AIMS: The 2021 European Society of Cardiology (ESC) guideline on cardiovascular disease (CVD) prevention categorizes moderate and severe chronic kidney disease (CKD) as high and very-high CVD risk status regardless of other factors like age and does not include estimated glomerular filtration rate (eGFR) and albuminuria in its algorithms, systemic coronary risk estimation 2 (SCORE2) and systemic coronary risk estimation 2 in older persons (SCORE2-OP), to predict CVD risk. We developed and validated an Add-on to incorporate CKD measures into these algorithms, using a validated approach. METHODS: In 3,054 840 participants from 34 datasets, we developed three Add-ons [eGFR only, eGFR + urinary albumin-to-creatinine ratio (ACR) (the primary Add-on), and eGFR + dipstick proteinuria] for SCORE2 and SCORE2-OP. We validated C-statistics and net reclassification improvement (NRI), accounting for competing risk of non-CVD death, in 5,997 719 participants from 34 different datasets. RESULTS: In the target population of SCORE2 and SCORE2-OP without diabetes, the CKD Add-on (eGFR only) and CKD Add-on (eGFR + ACR) improved C-statistic by 0.006 (95%CI 0.004-0.008) and 0.016 (0.010-0.023), respectively, for SCORE2 and 0.012 (0.009-0.015) and 0.024 (0.014-0.035), respectively, for SCORE2-OP. Similar results were seen when we included individuals with diabetes and tested the CKD Add-on (eGFR + dipstick). In 57 485 European participants with CKD, SCORE2 or SCORE2-OP with a CKD Add-on showed a significant NRI [e.g. 0.100 (0.062-0.138) for SCORE2] compared to the qualitative approach in the ESC guideline. CONCLUSION: Our Add-ons with CKD measures improved CVD risk prediction beyond SCORE2 and SCORE2-OP. This approach will help clinicians and patients with CKD refine risk prediction and further personalize preventive therapies for CVD.

We consider rotating finite size vortex arrays in Bose-Einstein condensates that are confined by cylindrically symmetric external potentials. We show that such systems possess two exact Nambu-Goldstone zero modes associated with two spontaneously broken continuous symmetries of the system. We verify our analytical result via direct numerical diagonalizations of the Bogoliubov-de Gennes equations. We consider geometric phases of mobile quantum vortices in superfluid Bose-Einstein condensates. Haldane and Wu [Phys. Rev. Lett. 55, 2887 (1985)] showed that the geometric phase, $\gamma_{\mathcal C}=2\pi N_{\mathcal C}$, of such a vortex is determined by the number of condensate atoms $N_{\mathcal C}$ enclosed by the vortex trajectory. Considering an experimentally realistic freely orbiting vortex leads to an apparent disagreement with this prediction. We resolve it using the superfluid electrodynamics picture, which allows us to identify two additional contributions to the measured geometric phase; (i) a topologically protected edge current of vortices at the condensate boundary, and (ii) a superfluid displacement current. Our results generalise to, and pave the way for experimental measurements of vortex geometric phases using scalar and spinor Bose--Einstein condensates, and superfluid Fermi gases., Comment: 8 pages, 7 figures, 5 videos. A cat-state is formed as the steady-state solution for the signal mode of an ideal, degenerate parametric oscillator, in the limit of negligible single-photon signal loss. In the presence of the signal loss, this is no longer true over timescales much longer than the damping time. However, for sufficient parametric nonlinearity, a cat-state can exist as a transient state. In this paper, we study the dynamics of the creation and decoherence of cat-states in degenerate parametric oscillation, both with and without the effect of a Kerr nonlinearity that applies to recent superconducting-circuit experiments generating cat-states in microwave cavities. We determine the time of formation and the lifetime of a cat-state in terms of three dimensionless parameters $\lambda$, $g$ and $\chi$. These relate to the driving strength, the parametric nonlinearity, and the Kerr nonlinearity, respectively. We find that the Kerr nonlinearity has little effect on the threshold parametric nonlinearity ($g>1$) required for the formation of cat-states, and does not significantly alter the decoherence time of the cat-state, but can reduce the time of formation. The quality of the cat-state increases with the value $g$, and can also improved by the Kerr nonlinearity. To verify the existence and quality of the cat-state, we consider several signatures, including interference fringes and negativity, and show how they can be computed. We simulate a superconducting-circuit experiment using published experimental parameters and found good agreement with experimental results, indicating that a nonclassical cat-like state with a small Wigner negativity is generated in the experiment. A stronger nonlinearity would lead to a cat-state with convincing cat-state signatures. Finally, we explore the feasibility of creating large cat-states with a coherent amplitude of 20, corresponding to 400 photons.

