The shaft capacity of driven piles in clay increases with time even after installation-induced consolidation is completed. However, existing studies have shown that these gains in capacity are variable and cannot be predicted reliably. Projects such as those involving life extension of existing offshore platforms (which often involve increased platform loads) as well as those considering foundation re-use provide the motivation for this paper, which investigates the potential for model-scale testing to replicate the ageing characteristic of shaft friction in a laboratory environment. The study employs a number of model piles installed in reconstituted, high-plasticity Onsoy clay in laboratory pressure chambers. The first-time tension load tests on these piles, performed over the period of 1 year after installation, are compared with a similar series of tests on full-scale driven piles in Onsoy, Norway. Good agreement between laboratory and field experiments is observed when allowance is made for the significantly longer consolidation periods of the full-scale piles. This finding implies that ageing effects of shaft friction in clay can be investigated over relatively short time periods using model piles in laboratory testing chambers.

This paper is based on experience obtained during design of Payara Project, offshore Guyana, where 10" production and 16" water injection pipelines were found to be potentially susceptible to walking. Offshore pipelines that are subject to HT/HP conditions, riser tensions and seabed slopes can be susceptible to walking, which may jeopardize the integrity of connected subsea structures. Should the cumulated walking during the operating life of the pipeline exceed the maximum allowed displacement, an anchoring system could be required to mitigate the movement. However, predicting pipeline walking is a complex matter, depending on parameters often affected by significant uncertainty, such as pressure and temperature conditions, heating and cooldown cycles, soil pipe interaction, planned buckle evolution and route modifications. For this reason, a walking assessment performed during detail design can lead to uncertain results, and question arises as to the best timing to install walking mitigation structures (WMS). A Wait-and-See approach represents a good compromise, mediating between CAPEX and OPEX needs, and allowing a risk-based decision process based on pipeline behavior as monitored during planned surveys. However, the installation of WMS during operation may require ancillary structures to be installed on ‘day one’ that could nullify the strategic advantage given by the Wait-and-See, not to mention the mob/demob cost of the vessel used for installation. The Pipe Clamping Mattress (PCM), patented by Shell and commercialized by MMA Offshore, has been proposed as a convenient and effective way to anchor the pipeline, either at day one or during operation. The PCM comprises a two-winged mattress fabricated by pivoting two concrete blocks around a central hinge, and which is shaped to engage and clamp the pipe. A log mattress is then positioned to provide additional clamping force. The overall weight of PCM provides the clamping grip required to prevent pipeline slippage as well as the additional axial resistance needed to mitigate or arrest walking. An advanced FE Model for PCM performance study has been developed in ABAQUS based on the CEL methodology (Coupled Eulerian-Lagrangian). In comparison to more conventional models, this technique models the seabed as a deformable Eulerian domain, in which PCM and pipeline can penetrate. Seabed settlement and soil displacement due to system motion can be fully calculated, estimating the evolution of axial resistance and the growth of berms at the PCM sides. The objective of this paper is to improve the understanding of pipeline/PCM interaction, in particular with respect to: Verifying the overall embedment of the pipeline/PCM system.Verifying and assessing the PCM restraining capacity against axial displacement.Assessing the minimum number of PCMs required to ensure walking prevention/arrest.

Plate anchors are an attractive technology for mooring floating facilities as relative to piles, suction caissons and drag anchors they provide a much higher capacity relative to their mass. Plate anchors may experience an extreme loading event that will cause geotechnical failure, although they will still retain a residual capacity. The displacement associated with bringing the anchor to failure will induce excess pore pressures that initially reduce soil strength, but will dissipate over time, leading to regains in soil strength and hence anchor capacity. This paper considers the time scales and magnitude of this anchor capacity regain through a series of model scale experiments conducted in a geotechnical centrifuge. The experiments involved vertical loading of pre-embedded horizontally orientated circular anchors in normally consolidated kaolin clay. The results show that anchor capacity regain is a function of consolidation time and the level of resistance maintained on the anchor, with the longest consolidation time and highest maintained resistance leading to a capacity regain of approximately 60%. These capacity increases are described here using a simple hyperbolic function, which provides a basis for estimating the time needed for the residual anchor capacity to regain sufficient capacity following a movement event.

