Abstract

Introduction: Reminiscence therapy (RT), which engages individuals to evoke positive memories, has been shown to be effective in improving psychological well-being in older adults suffering from PTSD, depression, and anxiety. However, its impact on brain function has yet to be determined. This paper presents functional magnetic resonance imaging (fMRI) data to describe changes in autobiographical memory networks (AMN) in community-dwelling older adults. Methods: This pilot study used a within-subject design to measure changes in AMN activation in 11 older adults who underwent 6 weeks of RT. In the scanner, participants retrieved autobiographical memories which were either recent or remote, rehearsed or unrehearsed. Participants also underwent a clinical interview to assess changes in memory, quality of life, mental health, and affect. Findings: Compared to pretreatment, anxiety decreased (z = −2.014, p =.040) and activated significant areas within the AMN, including bilateral medial prefrontal cortex, left precuneus, right occipital cortex, and left anterior hippocampus. Conclusion: Although RT had subtle effects on psychological function in this sample with no evidence of impairments, including depression at baseline, the fMRI data support current thinking of the effect RT has on the AMN. Increased activation of right posterior hippocampus following RT is compatible with the Multiple Trace Theory Theory (Nadel & Moscovitch, 1997). [ABSTRACT FROM AUTHOR]

Abstract

Cet ouvrage collectif réunissant chercheurs et cliniciens fait le point sur la littérature neuroscientifique actuelle, et en particlulier sur le courant de la neuroesthétique. La littérature cognitive tente depuis peu d'élucider les bases neuronales et suggère que la créativité artistique serait la résultante neurobiologique d'une explosion combinatoire. D'autre part, et parallèlement, le recours à l'art et aux supports artistiques comme thérapie peut être revisité à la lumière de ces travaux. Il s'agit de pratiques le plus souvent empiriques ou nées d'observations expérimentales. Si elles sont exercées par des thérapeutes expérimentés, elles se distinguent des activités occupationnelles ou récréationnelles artistiques, dont on reconnaît d'ailleurs aujourd'hui la charge motivationnelle et le potentiel de stimulation cognitive. Le « plaisir » que suscitent ces activités peut être scientifiquement évalué, mais des effets « directs » sont également envisagés, comme le montrent par exemple de plus en plus de travaux de neurosciences de la musique. Ces résultats ouvrent la perspective d'un soin neuropsychologique où identité et créativité auraient une plus grande part, en faisant appel aux capacités préservées des patients.

Abstract

International audience; Through this review of 25 clinical and experimental works on long-term musical memories in Alzheimer’s disease (AD) patients, we attempt to clarify the conceptual understanding of musical memories, identify their evolution across the stages of the pathology, and propose possible explanations concerning the neural and cognitive mechanisms that underpin the preservation and impairment of certain musical memories. After clarifying the different kind of musical memories, we investigated their alterations throughout AD’s progression from mild to severe stages. Both procedural and retrograde semantic memory seem relatively spared in AD, while episodic memory appears to be impaired early. Moreover, partial preservation of music encoding in AD can be revealed through paradigms that are especially designed for AD patients (relying on behavioral cues, using adapted settings, etc.). Although seldomly used, they would definitely help understanding the preserved capacities in every stage of AD. However, more research is needed to better understand this phenomenon and assess its specificity to music or other types of supports. These findings could lead to multiple applications in care settings and research designs, bringing more nuanced understanding of how long-term musical memory degrades throughout the course of AD, and should encourage us to prioritize patients’ preserved cognitive abilities in current AD recreational and care programs.

