First published: 20 May 2022, Key points Due to the slow growth of Low‐grade gliomas (LGGs), cognitive impairments can be quite difficult to detect at early stages There is a lack of staff specialized on the cognitive and emotional assessment of patients, such as neuropsychologists and speech therapists Brain tumor patients are often misclassified as asymptomatic at the moment of diagnosis An accurate and individually‐designed assessment of their cognitive status can significantly improve the treatment strategy for each patient, allowing long‐term planning and substantial differences in post‐operative outcomes Healthcare facilities treating LGG patients should give high priority to the formal inclusion of trained professionals capable of designing and implementing adequate pre‐ and post‐operative strategies. By addressing this challenge, it is possible to guide clinical interventions and close the gap between surgery and rehabilitation therapy

First published: 24 December 2020, Recent evidence suggests that damage to the language network triggers its functional reorganization. Yet, the spectro-temporal fingerprints of this plastic rearrangement and its relation to anatomical changes is less well understood. Here, we combined magnetoencephalographic recordings with a proxy measure of white matter to investigate oscillatory activity supporting language plasticity and its relation to structural reshaping. First, cortical dynamics were acquired in a group of healthy controls during object and action naming. Results showed segregated beta (13–28 Hz) power decreases in left ventral and dorsal pathways, in a time-window associated to lexico-semantic processing ( 250–500 ms). Six patients with left tumors invading either ventral or dorsal regions performed the same naming task before and 3 months after surgery for tumor resection. When longitudinally comparing patients' responses we found beta compensation mimicking the category-based segregation showed by controls, with ventral and dorsal damage leading to selective compensation for object and action naming, respectively. At the structural level, all patients showed preoperative changes in white matter tracts possibly linked to plasticity triggered by tumor growth. Furthermore, in some patients, structural changes were also evident after surgery and showed associations with longitudinal changes in beta power lateralization toward the contralesional hemisphere. Overall, our findings support the existence of anatomo-functional dependencies in language reorganization and highlight the potential role of oscillatory markers in tracking longitudinal plasticity in brain tumor patients. By doing so, they provide valuable information for mapping preoperative and postoperative neural reshaping and plan surgical strategies to preserve language function and patient's quality of life.

There is still no reliable testing instrument, as the available batteries have been developed for specific languages. Heterogeneity in the selection criteria for stimuli leads to differences, for example, in the size, color, image quality, and even names associated with pictures, making direct cross-linguistic comparisons difficult. Here we present MULTIMAP, a new multilingual picture naming test for mapping eloquent areas during awake brain surgery. Recognizing that the distinction between nouns and verbs is necessary for detailed and precise language mapping, MULTIMAP consists of a database of 218 standardized color pictures representing both objects and actions. These images have been tested for name agreement with speakers of Spanish, Basque, Catalan, Italian, French, English, German, Mandarin Chinese, and Arabic, and have been controlled for relevant linguistic features in cross-language combinations. The MULTIMAP test for objects and verbs represents an alternative to the Oral Denomination 80 (DO 80) monolingual pictorial set currently used in language mapping, providing an open-source, standardized set of up-to-date pictures, where relevant linguistic variables across several languages have been taken into account in picture creation and selection.

Nestin+cells from spheroid aggregates display typical histopathological features compatible with cell stemness. Nestin and CD133+cells found in glioblastomas, distributed frequently around aberrant vessels, are considered as potential cancer stem cells. They are possible targets for antitumoral therapy because they lead the tumorigenesis, invasiveness and angiogenesis. However, little is known about their role and presence in low-grade gliomas. The aim of this work is to localize and characterize the distribution of these cells inside tumors during the development of experimental endogenous glioma. For this study, a single dose of Ethyl-nitrosourea was injected into pregnant rats. Double immunofluorescences were performed in order to identify stem-like and differentiated cells. Low-grade gliomas display Nestin+cells distributed throughout the tumor. More malignant gliomas show, in addition to that, a perivascular location with some Nestin+cells co-expressing CD133 or VEGF, and the intratumoral spheroid aggregates of Nestin/CD133+ cells. These structures are encapsulated by welldifferentiated VEGF/GFAP+cells. Spheroid aggregates increase in size in the most malignant stages. Spheroid aggregates have morphological and phenotypic similarities to in vitro neurospheres and could be an in vivo analogue of them. These arrangements could be a reservoir of undifferentiated cells formed to escape adverse microenvironments.

38. LRRK2+ R1441G and G2019S-related Parkinson's disease: are these mutations a distinctive inclusion criteria for DBS surgery?