The impact of dietary phosphorus on chronic renal disease in cats, humans and other species is receiving increasing attention. As Ca and P metabolism are linked, the ratio of Ca:P is an important factor for consideration when formulating diets for cats and other animals. Here, we describe a fully randomised crossover study including twenty-four healthy, neutered adult cats, investigating postprandial responses in plasma P, ionised Ca and parathyroid hormone (PTH) following one meal (50 % of individual metabolic energy requirement) of each of six experimental diets. Diets were formulated to provide P at either 0·75 or 1·5 g/1000 kcal (4184 kJ) from the soluble phosphorus salt sodium tripolyphosphate (STPP, Na5P3O10), variable levels of organic Ca and P sources, and an intended total Ca:P of about 1·0, 1·5 or 2·0. For each experimental diet, baseline fasted blood samples were collected prior to the meal, and serial blood samples collected hourly for 6 h thereafter. For all diets, a significant increase from baseline was observed at 120 min in plasma PTH (P < 0·001). The diet containing the highest STPP inclusion level and lowest Ca:P induced the highest peaks in postprandial plasma P and PTH levels (1·8 mmol/l and 27·2 pg/ml, respectively), and the longest duration of concentrations raised above baseline were observed at 3 h for P and 6 h for PTH. Data indicate that Ca:P modulates postprandial plasma P and PTH. Therefore, when formulating diets containing soluble P salts for cats, increasing the Ca:P ratio should be considered.

In recent years, floating wind has been attracting increasing attention due to its ability to be deployed in greater water depths, which are usually associated with higher wind energy. At present, floating wind technology is more expensive than fixed offshore wind, with higher cost being driven by challenges such as the extended length of mooring lines, installation of anchors in deep water etc. For floating wind to become more competitive it will be necessary to devise methods that are able to bring down the overall cost by addressing areas such as these. This paper presents a relatively novel concept that may help reduce the capital cost by using an arrangement of ‘shared mooring lines’. This arrangement is able to significantly reduce the number of anchors used in a windfarm, compared to the current common industry practice of using 3 mooring lines to moor each individual turbine, by connecting some of the turbines to their neighbours, rather than the seabed. Furthermore, this study aims to address a scenario in which shared mooring lines are expected to be more favourable, i.e. deep water (1000 m) and large (10 MW) turbines. This numerical study provides some insights to a cost-saving mooring concept for a FOWT wind farm, by investigating loads in mooring lines and anchors, as well as relative displacement of the FOWTs in the windfarm. The cost saving is roughly estimated in terms of number of anchors and length of mooring line saved — serving as preliminary guidance for shared mooring line arrangements to be applied on larger windfarms or other geometric layouts (e.g. staggered rectangular array or hexagonally shared mooring connection).

Background Dietary protein and phosphorus (P) restriction is the mainstay for nutritional management of chronic kidney disease (CKD). However, adequate restriction levels for cats with early CKD remain unclear. Objectives To investigate responses in cats with early CKD to varying dietary protein, P, and calcium (Ca) : P ratio. Animals Nineteen research colony cats with International Renal Interest Society stages 1‐2 CKD. Methods In an opportunistic longitudinal case study, cats were fed a low protein (59 g/Mcal), low P (0.84 g/Mcal) dry diet (LP‐LP; Ca : P = 1.9) for 18 months and later transitioned onto a moderate protein (76‐98 g/Mcal), moderate P (1.4‐1.6 g/Mcal) dry‐wet diet regimen (MP‐MP; Ca : P = 1.4‐1.6) for 22 months. Fold‐changes in serum creatinine, total Ca (tCa) and P (primary outcomes) and fibroblast growth factor 23 (FGF23) were assessed by linear‐mixed models. Age-related changes in gastrointestinal function have been reported in companion animals, but the impact on digestive efficiency remains uncertain. Healthy dogs (n = 37, 2.6–14.2 years) received four diets varying in total dietary fibre (TDF, 6–29%, as fed). Healthy cats (n = 28, 1–13 years) received four diets with two fat (10–12%, 17–18%) and TDF (9 and 12%) levels. In a crossover design, diets were provided over four consecutive 10-day cycles, including a 4-day faecal collection. Apparent crude protein (CP), ether extract (EE), TDF, calcium (Ca), and phosphorus (P) digestibilities were determined. The effect of age was analysed as a continuous variable in dogs and as differences between adult (1–5 years) and senior (7–13 years) cats. In dogs, EE digestibility was unaffected by age (p &gt, 0.10). Dogs of 6–12 years had higher digestibility of CP (p = 0.032), TDF (p = 0.019), Ca (p = 0.019), and P (p = 0.024) when fed the 6% TDF diet. Senior cats had greater digestibility of TDF (p &lt, 0.01) and Ca (p = 0.024) but had lower EE and CP digestibility with one diet (17% fat, 9%TDF) (age, p &gt, 0.10, diet × age, p &lt, 0.001). Healthy ageing was associated with preserved nutrient digestibility in dogs and cats within the age ranges studied. The effect of ingredient sources in senior cats warrants further investigation.