Phase-space methods allow one to go beyond the mean-field approximation to simulate the quantum dynamics of interacting fields. Here, we obtain a technique for initializing either Wigner or positive-P phase-space simulations of Bose-Einstein condensates with quantum states at a finite temperature. As a means to calculate the initial states, we introduce the idea of a nonlinear chemical potential, which removes the zero-momentum phase-noise divergences of Bogoliubov theory to give a diagonal Hamiltonian. The resulting steady-state quantum theory is then directly applicable to the calculations of initial conditions for quantum simulations of BEC dynamics using phase-space techniques. These methods allow efficient and scalable simulation of large Bose-Einstein condensates. We suggest that nonlinear chemical potentials may have a general applicability to cases of broken symmetry., Comment: 23 pages

Rationale & objectiveChronic kidney disease (CKD) is complicated by abnormalities that reflect disruption in filtration, tubular, and endocrine functions of the kidney. Our aim was to explore the relationship of specific laboratory result abnormalities and hypertension with the estimated glomerular filtration rate (eGFR) and albuminuria CKD staging framework.Study designCross-sectional individual participant-level analyses in a global consortium.Setting & study populations17 CKD and 38 general population and high-risk cohorts.Selection criteria for studiesCohorts in the CKD Prognosis Consortium with data for eGFR and albuminuria, as well as a measurement of hemoglobin, bicarbonate, phosphorus, parathyroid hormone, potassium, or calcium, or hypertension.Data extractionData were obtained and analyzed between July 2015 and January 2018.Analytical approachWe modeled the association of eGFR and albuminuria with hemoglobin, bicarbonate, phosphorus, parathyroid hormone, potassium, and calcium values using linear regression and with hypertension and categorical definitions of each abnormality using logistic regression. Results were pooled using random-effects meta-analyses.ResultsThe CKD cohorts (n=254,666 participants) were 27% women and 10% black, with a mean age of 69 (SD, 12) years. The general population/high-risk cohorts (n=1,758,334) were 50% women and 2% black, with a mean age of 50 (16) years. There was a strong graded association between lower eGFR and all laboratory result abnormalities (ORs ranging from 3.27 [95% CI, 2.68-3.97] to 8.91 [95% CI, 7.22-10.99] comparing eGFRs of 15 to 29 with eGFRs of 45 to 59mL/min/1.73m2), whereas albuminuria had equivocal or weak associations with abnormalities (ORs ranging from 0.77 [95% CI, 0.60-0.99] to 1.92 [95% CI, 1.65-2.24] comparing urinary albumin-creatinine ratio > 300 vs < 30mg/g).LimitationsVariations in study era, health care delivery system, typical diet, and laboratory assays.ConclusionsLower eGFR was strongly associated with higher odds of multiple laboratory result abnormalities. Knowledge of risk associations might help guide management in the heterogeneous group of patients with CKD.

12. Seeking an Educational Utopia: An Alternative Model for Evaluating Student Learning Gain

- Abstract
Across the Higher Education sector, there is growing interest regarding the 'value for money' of a university education, and in response to this, universities may need to consider how to evaluate the perceived learning of students alongside their actual learning. "Learning Gain" maybe helpful in this regard, but there is currently no agreed method for evaluating the "Learning Gain" achieved, or perceived, by any particular student. This paper discusses a new model that can be used for evaluating such student perceptions. The model itself considers both the "Distance Travelled" (explicit knowledge) and the "Journey Travelled" (tacit knowledge) reported by students. The model employs a self-certified reflective survey method. A study, using a cohort of final year undergraduate dissertation students is undertaken to evaluate the potential usefulness of this model. The primary results obtained from this study provide an interesting perspective on how students perceive their own learning, which in turn has implications for understanding how our own teaching is viewed by students. The outcome of this research is that further testing is recommended using a wider variety of courses, and larger cohorts of students.

