IntroductionPolyneuropathy signs (Neuropathy Impairment Score, NIS), neurophysiologic tests (m+7Ionis ), disability, and health scores were assessed in baseline evaluations of 100 patients entered into an oligonucleotide familial amyloidotic polyneuropathy (FAP) trial.MethodsWe assessed: (1) Proficiency of grading neurologic signs and correlation with neurophysiologic tests, and (2) clinometric performance of modified NIS+7 neurophysiologic tests (mNIS+7Ionis ) and its subscores and correlation with disability and health scores.ResultsThe mNIS+7Ionis sensitively detected, characterized, and broadly scaled diverse polyneuropathy impairments. Polyneuropathy signs (NIS and subscores) correlated with neurophysiology tests, disability, and health scores. Smart Somatotopic Quantitative Sensation Testing of heat as pain 5 provided a needed measure of small fiber involvement not adequately assessed by other tests.ConclusionsSpecially trained neurologists accurately assessed neuropathy signs as compared to referenced neurophysiologic tests. We report an imported case of myositis caused by a rare parasite, Haycocknema perplexum, in Australia in a 37-year-old man who had progressive facial, axial, and limb weakness, dysphagia, dysphonia, increased levels of creatine kinase and hepatic aminotransferases, and peripheral eosinophilia for 8 years. Objective To determine whether histopathological, electromyographic and laboratory markers correlate with clinical measures in inclusion body myositis (IBM). Methods We reviewed our electronic medical records to identify patients with IBM according to European Neuromuscular Center (ENMC) 2011 criteria, seen between 2015 and 2020. We only included patients who had a muscle biopsy and needle electromyography (EMG) performed on the same muscle (opposite or same side). We used a detailed grading system [0 (normal) to 4 (severe)] to score histopathological and EMG findings. Clinical severity was assessed by the modified Rankin scale (mRS), muscle strength sum score (SSS), quadriceps strength and severity of dysphagia on swallow evaluation. Serum markers of interest were creatine kinase level and cN-1A antibodies. Results We included 50 IBM patients, with a median age of 69 years; 64% were males. Median disease duration at diagnosis was 51 months. On muscle biopsy, endomysial inflammation mainly correlated with dysphagia, and inversely correlated with mRS. Vacuoles and congophilic inclusions did not correlate with any of the clinical measures. On EMG, the shortness of motor un it potential (MUP) duration correlated with all clinical measures. Myotonic discharges, and not fibrillation potentials, correlated with the severity of inflammation. Serum markers did not have a statistically significant correlation with any of the clinical measures. Conclusions Dysphagia was the main clinical feature of IBM correlating with endomysial inflammation. Otherwise, inclusion body myositis clinical measures had limited correlation with histopathological features in this study. The shortness of MUP duration correlated with all clinical measures. Overuse injuries are common in elite athletes. The repetitive forces of sports activities lead to tendon, muscle, cartilage, bone, and nerve injuries and poor physical performance. An accurate and early diagnosis is necessary to design an objective and assertive rehabilitation program so that the athlete can return to his or her professional activities as soon as possible. This is the objective of this case report that used in a professional athlete of the Fluminense Futebol Clube, the M.A.C. Method - cicatricial acceleration method for the recovery of a muscular injury grade 2 of the semi-tendineus, verified after ultrasonography exam and that was positive compared to the magnetic resonance imaging. The treatment started on 03/25/2021 with the MAC methodology and ended on 03/28/2021. On 03/29/2021 the athlete was released to return to his club activities with the team. The M.A.C. application was done once a day, combining methylene blue with cryotherapy, which was used before and after the methodology. The M.A.C. Methodology doping therapy was used with the athlete: Red Laser:1200s/900s/600s, Amber Led:1800s/1200s/900s. The results of the therapy with the M.A.C. were qualified by the pain VAS scale where on the first day of care VAS = 7, on the third day VAS = 2 and on the fourth day VAS = 1. And they were quantified using ultrasound imaging from the first day of injury to the final response of the athlete's treatment with the M.A.C. methodology. Hamstring Muscle Injuries are prevalent in many sports, including soccer, and are often associated with prolonged time away from sport due to the long recovery process that a classic rehabilitation program takes, and where the athlete can course to functional decline and risk of new injuries. The scientific literature suggests that rehabilitation programs should be improved after ischiotibial muscle injury because there is still no consensus on the best methodology for faster recovery and return of the athlete to his/her activities. The M.A.C. methodology proved to be effective as part of the rehabilitation program in the recovery of the semi-tendinous muscle injury in high performance athletes in a short period of time, favoring the return of the professional to the game and minimizing possible disability. Introduction: Although peripheral neuropathy and cardiomyopathy are well-recognized manifestations of transthyretin (ATTR) amyloidosis, myopathy has been rarely reported.Methods: In this study we reviewed our muscle biopsy database (January 1998 to June 2018) to identify patients with ATTR amyloid myopathy confirmed by molecular or proteomic analysis. Clinical and laboratory findings were reviewed.Results: We identified eight ATTR amyloid myopathy patients (5 hereditary ATTR [ATTRv] and 3 wild-type ATTR [ATTRwt]). Myopathy was the initial manifestation in all ATTRwt patients and followed peripheral neuropathy (4 patients) or cardiomyopathy (1 patient) in ATTRv patients. One ATTRv patient developed myopathy after liver transplant. Peripheral neuropathy and cardiac amyloidosis occurred in seven and six patients, respectively. Muscle biopsy showed interstitial amyloid deposition in all patients, rare necrotic/regenerating fibers in six, and vacuoles in four.Discussion: Myopathy can be the initial manifestation of ATTRwt amyloidosis and can precede the peripheral neuropathy or occur after liver transplant in ATTRv amyloidosis. Objective: The present study compared the effects of gallium-aluminum-arsenide diode laser and healing oil on fibroblasts, blood vessels, and collagen maturation of skin wounds in Wistar rats., Materials and Methods: Twenty-four male rats weighing 325 ± 27 g were used. Five wounds, 12 mm in diameter, were made on the animals' backs. The rats were randomly divided into four groups with six animals in each group., Control Group: saline solution; L30 group: 30 J/cm(2) laser; L60 group: 60 J/cm(2) laser; Oil group: healing oil. Histomorphometric analysis was performed on the scar tissue removed from the different wounds every 4 d for 20 d., Results: On day 4, there were significantly more fibroblasts in the wounds treated with the laser and the healing oil compared to the controls. On day 8, there were significantly more fibroblasts in the oil group compared to the L30 and L60 groups. On the same day, the quantity of vessels was significantly greater in the L60 group compared to the other groups. On day 16, there was a significant increase in the number of blood vessels in the wounds treated with the 60 J/cm(2) laser compared to the other groups. Analysis of the collagen maturation index throughout the experiment showed significantly higher values in the L60 group compared to the other groups at all time points., Conclusion: The healing oil exerted a greater effect on fibroblast proliferation, whereas the 60 J/cm(2) laser was more effective in stimulating angiogenesis and scar-tissue maturation.
