We are reporting the use of three-dimensional transesophageal echocardiography as a supplement to two-dimensional transesophageal echocardiography in the percutaneous suture closure of the left atrial appendage.

Twenty-nine patients with different tricuspid valve (TV) pathologies were studied by both two-dimensional transthoracic (2DTTE) and live/real time three-dimensional transthoracic echocardiography (3DTTE). A major contribution of 3DTTE over 2DTTE was the en face visualization of all three leaflets of the TV in all patients. This allowed accurate assessment of TV orifice area in patients with TV stenosis and carcinoid disease. Loss of TV leaflet tissue, defects in TV leaflets and size of TV systolic non-coaptation could also be delineated and resulted in identifying the mechanism of tricuspid regurgitation (TR) in patients with Ebstein's anomaly and rheumatic heart disease. Prolapse of TV leaflets could also be well visualized and enabled us to develop a schema for systematic assessment of individual segment prolapse which could help in surgical planning. The exact sites of chordae rupture in patients with flail TV as well as right ventricular papillary muscle rupture could be well seen by 3DTTE. 3DTTE also permitted sectioning of various TV masses for more specific diagnosis of their nature. In addition, color Doppler 3DTTE provided an estimate of quantitative evaluation of TR severity, since the exact shape and size of the vena contracta could be accurately assessed. In conclusion, our preliminary experience with 3DTTE has demonstrated substantial incremental value over 2DTTE in the assessment of various TV pathologies.

In a patient with previously documented myocardial infarction, we assessed myocardial perfusion by using power contrast imaging and a newer intravenous echo contrast agent. The images were captured and stored digitally, and various image processing algorithms were used to assess myocardial perfusion. An apical perfusion defect was clearly visualized, and it correlated with radionuclide findings.

CECD results in complete delineation of the LV endocardium in almost all patients, and this should facilitate more accurate determination of LV volume and ejection fraction.

The aim of the study was to compare the accuracy of multiplane transesophageal echocardiography (TEE) with the more conventional biplane technique in the direct assessment of aortic valve area in patients with aortic stenosis. Short-axis images of the aortic valve adequate for measuring aortic valve area were obtained in all 81 patients studied by multiplane TEE but in only 56 of 64 patients (88%) using the biplane approach. The correlation coefficient for aortic valve area determined by multiplane TEE (r = 0.89; SEE = 0.04 cm 2 ) was higher (p 2 ). Correlations were higher for bicuspid valves (multiplane, r = 0.93; biplane, r = 0.75) than tricuspid valves (multiplane, r = 0.87; biplane, r = 0.75). Our study has demonstrated the superiority of multiplane TEE to both biplane TEE and transthoracic echocardiography (TTE) in the direct evaluation of aortic valve area in patients with aortic stenosis. Our study has demonstrated the superiority of multiplane transesophageal echocardiography over both biplane transesophageal echocardiography and transthoracic echocardiography in the direct evaluation of aortic valve area in patients with aortic stenosis.

Infected cardiac myxomas are extremely rare with only forty cases described in the literature. We report a case of an infected cardiac myxoma that presented in a manner similar to bacterial endocarditis. Our case is the first to be diagnosed using previously defined criteria, and is unusual in that transesophageal echocardiography was required to make the diagnosis.

Using longitudinal transesophageal echocardiography and the concept of three-dimensional transthoracic echocardiography, we performed three-dimensional reconstruction of transesophageal images of various clinico-pathologic cases, including atrial septal defect, mitral stenosis, mitral valve prolapse, and pulmonary stenosis. The hardware equipment and image processing flow chart of three-dimensional reconstruction of transesophageal echocardiographic images are described in this manuscript. Our present study indicates that three-dimensional reconstruction of transesophageal echocardiographic images could display multiregional three-dimensional structure of the heart and great vessels, including superior vena cava, ascending aorta, right ventricular outflow tract, pulmonary artery, and left heart, with clear, visual and stereoscopic imaging. The regional structures could be displayed at different levels of stereo-anatomic-sections and at different orientations of rotating stereo images, which could provide accurate three-dimensional anatomical information for cardiac stereo-morphological study and definition of spatial location and extent of cardiac abnormalities.