13. An Innovative Framework for Higher Education to Evaluate Learning Gain: A Case Study Based upon the Discipline of Marketing

- Abstract
The value for money of UK undergraduate degree courses is under increasingly critical scrutiny. Understanding the level of learning achieved by students on any particular course has therefore become of paramount importance as an indicator of teaching quality. The change to the learning that a student undertaking a course has acquired can be expressed as being their 'learning gain' and this paper applies a course level lens to investigate this using an innovative bottom-up approach which considers both the distance travelled by each student (explicit knowledge), and also their journey travelled (tacit understanding). Benchmarking learning gain data was collected from undergraduate marketing students, and gaps in perceived learning were identified. Changes to the teaching were implemented to address issues identified, and data was collected from the next cohort of students for comparison. Students reported significant improvements in learning after the changes in teaching had been implemented.

Koalas (Phascolarctos cinereus) are arboreal marsupials native to Australia that eat a specialized diet of almost exclusively eucalyptus leaves. Microbes in koala intestines are known to break down otherwise toxic compounds, such as tannins, in eucalyptus leaves. Infections by Chlamydia, obligate intracellular bacterial pathogens, are highly prevalent in koala populations. If animals with Chlamydia infections are received by wildlife hospitals, a range of antibiotics can be used to treat them. However, previous studies suggested that koalas can suffer adverse side effects during antibiotic treatment. This study aimed to use 16S rRNA gene sequences derived from koala feces to characterize the intestinal microbiome of koalas throughout antibiotic treatment and identify specific taxa associated with koala health after treatment. Although differences in the alpha diversity were observed in the intestinal flora between treated and untreated koalas and between koalas treated with different antibiotics, these differences were not statistically significant. The alpha diversity of microbial communities from koalas that lived through antibiotic treatment versus those who did not was significantly greater, however. Beta diversity analysis largely confirmed the latter observation, revealing that the overall communities were different between koalas on antibiotics that died versus those that survived or never received antibiotics. Using both machine learning and OTU (operational taxonomic unit) co-occurrence network analyses, we found that OTUs that are very closely related to Lonepinella koalarum, a known tannin degrader found by culture-based methods to be present in koala intestines, was correlated with a koala's health status. This is the first study to characterize the time course of effects of antibiotics on koala intestinal microbiomes. Our results suggest it may be useful to pursue alternative treatments for Chlamydia infections without the use of antibiotics or the development of Chlamydia-specific antimicrobial compounds that do not broadly affect microbial communities.

Evidence-informed decision making in clinical care and policy in nephrology is undermined by trials that selectively report a large number of heterogeneous outcomes, many of which are not patient centered. The Standardized Outcomes in Nephrology-Hemodialysis (SONG-HD) Initiative convened an international consensus workshop on November 7, 2015, to discuss the identification and implementation of a potential core outcome set for all trials in hemodialysis. The purpose of this article is to report qualitative analyses of the workshop discussions, describing the key aspects to consider when establishing core outcomes in trials involving patients on hemodialysis therapy. Key stakeholders including 8 patients/caregivers and 47 health professionals (nephrologists, policymakers, industry, and researchers) attended the workshop. Attendees suggested that identifying core outcomes required equitable stakeholder engagement to ensure relevance across patient populations, flexibility to consider evolving priorities over time, deconstruction of language and meaning for conceptual consistency and clarity, understanding of potential overlap and associations between outcomes, and an assessment of applicability to the range of interventions in hemodialysis. For implementation, they proposed that core outcomes must have simple, inexpensive, and validated outcome measures that could be used in clinical care (quality indicators) and trials (including pragmatic trials) and endorsement by regulatory agencies. Integrating these recommendations may foster acceptance and optimize the uptake and translation of core outcomes in hemodialysis, leading to more informative research, for better treatment and improved patient outcomes.

