary teeth could predict the location of cement-enamel junction (CEJ). Material and Methods: Two hundred extracted primary molars were selected and digital images were obtained. The teeth were set in arch models for clinical measurement. The cervical-occlusal (CO) and buccal-lingual/palatal (BL/P) cavities’ dimensions were obtained by digital (Image J) and clinical (periodontal millimeter probe) assessments. The cervical margin location was also determined. The thresholds (cut-off points) were determined by sensitivity, specificity and the areas under the receiver operating characteristics curves (Az) for the two methods. Pearson's correlation coefficient was used to investigate the correlation between clinical and digital measurements. Logistic regression analysis was performed to evaluate the association between the dimensions and cervical margin location. Results: There was a strong correlation between methods for all measurements (CO: r=0.90, VL/P: r=0.95). Cavities with BL/P distance higher than 4.5 mm and CO dimension higher than 3.5 mm had a lower chance of presenting the cervical limit above the CEJ, irrespective of the measurement method. Conclusion: CO and VL/P dimensions could be used to predict the CEJ location and, ultimately, as a clinical parameter for restorative decision-making. [ABSTRACT FROM AUTHOR]

The Prader-Willi Syndrome (PWS) is a complex genetic alteration affecting appetite, growth, metabolism, cognitive func-tion and behavior. It is characterized by low muscle tone, low height, incomplete sexual development, behavioral difficul-ties, and especially by chronic feelings of insatiable hunger and slow metabolism threatening life due to obesity. Changes in behaviour are very common and are related to the difficulty of dental treatment approach. We report the case of a 11-year-old patient, female, diagnosed with PWS, with high risk obesity and serious behavioural alterations. Clinically, she present-ed active caries lesions, periodontitis and dental extraction need. Conditioning for dental treatment was performed using techniques of ludotherapy and play therapy during 6 sessions, allowing the proposed interventions. It could be concluded that the use of both conditioning interventions decreased the anxiety facilitating the professional-patient bond, appearing as effective techniques for the outpatient care on pediatric patients dental control with severe behavioural alterations. El Síndrome de Prader-Willi (PWS) es una enfermedad genética compleja que afecta el apetito, crecimiento, metabolis-mo, función y comportamiento cognitivo. Está caracterizada por el bajo tono muscular, baja estatura, desarrollo sexual incompleto y problemas de conducta. El portador de este síndrome presenta características relacionadas a estados crónicos de insaciabilidad y de metabolismo lento que lo puede inducir a comer en exceso poniendo en riesgo su vida debido a la obesidad. Las alteraciones en la conducta son frecuentes dificultando el tratamiento odontológico de forma ambulatoria. Este es el reporte de caso de una paciente de 11 años de edad, diagnosticada con PWS, que presentó obesidad de alto ries-go y alteraciones severas en la conducta. Al examen clínico fueron observadas lesiones de caries dental, periodontitis y necesidad de extracciones dentarias. La paciente fue condicionada a través de las técnicas de Ludoterapia e Clownterapia durante 6 sesiones, permitiendo que los tratamientos fueran realizados. Estas dos terapias disminuyeron la ansiedad y fa-cilitaron el vínculo con el profesional, demostrando que son eficaces en el control de la conducta de pacientes pediátricos con alteraciones severas de conducta.

20. Perfil de problemas orales en infantes y preescolares atendidos en el servicio de urgencias odontológicas de un centro de salud infantil de Brasil

