Objective: Many stroke survivors still present with upper-limb paresis six months post-stroke, impacting their autonomy and quality of life (QoL). We designed an enriched Music-supported Therapy (eMST) program to reduce disability in this population. We evaluated the eMST's effectiveness in improving functional abilities and QoL in chronic stroke individuals compared to the conventional motor program Graded Repetitive Arm Supplementary Program (GRASP)., Methods: We conducted a pragmatic two-arm parallel-group randomized controlled trial with a 3-month follow-up and masked assessment. The eMST involved playing instruments during individual self-administered and group music therapy sessions. The GRASP consisted of self-administered motor exercises using daily objects. Both interventions were completed at home with telemonitoring and involved four one-hour weekly sessions for 10 weeks. The primary outcome was upper-limb motor function measured with the Action Research Arm Test. Secondary outcomes included motor impairment, daily life motor performance, cognitive functions, emotional well-being, QoL, self-regulation, and self-efficacy. Intention-to-treat (ITT) and per-protocol (PP) analyses were conducted including participants who discontinued the intervention and those who completed it entirely, respectively., Results: Fifty-eight chronic stroke patients were randomized to the eMST-group (n = 26; age: 64.2 ± 12.5; 6 [23.1%] females; 2.8 ± 2.9 years post-stroke), and the control group (n = 32; age: 62.2 ± 12; 8 [25%] females; 1.8 ± 6.2 years post-stroke). The eMST-group had more participants achieving a clinically relevant improvement in motor impairment post-intervention than the control group for the ITT (55% vs 21.6%; OR = 4.5 (95% CI 1.4-14); p = .019) and PP analyses (60% vs 20%; OR = 6 (95% CI 1.5-24.7); p = .024), sustained at follow-up. The eMST-group reported greater improvements in emotion (difference = 11.1 (95% CI 0.8-21.5; p = 0.36) and participation (difference = 10.3 (95% CI 0.6-25.9); p = 0.41) subscales of QoL, and higher enjoyment during the sessions (difference = 1 (95% CI 0.3-1.5); p = 0.12). Some patients with autism and severe intellectual disability may experience uncontrolled aggression, causing serious injury or harm to others, and the therapeutic ineffectiveness of traditional pharmacological and behavioral treatment may aggravate symptoms. Deep brain stimulation (DBS) has been tested in patients with little evidence in children and adolescents. Therefore, we analyzed the efficacy and safety of DBS in refractory aggression in pediatric subjects with autism (ASD) and severe intelligence deficit (ID).Methods A meta-analytic review of Web of Science (WOS) and Scopus articles, following Prisma criteria. A total of 555 articles were identified, but after applying the inclusion criteria, only 18 were analyzed. The review of the registries and the extraction of information was performed by 2 independent groups, to reduce the evaluator's bias. For the description of the results, pediatric patients with ASD or ID present in each registry, with an application of specialized scales (Overt aggression scale, OAS, and THE modified version of the OAS, MOAS) pre and post-DBS, with a clinical follow-up of at least 12 months, were considered valid. Clinical improvement was calculated using tests of aggressiveness. In each registry with available data and then pooling the means of all patients in the OAS and MOAS, the effect size of DBS (overall and per study) was estimated. Finally, the adapted NOS scale was applied to rate the studies' quality and level of bias.Results In the studies analyzed, 65/100 were pediatric patients, with a mean age of 16.8 years. Most of the studies were conducted in South America and Europe. In all teams, aggressive behavior was intractable, but only 9 groups (53/65) applied specialized scales to measure aggressiveness, and of these, only 51 subjects had a follow-up of at least 12 months. Thus, in 48/51 a clinical improvement of patients was estimated (94.2%), with a considerable overall effect size (OAS: d = 4.32; MOAS: d = 1.46). However, adverse effects and complications were found in 13/65 subjects undergoing DBS. The brain target with the most evidence and the fewest side effects was the posteromedial hypothalamic nuclei (pHypN). Finally, applying the adapted NOS scale, quality, and bias, only 9 studies show the best indicators.Conclusion An optimal level of efficacy was found in only half of the publications. This is mainly due to design errors and irrelevant information in the reports. We believe that DBS in intractable aggressiveness in children and adolescents with ASD and severe ID can be safe and effective if working groups apply rigorous criteria for patient selection, interdisciplinary assessments, objective scales for aggressiveness, and known surgical targets., (© 2024. The article, in the form of an essay, systematizes a 40-year-long professional trajectory of interdisciplinary and socially engaged experiences around the analysis and prevention of accidents and disasters. This study was mainly developed within the scope of research and postgraduate studies in Public Health in Brazil, driven by the sanitarian movement and the construction of Brazilian Unified National Health System (SUS) in its search for democracy and social and health justices. Its empirical basis involved workers' health and environmental surveillance actions organized in networks led by SUS in conjunction with universities, unions, social movements, environmental nongovernmental organizations (NGO), and Public Prosecutors' Offices. Events of greater socio-environmental complexity in sectors such as steel, petrochemicals, mining, agribusiness, and energy forged the search for new epistemic and interdisciplinary references that encompassed two new justices, i.e., environmental and cognitive. This essay systematizes this trajectory of conceptual contributions in three movements from the 1980s to the present day (each corresponding to a socio-political and institutional context) to reflect on paradigmatic transition movements in the analysis and prevention of accidents and disasters from an interdisciplinary perspective. It ends by suggesting abyssal and emancipatory prevention to face different current crises, including environmental, health, democratic, and civilizing ones.

