Until 2014, the management of invasive alien species has been poorly addressed in Europe. Recently, the Regulation 1143/14 on invasive alien species (IAS) was issued by the European Community. A first list of "IAS of Union Concern" was presented in 2016 and subsequently updated in 2017. The species included in that list are subjected to a number of commercial restrictions, and EU member states are required to control or eradicate them. The inclusion of an IAS in this list is based on the results of a risk-assessment procedure. At present, this list contains 49 IAS, none of which marine. Member states can decide either to adopt the EU list as it is, or to integrate it with additional "IAS of national concern". Italy has opted for the second way and is currently developing its own national list, which may eventually include marine IAS. Italy is a major "first hub" for marine introductions in the Mediterranean Sea, with over 200 multicellular marine alien species reported so far. Italy, with its central position in the Mediterranean Sea, shares several species with the Western European countries, carried by ships or with shellfish stocks, as well as a number of Indo-Pacific species with the Eastern Mediterranean countries, entered through the Suez Canal. Therefore, the roadmap that Italy is currently tracing towards the possible inclusion of marine species in the list of IAS to be managed and controlled, is also relevant for other European and Mediterranean countries. This presentation will show how the process has been organized and conducted, and the preliminary outcomes

Protozoan infections caused by Plasmodium, Leishmania, and Trypanosoma spp. contribute significantly to the burden of infectious diseases worldwide, causing severe morbidity and mortality. The inadequacy of available treatments calls for cost‐ and time‐effective drug discovery endeavors. To this end, we envisaged the triazole linkage of privileged structures as an effective drug design strategy to generate a focused library of high‐quality compounds. The versatility of this approach was combined with the feasibility of a phenotypic assay, integrated with early ADME‐tox profiling. Thus, an 18‐membered library was efficiently assembled via Huisgen cycloaddition of phenothiazine, biphenyl, and phenylpiperazine scaffolds. The resulting 18 compounds were then tested against seven parasite strains, and counter‐screened for selectivity against two mammalian cell lines. In parallel, hERG and cytochrome P450 (CYP) inhibition, and mitochondrial toxicity were assessed. Remarkably, 10‐((1‐(3‐([1,1'‐biphenyl]‐3‐yloxy)propyl)‐1H‐1,2,3‐triazol‐5‐yl)methyl)‐10H‐phenothiazine (7) and 10‐(3‐(1‐(3‐([1,1'‐biphenyl]‐3‐yloxy)propyl)‐1H‐1,2,3‐triazol‐4‐yl)propyl)‐10H‐phenothiazine (12) showed respective IC50 values of 1.8 and 1.9 mg mL−1 against T. cruzi, together with optimal selectivity. In particular, compound 7 showed a promising ADME‐tox profile. Thus, hit 7 might be progressed as an antichagasic lead.

The ESCA (Ecological Status of Coralligenous Assemblages) index was developed to assess the ecological quality of coralligenous habitat using macroalgae as a biological indicator. The aim of this study was to evaluate the response to human-induced pressures of macroalgae and sessile macro-invertebrates shaping the coralligenous habitat and to integrate their sensitivity into the ESCA index. Coralligenous assemblages were sampled at 15 locations of the NW Mediterranean Sea classified into three groups: i) marine protected areas; ii) low urbanized locations; and iii) highly urbanized locations. A sensitivity level value was assigned to each taxon/group on the basis of its abundance in each environmental condition, the data available in the literature and the results of an expert judgement survey. The index that includes the totality of the assemblages (named ESCA-TA), calculated using both macroalgae and sessile macro-invertebrates, detected the levels of human pressure more precisely than the index calculated with only macroalgae or with only invertebrates. The potential for assessing the ecological quality of marine coastal areas was thus increased with the ESCA-TA index thanks to the use of a higher variety of descriptors. El índice ESCA (Estado Ecológico de las Comunidades Coralígenas) ha sido desarrollado para determinar el estado ecológico de los hábitats coralígenos utilizando macro-algas como indicador biológico. El objetivo de este estudio fue evaluar la respuesta, ante presiones antropogénicas, de macro-algas y macro-invertebrados sésiles moldeadores de la comunidad coralígena y su sensibilidad al índice ESCA. Se muestrearon comunidades coralígenas en 15 localizaciones del Mediterráneo Nord-Occidental clasificadas en 3 grupos: i) áreas marinas protegidas; ii) poco urbanizadas; iii) muy urbanizadas. Un valor de Nivel de Sensibilidad fue asignado a cada taxón/grupo en base a su abundancia en cada condición medioambiental, a información bibliográfica disponible y a los resultados de juicios por parte de expertos. El índice que integra la totalidad de las comunidades (llamado ESCA-TA), calculado usando tanto macro-algas como macro-invertebrados sésiles, detectó los diferentes niveles de presión humana de manera más precisa que el índice calculado solo con macro-algas o solo con invertebrados. El potencial para determinar el estado ecológico de las áreas marinas protegidas se incrementó con el índice ESCA-TA gracias al uso de una mayor variedad de descriptores.

Objective. Celiac disease (CD) has strongly been established as associated with some site-specific gastrointestinal malignancies. On the contrary, according to the few reports available, the risk of colon carcinoma in CD patients has been described similar to that of general population. In this cohort study, we describe the risk of colon carcinoma in a group of Italian celiac patients. Materials and methods. The study population included all CD patients diagnosed at the Collaborating Centers of the Italian Registry of CD between 1st January 1982 and 31st December 2006. Upon diagnosis of CD and upon at every subsequent clinical control, the Collaborating Centers filled in a validated form for each CD patient reporting information about demographic data, possible occurrence of a neoplasm and adherence to a gluten-free diet. Results. Out of 1757 celiac patients enrolled, 6 developed a colon carcinoma during the follow-up period (mean: 18.1 years). The standardized incidence ratio (SIR) resulted 0.29 (95% CI = 0.07–0.45). Stratifying the risk for the dietary gluten intake, the SIR dropped to 0.07 (95% CI = 0.009–0.27) for CD patients with a strict adherence to a gluten-free diet. Conclusion. We confirm the previous finding that there is low risk to develop a colon cancer in celiac patients.