Objective: To determine the profile of oral problems in infants and preschool children treated at an emergency service from Clinic of Infantile Specialities / Clinical Baby of the State University of Londrina, Brazil. Materials and method: Retrospective study where 776 clinical histories of patients aged 0 to 72 months, treated between April 2007 and April 2008 were analysed. Data collection included gender, age, place of residence, parental occupation and educational level, reason for consultation, involved teeth and performed intervention. Chi-square test (χ2) and a significance level of 5% were used for variables associations. Results: A greater proportion of male patients (55.5%) attended the service and the main age group was the 0-24 months (27.3%). It was found that 62.9% lived in urban areas and most parents/caregivers had completed secondary school(30.5% parents, 33.8% mothers). The main reason for consultation was caries (38.9%) - pain as main complaint (42.0%) and dental trauma (28.9%). The most affected tooth by caries was the lower second primary molar and, regarding trauma, the upper central primary incisors were the most injured. Interventions varied from endodontic treatment (16.0%) and adequacy of oral environment (15.1%). There was a statistically significant association between mother educational level and reason for consultation (p = 0.001). Conclusion: The assessed health service was necessary and useful. However, oral health problems resolution in the paediatric population in Londrina will only occur after the implementation of reorganised dental services from the currently available. Objetivo: Determinar el perfil de los problemas orales en lactantes y preescolares atendidos en el servicio de urgencias de la Clínica de Especialidades infantil/ Clínica de Bebés de la Universidad Estatal de Londrina, Brasil. Materiales y métodos: Estudio retrospectivo donde se analizaron 776 historias clínicas de pacientes en el rango de edad de 0 a 72 meses, atendidos entre abril de 2007 a abril de 2008. Los datos recolectados fueron: Género, edad, lugar de residencia, ocupación y grado de escolaridad de los padres, motivo de consulta, dientes involucrados e intervenciones realizadas. Se utilizó la prueba de Chi2 (χ2) y un nivel de significancia de 5% para la asociación entre variables. Resultados: Se observó una mayor proporción de pacientes de género masculino en la búsqueda del servicio (55,5%) y la mayoría de pacientes se ubicaron en el rango de edad de 0 a 24 meses (27,3%). Se verificó que el 62,9% residían en la zona urbana y la mayoría de los padres/responsables habían cursado secundaria completa (30,5% padres, 33,8% madres). Los motivos de consulta más frecuentes en la atención fueron caries dental (38,9%) siendo el dolor la queja principal (42,0%) y trauma dental (28,9%). Los dientes más afectados por caries fueron los segundos molares inferiores primarios y, por trauma los incisivos centrales superiores primarios. Dentro de las intervenciones más comunes se encontraron el tratamiento endodóntico (16,0%) y la adecuación del medio bucal (15,1%). Se encontró una asociación estadísticamente significativa entre escolaridad de la madre y motivo de consulta en el servicio de urgencias (p=0,001). Conclusión: El servicio analizado es necesario y útil, sin embargo, la resolución de problemas orales en la población infantil de Londrina, ocurrirá por medio de una reorganización de los servicios odontológicos actuales.