Seventy-five healthy adult cats (n 25/group) were fed either a low P control (1·4 g/1000 kcal [4184kJ]; Ca:P ratio 0·97) or one of two test diets with 4 g/1000 kcal (4184 kJ); Ca:P 1·04 or 5 g/1000 kcal (4184kJ); Ca:P 1·27, both incorporating 1 g/1000 kcal (4184 kJ) sodium tripolyphosphate (STPP) – for a period of 30 weeks in a randomised parallel-group study. Health markers in blood and urine, glomerular filtration rate, renal ultrasound and bone density were assessed at baseline and at regular time points. At the end of the test period, responses following transition to a commercial diet (total P – 2·34 g/1000 kcal [4184kJ], Ca:P 1·3) for a 4-week washout period were also assessed. No adverse effects on general, kidney or bone (skeletal) function and health were observed. P and Ca balance, some serum biochemistry parameters and regulatory hormones were increased in cats fed test diets from week 2 onwards (P ≤ 0·05). Data from the washout period suggest that increased serum creatinine and urea values observed in the two test diet groups were influenced by dietary differences during the test period, and not indicative of changes in renal function. The present data suggest no observed adverse effect level for feline diets containing 1 g P/1000 kcal (4184 kJ) from STPP and total P level of up to 5 g/1000 kcal (4184 kJ) when fed for 30 weeks.

Background Using indicator amino acid oxidation methodology, the mean dietary requirement of adult dogs for methionine (Met) was estimated to be ∼66% of the current recommended allowance. Dogs fed a diet formulated to provide the estimated mean Met requirement for 32 wk maintained plasma Met, seemingly supported by betaine oxidation. Objective To gain a better understanding of the metabolic changes that were associated with supporting plasma Met when dogs were fed a limited Met diet over 32 wk, we analyzed plasma samples taken from that study using a data-driven metabolomics approach. Methods Labrador retrievers (20 females/13 males; mean age: 4.9 y; range: 2.0-7.9 y) were fed semi-purified, nutritionally complete diets. After 4 wk of feeding a control diet (DL-Met; 1.37 g/1000 kcal), 17 dogs remained on this diet and 16 were transitioned to a test diet formulated to the estimated mean Met requirement (0.55 g/1000 kcal), with dietary total sulfur amino acid maintained with additional l-cystine (Cys). Dogs were individually fed diets to maintain a stable body weight at an ideal body condition score for 32 wk. Plasma samples from fasted blood collected at baseline and 8 and 32 wk were analyzed using untargeted metabolic profiling. Results Analysis of metabolites (n = 593) confirmed our primary findings (increased Met, betaine, and dimethylglycine). Metabolite changes consistent with repartitioning choline to support Met cycling included reduced pools of lipids derived via phosphatidylethanolamine N-methyltransferase and enhanced fatty acid oxidation. Some changes were consistent with metabolomics studies reported in other species that used interventions known to extend life span (caloric- and Met-restricted diets or feeding strategy). Conclusions Changes in the plasma metabolome were consistent with reported adaptations to support Met-dependent activities. We propose that feeding a limited-Met, high-Cys diet using the estimated mean Met requirement in adult Labrador retrievers alters regulation of the Met cycle, thereby altering metabolism, similar to interventions that extend life span.

A novel solution to mitigate pipeline walking, namely the use of pipe-clamping mattresses (PCMs), was first developed for the Malampaya project (Frankenmolen et al. 2017). Comprising a hinged concrete structure designed to clamp onto a section of pipeline, and supporting (post-installed) ballast weight that is transferred directly to the pipeline, PCMs are considered a highly efficient alternative to more traditional solutions such as rock dump or concrete mattress. The original PCM geotechnical design was based primarily on analyses extrapolated from pipeline-seabed interaction, supported by a standard suite of classification and interface tests. Physical model testing was not performed. While observations (taken since installation) show that the PCMs have effectively mitigated pipeline walking in the seabed conditions at Malampaya, their performance in other soil types has not been investigated. To provide further evidence on the effectiveness of PCMs, and investigate their performance over time, a series of centrifuge tests were performed in a soil sample representative of deep-water Gulf of Mexico conditions. In each test, a model representing the PCM was installed on the pipeline section, which was then subjected to cyclic axial displacement. Settlement of the pipeline-PCM system, as well as changes in axial resistance, were directly measured and are reported in this paper.