Background: Anhedonia refers to the diminished capacity to experience pleasure. It has been described both as a symptom of depression and an enduring behavioral trait that contributes its development. Specifically, in stroke patients, anhedonia has been closely linked to depression, resulting in reduced sensitivity to everyday pleasures and intrinsic motivation to engage in rehabilitation programs and maintain a healthy active lifestyle. This condition may hinder patients' recovery, diminishing their autonomy, functioning, and quality of life., Objective: We aimed to explore the prevalence and level of anhedonia and those variables that might be associated in patients with both ischemic and hemorrhagic stroke at subacute and chronic phases of the disease., Methods: We conducted an exploratory cohort study with a sample of 125 patients with subacute and chronic stroke presenting upper-limb motor deficits. We measured participants' level of anhedonia with four items from the Beck Depression Inventory-II that describe the symptoms of this condition: loss of pleasure, loss of interest, loss of energy, and loss of interest in sex. We also collected demographic and clinical information and evaluated motor and cognitive functions as well as levels of depression, apathy, and various mood states. The results were compared to a sample of 71 healthy participants of similar age, sex, and level of education., Results: Stroke patients demonstrated a significantly higher prevalence (18.5-19.7%) and level of anhedonia compared to the healthy controls (4.3%), regardless of stroke phase, level of motor impairment, and other clinical variables. Furthermore, post-stroke anhedonia was associated with lower levels of motivation and higher levels of negative mood states such as fatigue and anger in the long term. Importantly, anhedonia level was superior in stroke patients than in healthy controls while controlling for confounding effects of related emotional conditions., Conclusion: This study provides novel evidence on the prevalence, level and factors related to anhedonia post-stroke. Background: Chronic non-cancer pain (CNCP) treatment's primary goal is to maintain physical and mental functioning while improving quality of life. Opioid use in CNCP patients has increased in recent years, and non-pharmacological interventions such as music listening have been proposed to counter it. Unlike other auditive stimuli, music can activate emotional-regulating and reward-regulating circuits, making it a potential tool to modulate attentional processes and regulate mood. This study's primary objective is to provide the first evidence on the distinct (separate) effects of music listening as a coadjuvant maintenance analgesic treatment in CNCP patients undergoing opioid analgesia., Methods and Analysis: This will be a single-centre, phase II, open-label, parallel-group, proof-of-concept randomised clinical trial with CNCP patients under a minimum 4-week regular opioid treatment. We plan to include 70 consecutive patients, which will be randomised (1:1) to either the experimental group (active music listening) or the control group (active audiobooks listening). During 28 days, both groups will listen daily (for at least 30 min and up to 1 hour) to preset playlists tailored to individual preferences.Pain intensity scores at each visit, the changes (differences) from baseline and the proportions of responders according to various definitions based on pain intensity differences will be described and compared between study arms. We will apply longitudinal data assessment methods (mixed generalised linear models) taking the patient as a cluster to assess and compare the endpoints' evolution. Introduction: The detection of anxiety symptoms among health workers who care for patients infected with COVID-19 is a current priority. Fast and valid instruments are required for this population group. The objective is to establish the construct validity and reliability of the Generalised Anxiety Disorder (GAD-7) scale in Colombian doctors during the COVID-19 lockdown., Methods: E-health study, in which cross-sectional data were collected online (n = 1030) from 610 COVID doctors and 420 non-COVID doctors, during the Colombian lockdown, between 20 April and 10 August 2020. Each subject was contacted, and they confirmed their participation, identity and professional role., Results: A single