23. Complications following replantation of primary teeth: a case report.

- Abstract
Introduction: the avulsion of primary teeth is a disturbing and unexpected event. Description: this report describes the clinical case of a three-year-old child who suffered an avulsion and replantation of the primary upper central incisors at the site of the injury. The guardians sought treatment for the child at the Federal University of Minas Gerais after the replantation. Four months later, the child suffered a new trauma and the replanted teeth presented advanced mobility, root resorption and fistula. The clinical conduct was extraction and rehabilitation with a fixed esthetic maintainer. Discussion: the literature describes two treatment options for avulsion of primary incisors: replantation and non-replantation. According to a recent systematic review, the difficulty in obtaining a consensus regarding the best clinical conduct is due, in part, to the scarcity of publications that present not only follow-ups with clinical success, but also with failures. The outcomes of replantation can be influenced by several factors. The time elapsed between replantation and splinting, and the new episode of trauma, negatively influenced the prognosis in the present case, leading to failure. Replantation of primary incisors is not yet evidence-based treatment. Therefore, this option must be chosen with caution and in ideal situations. Background: Atraumatic restorative treatment (ART) and chemomechanical removal of dental caries have been shown to be safe and effective restorative procedures. However, current literature lacks well-design studies comparing the pain perceived by infants using ART and chemo-mechanical removal techniques.Aims: To compare pain perception of children of two techniques (ART vs. chemomechanical removal) of restorative treatments for dental caries removal.Settings and Design: A randomized clinical trial with two parallels arms (1:1) was conducted in the cities of Manaus (AM) and Uberlândia (MG), Brazil.Methods: Children between 4 and 9 years old were selected. In both groups - ART (n = 20) and chemomechanical caries removal (Papaya® gel) (n = 20) - oximeter measurements (heart rate and blood oxygenation) were performed before the intervention and until the complete removal of the infected caries dentin. At the end of the treatment, a Wong-Baker scale was used to assess pain perception during the procedure. An independent researcher measured, in minutes, the duration of each technique.Statistical Analysis: Poisson regression analysis was used to evaluate the association between the outcome and the explicative variables.Results: No statistical difference in pain perception between the two procedures of carious tissue removal was observed. By contrast, a lower heart rate was observed when chemomechanical removal was used (P = 0.013).Conclusion: Children that underwent chemomechanical caries removal presented lower heart rate compared to those who underwent ART. - Abstract
Objetivo: Evaluar la longevidad de restauraciones de Tratamiento Restaurador Atraumático (TRA), utilizando dos materiales diferentes: un Cemento de Ionómero de Vidrio (CIV) de bajo costo y una Resina Compuesta de nanopartículas (RC), en cavidades ocluso-proximales de molares temporales. Materiales y métodos: Un total de 83 dientes fueron seleccionados en 43 niños de siete a diez años de edad, que presentaron al menos una lesión de caries ocluso proximal, en la ciudad de Cosmópolis / SP, Brasil. Los pacientes fueron asignados en dos grupos: CIV de bajo costo (Vitromolar - Nova DFL) y resina compuesta de nanopartículas (RC) (Filtek Z350 XT - 3M ESPE). Todos los procedimientos se realizaron de acuerdo con los principios del TRA. Las restauraciones fueron evaluadas después de 3, 6, 9 y 12 meses por un examinador entrenado. Para verificar la tasa de sobrevida de las restauraciones utilizamos el análisis de sobrevida de Kaplan-Meier y la prueba de Log-Rank; y, para evaluar la asociación entre el resultado y las variables del paciente se aplicó la prueba de regresión de Cox. El nivel de significancia para todas las pruebas fue del 5%. Resultados: Después de 12 meses de seguimiento, la sobrevida global de las restauraciones fue de 75,3%. Las tasas de sobrevida del CIV y de la RC fueron 62.2% y 93.7%, respectivamente. El análisis estadístico mostró diferencias significativas entre los materiales, donde la RC tuvo un mejor desempeño (HR = 0.15, IC 0.03 a 0.68, p = 0.014) que el CIV de bajo costo. No se encontraron diferencias estadísticas para otras variables (p >0.05). Conclusión: La utilización de resina compuesta de nanopartículas en las restauraciones de TRA tuvo un mejor rendimiento en comparación con el cemento de ionómero de vidrio de bajo costo, después de una evaluación de 12 meses. - Abstract
A infraoclusão de dentes decíduos pode ocorrer devido a fusão anatômica do cemento dentário com o osso alveolar, situação que denominamos como anquilose dentária. Esse evento, comum em molares decíduos, pode causar prejuízo no desenvolvimento da oclusão. O objetivo desse trabalho foi relatar a reabilitação de um molar decíduo infra-ocluído utilizando uma banda ortodôntica sobre o dente, cimento de ionômero de vidro modificado por resina encapsulado (Riva Ligh Cure, SDI, Brasil) e seu acompanhamento durante doze meses. Paciente de nove anos, do sexo feminino, compareceu ao consultório acompanhado de sua mãe com uma queixa estética. Após diagnostico de anquilose, foi realizada a restauração no elemento dentário em sessão única e realizado acompanhamento clínico e radiográfico durante doze meses. Ao final desse período observamos que o dente tratado permaneceu restaurado o que proporcionou a esfoliação das raízes do dente decíduo e consequentemente a erupção do dente permanente. Diante disso, concluímos que é possível tratar molares decíduos em infraoclusão com cimento de ionômero de vidro modificado por resina, no intuito de facilitar a realização do procedimento em sessão única e diminuir os riscos associados a um dente anquilosado não tratado precocemente.

Objective: To compare the capacity of silver diamine fluoride (30%) and the bioactive giomer light-curing varnish for arresting ICDAS score 5 non-retentive caries lesions. Material and Methods: An in vitro analytical study was conducted using 36 healthy primary teeth, in which a cavity was created, divided into 3 groups: control (distilled and deionized water), giomer-based light-curing varnish - Barrier Coat, and Silver Diamine Fluoride 30% - Cariestop (SDF). The samples were analyzed for mineral loss as measured by surface microhardness (SMH). The Knoop microhardness test was performed using the HVS-1000 microhardness tester. The data were compared by two-way analysis of variance - treatment and time (before and after treatment) followed by Tukey's post-test. The significance level adopted in all analyzes was 5%. Results: The specimens treated with giomer showed higher SMH compared to the untreated group but were similar to those of the SDF group. The SDF group, on the other hand, showed no statistically significant difference from the control group (p=0.010). Conclusion: The giomer varnish had a remineralizing effect on artificial dentinal caries. [ABSTRACT FROM AUTHOR]