BACKGROUND Color Doppler flow mapping is widely used as a marker of severity of valvular regurgitation, and the transesophageal approach has provided high-quality images in patients with poor acoustic windows. However, different instruments produce significantly variable images. Techniques that use jet spatial information to determine the severity of the lesion may need to be derived specifically for the instrument used. Given a lack of standardization of the many commonly used instruments, the goal of this study was to quantify variability between instruments by imaging well-defined jet flow fields created in vitro. METHODS AND RESULTS Pulsatile jets were created in vitro using a blood analogue fluid through physiological orifice diameters and imaged from a distal window using six commonly used color Doppler instruments. Transesophageal transducers (5.0 MHz) were used with all instruments studied. Peak jet areas were planimetered and averaged with systematic variations in Nyquist limit, color filter, and sector angle (which produced variations in frame rate). Changes in instrument settings produced significant variation in jet size for all instruments studied. Comparisons within instruments and among instruments were difficult because of preset and ambiguous setting levels. When comparisons were possible between similar settings, variability was dramatic (eg, 57% variability between instruments with very similar Nyquist limits). CONCLUSIONS A lack of standardized color Doppler instrument settings prohibits translation of jet area techniques from one instrument to another. This should be taken into consideration when using different machines for clinical study.

We present a 32-year-old male with metastatic thymic carcinoma invading the left atrium in whom two-dimensional transesophageal echocardiography was able to differentiate a thrombus superimposed on the tumor.

Our case illustrates the value of transesophageal echocardiography in the assessment of patients suspected or diagnosed to have prosthetic aortic valve endocarditis. In addition to making a correct diagnosis of the exact site and nature of the aortopulmonary fistula, the technique also correctly demonstrated the location and extent of all the abscess cavities. The two perforations in the anterior mitral leaflet as well as the disruption of the aortic prosthesis with paraprosthetic leak were also correctly characterized.

Continuous intravenous infusion of epoprostenol sodium in selected patients with primary pulmonary hypertension improves symptoms and survival. This report describes two patients with primary pulmonary hypertension treated with epoprostenol in whom intrapulmonary shunting and severe hypoxemia occurred. Intrapulmonary shunting was confirmed by contrast echocardiography showing delayed appearance of bubbles in the left cardiac chambers after peripheral venous injection of agitated saline solution.

In this report, we present 34 patients in whom surgical intervention was undertaken for severe mitral insufficiency due to mitral valve prolapse (MVP). Location and severity of MVP and regurgitation were assessed preoperatively by live/real time three-dimensional transthoracic echocardiography and closely agreed with the surgical findings.

Hemodynamic studies of regurgitant lesions in the heart focus on identifying a reliable noninvasive method of volumetric flow calculation. In these studies the influence of blood viscosity to the flowfield under pulsatile flow conditions and constraining wall geometry has not been examined in detail. Pulsatile flow studies in straight tubes have shown that viscous effects significantly influence the periodic flowfield, especially near the wall. The purpose of this study is to investigate the significance of transient effects in the flowfield proximal to a lesion under constraining wall geometry. The proximal flowfield was analyzed with computational fluid dynamics (CFD) computer simulations and color flow Doppler mapping (CFM). Three different stroke volumes and regurgitant waveforms were investigated for upstream wall orientations that varied from -64 degrees to +64 degrees (measured from the orifice plane). Results showed that for each upstream wall orientation, a single instantaneously normalized centerline velocity distribution characterized the flowfield throughout the cycle. The centerline distributions were in phase with the pressure gradient and almost identical to the corresponding steady-state distributions. Minor deviations were observed near the wall, where viscous effects were predominant. Transient flow effects such as blunt profiles and pressure velocity phase shifts, which were observed in straight circular tubes, were not observed in regurgitant orifice flowfields. This is true even under severe confinement conditions.

The clinical evaluation of blood flow regurgitation through a heart valve or stenotic lesion is an unresolved problem. The proximal flowfield region has been the study focus in the last few years; however, investigators have failed to identify an accurate and reliable calculation scheme due to lack of geometric information about the shape and size of the regurgitating or stenotic orifice. Presented here is a superior method of calculation, by using three-dimensional (3-D) echocardiography combined with Doppler velocimetry. The geometric structure of the orifice in a regurgitating porcine prosthetic valve in vitro was formulated by 3-D image construction of sequentially obtained 2-D images. The velocity flowfield was accessed by color Doppler flow mapping (CD) and continuous-wave Doppler (CW). Two accurate methods of calculation of regurgitant variables were developed. The first method calculated peak regurgitant flow rate from CD and the second method calculated regurgitant flow volume from CW. Both methods showed excellent correlation with the corresponding true values from an electromagnetic flowmeter. The promising preliminary results in such a realistic porcine model indicate the possibility of establishing a routine procedure to be tested in the clinical setting. Previous studies have indicated that velocity measurements from color Doppler (CD) instruments are susceptible to errors due to the interaction of the ultrasound beam and the proximal orifice flow field. This study examined the influence of high aspect ratio (AR) orifices on the CD velocity error. Center line velocity error distributions for orifices ranging from 7.07 to 78.5 mm2, varying in shape from circular to an AR = 8 ellipse, were evaluated using a numerical model of the ultrasound beam and the simulated regurgitant flow field. An in vitro study was also performed and confirmed the findings of the numerical model. The study showed that increasing AR does not significantly change the error characteristics. The study confirmed that orifice size is the dominant factor in the error distribution, and that corrections speculated for circular orifices can be extended to elliptical orifices without significant errors. Due to the unavoidable problem of aliasing, color flow signals from high blood flow velocities cannot be measured directly by conventional color Doppler. A new technology termed Quantitative Un-Aliased Speed Algorithm Recognition (Quasar) has been developed to overcome this limitation. Employing this technology, we used transesophageal color Doppler echocardiography to investigate whether the velocities detected by the Quasar would correlate with those obtained by continuous-wave Doppler both in vitro and in vivo. In the in vitro study, a 5.0 MHz transesophageal transducer of a Kontron Sigma 44 color Doppler flow system was used. Fourteen different peak velocities calculated and recorded by color Doppler-guided continuous-wave Doppler were randomly selected. In the clinical study, intraoperative transesophageal echocardiography was performed using the same transducer 18 adults (13 aortic valve stenosis, 2 aortic and 2 mitral stenosis, 2 hypertrophic obstructive cardiomyopathy and 1 mitral valve stenosis). Following each continuous-wave Doppler measurement, the Quasar was activated, and a small Quasar marker was placed in the brightest area of the color flow jet to obtain the maximum mean velocity readout. The maximum mean velocities measured by Quasar closely correlated with maximum peak velocities obtained by color flow guided continuous-wave Doppler in both in vitro (0.53 to 1.65 m/s, r = 0.99) and in vivo studies (1.50 to 6.01 m/s, r = 0.97). We conclude that the new Quasar technology can accurately measure high blood flow velocities during transesophageal color Doppler echocardiography. This technique has the potential of obviating the need for continuous-wave Doppler.

The authors previously published data that describe acute alterations in ventricular dimensions and in the severity of mitral and tricuspid regurgitation (MR/TR) after initiation of left ventricular assist device (LVAD) pumping. In the current study, measurements of ventricular size and regurgitant jet area acquired after LVAD implantation are presented. Eight patients had LVAD implanted pending cardiac transplantation (duration of assist 70-279 days; mean, 162 +/- 29 days). Echocardiograms were obtained at the time of LVAD implant and later during LVAD support (mean time for late echo, 95 +/- 32 days post-implant). Comparisons of pre-implant with late post-implant data showed: increased TR jet area (4.8 +/- 1.0 cm2 vs. 8.0 +/- 1.7 cm2 P < 0.05); increased right ventricular (RV) end-systolic dimension (31 +/- 4 vs 40 +/- 5 mm, P < 0.05); and increased RV end-diastolic dimension (35 +/- 4 vs. 45 +/- 5 mm, P < 0.065). Decreased MR jet area and decreased LV dimensions (P < 0.05) also were noted on comparison of pre-implant and late post-implant data. There were no significant differences between any immediate post-implant and late post-implant echocardiographic measurements. No patient had clinical evidence of RV failure. LV mechanical assist causes an acute increase in TR, presumably by volume loading the RV. TR and RV enlargement persisted but did not discernibly worsen on subsequent post-implant echocardiograms. LV dimensions and MR remained less than the pre-implant values on later post-implant determinations.

The commercially manufactured contrast agents, Echovist and Albunex, were compared with sonicated conventional agents, indocyanine green, 29% renografin-60, 0.9% normal saline and 25% mannitol in their ability to enhance color Doppler flow signals. In a pneumatically regulated pulsatile flow system, a glycerine, saline (0.9%) and sand (5 microns particle size) solution was imaged using a 2.5 MHz phased-array transducer. Four different flow velocities (0.40, 0.35, 0.30 and 0.25 m/s) as measured by color Doppler guided pulse Doppler were utilized. All color Doppler settings were kept constant throughout the study. Utilizing a power injector, four different volumes (1.0, 1.5, 2.0 and 2.5 mL) of each contrast agent were injected into the flow medium at various transducer angles (20, 30, 40 and 60 degrees) and various distances from the transducer (3.38, 5.5, 6.76 cm). For Echovist and Albunex, several concentrations varying from 2% to 100% were used. Keeping instrument settings constant, color flow areas obtained before and after each contrast injection were planimeterized and the percent increase in the color flow area computed and compared. At full (100%) concentration, 20 degrees transducer angle and a flow velocity of 0.40 m/s, the maximum increase in the color flow area was 568%, 251%, 180%, 110%, 71%, and 38% for Echovist, Albunex, sonicated indocyanine green, renografin, normal saline and mannitol, respectively. However, a significant reduction in the degree of color flow enhancement was observed, with decreases in the concentration of these agents, and increases in the Doppler beam incident angle or distance from the transducer. Increasing the flow velocity of the medium into which contrast was injected did not produce significant changes in the contrast enhancement effect for all agents except Echovist. Increasing the injection volume significantly increased the color flow area for sonicated agents but not for Echovist or Albunex. This preliminary in vitro study shows that the commercially manufactured contrast agents, Echovist and Albunex, are much superior to sonicated conventional contrast agents in the enhancement of color Doppler flow signals. Of the sonicated agents, indocyanine green had the best enhancement capability.

We evaluated the clinical applicability of a prototype tomographic transesophageal echocardiographic (TEE) system, which not only provides conventional TEE images but also three-dimensional tissue reconstruction and four-dimensional display capabilities. The probe was used in 16 patients in the echocardiographic laboratory, intensive care unit, and the operating room. The instrument is a 5-MHz, 64-element, phased array unit mounted on a sliding carriage within a casing. After appropriate probe placement within the esophagus, the probe is straightened, a balloon surrounding the probe is inflated, and data acquisition begun with ECG and respiration gating. With computer controlled transducer movement at 1-mm increments, a complete cardiac cycle is recorded at each tomographic level. These are processed using a dedicated four-dimensional software, and displayed as a dynamic three-dimensional tissue image of the heart. We were able to see the dynamic motion of the ventricles and all the valves in the four-dimensional format. In addition to four-dimensional display, we were able to cut and visualize the heart in dynamic mode in any desired plane and also in multiple planes. Patients tolerated the procedure well. We conclude that this tomographic four-dimensional approach, which does not require tedious off-line processing, can easily be performed in patients and has a strong clinical potential.

This study validates the accuracy of the newly-developed Quasar technique in the assessment of high flow velocities using color Doppler flow mapping.

The severity of valvular aortic stenosis was assessed by Doppler color flow mapping in 100 consecutive patients who underwent successful cardiac catheterization within 2 weeks of the Doppler study. The maximal width of the aortic stenosis jet seen in 61 of these patients (Group A) was measured at the aortic valve. Color-guided continuous wave Doppler examination was used to measure the mean transaortic pressure gradient, and the aortic valve area was estimated using the simplified continuity equation. The aortic stenosis jet was not seen in 39 patients (Group B), and the mean pressure gradient and aortic valve area in these patients were assessed by conventional Doppler echocardiography alone. The mean pressure gradient obtained by continuous wave Doppler study and cardiac catheterization in the 61 Group A patients correlated well (r = 0.90); the correlation was lower in the 39 Group B patients (r = 0.70). The overall correlation for the combined Groups A and B was good (r = 0.82). The aortic valve area estimated by continuous wave Doppler study and cardiac catheterization in 54 Croup A patients correlated well (r = 0.92); the correlation in 22 Group B patients was lower (r = 0.71). The correlation for all 76 patients (Groups A and B) was good (r = 0.80). The maximal nortic stenosis jet width also correlated well with the aortic valve area estimated at catheterization in 54 patients (r = 0.90). Group C represented an additional 14 patients in whom the left ventricle could not be entered during cardiac catheterization. All of these patients had severe aortic stenosis as detected by Doppler color flow mapping and had a calcified aortic valve with virtually immobile leaflets at subsequent surgery. These results show that Doppler color flow imaging combined with conventional Doppler study provides a reliable assessment of the severity of aortic stenosis and that the aortic stenosis jet width accurately estimates the degree of the aortic stenosis.

Doppler color flow mapping and color-guided conventional Doppler studies were performed on 119 patients with 126 prosthetic valves (mitral alone in 60, aortic alone in 52 and both mitral and aortic in 7 patients) within 2 weeks of the catheterization study or surgery, or both. The mean pressure gradients derived by color-guided continuous wave Doppler ultrasound correlated well with those obtained at catheterization for both the tissue and mechanical mitral and aortic prostheses (r = 0.85 to 0.87). For the effective prosthetic orifice areas, better correlation with catheterization results were obtained with the tissue mitral (r = 0.94) and tissue aortic (r = 0.87) prostheses than with the mechanical mitral (r = 0.79) and mechanical aortic (r = 0.76) prostheses. The maximal width of the color flow signals at their origin from the tissue mitral prostheses also correlated well with the effective prosthetic orifice area at catheterization (r = 0.81). Doppler color flow mapping identified prosthetic valvular regurgitation with a sensitivity and specificity of 89% and 100%, respectively, for the mitral and 92% and 83% for the aortic prostheses. There was complete agreement between the Doppler color flow mapping and angiographic grading of the severity of prosthetic valvular regurgitation in 90% of mitral and 73.5% of the aortic regurgitant prostheses with under- or overestimation by greater than 1 grade in only two cases. Valvular and paravalvular regurgitation was correctly categorized by Doppler color flow mapping in relation to the surgical findings in 94% of the mitral and 80.5% of the aortic prostheses.

To determine the reproducibility of two-dimensional exercise echocardiography, duplicate studies were performed on the same patients a median of 14 days apart. Because measurements are operator-dependent, interobserver variability was calculated for two experienced readers who interpreted the findings independently in a blinded manner. A high degree of interobserver agreement was found in evaluation of both ejection fraction measurements and wall motion abnormalities. Readings for ejection fraction immediately after exercise taken on different days could be estimated within 4% of the values measured in the first test; similarly measured wall motion score index was within 6% of that in the first test. Ejection fractions and wall motion scores were highly correlated between tests 1 and 2. The correlation coefficients between tests 1 and 2 were 0.92 for both the pre- and postexercise ejection fractions and 0.98 for both the pre- and postexercise wall motion scores. Quantitative two-dimensional echocardiography immediately after exercise is highly reproducible, providing a valuable tool for assessing serial changes in left ventricular function.