factor factorial structure was found, made up of the 7 items of the instrument, which managed to explain 70% of the variance. The goodness of fit indices (RMSEA = 0.080; CFI = 0.995; SRMR = 0.053; p < 0.001) showed an "acceptable" unidimensionality and adequate factor loadings in each item of the GAD-7, >0.070. Finally, the internal consistency of the instrument was good, with a Cronbach's alpha of 0.920 (95%IC, 8.80-9.71)., Conclusions: The GAD-7 is an instrument that presents adequate indicators of validity and reliability. Introduction: The effects of COVID-19 confinement have been severe, especially in older adults. Therefore, we analyzed the factors associated with cognitive impairment (CI) in Latin America (LA)., Methods: We conducted a cross-sectional observational study with a total of 5245 older adults from 10 countries in LA., Measurement: We used the Telephone Montreal Cognitive Assessment (T-MoCA) and the Eight-item Informant Interview to Differentiate Aging and Dementia (AD8) scale., Results: We found that age, depressive symptomatology, bone fractures, being widowed, having a family member with dementia, and unemployment were associated with an increased risk of CI. In contrast, higher education, hypertension with continuous treatment, quarantine, and keeping stimulating cognitive and physical activities were associated with a lower probability of CI. No significant association was found between suffering from diabetes or being retired and CI., Discussion: It is essential to conduct follow-up studies on these factors, considering their relationship with CI and the duration of confinement., Competing Interests: The authors declare no conflicts of interest. Introduction: The coronavirus pandemic continues to affect the mental health of healthcare personnel in Latin America (LA)., Objective: To estimate the prevalence of psychological disturbances and associated risk factors for mental health in healthcare personnel in LA during the second year of the COVID-19 pandemic., Method: This multicenter cross-sectional study included a total sample of 5437 healthcare professionals from Argentina, Bolivia, Chile, Colombia, Ecuador, and Peru. The PHQ-9, GAD-7, and a brief demographic questionnaire were used. The prevalence of anxiety and depression was estimated based on the cut-off points of the instruments. Two multivariate logistic regressions were performed., Results: A population burden of anxiety (40.1%) and depression (62.2%) was found in healthcare personnel in LA. Among professionals in Argentina (OR = 1.374; P <.001), those working in state hospitals (OR = 1.536; P <.003), frontline healthcare workers for COVID patients (OR = 1.848; P <.001), general practitioners (OR = 1.335; P <.001), and specialists (OR = 1.298; P <.001), a higher risk of experiencing mental disorders was observed. Among women, younger personnel, and administrative staff, a higher probability of experiencing anxiety and depression was identified., Conclusions: The burden of mental disorders on healthcare personnel in Latin America is alarming. Background: Alzheimer's disease (AD) is a neurodegenerative disease that causes a gradual loss of cognitive functions and limits daily activities performance. Early diagnosis of AD is essential to start timely treatment. This study aimed to validate the Uniform Data Set neuropsychological battery version 3.0 (UDS 3.0) in a Colombian cohort., Methods: This study is a cross-sectional type, consecutive, incidental, with 143 persons, divided into two groups: 48 diagnosed AD cases and 95 healthy controls, between the ages of 50 and 80+, and between 1 and 19+ years of education., Results: The results indicate differences between the control group and the AD group in most battery tests. A significant correlation was found between the Montreal Cognitive Assessment (MoCA), Multilingual Naming Test (MINT), Craft Story, Benson Figure Test, P-word and F-word Phonemic Fluency Test, and their respective reference tests. Cutoff points were found based on the Youden index for each sub-test. The results indicate that all sub-tests are above the reference line of the ROC curve., Conclusion: The use of the UDS 3.0 in Colombia would help improving clinical diagnostic routes because of its high accuracy and high correlation with tests that measure general impairment; it has good sensitivity and specificity, and it can be a useful tool for AD.