Purpose: To evaluate if using the laser fluorescence (LF) device improves the detection of early or more advanced occlusal caries lesions in primary teeth. Materials and Methods: A total of 181 occlusal sites in primary teeth were assessed by one examiner using DIAGNOdent. The same examiner and a second examiner evaluated 72 of the sites again to assess intra- and inter-examiner reproducibility. After histological validation, lesions were divided at three thresholds according to the lesion depth: outer half of the enamel (D1), inner half of the enamel (D2) and dentine (D3). The sensitivity, specificity, accuracy and area under ROC curve were calculated for cut-off points for the sample considering the aforementioned thresholds. The parameters were compared using chi-square test and by comparison between unpaired ROC curves. Results: The overall LF performance was better at dentine threshold than at enamel threshold. The higher specificity was found at D3 and D2, and higher accuracy at D3 (P < 0.001). The intra- and inter-examiner agreements were classified as good or excellent for all thresholds. Conclusions: The LF device performs better at the dentine threshold than at the enamel threshold. This method does not perform well in detecting initial enamel caries lesions. [ABSTRACT FROM AUTHOR]

Objectives: This study aimed to evaluate the knowledge of Pediatric Dentists in the city of Fortaleza-CE about the practice of sedation. Methods: Participated 66 professional's specialists registered in the Regional Council of Dentistry - Ceara who worked in public and private service. Data collection was performed through a structured and self-administered questionnaire. The data obtained were tabulated in the IBM SPSS Statistics program and analyzed through a descriptive and exploratory statistical analysis. Results: The results showed that 36.4% of the professionals had 5 to 10 years of specialization, 93.9% were female and 59.1% both worked in the public and private sectors. Only 21.1% had a sedation course and 33.3% used the technique. Midazolam and Cloral Hydrate were the most commonly cited drugs in the sedation technique. Allergic reactions were the most reported as risk of sedation. Conclusion: Considering the limitations of this study, it is possible to consider that the professionals have reasonable knowledge and few use the technique of sedation, mainly because they do not feel safe. Los dientes supernumerarios se describen como anormalidades en el número de dientes debido a la actividad excesiva de la lámina dental. Estos pueden ser únicos o múltiples, unilateral o bilateral, con malformación o no en tamaño y forma, en proceso de erupción o retenidos. Entre las posibilidades estéticas de tratamiento para los dientes anatómicamente comprometidos tenemos la rehabilitación estética. Paciente, 4 años de edad de sexo femenino, llegó a la clínica de la Facultad de Odontología de São Leopoldo Mandic acompañada por su madre, después de la anamnesis fue realizado el examen clínico en el que se observó una movilidad anormal del diente 61. Para un mejor diagnóstico, se realizó una radiografía oclusal modificada donde se detectó la presencia de un diente supernumerario en la región de los incisivos superiores. En la siguiente consulta, el paciente tenía el diente 61 exfoliado y el diente supernumerario en proceso de erupción presentando características de diente conoide, el tratamiento propuesto fue rehabilitación estética del supernumerario con resina compuesta por medio de las coronas de celuloide. Este procedimiento hizo posible restablecer la armonía de la sonrisa de forma simple y eficiente, además de la satisfacción del paciente y del responsable con respecto a la sonrisa estética.

El Síndrome de Prader-Willi (PWS) es una enfermedad genética compleja que afecta el apetito, crecimiento, metabolismo, función y comportamiento cognitivo. Está caracterizada por el bajo tono muscular, baja estatura, desarrollo sexual incompleto y problemas de conducta. El portador de este síndrome presenta características relacionadas a estados crónicos de insaciabilidad y de metabolismo lento que lo puede inducir a comer en exceso poniendo en riesgo su vida debido a la obesidad. Las alteraciones en la conducta son frecuentes dificultando el tratamiento odontológico de forma ambulatoria. Este es el reporte de caso de una paciente de 11 años de edad, diagnosticada con PWS, que presentó obesidad de alto riesgo y alteraciones severas en la conducta. Al examen clínico fueron observadas lesiones de caries dental, periodontitis y necesidad de extracciones dentarias. La paciente fue condicionada a través de las técnicas de Ludoterapia e Clownterapia durante 6 sesiones, permitiendo que los tratamientos fueran realizados. Estas dos terapias disminuyeron la ansiedad y facilitaron el vínculo con el profesional, demostrando que son eficaces en el control de la conducta de pacientes pediátricos con alteraciones severas de conducta.