A procedure is described whereby a linearly coupled spinor Bose condensate can be used as a physically accessible quantum simulator of the early universe. In particular, an experiment to generate an analog of an unstable vacuum in a relativistic scalar field theory is proposed. This is related to quantum theories of the inflationary phase of the early universe. There is an unstable vacuum sector whose dynamics correspond to the quantum sine-Gordon equations in one, two or three space dimensions. Numerical simulations of the expected behavior are reported using a truncated Wigner phase-space method, giving evidence for the dynamical formation of complex spatial clusters. We review recent developments in the theory of quantum dynamics in ultra-cold atomic physics, including exact techniques, but focusing on methods based on phase-space mappings that are appli- cable when the complexity becomes exponentially large. These phase-space representations include the truncated Wigner, positive-P and general Gaussian operator representations which can treat both bosons and fermions. These phase-space methods include both traditional approaches using a phase-space of classical dimension, and more recent methods that use a non-classical phase-space of increased dimensionality. Examples used include quantum EPR entanglement of a four-mode BEC, time-reversal tests of dephasing in single-mode traps, BEC quantum collisions with up to 106 modes and 105 interacting particles, quantum interferometry in a multi-mode trap with nonlinear absorp- tion, and the theory of quantum entropy in phase-space. We also treat the approach of variational optimization of the sampling error, giving an elementary example of a nonlinear oscillator.

We present a new model for the continuous measurement of a coupled quantum dot charge qubit. We model the effects of a realistic measurement, namely adding noise to, and filtering, the current through the detector. This is achieved by embedding the detector in an equivalent circuit for measurement. Our aim is to describe the evolution of the qubit state conditioned on the macroscopic output of the external circuit. We achieve this by generalizing a recently developed quantum trajectory theory for realistic photodetectors [P. Warszawski, H. M. Wiseman and H. Mabuchi, Phys. Rev. A_65_ 023802 (2002)] to treat solid-state detectors. This yields stochastic equations whose (numerical) solutions are the ``realistic quantum trajectories'' of the conditioned qubit state. We derive our general theory in the context of a low transparency quantum point contact. BackgroundAdding the measurement of cystatin C to that of serum creatinine to determine the estimated glomerular filtration rate (eGFR) improves accuracy, but the effect on detection, staging, and risk classification of chronic kidney disease across diverse populations has not been determined.MethodsWe performed a meta-analysis of 11 general-population studies (with 90,750 participants) and 5 studies of cohorts with chronic kidney disease (2960 participants) for whom standardized measurements of serum creatinine and cystatin C were available. We compared the association of the eGFR, as calculated by the measurement of creatinine or cystatin C alone or in combination with creatinine, with the rates of death (13,202 deaths in 15 cohorts), death from cardiovascular causes (3471 in 12 cohorts), and end-stage renal disease (1654 cases in 7 cohorts) and assessed improvement in reclassification with the use of cystatin C.ResultsIn the general-population cohorts, the prevalence of an eGFR of less than 60 ml per minute per 1.73 m(2) of body-surface area was higher with the cystatin C-based eGFR than with the creatinine-based eGFR (13.7% vs. 9.7%). Across all eGFR categories, the reclassification of the eGFR to a higher value with the measurement of cystatin C, as compared with creatinine, was associated with a reduced risk of all three study outcomes, and reclassification to a lower eGFR was associated with an increased risk. The net reclassification improvement with the measurement of cystatin C, as compared with creatinine, was 0.23 (95% confidence interval [CI], 0.18 to 0.28) for death and 0.10 (95% CI, 0.00 to 0.21) for end-stage renal disease. Results were generally similar for the five cohorts with chronic kidney disease and when both creatinine and cystatin C were used to calculate the eGFR.ConclusionsThe use of cystatin C alone or in combination with creatinine strengthens the association between the eGFR and the risks of death and end-stage renal disease across diverse populations. A mechanical electroscope based on a change in the resonant frequency of a cantilever one micron in size in the presence of charge has recently been fabricated. We derive the decoherence rate of a charge superposition during measurement with such a device using a master equation theory adapted from quantum optics. We also investigate the information produced by such a measurement, using a quantum trajectory approach. We investigate the efficacy with which entanglement can be teleported using a continuous measurement scheme. We show that by using the correct gain for the classical channel the degree of violation of locality that can be demonstrated (using a CH type inequality) is {\it not} a function of the level of entanglement squeezing used in the teleportation. - Abstract
Understanding the health consequences associated with exposure to risk factors is necessary to inform public health policy and practice. To systematically quantify the contributions of risk factor exposures to specific health outcomes, the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021 aims to provide comprehensive estimates of exposure levels, relative health risks, and attributable burden of disease for 88 risk factors in 204 countries and territories and 811 subnational locations, from 1990 to 2021.