Oligosaccharides are important components of milk, serving as substrates for the intestinal microbiota, acting as antimicrobials that prevent pathogen colonization, and supporting the developing gastrointestinal immune system of neonates. Nutrient composition of canine and feline milk samples has been described previously, but little is known about the oligosaccharide content. Therefore, the objective of this study was to characterize canine and feline milk samples using a high-throughput glycomics approach. 23 dogs (9 Labrador retriever and 14 Labrador retriever x golden retriever crossbreed) and 6 domestic shorthair cats were recruited to the study. Milk samples were collected by manual expression at time points after parturition. Samples were collected across 2 phases per species, differentiated by maternal diet. Following extraction, oligosaccharide content was determined by liquid chromatography-mass spectrometry (LC-MS). In canine milk samples, 3 structures accounted for over 90% of all oligosaccharides detected across two diet groups. These were 3’-sialyllactose, 6’-sialyllactose, and 2’-fucosyllactose. In feline samples, a more diverse range of oligosaccharides was detected, with up to 16 structures present at relative abundance >1% of the total. Difucosyllactose-N-hexaose b, 3’-sialyllactose and lacto-N-neohexaose were all detected at abundances >10% in feline milk samples. Statistically significant differences (p

Renal disease has a high incidence in cats, and some evidence implicates dietary P as well. To investigate this further, two studies in healthy adult cats were conducted. Study 1 (36 weeks) included forty-eight cats, stratified to control or test diets providing 1·2 or 4·8 g/1000 kcal (4184 kJ) P (0 or approximately 3·6 g/1000 kcal (4184 kJ) inorganic P, Ca:P 1·2, 0·6). Study 2 (29 weeks) included fifty cats, stratified to control or test diets, providing 1·3 or 3·6 g/1000 kcal (4184 kJ) P (0 or approximately 1·5 g/1000 kcal (4184 kJ) inorganic P, Ca:P 1·2, 0·9). Health markers, glomerular filtration rate (GFR) and mineral balance were measured regularly, with abdominal ultrasound. Study 1 was halted after 4 weeks as the test group GFR reduced by 0·4 (95 % CI 0·3, 0·5) ml/min per kg, and ultrasound revealed changes in renal echogenicity. In study 2, at week 28, no change in mean GFR was observed (P >0·05); however, altered renal echogenicity was detected in 36 % of test cats. Phosphorus is present in diets as naturally occurring P from raw materials or added as an inorganic salt. However, little is known about postprandial kinetics of P absorption in cats. Here, we describe several studies quantifying postprandial kinetics following the ingestion of diets of varying composition. Briefly, cats were fed a meal consisting of 50 % of their metabolic energy requirement in a randomised crossover design. A pre-meal baseline blood sample was taken via cephalic catheter and repeated measurements taken regularly up to 6 h post-meal to assess the whole blood ionised Ca, plasma P and parathyroid hormone concentrations. A diet containing 4·8 g total P/4184 kJ (1000 kcal), 3·5 g P from sodium dihydrogen phosphate (NaH2PO4)/4184 kJ (1000 kcal) and Ca:P 0·6 caused a marked increase in plasma P from baseline to a peak of 1·976 (95% CI 1·724, 2·266) mmol/l (P P = 0·008). Subsequent data indicate that added inorganic P salts in the diet above 0·5 g P/4184 kJ (1000 kcal) cause an increase in plasma P in cats, while diets below this do not. The data presented here demonstrate that sources of added inorganic P salts cause a temporary postprandial increase in plasma P in a dose-dependent manner, prolonged in diets with Ca:P

Although the implications of long-term high Ca intakes have been well documented in growing dogs, the health consequences of Ca excess in adult dogs remain to be established. To evaluate the impact of feeding a diet containing 7·1 g/4184 kJ (1000 kcal) Ca for 40 weeks on Ca balance and health parameters in adult dogs, eighteen neutered adult Labrador Retrievers, (nine males and nine females) aged 2·5–7·4 years were randomised to one of two customised diets for 40 weeks. The diets were manufactured according to similar nutritional specifications, with the exception of Ca and P levels. The diets provided 1·7 and 7·1 g/4184 kJ (1000 kcal) (200(SD26) and 881(SD145) mg/kg body weight0·75 per d, respectively) Ca, respectively, with a Ca:P ratio of 1·6. Clinical examinations, ultrasound scans, radiographs, health parameters, metabolic effects and mineral balance were recorded at baseline and at 8-week intervals throughout the study. Dogs in both groups were healthy throughout the trial without evidence of urinary, renal or orthopaedic disease. In addition, there were no clinically relevant changes in any of the measures made in either group (all P>0·05). The high-Ca diet resulted in a 3·3-fold increase in faecal Ca excretion (PP>0·05). Ca intakes of up to 7·1 g/4184 kJ (1000 kcal) are well tolerated over a period of 40 weeks, with no adverse effects that could be attributed to the diet or to a high mineral intake.