Background: Clinical studies are being conducted in less strict conditions in order to establish an adequate scientific basis for decision making. The aim of this pragmatic randomized clinical trial is to evaluate the effectiveness of single and multiple-surfaces restorations performed following the Atraumatic Restorative Treatment (ART) premises compared with Conventional Treatment (CT) using bulk fill composite restorations in primary and permanent teeth. Methods/design: A total of 1,214 5-to-13 year-old children with at least one single or multiple-surface dentin caries lesion in primary or permanent molars will be selected in public schools of Barueri-SP, Brazil. The participants will be randomly assigned into 2 groups: CT (caries removal with bur and restoration performed with Scotchbond™ Universal Adhesive system associated with Filtek Bulk Fill -- 3 M/ESPE) and ART (Caries removal with hand instruments and restoration with high viscosity glass ionomer cement Ketac Molar Easy Mix -- 3 M/ESPE). Ten untrained dentists will perform the treatment in in dental offices located at public schools. The restorations will be evaluated after 6, 12 and 24 months by an independent trained and calibrated examiner. The restoration and tooth survival, the cost- effectiveness analysis between the two groups and the operators' preferences regarding the techniques will be also evaluated. Kaplan-Meier survival analysis and log-rank test will be applied for the restoration and tooth survival. All the average event rates in the two groups will be modelled and compared with a Cox proportional hazard shared frailty model since there is an operator-cluster effect. The significance level for all analyses will be 5 %. Discussion: Our hypothesis is that despite similar expected effectiveness between ART using high viscosity GIC and conventional treatment using bulk fill composite resin when treating single or multiple-surface in posterior primary and permanent teeth, ART will present superior cost-effectiveness. Background: In literature there are few data on total-etch adhesives in primary teeth with regard to different etching times and application of multiple layers. Objective: to assess in vitro the influence of acid etching at different times and number of layers of an adhesive system in the marginal adaptation of resin composite in deciduous teeth by a liner computerized method. Study Design: Eighty deciduous teeth were selected and Class V cavity preparations in enamel and dentin were made. The specimens were randomly divided into 4 groups (n=20): G1 (control): acid etching for 15 sec and application of 2 layers of Single Bond adhesive system (SB); G2: 15 sec and 1 layer; G3: 15 sec and 3 layers; G4: 10 sec and 2 layers. After performing restoration, the specimens were immersed in 0.5% methylene blue for 4 hours and were then sectioned and photographed. The microleakage evaluation was realized by 3 calibrated blind examiners through pixel assessment using the TPSDig program. The extension of the cavity wall and extension of the microleakage was measured in pixels for the leakage percentage calculation. The results were submitted to the descriptive analysis and Kruskal-Wallis test. Results: Ten seconds of etching and application of 3 layers of SB on the gingival wall on dentin resulted in greater microleakage. Conclusion: The application protocol recommended by the manufacturer in vitro (15s of acid etching and 2 layers of adhesive) was the one that showed less microleakage in deciduous teeth. Objetivo: Realizar estudo retrospectivo por meio de exames radiográficos de crianças apresentando 21 molares decíduos com cavidades profundas, em fase de maturação e regressão pulpar do ciclo biológico, nos quais foi realizado tratamento pulpar indireto em sessão única, sem posterior reabertura para a reavaliação da dentina remanescente. Método: De uma amostra inicial de 60 prontuários, somente 12 foram selecionados. A idade média das crianças parti cipantes foi de 7 anos no início do tratamento, totalizando, dessa forma, 21 dentes decíduos com lesões de cárie profundas tratados previamente com tratamento pulpar indireto. Os critérios de inclusão foram: molares decíduos, lesões de cárie profundas, dentes decíduos restaurados com tratamento pulpar indireto utilizando resina composta ou cimento de ionômero de vidro, tempomínimo de acompanhamento de 6 meses, pacientes que retornaram para exame radiográfi co, plano de tratamento proposto para os dentes em questão, datas das intervenções, datas das radiografias iniciais e finais presentes nos prontuários. Resultados: A análise das 21 radiografias selecionadas revelou que com a aplicação da técnica de TPI, nenhum dos dentes avaliados apresentou processos de reabsorção patológica interna ou externa e tão pouco ocorreram lesões periapicais e intra-radiculares durante os períodos de acompanhamento deste estudo. Conclusão: O TPI em única sessão é uma alternativa para o tratamento de lesões profundas em dentes decíduos nas fases de maturação e regressão pulpar. São necessários mais estudos prospectivos para obtenção de informações adicionais para a resposta do complexo denti no-pulpar após a remoção parcial do tecido cariado. Objective: To perform a retrospective study by the examination of radiographs of children presenting 21 primary molars with deep cavities, in the pulp maturation and regression phase of the biological cycle, in which indirect pulp treatment was performed in a single session, without further reopening for reevaluation of the remaining dentin. Method: From an initial sample of 60 charts, only 12 were selected. The mean age of the children enrolled in the study was 7 years in the beginning of the treatment, totalizing 21 primary teeth with deep caries lesions previously treated with indirect pulp treatment. The inclusion criteria were: primary molars, deep caries lesions, primary teeth restored by indirect pulp treatment using composite resin or glass ionomer cement, minimal follow-up period of 6 months, patients that returned for the radiographic exam, treatment plan proposed for the target teeth, date of the interventions, dates of the initial and final radiographs present in the charts. Results: The analysis of the 21 radiographs selected for the study revealed that none of the teeth subjected to indirect pulp treatment presented internal or external pathological resorpti on processes and no periapical or intraradicular lesions were found during the follow-up periods of the study. Conclusion: The indirect pulp treatment in a single session is an alternative for the treatment of deep caries lesions in primary teeth in the pulp maturation and regression phases. Further prospective studies are needed to obtain additional information for the pulp-denti n complex response after partial removal of the carious tissue. [ABSTRACT FROM AUTHOR]

