Abstract

Multiple randomized, controlled clinical trials have yielded discordant results regarding the efficacy of convalescent plasma in outpatients, with some showing an approximately 2-fold reduction in risk and others showing no effect. We quantified binding and neutralizing antibody levels in 492 of the 511 participants from the Clinical Trial of COVID-19 Convalescent Plasma in Outpatients (C3PO) of a single unit of COVID-19 convalescent plasma (CCP) versus saline infusion. In a subset of 70 participants, peripheral blood mononuclear cells were obtained to define the evolution of B and T cell responses through day 30. Binding and neutralizing antibody responses were approximately 2-fold higher 1 hour after infusion in recipients of CCP compared with saline plus multivitamin, but levels achieved by the native immune system by day 15 were almost 10-fold higher than those seen immediately after CCP administration. Infusion of CCP did not block generation of the host antibody response or skew B or T cell phenotype or maturation. Activated CD4+ and CD8+ T cells were associated with more severe disease outcome. These data show that CCP leads to a measurable boost in anti-SARS-CoV-2 antibodies but that the boost is modest and may not be sufficient to alter disease course.

Abstract

Chronic lung allograft dysfunction (CLAD) remains the major complication limiting long-term survival among lung transplant recipients (LTRs). Limited understanding of CLAD immunopathogenesis and a paucity of biomarkers remain substantial barriers for earlier detection and therapeutic interventions for CLAD. We hypothesized the airway transcriptome would reflect key immunologic changes in disease. We compared airway brush-derived transcriptomic signatures in CLAD (n = 24) versus non-CLAD (n = 21) LTRs. A targeted assessment of the proteome using concomitant bronchoalveolar lavage (BAL) fluid for 24 cytokines/chemokines and alloimmune T cell responses was performed to validate the airway transcriptome. We observed an airway transcriptomic signature of differential genes expressed (DGEs) in CLAD marked by Type-1 immunity and striking upregulation of two endogenous immune regulators: indoleamine 2, 3 dioxygenase 1 (IDO-1) and tumor necrosis factor receptor superfamily 6B (TNFRSF6B). Advanced CLAD staging was associated with a more intense airway transcriptome signature. In a validation cohort using the identified signature, we found an area under the curve (AUC) of 0.77 for CLAD LTRs. Targeted proteomic analyses revealed a predominant Type-1 profile with detection of IFN-γ, TNF-α, and IL-1β as dominant CLAD cytokines, correlating with the airway transcriptome. The airway transcriptome provides novel insights into CLAD immunopathogenesis and biomarkers that may impact diagnosis of CLAD.

7. Assessing the effect of tissue structural changes during cardiac deformation using magnetic resonance imaging techniques

Abstract

Global assessment of the myocardial tissue wall motion from cine MRI (Magnetic Resonance Imaging) is widely used in clinical practice, however, the estimation is done, in the majority of cases, visually rather than quantitatively. This doctoral thesis proposes steps forward in the local assessment of the left ventricular (LV) function in order to provide a better estimation of the damage post acute myocardial infarction (MI). Accurate post acute MI tissue damage estimation from cine MR tracking might help clinicians to perform, better, more personalised, revascu larisation treatments. This thesis makes three technical contributions. The first technical contribution focuses on LV scar segmentation from Late Gadolium Enhancement (LGE) using supervoxels. LGE represents the gold standard for assessing the viability of the heart muscle. However, considerable user input is needed in order to correctly segment the scar. A global intensity threshold is used, and a further, remote, area of "healthy myocardium" also needs to be segmented. Furthermore, an additional number of corrections are required to remove false positive areas of enhancement. We found that by using a strong regulariser such as a modified version of the Single Linear Iterative Clustering (SLIC) supervoxels, we can eliminate the false positive enhancement areas. Furthermore, there is no need for a remote "healthy myocardium" area to be contoured. The second technical contribution is to provide a 4D map of supervoxels (voxels clustered together based on local similarity) computed using similarity in strain curves. Our study found that while LGE performs extremely well in highlighting the area where scarring occurs, at the same time it underestimates the area of affected myocardium when compared with strain analysis. Here we investigated various registration methods, based on optical flow deformable registration, in order to evaluate the accuracy of computing the displacement curves, and subsequently validated the registration method against manually placed landmarks in the LV. The third technical contribution is a modification to the maskSLIC algorithm to generate six cluster centers in each short axis, basal and mid-ventricular slice, equally distributed at 60◦ from each other in order to mimic the AHA 16 segments model used in clinical practice. Considering clinical perspectives centered approach, these technical advances will contribute to the field of regional assessment of the cardiac function, therefore potentially improving patient outcome.