- Abstract
Regular, detailed reporting on population health by underlying cause of death is fundamental for public health decision making. Cause-specific estimates of mortality and the subsequent effects on life expectancy worldwide are valuable metrics to gauge progress in reducing mortality rates. These estimates are particularly important following large-scale mortality spikes, such as the COVID-19 pandemic. When systematically analysed, mortality rates and life expectancy allow comparisons of the consequences of causes of death globally and over time, providing a nuanced understanding of the effect of these causes on global populations.

33. Relationships between Childhood and Adult Physical Activity Patterns in a Community Sample.

- Abstract
This study examined the relationship between adult physical activity levels and patterns of activity that were established during childhood. A random digit telephone survey was conducted of noninstitutionalized residents in a medium sized California city. Subjects ages 18 and over who volunteered to participate were questioned about their involvement in both formal and informal leisure-time physical activities during their childhood and teen years. Self-reported adult levels of aerobic activity were recorded along with reasons for participation in physical activity for all three time periods. Relevant demographic data were also collected. The results indicated that aerobically active adults were significantly more likely to have participated in organized physical activities during childhood. They were also significantly more likely to have been involved with informal physical activities during high school. Self-evaluation of athletic ability during childhood was significantly more favorable among those children who became aerobically active adults. (Author)

36. Considering the Marketing of Higher Education: The Role of Student Learning Gain as a Potential Indicator of Teaching Quality

- Abstract
The marketization of higher education has ensured that students have become consumers. As a result, students are demanding increased levels of information regarding potential university courses so that they can make informed decisions regarding how best to invest their money, time and opportunity. A comparison of the teaching quality delivered on different programs of study will be an important element of this decision-making process. The Teaching Excellence Framework proposes that teaching quality will be assessed by measures including the evaluation of student learning gain. This paper reflects on an analysis of consultation responses from key stakeholders across the UK higher education sector to determine how evaluating learning gain could be effectively achieved. Synthesizing these responses, ten key considerations regarding evaluating learning gain have been identified that together provide a unique perspective to ensure that any evaluation of student learning gain subsequently undertaken is relevant to the marketing of higher education.

- Abstract
Aim: To describe and compare de novo arteriovenous fistula (AVF) failure rates between Australia and New Zealand (ANZ), and Malaysia.Background: AVFs are preferred for haemodialysis access but are limited by high rates of early failure.Methods: A post hoc analysis of 353 participants from ANZ and Malaysia included in the FAVOURED randomised-controlled trial undergoing de novo AVF surgery was performed. Composite AVF failure (thrombosis, abandonment, cannulation failure) and its individual components were compared between ANZ (n= 209) and Malaysian (n= 144) participants using logistic regression adjusted for patient- and potentially modifiable clinical factors.Results: Participants’ mean age was 55 ± 14.3 years and 64% were male. Compared with ANZ participants, Malaysian participants were younger with lower body mass index, higher prevalence of diabetes mellitus and lower prevalence of cardiovascular disease. AVF failure was less frequent in the Malaysian cohort (38% vs 54%; adjusted odds ratio (OR) 0.53, 95% confidence interval (CI) 0.31–0.93). This difference was driven by lower odds of cannulation failure (29% vs 47%, OR 0.45, 95% CI 0.25–0.80), while the odds of AVF thrombosis (17% vs 20%, OR 1.24, 95% CI 0.62–2.48) and abandonment (25% vs 23%, OR 1.17, 95% CI 0.62–2.16) were similar.Conclusions: The risk of AVF failure was significantly lower in Malaysia compared to ANZ and driven by a lower risk of cannulation failure. Differences in practice patterns, including patient selection, surgical techniques, anaesthesia or cannulation techniques may account for regional outcome differences and warrant further investigation.