Relations between sensory functions and Alzheimer's disease are still under explored.To understand them better, theFondation MédfJricAlzheimer has brought together a multi-disciplinary expert group. Aristote's five senses must be enhanced by today's know ledge of proprioception, motor cognition and pain perception. When cognition breaks dawn, the persan with dementia perceives the world around her with her sensory experience, yet is unable to intagratB all this information to understand the contaxt.The trBBtment of mul tiple sensory inputs by the brain is closely linked to cognitive processes. Sensory deficits reduce considerably the autonomy of people with dementia in their daily fife and their rela tions with others, increase their social isolation and the risk of accidents. Professionals involved with neurodegenerative diseases remain poorly aware of sensory deficits, which can bias the results of cognitive tests. However. there are simple tools to detect these deficits, notably for vision, hearing and balance disorders, which can be corrected. Many interventions for cognitive rehabilitation or quality of fife improvement are based on sen sory functions. The environment of people with dementia must be adapted to become understandable, comfortable, safe and eventually therapeutic.; Afin de mieux comprendre les relations, encore insuffisamment étudiées, entre les fonctions sensorielles et la maladie d'Alzheimer, la Fondation Médéric Alzheimer a réuni un groupe d'experts multi-disciplinaire. Aux cinq sens d'Aristote, il faut aujourd'hui ajouter la proprioception, la cognition motrice et la perception de la douleur. Lorsque la cognition se détériore, la personne atteinte de la maladie d'Alzheimer voit le monde qui l'entoure avec son expérience sensorielle, sans qu'elle puisse toutefois intégrer toutes ces informations pour comprendre le contexte. Le traitement des multiples informations sensorielles par le cerveau est étroitement lié à des processus cognitifs. Les déficits sensoriels réduisent considérablement l'autonomie des personnes malades dans la vie quotidienne, leurs relations avec autrui, augmentent leur isolement social et le risque d'accidents. Les professionnels impliqués dans les maladies neurodégénératives restent insuffisamment sensibilisés aux déficits sensoriels, qui peuvent fausser l'évaluation de la cognition. Il existe pourtant des outils de repérage simples de ces déficits, notamment pour les troubles de la vision, de l'audition et de l'équilibre qui peuvent être corrigés. De nombreuses interventions pour la réhabilitation cognitive ou l'amélioration de la qualité de vie s'appuient sur les fonctions sensorielles. L'environnement des personnes malades doit être adapté pour le rendre compréhensible, confortable, sûr et si possible thérapeutique.

50. Les impacts d’un programme de chant choral sur le bien-être et la cognition de personnes âgées en perte d’autonomie

Abstract

Résumé : Le chant choral serait bénéfique à tout âge : cette étude choisit d’en mesurer les impacts auprès de personnes très âgées. Un devis quantitatif quasi-expérimental à trois groupes fut adopté : la Chorale, l’Hebdo-Bistro (ateliers et conférences, groupe de comparaison), et le groupe Témoin. L’étude longitudinale, intergénérationnelle, comporta trois saisons. La cognition (Mattis, 3MS, Trail Making, empan numérique, fluences formelle, catégorielle), l’humeur (bien-être général, dépression (GDS)), l’autoefficacité (GSES) et l’autonomie (QAF) furent mesurées à trois reprises (pré, post, 2e post). En outre, des mesures hebdomadaires furent administrées concernant la santé physique (consultations médicales, médicaments, chutes) et la participation sociale (activités). L’analyse intergroupe ne rapporta aucune différence significative. Les comparaisons intragroupe montrèrent une amélioration significative pour la Chorale (3MS et activités sociales), et une tendance d’amélioration pour la Chorale et l’Hebdo-Bistro (fluence formelle). Bien que le petit échantillon (n=21) exclue toute généralisation, les résultats demeurent inspirants en contexte de vieillissement populationnel., Choir singing is known to be beneficial : this study chose to measure its impacts on elderly people. The quantitative and quasi-experimental design included three groups : the Choir, Hebdo-Bistro (conference workshops), the comparison group, and the Control group. This longitudinal, intergenerational study lasted for three seasons. Cognition (Mattis, 3MS, Trail Making, Numeral WAIS Scale, Formal and Categorical Fluences), humor (Psychological Well-being of American Adults, Geriatric Depression Scale), self-efficacy (Schwartzer), and autonomy (QAF questionnaire) were administered three times (pre, post, 2nd post). In addition, weekly measurements were administered on health related topics (medical consultations, medication, falls) and social participation (activities). The analyses showed no significant differences between groups. However, the intra-group analyses showed a significant improvement for the Choir (3MS, social activities), and a tendency for improvement (Formal Fluence) for the Choir and Hebdo-Bistro groups. The small sample (n =21) excludes any generalization, but the results are promising in today’s aging context.