- Abstract
Introduction: Mutations in LRRK2+ gene in chromosome 12 (also called dardarin, from the Basque word dardara, meaning tremor) have been related to the appearance of Parkinson's disease in several families. One particular mutation on this gene (R1441G) is specific to subjects of a Basque inheritance, whereas others have been described in families around the world, being G2019S the most frequent one. A previous study in our unit with a small sample size suggested that R1441G carriers had a worse outcome after DBS compared to an idiopathic PD group. Objective: To determine if DBS is equally valuable and has the same outcome to patients with PD with G2019S or R1441G mutation in LRRK2+ gene as is to idiopathic PD patients. Methods: Patients with LRRK2+ anomalies who have had surgery in Cruces University Hospital (Barakaldo, Basque Country) have been included in this analysis with a minimum of one year after procedure follow-up. As a control, patients matched 2:1 have been selected being similar in age at surgery and time of evolution of disease. Clinical data included variation of UPDRS and Schwab-England at onset and one year postoperatively, as well as variation of levodopa equivalent daily dose (LEDD). Results: Seventeen patients have been found in our cohort with LRRK2+ mutations, 12 of these with the R1441G mutation and 5 with G2019S one. Mean age at surgery in the case group was 60.05 (61.8 in the control group) and patients had a mean of 12.5 years of evolution of disease in the case group (12.3 in the control group). Implementing a linear mixed model, no differences were found related to UPDRS-III reduction after one year of surgery (31.05% in the genetic group and 30.29% in the control group) (p=0.857). No differences were found in the reduction of the Schwab-England scale (18.82 point reduction in the genetic group versus 17.71 point reduction in the control group) (p=0.886). LEDD was reduced in mean 572.53 mg in the genetic group versus 530.44 mg in the control group, which also was non-significant (p=0.734). Comparing between both mutated groups, no differences have been found in any parameter. Conclusion: There is no evidence in our study that DBS outcomes may be different in the LRRK2+ mutated group including G2019S or R1441G mutations PD than in the idiopathic PD group. Therefore, contrary to our previous report, LRRK2+ mutation carrier status should not be relevant criteria to select patients to surgery. [ABSTRACT FROM AUTHOR]

- Abstract
The O-arm surgical imaging system® is a multidimensional platform that provides the opportunity of visualizing the electrodes in real time in deep brain stimulation surgery. The objective of this study is to investigate in our center the value of this tool, by comparing the clinical improvement of a group operated on with this technology and a control group. After one year follow-up, the results were analyzed. Material and Methods: Twenty consecutive Parkinson disease patients that received deep brain stimulation with one year follow-up were selected. Electrode placement was optimized using the O-arm imaging system. They were compared to 20 consecutive patients operated on under 2D fluoroscopy as a control group. In both groups variation of the motor part of the Unified Parkinson's Disease Rating Scale (UPDRS-III) was measured, in addition to the number of microrecording tracks and the length of the surgery. Results: A median basal off reduction of 42% was observed in the O-arm group vs. 24% in the control at one year follow-up, which was statistically significant (p < 0.005). The reduction of the length of the surgery was statistically significant, being 1 h and 10 minutes less in the O-arm group. The number of tracks needed per electrode was reduced from 2.47 to 1.99. Conclusions: The O-arm can be easily included in the workflow of deep brain stimulation surgery. It gives the opportunity of visualizing the electrodes in real time. In our center it has helped to improve our results and refine our procedure. It has helped reducing the surgical time and number of tracks, as well as improving the clinical state of our patients. - Abstract
Recent evidence suggests that damage to the language network triggers its functional reorganization. Yet, the spectro-temporal fingerprints of this plastic rearrangement and its relation to anatomical changes is less well understood. Here, we combined magnetoencephalographic recordings with a proxy measure of white matter to investigate oscillatory activity supporting language plasticity and its relation to structural reshaping. First, cortical dynamics were acquired in a group of healthy controls during object and action naming. Results showed segregated beta (13-28 Hz) power decreases in left ventral and dorsal pathways, in a time-window associated to lexico-semantic processing (~250-500 ms). Six patients with left tumors invading either ventral or dorsal regions performed the same naming task before and 3 months after surgery for tumor resection. When longitudinally comparing patients' responses we found beta compensation mimicking the category-based segregation showed by controls, with ventral and dorsal damage leading to selective compensation for object and action naming, respectively. At the structural level, all patients showed preoperative changes in white matter tracts possibly linked to plasticity triggered by tumor growth. Furthermore, in some patients, structural changes were also evident after surgery and showed associations with longitudinal changes in beta power lateralization toward the contralesional hemisphere. Overall, our findings support the existence of anatomo-functional dependencies in language reorganization and highlight the potential role of oscillatory markers in tracking longitudinal plasticity in brain tumor patients. By doing so, they provide valuable information for mapping preoperative and postoperative neural reshaping and plan surgical strategies to preserve language function and patient's quality of life., (© 2020 The Authors.