Short-term feeding studies have highlighted a phenomenon in Ca regulation that raises concerns around Ca absorption in dogs that may make an impact on commercial diets near to the maximum recommended level. A recent study to determine responses in dogs fed one of two diets differing in dietary Ca over 40 weeks found no evidence to suggest a concern across a range of biological parameters hypothesised to be affected by Ca. Unforeseen consequences of dietary Ca could have occurred and metabolic profiling was deemed a suitable data-driven approach to identify effects of dietary Ca. The objectives were to compare the fasted plasma metabolome (sampled at 8-week intervals over 40 weeks) of dogs fed one of two diets, near to the minimum and maximum recommended levels of dietary Ca. Comparisons with the control diet were also investigated across the postprandial time course (1–4 h) following acute (1 d) and long-term (24 weeks) feeding of the test diet. Comparing fasted plasma samples at each time point, no significant effect (adjusted P < 0·05) of diet on metabolites was observed. In the postprandial state, only phosphate was consistently different between diets and was explained by additional dietary P to maintain Ca:P. Metabolic profiling analysis supports the view that the dietary Ca upper limit is safe. Additionally, the canine plasma metabolome was characterised, providing insights into the stability of individual profiles across 40 weeks, the response to consumption of a nutritionally complete meal over a 4 h postprandial time course and different kinetic categories of postprandial absorption.

The first 67% of the data were used to build a prediction model, which included feature selection and identification of the optimal neural network type and architecture. The remaining unseen EHRs were used to evaluate the model performance.ResultsThe final model was a recurrent neural network (RNN) with 4 features (creatinine, blood urea nitrogen, urine specific gravity, and age). When predicting CKD near the point of diagnosis, the model displayed a sensitivity of 90.7% and a specificity of 98.9%. Model sensitivity decreased when predicting the risk of CKD with a longer horizon, having 63.0% sensitivity 1 year before diagnosis and 44.2% 2 years before diagnosis, but with specificity remaining around 99%.Conclusions and clinical importanceThe use of models based on machine learning can support veterinary decision making by improving early identification of CKD.

The implications of long-term high calcium (Ca) intake are well documented in growing dogs and in adult dogs of large breed size, however, the consequences on other breeds and breed sizes are yet to be determined. Eighteen neutered adult beagles, nine males and nine females aged 1.4–4.4 years, were randomized to control or test diets providing in g∙4184 kJ−1 (1000 kcal−1): 1.44 and 7.19 total Ca balanced with 1.05 and 4.25 total phosphorus, respectively, for 40 weeks. Health parameters, ultrasound scans, radiographs, glomerular filtration rate, and mineral balance were measured at eight-week intervals. All dogs remained healthy with no measured evidence of orthopedic, urinary, or renal disease. The test diet resulted in a 5.2 fold increase in fecal Ca excretion. Apparent Ca digestibility (%) and Ca balance (g/d) did not significantly (p &gt, 0.05) change from baseline in the test diet group, although dogs displayed a positive Ca balance (maximum at week 8, 1.11 g/d with 95% CI (0.41, 1.80)) before a neutral Ca balance was restored at week 32. Despite an initial positive Ca balance, we can conclude that no measurable adverse health effects were observed as a result of the test diet fed in this study in beagles over a period of 40 weeks.

Simple Summary Obesity and related diseases are common problems for dogs and inappropriate feeding during development is a contributor to life-long weight issues. Judging the right amount of food to give a growing puppy is challenging and providing a simple recommendation to owners is essential. However, differences in dog size, activity, and many other factors such as neutering can all have a role in impacting the actual energy requirements for growth. Yet, the current feeding guideline for growth (NRC 2006) does not accommodate these factors. Therefore, this study investigated how much a small breed (Norfolk Terrier puppies) requires to maintain growth and a healthy body condition score (BCS) through their first year of life. We found that they required significantly less than suggested by the NRC. Changes in the assessment of appropriate feeding during development are required and this study supports the need to revise the NRC (2006) equation for small breed dogs. Abstract An appropriate energy intake for healthy growth can reduce the risk of obesity and co-morbidities, such as orthopaedic diseases. The 2006 National Research Council (NRC) universal equation calculates the energy requirement of growing dogs based on predicted adult body weight, but evidence suggests a revision may be required. This study investigates the energy requirements of seventeen Norfolk terrier puppies over their first year (10 to 52 weeks). Puppies were individually fed complete and balanced diets in amounts to maintain an optimal body condition score (BCS), recording intake daily and body weight and BCS weekly. To monitor health a veterinary examination, haematology and plasma biochemistry and serum measures of bone turnover were undertaken every 12 weeks. Skeletal development was assessed using dual-energy X-ray absorptiometry (26 and 52 weeks). Puppies were clinically healthy with normal skeletal development and healthy growth throughout. The energy intake to achieve this was significantly lower than that predicted by the NRC (2006) equation at all time points, with largest mean difference of 285 kJ/kg0.75 per day at 10 weeks. If fed according to the NRC 2006 equation, dogs would have been in positive energy balance, possibly leading to obesity. These data support a revision to the NRC (2006) equation.