Abstract

Aims Cardiovascular magnetic resonance parametric mapping enables non-invasive quantitative myocardial tissue characterization. Human myocardium has normal ranges of T1 and T2 values, deviation from which may indicate disease or change in physiology. Normal myocardial T1 and T2 values are affected by biological sex. Consequently, normal ranges created with insufficient numbers of each sex may result in sampling biases, misclassification of healthy values vs. disease, and even misdiagnoses. In this study, we investigated the impact of using male normal ranges for classifying female cases as normal or abnormal (and vice versa). Methods and results One hundred and forty-two healthy volunteers (male and female) were scanned on two Siemens 3T MR systems, providing averaged global myocardial T1 and T2 values on a per-subject basis. The Monte Carlo method was used to generate simulated normal ranges from these values to estimate the statistical accuracy of classifying healthy female or male cases correctly as 'normal' when using sex-specific vs. mixed-sex normal ranges. The normal male and female T1- and T2-mapping values were significantly different by sex, after adjusting for age and heart rate. Conclusion Using 15 healthy volunteers who are not sex specific to establish a normal range resulted in a typical misclassification of up to 36% of healthy females and 37% of healthy males as having abnormal T1 values and up to 16% of healthy females and 12% of healthy males as having abnormal T2 values. This paper highlights the potential adverse impact on diagnostic accuracy that can occur when local normal ranges contain insufficient numbers of both sexes. In addition, because the site presents the zone of the lagoon where an environmental regeneration was attempted, my proposal will make use of the new “nature” that has been designed. Abstract

Introduction. Stroke is one of the main reasons for acquired disability, and the past 2 years have demonstrated that COVID-19 is a major cause of thromboembolic accidents, which usually determine cardio-pulmonary infraction, as it has a higher incidence on larger blood vessels, but there are also numerous cases reported of stroke in patients after or, more rarely, during COVID-19 active infection. Material and Methods. We present the case of a 52 year old man diagnosed with stroke during hospitalisation for a moderate type of COVID-19(Coronavirus 2019), which after several days from admittance, was found with left hemiplegia in his chamber. A comprehensive sensitive-motor rehabilitation program has been established after two and a half months after discharge, as the deficit did not regress. Results and discussion. The evolution of the patient was favorable, with an improvement of the symptoms after treatment, especially the pain syndrome, but also on improved muscle tonus and slight muscle strength increase and improved disability scores The patient rehabilitation should continue for a longer period of time, as new cerebral pathways take time to be established. Conclusions. In patients with stroke, independent of the mechanism of occurring, complex sensitive-motor rehabilitation should be started as soon as possible, in good condition, so that the benefits outbalance the risks. Extra precautions should be taken in regards to patients with COVID-19 related stroke, as they are more inclined to have a thrombotic related development, even after fully medically recovering from the disease.