OBJECTIVES: Prospective studies have identified a number of patient- and procedure-related independent risk factors for post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis, with different conclusions, so various questions are still open. The endoscopist's expertise, case volume, and case mix can all significantly influence the outcome of ERCP procedures, but have been investigated little to date. METHODS: We identified patient- and procedure-related risk factors for post-ERCP pancreatitis and the impact of the endoscopist's experience and the center's case volume, using univariate and multivariate analysis, in a multicenter, prospective study involving low- and high-volume centers, over a 6-month period. RESULTS: A total of 3,635 ERCP procedures were included; 2,838 (78%) ERCPs were performed in the 11 high-volume centers (median 257 each) and 797 in the 10 low-volume centers (median 45 each). Overall, 3,331 ERCPs were carried out by expert operators and 304 by less-skilled operators. There were significantly more grade 3 difficulty procedures in high-volume centers than in low-volume ones (P10 attempts to cannulate the Vater's papilla, pancreatic duct cannulation, contrast injection of the pancreatic ductal system, pre-cut technique, and pancreatic sphincterotomy, among procedure-related risk factors. Multivariate analysis also showed that a history of post-ERCP pancreatitis, biliary pain, >10 attempts to cannulate the Vater's papilla, main pancreatic duct cannulation, and pre-cut technique were significantly associated with the complication. CONCLUSIONS: A history of pancreatitis among patient-related factors, and multiple attempts at cannulation among procedure-related factors, were associated with the highest rates of post-ERCP pancreatitis. Pre-cut sphincterotomy, although identified as another significant risk factor, appeared safer when done early (fewer than 10 attempts at cannulating), compared with repeated multiple cannulation. The risk of post-ERCP pancreatitis was not associated with the case volume of either the single endoscopist or the center; however, high-volume centers treated a larger proportion of patients at high risk of pancreatitis and did a significantly greater number of difficult procedures

Patients with celiac disease have an increased rate of enteropathy-associated T-cell lymphoma, but conflicting data are available about the protective role of a gluten-free diet with regard to the development of this malignancy. We followed 1,757 celiac patients for a total period of 31,801 person-years, collecting data about the frequency of gluten intake and the incidence of the enteropathy-associated T-cell lymphoma. Out of the nine celiac patients who developed an intestinal lymphoma [standard morbidity ratio of 6.42 (95% CI = 2.9-12.2; P < 0.001)], only two kept a strict gluten-free diet after the diagnosis of celiac disease and developed the malignancy after the peridiagnosis period of 3 years, dropping therefore the standard morbidity ratio to 0.22 (95%CI = 0.02-0.88; P < 0.001). The risk of developing an intestinal lymphoma for the celiac patients that used to have dietary gluten was significant (X(2 )= 4.8 P = 0.01). These results show that a strict gluten-free diet is protective towards the development of enteropathy-associated T-cell lymphoma.

Background The association between coeliac disease (CD) and neoplasms has been long established, but few data are available about the risk factors. The aim of this paper is to estimate the risk of developing a neoplasm among non diagnosed coeliac patients and to evaluate if this risk correlates with the age of patients at diagnosis of coeliac disease. Methods The study population consists of patients (n = 1968) diagnosed with CD at 20 Italian gastroenterology referral Centers between 1st January 1982 and 31st March 2005. Results The SIR for all cancers resulted to be 1.3; 95% CI = 1.0–1.7 p < 0.001. The specific SIRs for non Hodgkin lymphoma was 4.7; 95% CI = 2.9–7.3 p < 0.001, for the small bowel carcinoma 25; 95% CI = 8.5–51.4 p < 0.001, for non Hodgkin lymphoma 10; 95% CI = 2.7–25 p = 0.01, finally for the stomach carcinoma 3; 95% CI = 1.3–4.9 p < 0.08. The mean age at diagnosis of CD of patients that developed sooner or later a neoplasm was 47,6 ± 10.2 years versus 28.6 ± 18.2 years of patients who did not. Conclusion Coeliac patients have an increased risk of developing cancer in relation to the age of diagnosis of CD. This risk results higher for malignancies of the gastro-intestinal sites. An accurate screening for tumors should be performed in patients diagnosed with CD in adulthood and in advancing age.

Tuscany, The conservation of the coastal marine environment requires the possession of information that enables the global quality of the environment to be evaluated reliably and relatively quickly. The use of biological indicators is often an appropriate method. Seagrasses in general, and Posidonia oceanica meadows in particular, are considered to be appropriate for biomonitoring because of their wide distribution, reasonable size, sedentary habit, easy collection and abundance and sensitivity to modifications of littoral zone. Reasoned management, on the scale of the whole Mediterranean basin, requires standardized methods of study, to be applied by both researchers and administrators, enabling comparable results to be obtained. This paper synthesises the existing methods applied to monitor P. oceanica meadows, identifies the most suitable techniques and suggests future research directions. From the results of a questionnaire, distributed to all the identified laboratories working on this topic, a list of the most commonly used descriptors was drawn up, together with the related research techniques (e.g. standardization, interest and limits, valuation of the results). It seems that the techniques used to study meadows are rather similar, but rarely identical, even though the various teams often refer to previously published works. This paper shows the interest of a practical guide that describes, in a standardized way, the most useful techniques enabling P. oceanica meadows to be used as an environmental descriptor.