Catecholamine reuptake inhibition improves the performance of male volunteers exercising in warm conditions, but sex differences in thermoregulation, circulating hormones, and central neurotransmission may alter this response. With local ethics committee approval, nine physically active women (mean ± SD age 21 ± 2 years; height 1.68 ± 0.08 m; body mass 64.1 ± 6.0 kg; VO2peak 51 ± 7 mL/kg/min) were recruited to examine the effect of pre-exercise administration of Bupropion (BUP; 4 × 150 mg) on prolonged exercise performance in a warm environment. Participants completed a VO2peak test, two familiarization trials, and two randomized, double-blind experimental trials. All trials took place during the first 10 days of the follicular phase of the menstrual cycle. Participants cycled for 1 h at 60% VO2peak followed by a 30-min performance test. Total work done was greater during the BUP trial (291 ± 48 kJ) than the placebo trial (269 ± 46 kJ, P = 0.042, d = 0.497). At the end of the performance test, core temperature was higher on the BUP trial (39.5 ± 0.4 °C) than on the placebo trial (39.2 ± 0.6 °C, P = 0.021; d = 0.588), as was heart rate (185 ± 9 vs 179 ± 13, P = 0.043; d = 0.537). The results indicate that during the follicular phase of the menstrual cycle, an acute dosing protocol of BUP can improve self-regulated performance in warm conditions.

Summary Background & aims Dehydration of as little 2% of total body weight may impair physical and cognitive performance. The aim of this study was to investigate the prevalence of dehydration at the start and end of shifts in nurses and doctors on-call. The secondary aims were to assess the relation between hydration status and cognitive function. Methods This prospective cohort study was conducted on nurses and doctors working on medical and surgical admissions wards at a university teaching hospital. Participants arrived on the ward approximately 20 min before their shift and were asked to provide a urine sample. Height and weight were then measured. A 10 mL blood sample was analysed for full blood count, serum urea and electrolytes, and blood glucose. Cognitive function was assessed using a series of computer-based tests including the Stroop Colour Naming Interference Test and Sternberg Memory Paradigm. Participants then worked normally but were asked to keep a fluid diary for the duration of their shift and fluid balance was estimated. Tests were repeated at the end of the shift. Dehydration was defined as urine osmolality >800 mOsmol/kg and oliguria was defined as urine output Results We recruited 92 nurses and doctors, of whom 88 completed the study, amounting to 130 shifts. 52% participated for one shift, and 48% for two shifts. Thirty-six percent of participants were dehydrated at the start of the shift and 45% were dehydrated at the end of their shift. Mean (SD) urinary osmolality was significantly greater at the end of the shift when compared with the start [720 (282) vs. 622 (297) mOsm/kg, P = 0.031). Moreover, 41% were oliguric at the end of the shift. Single number and five-letter Sternberg short-term memory tests were significantly impaired in dehydrated participants ( P Conclusions This study highlights that a significant proportion of nurses and doctors were dehydrated at the start and end of medical and surgical shifts. Dehydration was associated with some impairment of cognitive function.