Background and objectives: An increase in emotional disturbances and complaints about cognitive performance has been observed in Latin American healthcare workers during the SARS-CoV-2 pandemic, which can affect attention capacity and increase the levels of stress and burnout of these professionals. The objective was to analyse subjective cognitive complaints (SCC) and associated factors in health personnel during the COVID-19 pandemic in five Latin American countries. Methods: Multicentre cross-sectional study, which included 3,738 professionals from Colombia, Chile, Argentina, Ecuador, Bolivia, and Peru. The Generalized Anxiety Disorder Scale (GAD-7) and the Patient Health Questionnaire (PHQ-9) was used to assess depression, and the Mini-Z to assess Burnout. For the SCC, an item on cognitive concerns in attention and memory was used. Results: The prevalence of cognitive complaints was 69.2%. The factors associated with a higher risk of SCC were the scores in the GAD-7, PHQ and Mini-Z, in addition to being part of the Ecuadorian health personnel. The COVID-19 pandemic has shown the need for neuropsychological care for older adults with memory complaints in different contexts, including rural areas or areas with difficult access., Objective: This study aimed to analyze the clinical utility of the Phototest, through telemedicine, to identify mild cognitive impairment in rural older adults with memory complaints, during the COVID-19 pandemic., Methods: We performed a cross-sectional, case-control, and clinical utility comparison of brief cognitive tests (BCTs). The sample included 111 rural elderly people with mild cognitive impairment (MCI) and 130 healthy controls from the Los Lagos region, Chile. The instruments adopted were modified Mini-Mental State Examination (MMSEm) and adapted version of the Phototest (PT) for Chile., Results: To identify mild cognitive impairment, using a cutoff score of 27-28 points, the Phototest showed a sensitivity of 96.6% and a specificity of 81.8%; indicators superior to those of the MMSEm., Conclusions: The Phototest is more accurate than the MMSEm in identifying cognitive alterations in rural older adults with cognitive memory complaints through telemedicine. Background: The impact of quarantine in older adults have been reported in several studies with contradictory results, reporting from negative effects to no significant outcomes or even beneficial consequences. Heterogeneity in aging plays a role in each region, the aim of this study is to analyze the impact of quarantine on health conditions (physical and mental) and lifestyle in older adults in five Centro American countries during COVID-19 pandemic., Method: In this cross-sectional study, n = 712 older adults 60 years and older from Mexico, Guatemala, El Salvador, Honduras and Costa Rica were assessed by telephone. Sociodemographic data, physical and mental health, lifestyle and quarantine conditions were asked previous informed consent., Results: In general, mean of days in quarantine at the moment of the study was 142 days (approximately four months and three weeks). In the analysis of the impact of the days in quarantine effects were found on the frequency of falls, functional ability in Activities of Daily Living (ADL), general cognitive function, memory, orientation, language, frequency of drinking alcohol, having a balanced diet, and being active cognitively. Some differences were found between countries., Conclusions: Effects of quarantine on older adults in Centro America, requires attention of governments and healthcare to prevent long term morbidity and disability, and to promote healthy aging., Competing Interests: The authors have declared that no competing interests exist.