Abstract

This study investigates the interwar development of a new typology of building- the department store, focusing on the case study of “Mgazinul Victoria”, first of its kind in the city of Bucharest, Romania. Rising to fame during the first half of the 20th century, surviving Second World War, and thriving during the Communist period (1947-1989), earning the title of protected monument from the Patrimony of Romania, today the building shows clear signs of economic decline and architectural decay. In this paper I explain and contextualize the important role that the store had in society, how the attitudes changed and how this change is reflected in the architecture of the store. Furthermore, by analyzing the architecture of the building I classify it within the Art Deco movement and argument that this classification together with its location within the city support the premise that the addition of the department store typology is representative for the public wish of alignment with the Western European countries, its cultures, formal urban structures, and architectural aesthetics. Abstract

Background: Quantitative cardiovascular magnetic resonance T1-mapping is increasingly used for myocardial tissue characterization. However, the lack of standardization limits direct comparability between centers and wider roll-out for clinical use or trials. Purpose: To develop a quality assurance (QA) program assuring standardized T1 measurements for clinical use. Methods: MR phantoms manufactured in 2013 were distributed, including ShMOLLI T1-mapping and reference T1 and T2 protocols. We first studied the T1 and T2 dependency on temperature and phantom aging using phantom datasets from a single site over 4 years. Based on this, we developed a multiparametric QA model, which was then applied to 78 scans from 28 other multi-national sites. Results: T1 temperature sensitivity followed a second-order polynomial to baseline T1 values (R2 > 0.996). Some phantoms showed aging effects, where T1 drifted up to 49% over 40 months. The correlation model based on reference T1 and T2, developed on 1004 dedicated phantom scans, predicted ShMOLLI-T1 with high consistency (coefficient of variation 1.54%), and was robust to temperature variations and phantom aging. Using the 95% confidence interval of the correlation model residuals as the tolerance range, we analyzed 390 ShMOLLI T1-maps and confirmed accurate sequence deployment in 90%(70/78) of QA scans across 28 multiple centers, and categorized the rest with specific remedial actions. Conclusions: The proposed phantom QA for T1-mapping can assure correct method implementation and protocol adherence, and is robust to temperature variation and phantom aging. This QA program circumvents the need of frequent phantom replacements, and can be readily deployed in multicenter trials.

Abstract

Rationale: Lymphopenia is common in severe coronavirus disease (COVID-19), yet the immune mechanisms are poorly understood. As inflammatory cytokines are increased in severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection, we hypothesized a role in contributing to reduced T-cell numbers. Objectives: We sought to characterize the functional SARS-CoV-2 T-cell responses in patients with severe versus recovered, mild COVID-19 to determine whether differences were detectable. Methods: Using flow cytometry and single-cell RNA sequence analyses, we assessed SARS-CoV-2-specific responses in our cohort. Measurements and Main Results: In 148 patients with severe COVID-19, we found lymphopenia was associated with worse survival. CD4+ lymphopenia predominated, with lower CD4+/CD8+ ratios in severe COVID-19 compared with patients with mild disease (P < 0.0001). In severe disease, immunodominant CD4+ T-cell responses to Spike-1 (S1) produced increased in vitro TNF-α (tumor necrosis factor-α) but demonstrated impaired S1-specific proliferation and increased susceptibility to activation-induced cell death after antigen exposure. CD4+TNF-α+ T-cell responses inversely correlated with absolute CD4+ counts from patients with severe COVID-19 (n = 76; R = -0.797; P < 0.0001). In vitro TNF-α blockade, including infliximab or anti-TNF receptor 1 antibodies, strikingly rescued S1-specific CD4+ T-cell proliferation and abrogated S1-specific activation-induced cell death in peripheral blood mononuclear cells from patients with severe COVID-19 (P < 0.001). Single-cell RNA sequencing demonstrated marked downregulation of type-1 cytokines and NFκB signaling in S1-stimulated CD4+ cells with infliximab treatment. We also evaluated BAL and lung explant CD4+ T cells recovered from patients with severe COVID-19 and observed that lung T cells produced higher TNF-α compared with peripheral blood mononuclear cells. Conclusions: Together, our findings show CD4+ dysfunction in severe COVID-19 is TNF-α/TNF receptor 1-dependent through immune mechanisms that may contribute to lymphopenia.