BACKGROUND: The identification of beverages that promote longer-term fluid retention and maintenance of fluid balance is of real clinical and practical benefit in situations in which free access to fluids is limited or when frequent breaks for urination are not desirable. The postingestion diuretic response is likely to be influenced by several beverage characteristics, including the volume ingested, energy density, electrolyte content, and the presence of diuretic agents. OBJECTIVE: This study investigated the effects of 13 different commonly consumed drinks on urine output and fluid balance when ingested in a euhydrated state, with a view to establishing a beverage hydration index (BHI), i.e., the volume of urine produced after drinking expressed relative to a standard treatment (still water) for each beverage. DESIGN: Each subject (n = 72, euhydrated and fasted male subjects) ingested 1 L still water or 1 of 3 other commercially available beverages over a period of 30 min. Urine output was then collected for the subsequent 4 h. The BHI was corrected for the water content of drinks and was calculated as the amount of water retained at 2 h after ingestion relative to that observed after the ingestion of still water. RESULTS: Total urine masses (mean +/- SD) over 4 h were smaller than the still-water control (1337 +/- 330 g) after an oral rehydration solution (ORS) (1038 +/- 333 g, P < 0.001), full-fat milk (1052 +/- 267 g, P < 0.001), and skimmed milk (1049 +/- 334 g, P < 0.001). Cumulative urine output at 4 h after ingestion of cola, diet cola, hot tea, iced tea, coffee, lager, orange juice, sparkling water, and a sports drink were not different from the response to water ingestion. The mean BHI at 2 h was 1.54 +/- 0.74 for the ORS, 1.50 +/- 0.58 for full-fat milk, and 1.58 +/- 0.60 for skimmed milk. CONCLUSIONS: BHI may be a useful measure to identify the short-term hydration potential of different beverages when ingested in a euhydrated state. Background: older adults are susceptible to dehydration due to age-related pathophysiological changes. We aimed to investigate the prevalence of hyperosmolar dehydration (HD) in hospitalised older adults, aged ≥65 years, admitted as an emergency and to assess the impact on short-term and long-term outcome. Methods: this prospective cohort study was performed on older adult participants who were admitted acutely to a large UK teaching hospital. Data collected included the Charlson comorbidity index (CCI), national early warning score (NEWS), Canadian Study of Health and Aging (CSHA) clinical frailty scale and Nutrition Risk Screening Tool (NRS) 2002. Admission bloods were used to measure serum osmolality. HD was defined as serum osmolality >300 mOsmol/kg. Participants who were still in hospital 48 h after admission were reviewed, and the same measurements were repeated. Results: a total of 200 participants were recruited at admission to hospital, 37% of whom were dehydrated. Of those dehydrated, 62% were still dehydrated when reviewed at 48 h after admission. Overall, 7% of the participants died in hospital, 79% of whom were dehydrated at admission (P = 0.001). Cox regression analysis adjusted for age, gender, CCI, NEWS, CSHA and NRS demonstrated that participants dehydrated at admission were 6 times more likely to die in hospital than those euhydrated, hazards ratio (HR) 6.04 (1.64–22.25); P = 0.007. Conclusions: HD is common in hospitalised older adults and is associated with poor outcome. Coordinated efforts are necessary to develop comprehensive hydration assessment tools to implement and monitor a real change in culture and attitude towards hydration in hospitalised older adults.

Heat stress and exercise increase water loss from the body, primarily in the form of sweat. For some occupational groups, including miners, construction workers in hot climates, soldiers, and some athletes, daily water losses can reach 10-12 L. These losses must be replaced on a daily basis to maintain functional capacity. Both hyperhydration and hypohydration will, if sufficiently severe, impair all aspects of physiological function. Tests of strength and power are largely unaffected by dehydration of up to about 2%-4% of body mass. However, decrements in the performance of endurance tests may occur at these levels, especially in warm environments. Body water deficits, if sufficiently severe, also have adverse effects on measures of mood and on some elements of cognitive function and result in an increased subjective rating of the perception of effort. Beverages consumed during exercise can provide carbohydrates and electrolytes that may be beneficial in some situations; however, drinking in volumes required to match sweat loss may cause gastrointestinal discomfort that will generally impair performance.

This study examined the effects of variations in air velocity on time to exhaustion and thermoregulatory and perceptual responses to exercise in a hot environment. Eight male volunteers completed stationary cycle exercise trials at 70% peak oxygen uptake until exhaustion in an environmental chamber maintained at 30 °C and 50% relative humidity. Four air velocity conditions, 30, 20, 10, and 0 km/h, were tested, and the headwind was directed at the frontal aspect of the subject by 2 industrial fans, with blade diameters of 1 m and 0.5 m, set in series and positioned 3 m from the subject’s chest. Mean ± SD time to exhaustion was 90 ± 17, 73 ± 16, 58 ± 13, and 41 ± 10 min in 30-, 20-, 10-, and 0-km/h trials, respectively, and was different between all trials (P < 0.05). There were progressive elevations in the rate of core temperature rise, mean skin temperature, and perceived thermal sensation as airflow decreases (P < 0.05). Core temperature, heart rate, cutaneous vascular conductance, and perceived exertion were higher and evaporative heat loss was lower without airflow than at any given airflow (P < 0.05). Dry heat loss and plasma volume were similar between trials (P > 0.05). The present study demonstrated a progressive reduction in time to exhaustion as air velocity decreases. This response is associated with a faster rate of core temperature rise and a higher skin temperature and perceived thermal stress with decreasing airflow. Moreover, airflow greater than 10 km/h (2.8 m/s) might contribute to enhancing endurance exercise capacity and reducing thermoregulatory, cardiovascular, and perceptual strain during exercise in a hot environment.