This article, an essay, and narrative review, analyzes the relationship between the 2030 Agenda, food systems, and their relevance to global and collective health. The concept of syndemics contextualizes the COVID-19 pandemic in relation to poverty and social injustice, as it also reveals the synergy with other pandemics related to the advancement of the global food system: malnutrition, obesity, and climate change, which all have strong influence of the dominant model of agriculture. We also use four strategic concepts to think about the transition towards healthy and sustainable food systems: food system, food and nutrition security (FNS), human right to adequate food (HRAF) and agroecology. Then, we gather international reports and data that systematize studies on the growing threats imposed by the dominant agricultural model, often denied by powerful economic sectors and neoconservative groups. We also highlight challenges imposed at different scales, from global to local, so that public policies and social mobilizations developed in the last two decades can resist and reinvent themselves in the construction of fairer societies.

The article, a critical essay, presents the notion of an emancipatory promotion of health (EPH), which seeks to relate the field of public health in theoretical and practical terms with the reinvention of utopias and relevant social struggles of our time principally in the Global South, a metaphor designating the peripheral or semiperipheral regions of the modern world-system. This is a relevant topic in the face of the planetary socio-ecological crisis and the debate on economic development, democracy and sustainability at a moment especially critical for the Brazilian reality and other countries. In particular, the contributions of peoples and social movements from rural, forests and water-dominated regions in Brazil to the emancipatory practices of health are discussed. The article is mainly based on two theoretical and methodological references, the Political Ecology and post-colonial approaches. The first focuses its analysis on the intensification of the social metabolism and environmental conflicts in the global capitalist economy, based on an unfair and unsustainable international trade that generates numerous socio-environmental conflicts, mainly in the Global South, that is, the peripheral countries that export agricultural and mineral commodities to richer ones. The second reference broadens and integrates the critique of capitalism with colonialism and patriarchy, understood as the three axes of oppression resulting from the Eurocentric modernity project. The challenge here is to deconstruct and reconstruct through an intercultural dialogue new conceptions of society, economy, nature, development, work and health. Such references help us to understand the importance of indigenous, peasant and Afro-descendant struggles for an emancipatory health promotion., (© The Author(s) 2019. This article explores the applicability of fluorescence and absorbance spectroscopy for estimating organic pollution in polluted rivers. The relationship between absorbance, fluorescence intensity, dissolved organic carbon, biochemical oxygen demand (BOD), chemical oxygen demand (COD), and other water quality parameters were used to characterize and identify the origin and the spatial variability of the organic pollution in a highly polluted watershed. Analyses were performed for the Iguassu River, located in southern Brazil, with area about 2,700 km 2 and ∼3 million inhabitants. Samples were collect at six monitoring sites covering 107 km of the main river. BOD, COD, nitrogen, and phosphorus concentration indicates a high input of sewage to the river. Specific absorbance at 254 and 285 nm (SUVA 254 and A 285 /COD) did not show significant variation between sites monitored, indicating the presence of both dissolved compounds found in domestic effluents and humic and fulvic compounds derived from allochthonous organic matter. Correlations between BOD and tryptophan-like fluorescence peak (peak T 2 , r =0.7560, and peak T 1 , r =0.6949) and tyrosine-like fluorescence peak (peak B, r =0.7321) indicated the presence of labile organic matter and thus confirmed the presence of sewage in the river. Results showed that fluorescence and absorbance spectroscopy provide useful information on pollution in rivers from critical watersheds and together are a robust method that is simpler and more rapid than traditional methods employed by regulatory agencies.