The authors describe a clinical case of oral rehabilitation, in a 4-year-old patient, using indirect composite onlay restorations. Eight severely decayed primary molars were restored. The clinical findings after 4 years demonstrated the efficiency of the technique as well as its indication in extensive restorations in Pediatric Dentistry, reestablishing function and aesthetics of these primary teeth. Purpose: To evaluate if high-powered magnification could improve the performance of visual inspection and compare with other methods to detect occhisal caries lesions in primary teeth. Methods: 110 suspected sites were analyzed by two examiners with four methods: visual inspection, visual inspection aided with high-powered magnification (x20), laser fluorescence (DIAGNOdent) and radiographs. The first examiner performed the examinations in a second session to assess the intra-examiner reproducibility. Then, the teeth were cut and the sections were evaluated in stereomicroscope. Sensitivity, specificity and accuracy were calculated at enamel and dentin caries thresholds, and compared using McNemar change test. The ROC analysis was performed and the mean of areas under ROC curves were compared using ANOVA and Student-Newman-Keuls test. Inter- and intra-examiner reproducibility was assessed by calculating Cohen's Kappa. Results: The visual inspection with magnification did not change the accuracy and reproducibility of the visual inspection alone. Purpose: To assess the total viable bacteria in infected dentin after sealing with glass-ionomer cement containing 1% metronidazole, 1% ciprofloxacin and 1% cefaclor. Methods: 40 children were selected, with ages ranging from 4-10 years, with primary teeth with active carious lesions without any pulpal involvement The patients were divided randomly into two groups in accordance to the glass-innomer cement used to seal the carious lesions: Group l (control, n=20), sealing with conventional glass-ionomer cement; and Group 2 (test, n=20) scaling with glass-ionomer cement including antibiotics, The infected dentin was collected both before and 24 hours after the sealing of the cavities and, after inoculation in blood agar, all the plates were incubated in containers kept at 37°C for 5 days in an atmosphere of 85% N
2 , 10% CO2 and 5% H2 , for subsequent counting of total viable bacteria. Results: The results were analyzed using the Wilcoxon and Mann Whitney tests. The glass-ionomer cement with 1% of metronidazole, 1% of ciprofloxacin and 1% of cefaclor showed a significantly greater reduction in microbiota in the infected dentin in comparison to the reduction with the conventional ionomer cement (P< 0.01), with a mean reduction of 98.65% of all viable bacteria. The infected dentin after sealing with glass-ionomer cement with antibiotics showed, using scanning electron microscopy, the presence of bacterial aggregates, intermbular dentin with exposure of collagen fibers, and dentin tubules. The purpose of this in vitro study is to assess the bond strength of three different posts used to restore primary anterior teeth. These posts were made by composite resin, 0.7 mm alpha-shaped orthodontic wire and dentin posts. Thirty roots of extracted primary anterior teeth, with initial physiological resorption, were mounted in a container filled with self cured resin and were endodontically treated Thirty posts were prepared using three different materials: 10 composite resin posts (Z100), 10 alpha-shaped orthodontic wire and 10 dentin posts. A bulk of composite resin (Z100) was made over the posts using a circular Teflon bisected strip. The specimens were stored in 37°C water solution for 24 hours before tensile bond strength tests in 0.5 mm/min speed were performed (Instron model 4442 test machine-Instron Corporation). The results were submitted to the Kruskal-Wallis test and there were no significant statistical differences among the three types of posts assessed (p = 0.8666). The dentin posts technique might be considered an alternative for rehabilitation in pediatric dentistry, because of the possibility of natural resorption. [ABSTRACT FROM AUTHOR]