Ten endurance-trained males were recruited to examine the possible role of carbohydrate (CHO) receptors in the mouth influencing exercise performance in the heat. Volunteers completed an incremental test to exhaustion to determine peak oxygen uptake, a familiarisation trial, followed by 2 experimental trials. Trials consisted of a 1-h time trial undertaken in a climatic chamber maintained at 30 °C, 60% relative humidity. Immediately before, and at regular intervals throughout exercise, subjects ingested a bolus of water and then were provided with either a placebo (PLA) or a 6.4% glucose (CHO) solution to rinse in the mouth for 10 s before being expectorated. There was no difference in total work done between the PLA and CHO trials (758.8 ± 149.0 kJ; 762.6 ± 141.1 kJ; P = 0.951). Pacing was also similar, with no differences in power output apparent during the experimental trials (P = 0.546). Core temperature (P = 0.615), heart rate (P = 0.505), ratings of perceived exertion (P = 0.181), and perceived thermal stress (P = 0.416) were not influenced by the nature of the intervention. Blood glucose concentrations were similar during the CHO and PLA trials (P = 0.117). In contrast to the findings of several studies undertaken in temperate conditions, the present investigation failed to support role of oral sensing of CHO in influencing performance during prolonged exercise in warm conditions.

This investigation quantified the effect of changes in plasma osmolality on the measurement of hematocrit (Hct) and the implications for the subsequent use of these data to calculate changes in plasma volume and application to the World Anti-Doping Agency Athlete Biological Passport.Two groups of eight male volunteers visited the laboratory after an overnight fast. In study 1, a 20-mL blood sample was collected and aliquoted into collection tubes containing saline of varying concentrations to alter the sample osmolality. In study 2, plasma osmolality was manipulated in vivo through prolonged exercise. Samples were analyzed for hemoglobin concentration and Hct using manual methods and using an automated hematology analyzer (AHA).Changes in blood, plasma, and red cell volumes were calculated. Although AHA Hct values did not change (P = 0.652), spun packed cell volume fell progressively as the osmolality of the sample increased (P0.001, study 1). Consequently, there was a significant increase in apparent plasma volume as osmolality increased (P0.001): regression analysis revealed that a 10 mOsm·kg change in plasma osmolality produced a difference of 0.8 Hct units and a 1.6% change in plasma volume. In study 2, exercise produced a 12 ± 3 mOsm·kg increase in plasma osmolality. No difference in Hct was apparent at rest (P = 0.659), but spun packed cell volume was 1.0 ± 0.9 Hct units lower during exercise compared with AHA data (P0.001). There was a difference in the degree of plasma volume change calculated, with a reduction of 8.7% ± 3.4% and 11.3% ± 3.5% reported with the manual and AHA methods, respectively (P = 0.002).Conditions or interventions that result in a marked change in plasma osmolality produce a discrepancy in Hct measured using an AHA, consequently introducing errors into any calculation of changes in plasma volume using these data. These findings may also have implications for the measurement of Hct by World Anti-Doping Agency-accredited laboratories.

A method has been developed for metabolite profiling of the salivary metabolome based on protein precipitation and ultra-high performance liquid chromatography coupled with ion mobility-mass spectrometry (UHPLC–IM–MS). The developed method requires 0.5 mL of human saliva, which is easily obtainable by passive drool. Standard protocols have been established for the collection, storage and pre-treatment of saliva. The use of UHPLC allows rapid global metabolic profiling for biomarker discovery with a cycle time of 15 min. Mass spectrometry imparts the ability to analyse a diverse number of species reproducibly over a wide dynamic range, which is essential for profiling of biofluids. The combination of UHPLC with IM–MS provides an added dimension enabling complex metabolic samples to be separated on the basis of retention time, ion mobility and mass-to-charge ratio in a single chromatographic run. The developed method has been applied to targeted metabolite identification and untargeted metabolite profiling of saliva samples collected before and after exercise-induced physiological stress. δ-Valerolactam has been identified as a potential biomarker on the basis of retention time, MS/MS spectrum and ion mobility drift time.

This study examined effects of 4 weeks of caffeine supplementation on endurance performance. Eighteen low-habitual caffeine consumers ( 0.05). Before supplementation, all participants completed one VO2peak test, one practice trial and 2 experimental trials (acute 3 mg · kg−1 caffeine [precaf] and placebo [testpla]). During the supplementation period a second VO2peak test was completed on day 21 before a final, acute 3 mg · kg−1 caffeine trial (postcaf) on day 29. Trials consisted of 60 min cycle exercise at 60% VO2peak followed by a 30 min performance task.