The contemporary workforce requires technical and reading skills (Shanahan & Shanahan, 2008; The Conference Board, Corporate Voices for Working Families, Partnership for 21st Century Skills, Society of Human Resource Management, 2006). Technical skills allow workers to perform specific workforce tasks, while reading skills are generally needed by all workers. The model used in this study emphasizes the integration of technical and reading skills in the delivery of business content. The effective instruction of both technical and reading skills require that the teacher be knowledgeable in the content, have an appropriate teaching and learning context, and implement instructional strategies (Kehe, 2003). Business teachers indicated that they could provide more effective interdisciplinary instruction if they were provided additional training (Hite & Polkinghorne, 2006). Instructional reading skill strategies call for the teacher to recognize student reading experiences, deliberately select instructional strategies, explicitly alert students to methods of reading, and guide instruction with knowledge unique to the content-area (National Institute of Child Health and Human Development, 2000). This study posits Forget's (2004) MAX Learning instructional strategy for the interdisciplinary teaching of technical and reading skills in business courses.

47. Reading Skill Development: A Survey of Need and Responsibility

Background: According to research, adolescent reading skills tend to be poor, and increased pressure from Federal legislation has been placed on educators to improve these skills. Research lacks on business teacher educators' preparation to provide reading skill instruction. Purpose: The research purpose was to better understand the perceptions of business teacher educators regarding the integration of reading instruction into high school business courses and the influence of selected variables on their perceptions. Method: A quantitative descriptive design was used to determine the perceptions of 191 business teacher educators responsible for teaching the methods courses to business teacher candidates. Over half (111, 58.12%) responded to the mailed survey. Results: Although most (99, 89.1%) business teacher educators agreed or strongly agreed that high school business teachers should integrate reading instruction in business courses, 64 (58.7%) indicated they disagreed or strongly disagreed that business teachers should be held accountable for teaching reading in business courses. Length of teaching and focus of employing college were found to significantly influence perceptions. Conclusions and Recommendations: The majority, except those with 16 to 20 years of experience and those employed at universities primarily focused on teaching, had favorable perceptions towards the integration of reading skills. However, they did not support holding business teachers accountable or responsible for the development of student reading skills. Additional research needs to be conducted to determine factors contributing to the statistically significant differences in perceptions based on demographics factors such as length of teaching career and focus of institution where employed. Additional research also needs to be conducted to determine the extent to which business teachers are prepared to develop their students' reading skills. (Contains 3 tables.)

Attention to the judgments about the validity of research-generated knowledge claims is integral to all social science research. During the past several decades, knowledge development has been split into two communities: conventional researchers and reformist researchers. Narrative research is positioned within the reformist community. The two communities use different kinds of data and employ different analytic processes. In both communities, researchers develop arguments to convince readers of the validity of their knowledge claims. Both need to respond to threats to validity inherent in their designs. The threats particular to narrative research relate to two areas: the differences in people's experienced meaning and the stories they tell about this meaning and the connections between storied texts and the interpretations of those texts.

Qualitative research is inquiry aimed at describing and clarifying human experience as it appears in people's lives. Researchers using qualitative methods gather data that serve as evidence for their distilled descriptions. Qualitative data are gathered primarily in the form of spoken or written language rather than in the form of numbers. Possible data sources are interviews with participants, observations, documents, and artifacts. The data are usually transformed into written text for analytic use. Selection of interview participants requires purposive and iterative strategies. Production of interview data requires awareness of the complexity of self-reports and the relation between experience and languaged expression. To generate interview data of sufficient breadth and depth requires practiced skill and time. Production of useful data from other sources is addressed.

- Abstract
This article describes an alternative methodology for conducting research that is intended to bring about institutional change. This process involves developing deeper awareness among faculty members, administrators, or counselors, of a problem that exists in their local context. In some instances these individuals may be unaware that the problem exists; in others, they may be aware of the problem but not of its magnitude; or they may perceive its broad outline but not the details. To differentiate between this alternative methodology and the traditional way of conducting research, the authors call the former the "practitioner-as-researcher" model. The principal distinction between the two models is in their approach to knowledge production. In the traditional model the individual identified as the researcher controls the production of knowledge; in the practitioner-as-researcher model, stakeholders produce knowledge within a local context in order to identify local problems and take action to solve them. This article contains four parts that serve to delineate the distinctiveness and utility of the practitioner-as-researcher model. In the first section the authors contrast the traditional model of research with the practitioner-as-researcher model. Second, they provide details about a project in which the authors have utilized the practitioner-as-researcher approach, the Diversity Scorecard project. Next, the outcomes that the practitioners who engaged in research experienced are discussed. Finally, the article provides concluding thoughts and reflections on the process. (Contains 5 notes.)