Purpose: The purpose of this study was to verify the influence of individual calibration on sound tooth surfaces and the number of readings on in vitro performance of a laser fluorescence (LF) device to detect occlusal caries in primary teeth. Methods: After standard calibration (on the porcelain reference object), 72 clinically suspect sites on occlusal surfaces of 54 exfoliated or extracted (for orthodontic purposes) primary molars were assessed using the DIAGNOdent. First, after individual calibration (on a sound surface of each tooth), 3 readings were performed for each site. Subsequently, another 3 readings were taken, but without individual calibration on a sound surface of the tooth. After these assessments, sites were also evaluated with only one reading after individual calibration. Histological validation was performed as the gold standard. LF values, sensitivity, specificity, accuracy (number of correct diagnosis in both sound and diseased teeth) were calculated and compared using the McNemar change test. The area under receiver operating characteristics (ROC) curves was also compared. Results: Readings with standard calibration only (mean=11.7±10.6 SD) were significantly higher statistically than assessment with 3 readings after individual calibration (10.7±10.7) and with 1 reading (10.2±8.3) after individual calibration. Nevertheless, sensitivity, specificity, accuracy, and area under ROC curve did not change significantly. Conclusion: Absence of individual calibration does not affect the laser fluorescence device's performance in detecting occlusal caries in primary teeth. [ABSTRACT FROM AUTHOR]

49. Bonding Performance of Universal and Contemporary Adhesives in Primary Teeth: A Systematic Review and Network Meta-Analysis of In Vitro Studies.

Purpose: To systematically review the literature to compare the bond strength of universal adhesives with etch-and-rinse and self-etch systems to primary teeth. Methods: The search was carried out in PubMed®/MEDLINE, Scopus®, LILACS, Embase®, and Web of Science™ databases with no restrictions. Two reviewers independently selected the studies, extracted the data, and assessed the risk of bias. Direct comparisons among universal adhesive in etch-and-rinse (UER) and self-etch (USE) modes and etch-and-rinse (ER) and self-etch (SE) systems were performed considering different substrates (sound enamel and dentin, and carious dentin) through meta-analyses of random effects. A mixed treatment comparisons meta-analysis was also performed comparing the bond strength of all adhesive approaches on sound dentin. Results: From 3,276 potentially eligible studies, 18 were selected for full-text analysis, and eight were included in the systematic review. All studies included in the meta-analyses evaluated a mild universal adhesive (Scotchbond™ Universal). In direct comparisons, there was no difference between USE and SE to sound enamel (mean difference [MD] equals 5.22; 95 percent confidence interval [95% CI] equals -9.09 to 19.52). In carious dentin, the results favored only ER over USE (MD equals -3.88; 95% CI equals -7.40 to -0.37). In sound dentin, the bond strength values of UER were higher than ER (MD equals 5.50; 95% CI equals 4.03 to 6.96). The rank probability showed that the best treatment on sound dentin was UER. Conclusion: Pooled in vitro data suggest that a mild universal adhesive system can substitute the etch-and-rinse and self-etch systems for restoring primary teeth. [ABSTRACT FROM AUTHOR]
