Objective: To apply a socioecological approach to identify risk and protective factors across levels of the “sports-ecosystem,” which are associated with mental health outcomes among athletes in para-sports and non-para sports. A further aim is to determine whether para athletes have unique risks and protective factor profiles compared to non-para athletes. Methods: A cross-sectional, anonymous online-survey was provided to all categorized (e.g., highest level) athletes aged 16 years and older, registered with the Australian Institute of Sport (AIS). Mental health outcomes included mental health symptoms (GHQ-28), general psychological distress (K-10), risky alcohol consumption (AUDIT-C) and eating disorder risk (BEDA-Q). Risk and protective factors across multiple levels of the socioecological model, including individual, microsystem, exosystem and macrosystem level factors were assessed via self-report. Results: A total of 427 elite athletes (71 para and 356 non-para athletes) participated in the study. No significant differences in the rates of mental health problems were observed between para and non-para athletes. Both differences and similarities in risk and protective factor profiles were found across the multiple levels of the sports-ecosystem. Weak evidence was also found to support the hypothesis that certain risk factors, including experiencing two or more adverse life events in the past year, sports related concussion, high self-stigma, inadequate social support and low psychological safety conferred a greater risk for poorer mental health outcomes for para athletes in particular. Conclusion: Risk factors occurring across various levels of the sports ecosystem, including individual, interpersonal and organizational level risk factors were found to be associated with a range of poorer mental health outcomes. The association between mental ill-health and certain risk factors, particularly those at the individual and microsystem level, appear to be greater for para athletes. These findings have important implications for policy and mental health service provision in elite sports settings, highlighting the need for more nuanced approaches to subpopulations, and the delivery of mental health interventions across all levels of the sports ecosystem. Aim: The concept of severe and complex youth depression has evolved from clinical experience. Here we conducted a file audit study to evaluate supporting data for the construct. Methods: A retrospective file audit was undertaken with 84 consecutively discharged patients over 6‐months from Orygen's Youth Mood Clinic (YMC; 50% female). Results: Over a third (36.9%) were disengaged from vocation and education, with exposure to traumatic events common (84.5%). Almost all patients (91.7%) reported past 2‐week suicidal ideation at clinic entry. Hospital emergency departments were the most frequent referral source (31.0%). Most (72.6%) had received previous mental health treatment. Multimorbidity was frequently observed. The presence of a substance use disorder, or ≥ 3 comorbidities were both associated with a greater likelihood of prior suicide attempt. Conclusions: These data highlight the clinical needs associated with severe and complex depression. Findings have implications for youth experiencing mood disorders attending tertiary and community mental health settings. [ABSTRACT FROM AUTHOR]

Objective: To review the literature on the experiences of boys and men exposed to childhood sexual abuse, and to assess the implications of this literature for trials of interventions and tailored services for this population., Method: We conducted a narrative review of papers pertaining to boys and men exposed to childhood sexual abuse. Implications of this literature for treatment were critically appraised., Results: Boys and men suffer the negative sequelae of childhood sexual abuse to the same (and sometimes greater) extent as girls and women. Boys and men also experience a number of unique challenges, as the abuse experience may undermine masculine identities and relations. This conflict may contribute to the underreporting of childhood sexual abuse among boys and men. Boys and men are less likely to disclose their abuse experience and wait longer to disclose compared to girls and women. Existing estimates therefore likely underestimate the prevalence of childhood sexual abuse among boys and men. Additionally, to date, intervention trials for individuals exposed to childhood sexual abuse have included a disproportionately low number of boys and men, even based on existing prevalence estimates., Conclusions: Further investigation into the treatment needs of boys and men exposed to childhood sexual abuse is critically important. To facilitate a better understanding of their needs, intervention studies for this cohort should include a greater proportion of boys and men. Studies should also assess the influence of boys' and men's alignments to masculine norms for moderating treatment outcomes as a means to guide gender-sensitive treatments.
