The National Lung Screening Trial (NLST) is a randomized multicenter study comparing low-dose helical computed tomography (CT) with chest radiography in the screening of older current and former heavy smokers for early detection of lung cancer, which is the leading cause of cancer-related death in the United States. Five-year survival rates approach 70% with surgical resection of stage IA disease; however, more than 75% of individuals have incurable locally advanced or metastatic disease, the latter having a 5-year survival of less than 5%. It is plausible that treatment should be more effective and the likelihood of death decreased if asymptomatic lung cancer is detected through screening early enough in its preclinical phase. For these reasons, there is intense interest and intuitive appeal in lung cancer screening with low-dose CT. The use of survival as the determinant of screening effectiveness is, however, confounded by the well-described biases of lead time, length, and overdiagnosis. Despite previous attempts, no test has been shown to reduce lung cancer mortality, an endpoint that circumvents screening biases and provides a definitive measure of benefit when assessed in a randomized controlled trial that enables comparison of mortality rates between screened individuals and a control group that does not undergo the screening intervention of interest. The NLST is such a trial. The rationale for and design of the NLST are presented.

A ceramic ytterbium:yttrium aluminum garnet (Yb:YAG) thin-disk laser is investigated at 15°C (288 K) and also at 80 K, where it behaves as a four-level laser. We introduce a new two-phase spray cooling method to cool the Yb:YAG. One system relies on R134a refrigerant while the other uses liquid nitrogen (LN2). The use of two systems allows the same disk to be tested at the two temperatures. When the Yb:YAG is cooled from room to cryogenic temperatures, the lasing threshold drops from 155 W to near 10 W, while the slope efficiency increases from 54% to a 63%. A 277 W laser with 520 W of pump is demonstrated. We also model the thermal and structural properties at these two temperatures and estimate the beam quality.

We derive equations for the ASE intensity, decay time, and heat load. The crux of our development is frequency integration over the gain lineshape followed by a spatial integration over the emitters. These integrations result in a gain length that is determined from experiment. We measure the gain as a function of incident pump power for a multi-pass pumped Yb:YAG disk doped at 9.8 at.% with an anti-ASE cap. The incident pump powers are up to 3kW. Our fit to the measured gain is within 10% of the measured gain up to pump powers where the gain starts to flatten out and roll over. In this comparison we extract the gain length that turns out to be 43% of the pump spot size of 7mm.

Stability in a Raman ring resonator is studied by using the adiabatic approximation. The analysis is based on far-off resonance Raman scattering in hydrogen. A medium-power approximation is employed, which is good for intensities less than 30 MW/cm2. The resulting differential equations retain their standard low-power Raman gain dependance in addition to an intensity-dependent phase. The steady-state intensity input–output behavior, as well as the linear stability analysis, is accomplished analytically without invoking the mean-field approximation. Feedback is applied to the Stokes beam, a gain situation, or to the depleted pump beam. The Stokes frequency is assumed to be perfectly tuned to the atomic and cavity resonances. It is shown that both situations are multistable and that the power-dependent phase largely determines the stability characteristics. Furthermore, we show that the negative slope branches can be stable when feedback is applied to the pump if the output pump intensity is decreasing with increasing input pump intensity.

We investigate the saturation effects of power broadening, Stark shifting, and population transfer on Stokes conversion in stimulated Raman scattering. We do not make the usual rotating wave approximation because the detuning from the next electronic state is assumed to be in the optical regime. Retaining the counter-rotating terms allows an exact determination of the pump and Stokes indexes of refraction. Steady-state solutions for the Stokes intensity and phase are obtained and the effects of making the rotating wave approximation (RWA) are discussed. Finally, we examine the behavior of these solutions for Stokes conversion in hydrogen gas when geometric propagation is appropriate.

Steady-state thermal blooming in perfectly conductive rectangular waveguides is described by the coupling between the Helmholtz equation and the diffusion equation without convection. The fields are pinned at the walls by Dirichlet and Neumann conditions and the temperature by Dirichlet conditions. The equations are solved by Green’s functions in a perturbation scheme employing the Born approximation. The first-order solutions indicate that only about 4% of the energy is transferred from the unperturbed mode, as defined by a constant index of refraction, to all the higher-order modes for moderate absorption.