The phenomenon of globalization and the increase in neo-extractivism in the global periphery intensify the search for new territories and natural resources for the economy, resulting in significant impacts on ecosystems and on the lives of vulnerable populations. It is considered that the environmental crisis imposes new challenges and requires an updating of the theoretical and methodological foundations of collective health and the social determinants of health. The scope of this paper is to present theoretical contributions to the construction of a critical socio-environmental approach from a review of the literature structured around previous work on the mapping of environmental conflicts, and conducting empirical studies in conflicting areas. The contributions of sociology, political ecology, postcolonial studies and geography is summarized for the discussion of the socio-environmental determinants of health, as well as experiences that integrate emancipatory knowledge, political subjects, resistances and alternatives for society.

'Health and Environment' is the nucleus of knowledge and practices surrounding the relations between society and nature, mediated by the mode of production and human labor, which help to understand the determination of the health-disease process of different social classes and groups. This paper discusses the challenges to build this field from the perspective of its Thematic Group of the Association of Collective Health. The three core themes of the 2nd Brazilian Symposium on Environmental Health are taken as the theoretical framework for analysis. A timeline of the group's trajectory, in its historical context, helps to identify the contemporary challenges. Among the main challenges identified, the challenge of building an interdisciplinary, intersectoral and participatory scientific practice, which could build knowledge and dialogue with social movements, is highlighted.

This article assesses aluminum production in Brazil and its social, environmental and public health impacts. The effects of the aluminum production chain challenge the idea of sustainable growth affirmed by business groups that operate in the sector. This article upholds the theory that the insertion of Brazil in the global aluminum market is part of a new configuration of the International Division of Labor (IDL), the polluting economic and highly energy dependent activities of which - as is the case of aluminum - have been moving to peripheral nations or emerging countries. The laws in such countries are less stringent, and similarly the environmental movements and the claims of the affected populations in the territories prejudiced in their rights to health, a healthy environment and culture are less influential. The competitiveness of this commodity is guaranteed in the international market, from the production of external factors such as environmental damage, deforestation, emissions of greenhouse gases and scenarios of environmental injustice. This includes undertakings in the construction of hydroelectric dams that expose traditional communities to situations involving the loss of their territories.

This article discusses the role of populations affected by environmental injustice situations in the production of knowledge about environmental health stemming from inequalities and discrimination in the distribution of risks and benefits of economic development. Special attention is given to the epistemological and political limits to producing knowledge and alternatives that enable advances in building more just and sustainable societies are highlighted. Based on a broader view of health, the limits of scientific approaches are called into question by acknowledging the importance of local knowledge are discussed, either to analyze environmental risks or their effects on health, including epidemiological studies. These limits are linked primarily to the concealment of conflicts and uncertainties, the lack of contextualization of exposure to risk and effects on health, as well as the difficulties of dialogue with the communities. The article also presents contributions and advances presented by environmental justice movements. The conclusion is that a constructivist, procedural and democratic perspective of confronting forms of knowledge and practices can guide the scientific production to benefit of environmental justice.

This article discusses the limits, alternatives and challenges of environmental management in contemporary globalized capitalist societies. It is based on a critical analysis supported by authors from social sciences, political ecology and public health. To this end, we systematize the meaning of hegemonic environmental management in terms of eco-efficiency and its limits to tackle environmental risks and construct democratic processes and societies. We developed four ideal scenarios involving possible combinations of environmental management and democracy. This model served as a base, together with academic studies and the theoretical and militant experience of the authors, for a reflection on the current characteristics and future trends of environmental management and democracy, with emphasis on the reality of Latin America, specifically Brazil. Lastly, we discuss possibilities for social transformation taking into consideration the contradictions and emancipatory alternatives resulting from confrontations between hegemonic tendencies of the market and counter-hegemonic utopias and social movements. The latter assume principles of environmental justice, economic solidarity, agro-ecology and sustainability as well as the construction of new epistemologies.

Objective: To estimate the externalities associated with acute poisoning from pesticides., Methods: The probabilities of acute poisoning were estimated according to characteristics of rural properties and cities in the state of Paraná, Southern Brazil. Information about acute poisoning obtained from the 1998-1999 Harvest Forecast Survey was used. The expected costs with poisoning in these properties were calculated from the sum of medical-hospital expenses and days spent on sick leaves, required for the recovery of intoxicated individuals. A multilevel model was constructed for the analysis., Results: The costs associated with acute poisoning can total up to US$ 149 million for the state of Paraná, i.e. for each dollar spent to purchase pesticides in this state, approximately US$ 1.28 may be spent with the external costs of poisoning. This situation could be changed with the implementation of public policies, such as the adoption of an organic agriculture promotion program in the cities where the social cost with acute poisoning could be reduced by approximately US$ 25 million., Conclusions: Society, especially the populations mainly affected by pesticides, could be benefited by the identification and elimination of the risks of acute intoxication associated with the current model. It is necessary to implement public policies and integrated actions that involve the fields of economics, public health, agronomy, environmental issues, education, and science and technology, among others.

One of the most important causes for poor water quality in urban rivers in Brazil is the low collection efficiency of the sewer system due to unforeseen interconnections with the stormwater drainage system. Since the beginning of the 20th century, Brazilian cities have adopted separate systems for sanitary sewers and stormwater runoff. Gradually these two systems became interconnected. A major challenge faced today by water managers in Brazil is to find efficient and low cost solutions to deal with this mixed system. The current situation poses an important threat to the improvement of the water quality in urban rivers and lakes. This article presents an evaluation of the water quality parameters and the diffuse pollution loads during rain events in the Pinheiros River, a tributary of the Tietê River in São Paulo. It also presents different types of integrated solutions for reducing the pollution impact of combined systems, based on the European experience in urban water management. An evaluation of their performance and a comparison with the separate system used in most Brazilian cities is also presented. The study is based on an extensive water quality monitoring program that was developed for a special investigation in the Pinheiros River and lasted 2.5 years. Samples were collected on a daily basis and water quality variables were analyzed on a daily, weekly or monthly basis. Two hundred water quality variables were monitored at 53 sampling points. During rain events, additional monitoring was carried out using an automated sampler. Pinheiros River is one of the most important rivers in the São Paulo Metropolitan Region and it is also a heavily polluted one.

Despite bans on certain pesticides and their replacement by others considered less hazardous, the widespread use of these substances in agriculture continues to threaten the environment and the health of millions of people. This article discusses the current double standard in the international trade of pesticides and focuses on Brazil, one of the main users of pesticides in the world, analyzing the trends in foreign trade (imports and exports) of selected pesticides as a function of changes in legislation in the United States, the European Union, and Brazil from 1989 to 2006. We applied time line analysis to eight organochlorines already banned in Brazil and conducted a case-by-case qualitative and quantitative analysis of nine other pesticides. The results indicate the existence of double standards, as demonstrated by the continued exports to Brazil of some pesticides banned in the United States and Europe.

The aim of this article was to evaluate socio-environmental vulnerability by selecting and analyzing socioeconomic, demographic, and infrastructural indicators around the Fazenda Botafogo Industrial Park in the city of Rio de Janeiro, Brazil, taking this work as a case study. The study aimed to determine whether there is a correlation between socially vulnerable groups and environmental risk, thus shaping a situation of environmental injustice. The study features a quantitative and spatial analysis based on the use of a geographic information system (GIS). The analyses show a strong correlation between social vulnerability and environmental risk, based on residents' proximity to factories in the target area. The indicators show that the populations with the worst socioeconomic conditions tend to live closest to the industrial sources of environmental risks. Inversely, the population groups with the best socioeconomic conditions tend to live the farthest from such sources.

In this article we discuss the development of hazardous waste co-incineration in cement kilns in Brazil as well as its impacts on health and the environment. Information was gathered through an extensive review on social and environmental impacts of co-incineration, and case studies, chosen after discussion with social movement representatives concerned with the co-incineration issue and related to the Brazilian Network on Environmental Justice, are described to illustrate the reality of co-incineration in Brazil. