Abstract

The motivations for the National Research Project on the Short Wood Supply Chain stem from the recognition of the great importance of the wood-based bio-economy for the future carbon-free society. In addition to the essential ecological benefits provided by forests, natural wood resources are expected to play a significant role due to the increasing wood demand for structural and non-structural uses. However, in Italy most industrial wood products are made from imported timber, which results in poor attention to local forestry, loss of traditional silvicultural know-how and reduced forest employment in most mountain areas. The project specifically aims: (i) to assess the feasability and sustainability of wood short supply chains; (ii) to promote new models of environmental certification and energy efficiency analysis; (iii) to create innovative products for bio-building (CrossLam, laminated timber, wood panels) using wood assortments from sustainable forest management in Italy; (iv) to characterise the functionality of wood products (with attention paid to architectural issues), predict wood degradation rate and assess conservation methods. Attention is specifically devoted to most common forest species in Italy, such as beech, pines, chestnut, deciduous oaks and poplar and to test thermal and surface treatments with natural substances and nano-materials.

Abstract

Opioid maintenance treatment (OMT) is the most widespread therapy for both females and males opioid addicts. While many studies have evaluated the OMT impact on men’s sexuality, the data collected about the change in women’s sexual functioning is still limited despite the fact that it is now well-known that opioids - both endogenous and exogenous - affect the endocrine system and play an important role in sexual functioning. The present study aims to determine how OMT with buprenorphine (BUP) or methadone (MTD) affects sexual health in women; examining also any possible emerging correlation between sexual dysfunction (SD), type of opioid and patients’ mental health. This multi-center study case recruited 258 female volunteers attending Italian public Addiction Outpatients Centers that were stabilized with OMT for at least 3 months. SD was assessed with the Arizona Sexual Experience Scale. The twelve-item General Health Questionnaire was used to assess participants’ mental health conditions. The results show that 56.6% of women receiving OMT for at least 3 months presented SD without significant differences between MTD e BUP groups. The majority of the subjects with SD have a poorer quality of intimate relationships and worse mental health than the average. To the best of our knowledge, the present study is the largest report on the presence of SDs in women as a side effects of MTD and BUP used in OMT. Since SDs cause difficulties in intimate relationships, lower patients’ quality of life and interfere with OMT beneficial outcomes, we recommend that women undertaking an opioid therapy have routine screening for SD and we highlight the importance to better examine opioid-endocrine interactions in future studies in order to provide alternative potential treatments such as the choice of opioid, opioid dose reduction and hormone supplementation.

Abstract

BackgroundTraumatic events are common globally; however, comprehensive population-based cross-national data on the epidemiology of posttraumatic stress disorder (PTSD), the paradigmatic trauma-related mental disorder, are lacking.MethodsData were analyzed from 26 population surveys in the World Health Organization World Mental Health Surveys. A total of 71 083 respondents ages 18+ participated. The Composite International Diagnostic Interview assessed exposure to traumatic events as well as 30-day, 12-month, and lifetime PTSD. Respondents were also assessed for treatment in the 12 months preceding the survey. Age of onset distributions were examined by country income level. Associations of PTSD were examined with country income, world region, and respondent demographics.ResultsThe cross-national lifetime prevalence of PTSD was 3.9% in the total sample and 5.6% among the trauma exposed. Half of respondents with PTSD reported persistent symptoms. Treatment seeking in high-income countries (53.5%) was roughly double that in low-lower middle income (22.8%) and upper-middle income (28.7%) countries. Social disadvantage, including younger age, female sex, being unmarried, being less educated, having lower household income, and being unemployed, was associated with increased risk of lifetime PTSD among the trauma exposed.ConclusionsPTSD is prevalent cross-nationally, with half of all global cases being persistent. Only half of those with severe PTSD report receiving any treatment and only a minority receive specialty mental health care. Striking disparities in PTSD treatment exist by country income level. Increasing access to effective treatment, especially in low- and middle-income countries, remains critical for reducing the population burden of PTSD.

Abstract

There is limited evidence available on the long-term impact of SARS-CoV-2 infection in children. In this article, the authors analyze the recent evidence on pediatric long Covid and lessons learnt from a pediatric post-Covid unit in Rome, Italy. To gain a better understanding of the concerns raised by parents and physicians in relation to the potential long-term consequences of this novel infection, it is important to recognize that long-term effect of a post-infectious disease is not a new phenomenon.The authors analyze the recent evidence on pediatric long Covid and lessons learnt from a pediatric post-Covid unit in Rome, Italy. Also, we analyze the long-term effects of other infectious diseases.

Abstract

The modern-day decline of coral reefs due to bleaching events has been recognized as one of the major consequences of man-driven climate change. However, also eutrophication has been highlighted as an equally great danger for coral reefs and as such for biodiversity hotspots. In the latest years this phenomenon has moved to the forefront in the scientific community. Fossil reefs play a key role in studying the emergence, development and faunal/floral diversity of reef environments under eutrophic conditions. Their importance as valuable data sources for studying long-term changes of coral reef environments and their resilience cannot be disputed, especially since they may record the complete life cycle of a reef complex. In this study, nine sections nearby the town of Dego (Savona Province, NW Italy) are presented and discussed with regards to their lithostratigraphic and paleontological contents. Due to the extensive amount of data, the original morphology of a fringing reef, consisting of core, flank and fore reef, under strong fluviatile influence could be reconstructed. This study emphasizes the importance of the coralline red algae association in such biocarbonatic build-ups as major constituent and as substrate stabilizers. The sections record the original colonization event of the local basement by the builder community, the emergence of the coral reef and finally the suffocation by the fluviatile sediments. The variation of the red algae association reflects a deepening trend and is possibly correlated to enhanced fluvial input, which tends to deteriorate ecological conditions and functions as a major trigger for initial reef suffocation.

Abstract

We evaluated the outcomes of revascularization in patients with chronic limb-threatening ischemia (CLTI) treated in real-world settings. This is a prospective multicenter cohort study with 12-month follow-up enrolling patients (n = 287) with CLTI undergoing open, endovascular, or hybrid lower extremity revascularization. The primary end point was amputation-free survival (AFS) at 12 months. Cox proportional analysis was used to determine independent predictors of amputation and restenosis. At 30 days, major adverse cardiovascular and major adverse limb events (MALE) rates were 3.1% and 2.1%, respectively. At 1 year, the overall survival rate was 88.8%, the AFS was 86.6%, and the primary patency was 70.5%. Freedom from MALE was 62.5%. After multivariate analysis, smoking (hazard ratio [HR] = 2.2, P = 0.04), renal failure (HR = 2.3, P = 0.03), Rutherford class (≥5) (HR = 3.2, P = 0.01), and below-the-knee disease (HR = 2.0, P = 0.05) were significant predictors of amputation; iloprost infusion (>10 vials) (HR = 0.64, P = 0.05) was a significant protective factor. Cilostazol administration (HR = 0.77, P = 0.05) was a significant protective factor for restenosis. Results from this prospective multicenter registry offer a consistent overview of clinical outcomes of CLTI patients at 1 year when adequately revascularized. Medical treatment, including statins, cilostazol and iloprost, were associated with improved 1-year freedom from restenosis and amputation.
