6. Fundamental studies to assess and restore vestibular function in patients with severe bilateral vestibular loss

Abstract

This research was focused around the artificial balance organ – the vestibular implant (VI). At first, diagnostic tools for vestibular function were studied: electro-oculography (EOG) showed good results as compared with the ‘golden standard’ of vestibular diagnostics, which is video-oculography (VOG). The latter is a non-invasive video-based method recording the eye movements, whereas the first measures the movements by placing electrodes around the eyes. Hence, EOG can serve as a more affordable alternative for VOG. Next, the selection criteria for VI were studied in a group of patients with bilateral vestibular loss. It was shown that only 76% of patients are eligible to get the VI, and no association between etiology of the disease and reflex test outcomes was found. Nevertheless, self-motion perception can be tested in a quick way in clinics and can thus complement vestibular reflex tests in diagnosing dysfunction and evaluating the VI efficiency. Finally, 3D model of electrical conductivity in the inner ear was constructed based on the animal experiments to improve electrical stimulation of the VI.

Abstract

There are various eye-tracking methods and the most common among them are methods of videooculography (VOG) and electrooculography (EOG). These methods are based on completely different physical principles, and accordingly, the signals received during the eye movements recording have different noise sources. Eventually, both methods register the same mechanical movement, and as respect, they should give consistent results. In this regard, the experimental study and analysis of accuracy of VOG and EOG while visual saccades registration performed using these methods of oculography simultaneously were carried out. 12 horizontal saccades to the left with an average amplitude of V-left = 150 +/- 50 deg/s and 10 saccades to the right with an average amplitude of V-right = 160 +/- 50 deg/s were recorded simultaneously by these two methods. The signals were processed by a low-pass filter with a cut-off frequency of 10 Hz and synchronized in time using crosscorrelation. Further, the saccades were averaged separately for each system and the left-right direction. The comparison of visual saccades was carried out using a linear regression model of the dependence of the eye velocity, recorded by EOG, on the eye velocity, recorded by VOG. A statistically significant good agreement of the eye velocities recorded using EOG and VOG during horizontal saccades was found. The obtained results of experimental studies have shown that despite the completely different physical principles these two methods based on and the presence of measurement errors of different nature, on average, the results of the eye movement registration by VOG and EOG systems are in excellent agreement with each other. The advantage of using EOG for measuring saccades and evaluating the vestibulo-ocular reflex (VOR)is shown.

Abstract

Introduction: The vestibular implant could become a clinically useful device in the near future. This study investigated the feasibility of restoring the high-frequency dynamic visual acuity (DVA) with a vestibular implant, using the functional Head Impulse Test (fHIT).Methods: A 72-year-old female, with bilateral vestibulopathy and fitted with a modified cochlear implant incorporating three vestibular electrodes (MED-EL, Innsbruck, Austria), was available for this study. Electrical stimulation was delivered with the electrode close to the lateral ampullary nerve in the left ear. The high-frequency DVA in the horizontal plane was tested with the fHIT. After training, the patient underwent six trials of fHIT, each with a different setting of the vestibular implant: (1) System OFF before stimulation; (2) System ON, baseline stimulation; (3) System ON, reversed stimulation; (4) System ON, positive stimulation; (5) System OFF, without delay after stimulation offset; and (6) System OFF, 25 min delay after stimulation offset. The percentage of correct fHIT scores for right and left head impulses were compared between trials.Results: Vestibular implant stimulation improved the high-frequency DVA compared to no stimulation. This improvement was significant for "System ON, baseline stimulation" (p = 0.02) and "System ON, positive stimulation" (p < 0.001). fHIT scores changed from 19 to 44% (no stimulation) to maximum 75-94% (System ON, positive stimulation).Conclusion: The vestibular implant seems capable of improving the high-frequency DVA. This functional benefit of the vestibular implant illustrates again the feasibility of this device for clinical use in the near future. [ABSTRACT FROM AUTHOR]
