Abstract

This article reports on results of the administration of the Alcohol, Smoking and Substance Involvement Screening Test (ASSIST) to 500 high school students (aged 16–19) in the Peruvian Amazon. Results indicate 68.6% of the sample reported consumption of alcohol in the past three months. One out of four students reported high risk involvement with at least one substance while 1 out of 3 reported moderate risk. This is one of the few instances in which ASSIST was administered to adolescents and offers possibilities for further reflection on drug use at early ages. [ABSTRACT FROM AUTHOR]

Abstract

Research on psychedelic-assisted psychotherapy has shown that music affects therapeutic outcomes at a fundamental level. The development of such therapies calls for research on the use of music with consciousness-altering substances, especially in contexts informed by their traditional use. Informed by ethnographic reports, our project answers this call, investigating the phenomenology of listening to icaros (medicine songs) during ayahuasca ceremonies as reported by addiction rehabilitation patients at Peru's Centro Takiwasi. We found that icaros were therapeutically significant. They elicited experiences of healing by modulating patients' emotions and the altered state induced by ayahuasca. They helped patients feel safe while guiding them through difficult memories and emotions, and facilitating experiences of healing and learning about their addictions. Thus, similar therapies must carefully consider how they incorporate music in their protocols. We suggest further research to better understand the synergistic, therapeutic effects of music and altered states of consciousness, especially in cross-cultural environments. [ABSTRACT FROM AUTHOR]

Abstract

In the Peruvian Amazon, tobacco (Nicotiana rustica) is considered a master plant and is the main curing tool of local healers. Among its several medicinal uses, we find drinking tobacco juice with the purpose of purging in order to heal on a physical, mental, and spiritual level. This specific practice is part of the addiction treatment protocol developed at Takiwasi Center. The goal of this investigation was to focus on the effects of the tobacco purge as reported by therapists at Takiwasi and to elaborate its relevance within the context of addiction treatment. In order to obtain information on this topic, we performed research fieldwork based on a participant-observation approach, and retrospective data analysis on information from interviews conducted by psychotherapists with patients. As alleged effects of the tobacco purge, therapists reported that patients experienced feelings of mental clarity with the greatest frequency (n = 80), followed by noteworthy physical effects including discomfort and intoxication (n = 63). An effect on sleep and dreams was also common (n = 36) and many found the experience directly related to addiction treatment and the desire to consume substances (n = 20). [ABSTRACT FROM AUTHOR]

Abstract

Information and communication technologies (ICT) have a growing impact in the health sector, are oriented towards the optimization of processes and their application covers fields such as psychotherapy and occupational therapy. The objective of this article is to describe the information system Plus, an initiative of the therapeutic community Takiwasi that arises from the need to establish a platform for the collection, storage and management of the different clinical processes and data that are generated within the institution throughout the treatment of drug-addict patients. The peculiarity of this system is closely linked to the uniqueness of the treatment protocol for addiction proposed in Takiwasi, where the practices of traditional Amazonian medicine are associated with modern psychotherapy, classic biomedical follow-up and other therapeutic spaces belonging to alternative and complementary medicine. The different modules by which the system is constituted are presented, as well as the information registration process. This ICT represents a fundamental tool to promote research on the therapeutic applications of traditional medicine and serves as key feedback for therapeutic interventions. Las tecnologías de información y comunicación (TIC) tienen un impacto creciente en el sector de la salud orientado a la optimización de procesos y su aplicación abarca varios campos como la psicoterapia y la terapia ocupacional. El objetivo del presente estudio es describir el sistema de información Plus, una iniciativa de la comunidad terapéutica Takiwasi que nace de la necesidad de instaurar una plataforma de recopilación, almacenamiento y gestión de los diferentes procesos y datos clínicos que se generan en la institución a lo largo del tratamiento de pacientes drogodependientes. Las peculiaridades de este sistema son estrechamente vinculadas a la unicidad del protocolo de tratamiento de adicciones propuesto en Takiwasi, donde las prácticas de la medicina tradicional amazónica son asociadas a la psicoterapia moderna, al seguimiento biomédico clásico y a otros espacios terapéuticos de medicina alternativa y complementaria. Se presentan los diferentes módulos por el cual el sistema está constituido, así como el proceso de registro de la información. Esta TIC representa una herramienta fundamental para promover la investigación en las aplicaciones terapéuticas de la medicina tradicional y como retroalimentación para mejorar las intervenciones terapéuticas.

Este capítulo se centra en las experiencias de dieta terapéutica a base de plantas medicinales amazónicas en Takiwasi, Centro de Rehabilitación de Toxicómanos y de Investigación de Medicinas Tradicionales, Perú. La dieta es uno de los métodos claves de la MTA. Podemos distinguir dos tipos de la dieta: la chamánica, que tiene como objetivo el aprendizaje de los conocimientos necesarios para el manejo de las plantas, y la dieta terapéutica, cuyo fin principal es la curación de la persona. Esta investigación estaba enfocada en la dieta terapéutica que se realiza en el Centro Takiwasi. El objetivo principal del Centro Takiwasi es el tratamiento de los TCS, en el cual la dieta también tiene su lugar. La dieta también está se organiza para las personas que están viniendo por diferentes motivaciones, sin sufrir de los TCS. En Takiwasi, la dieta es el trabajo con plantas (se empieza con una sesión purgativa, después una sesión de ayahuasca y toma de las plantas maestras) con el acompañamiento psicoterapéutico y un retiro de 8 días en la selva. El propósito de este texto es presentar los motivos de 80 personas que vinieron al Centro Takiwasi para hacer el tratamiento de dieta, y no el de adicciones. El enfoque de la investigación es sobre los efectos físicos, psíquicos y aspectos positivos y negativos que los participantes sintieron durante la dieta. En esta investigación se usan las cartas de motivación que cada persona tiene que escribir antes de la dieta y los protocolos que les piden generar después de la participación., This chapter focuses on the experiences with therapeutic diet based on Amazonian medicinal plants as performed in the Takiwasi Center for Rehabilitation of Drug Addicts and Research on Traditional Medicines, located in Peru. The diet is one of the key methods of traditional Amazonian medicine. Two types of diets can be distinguished: the shamanic diet, which aims to learn the necessary knowledge for the management of plants; and the therapeutic diet, whose main purpose is the recovery of the person. This research is focused on the therapeutic diet that is implemented in the Takiwasi Center. The main objective of the Center is the treatment of substance use disorders. The diet is also made for people who are coming for different nonsubstance use related reasons. In Takiwasi, the diet is based on the work done with the medicinal plants (it begins with a purgative session, then an ayahuasca ingesting session, to finish with the preparation of "master plants") along with psychotherapy help followed by an 8-day retreat in the jungle. The purpose of this text is to present the reasons of 80 people who came to the Takiwasi Center for diet, and not for an addiction treatment. The focus of the research is based on the physical, psychological, and positive and negative effects that participants felt during the diet. For this research the motivation letters that each person filled out before the diet and the protocols that they were asked to fill out after participation were used.

Objetivo. Este artículo tiene como objetivo ofrecer una descripción general del papel de la espiritualidad, de la fe y de las experiencias místicas en el tratamiento de los trastornos por uso de sustancias (TUS) presentando el caso de la comunidad terapéutica Takiwasi, donde se utilizan plantas psicoactivas, y considerando otras terapias pertenecientes a las medicinas complementarias y alternativas (MCA) que derivan de o tienen conexión con prácticas religiosas o espirituales. Metodología. La investigación se ha basado en la revisión de la literatura científica y gris y en una entrevista realizada con el presidente y fundador del Centro Takiwasi. Resultados y discusión. El sinergismo entre la espiritualidad cristiana y amazónica parece relevante dentro del protocolo Takiwasi. La literatura pone en evidencia el uso de MCA, incluyendo Mindfulness, Respiración Holotrópica y Yoga. Conclusión. La experiencia clínica muestra que la espiritualidad es un factor clave que se debe tener en cuenta en el tratamiento de la adicción y varias prácticas de MCA con connotaciones espirituales muestran un potencial prometedor para el tratamiento de los TUS. This article aims to give an overview on the role of spirituality, faith and mystical experiences in the treatment of Substance Use Disorders (SUDs) by presenting the case of the therapeutic community Takiwasi, where psychoactive plants are used, and by considering other Complementary and Alternative Medicine (CAM) therapies that are derived from or have connection with religious or spiritual practices. Methodology. The research has been based on the review of scientific and grey literature and on an interview performed with the president and founder of Takiwasi. Results and discussion. Synergism between Christian and Amazonian spirituality appears relevant within the Takiwasi protocol. The use of CAM including Mindfulness, Holotropic Breathwork and Yoga emerge from the literature. Conclusions. Clinical experience show that spirituality is a key factor to be taken into account when considering addiction treatment and several CAM practices with spiritual connotations show promising potential for the treatment of SUDs.

Abstract

Background: Global challenges related to access and benefit sharing (ABS) of biological resources have become a key concern in the area of research on herbal medicines, ethnopharmacology, drug discovery, and the development of other high value products for which Intellectual Property protection can be secured. While the Convention on Biological Diversity (CBD, Rio 1992) has been recognized as a huge step forward, the implementation of the Nagoya Protocol (NP) and of new forms of collaboration often remain unresolved, especially in the context of “the fair and equitable sharing of benefits arising from the utilization of genetic resources” (Convention on Biological Diversity, 2011). The vision and the specific implementation of this international treaty vary from country to country, which poses additional challenges. Aims: Using a case study approach, in this analysis we aim at understanding the specific opportunities and challenges for implementing international collaborations regarding ABS in six Latin American countries—Chile, Colombia, Guatemala, México, Panama, and Peru. Based on that analysis, we provide recommendations for the path ahead regarding international collaborations under ABS agreements in ethnopharmacological research. Results and Discussions: The implementation of the NP varies in the six countries; and while they are all rich in biodiversity, access and benefit sharing mechanisms differ considerably. There is a need to engage in a consultation process with stakeholders, but this has often come to a halt. Institutional infrastructures to implement national policies are weak, and the level of knowledge about the NP and the CBD within countries remains limited. Conclusions: Different policies in the six countries result in very diverse strategies and opportunities relating to the equitable use of biodiversity. A long-term strategy is required to facilitate a better understanding of the treaties and the resulting opportunities for a fairer development

Abstract

The herbal preparation ayahuasca has been an important part of ritual and healing practices, deployed to access invisible worlds in several indigenous groups in the Amazon basin and among mestizo populations of South America. The preparation is usually known to be composed of two main plants, Banisteriopsis caapi and Psychotria viridis , which produce both hallucinogenic and potent purging and emetic effects; currently, these are considered its major pharmacological activities. In recent decades, the psychoactive and visionary effect of ayahuasca has been highly sought after by the shamanic tourism community, which led to the popularization of ayahuasca use globally and to a cultural distancing from its traditional cosmological meanings, including that of purging and emesis. Further, the field of ethnobotany and ethnopharmacology has also produced relatively limited data linking the phytochemical diversity of ayahuasca with the different degrees of its purging and emetic versus psychoactive effects. Similarly, scientific interest has also principally addressed the psychological and mental health effects of ayahuasca, overlooking the cultural and pharmacological importance of the purging and emetic activity. The aim of this review is therefore to shed light on the understudied purging and emetic effect of ayahuasca herbal preparation. It firstly focuses on reviewing the cultural relevance of emesis and purging in the context of Amazonian traditions. Secondly, on the basis of the main known phytochemicals described in the ayahuasca formula, a comprehensive pharmacological evaluation of their emetic and purging properties is presented. Abstract

The medicinal plant diet is a healing process used in traditional Amazonian medicine (TAM), and it is poorly described within the scientific literature. This work analyzes the experience of seven participants in this therapy performed at the Takiwasi Center in Peru. Semistructured interviews were performed before and after treatment, documenting participants' motivation, psychological experience, and perceived personal changes (physically, psychologically, socially, and spiritually), as well as the role played by each medicinal plant. All the interviews were recorded, transcribed, and analyzed using interpretative phenomenological analysis. Reasons to participate in the plant diet included self‐discovery, personal development, interest in plant medicine, and professional realization. The experience was perceived as intense and allowed participants to experience self‐acceptance, self‐discovery, mental balance, rest, cleansing, and connection with nature. Three months after the experience, participants felt physical changes (n = 6), psychological changes (n = 7), social changes (n = 5), and spiritual changes (n = 5). [ABSTRACT FROM AUTHOR]

Abstract

Phlomis fruticosa L. and P. herba-venti are species belonging to the Lamiaceae family, which have been traditionally used to prepare tonic and digestive drinks. Multiple studies also demonstrated the inhibitory effects of P. fruticosa extracts and essential oil against oxidative/proinflammatory pathways and bacterial strains deeply involved in ulcerative colitis. Considering these findings, the present study evaluated the effects of alcoholic P. fruticosa and P. herba-venti leaf extracts in isolated rat colon challenged with Escherichia coli lipopolysaccharide (LPS), an ex vivo experimental paradigm of ulcerative colitis. In this context, we assayed colon levels of pro-oxidant and proinflammatory biomarkers, including nitrites, malondialdehyde (MDA), lactate dehydrogenase (LDH), and serotonin (5-HT). Additionally, the extracts have been tested in order to evaluate possible inhibitory effects on specific bacterial and fungal strains involved in ulcerative colitis. Alcoholic P. fruticosa and P. herba-venti extracts were able to blunt LPS-induced nitrite, MDA, 5-HT, and LDH levels in isolated rat colon. The same extracts also inhibited the growth of Pseudomonas aeruginosa, E. coli, Staphylococcus aureus, Candida albicans and Candida tropicalis. In conclusion, our findings show a potential role exerted by alcoholic P. fruticosa and P. herba- venti in managing the clinical symptoms related to ulcerative colitis.

The Takiwasi Center is a therapeutic community unique in its kind for the integration of psychotherapy and modern medicinewith the use of practices derived from Traditional Amazonian Medicine (TAM). The central and innovative axis is the therapeutic use of medicinal plants with psychosomatic effects on the individual, in ritual sessions or in a context of isolationcommonly called diet. The main objective of this work is to describe: the natural medicines used; the sociodemographicprofile of the users; the accompaniment in the process of integration of the ritual ceremonies of Ayahuasca., El Centro Takiwasi es una comunidad terapéutica única en su género por la integración de psicoterapia y medicina modernacon el uso de prácticas derivadas de la Medicina Tradicional Amazónica (MTA). El eje central e innovador es el uso terapéutico de plantas medicinales con efectos psicosomáticos sobre el individuo, en sesiones rituales o en un contexto deaislamiento comúnmente llamado dieta. Abstract

Objetivo. Este artículo tiene como objetivo dar una visión de los centros en las Américas que utilizan plantas medicinales tradicionales o sus derivados en el tratamiento de la dependencia a sustancias. Metodología. A tal fin, hemos considerado un número reducido de plantas provenientes de América del Sur, América Central y América del Norte. La investigación se ha basado en la literatura científica, el intercambio de información con los centros de tratamiento, las búsquedas en internet y la experiencia personal de los autores. Resultados y discusión. Los resultados muestran la relevancia de ciertas plantas psicoactivas bien conocidas también por inducir estados modificados de conciencia (EMC) como ayahuasca, coca, wachuma, tabaco, hongos psilocibios, Salvia divinorum y peyote. Conclusiones. La terapia asistida por plantas para el tratamiento de trastornos por uso de sustancias parece ser un campo de investigación prometedor, aunque la validación de los resultados clínicos debe mejorarse para la mayoría de los casos analizados. Objective. This article aims to give an overview of the major American centers using traditional herbal medicine or their derivatives in the treatment of substance dependence. Methodology. For the purpose of this article we have considered a small number of plants hailing from South, Central and North America. The research has been based on scientific literature, information exchange with treatment centers, internet searches and the personal experience of the authors. Results and discussion. Results show the relevance of certain psychoactive plants well known also for inducing modified states of consciousness (MSCs) including Ayahuasca, Coca, Wachuma, Tobacco, Psilocybe mushrooms, Salvia divinorum and Peyote. Conclusions. Plant based assisted therapy for the treatment of substance use disorders appear to be a promising field of research, although validation of the clinical outcomes need to be improved for the majority of the analyzed cases.

Abstract

Takiwasi is a therapeutic community for the treatment of Substance Use Disorders (SUDs) that combines traditional Amazonian medicine (TAM) with modern psychotherapy. One of the plant medicines from TAM used in this protocol is purgahuasca. It is a decoction of the vine Banisteriopsis caapi alone, whose use is traditional among the Awajún and other ethnic groups in Peru. The research began with a field trip to Awajún territory to explore the indigenous use of purgahuasca as an initiation rite. Then, analysis of its application was conducted in the clinical context of Takiwasi. Open-ended and semi-structured interviews with Awajún informants and Takiwasi's therapeutic staff were performed and analyzed following the narrative methodological approach. Further clinical data on the ingestion of purgahuasca by Takiwasi's SUD patients were obtained from the internal repository. These indicate that 359 (92.1%) patients reported having had the so-called mareación (dizziness), 299 (76.7%) experienced physical sensations, and 208 (53.3%) had visions. These effects can be related to the psychoactivity of β-Carbolines alkaloids from B. caapi, a medicinal plant that seems to have potential benefits also for SUD, especially giving a key contribution to the patients' therapeutic process of becoming aware of the personal reasons behind addictive behaviors. Abstract

The association between spirituality and medicine is unfolding as a research theme that may have increasing practical implications in healthcare systems. Both spiritual and scientific dimensions are present within the treatment protocol for addiction applied at the Takiwasi Center, a pioneer therapeutic community that combines western approaches, including psychotherapy, biomedicine, and Catholic practices, with traditional Amazonian medicine. Through a series of open-ended and semi-structured interviews conducted on nine workers of the center during fieldwork research and comparison with the information obtained from literature review, the present article aims at testing the existence of an effective synergy between Catholic religiosity and indigenous-mestizo spirituality within the therapeutic process performed at the Takiwasi Center and puts in evidence some stimulating and problematic issues that arise from this synergy.
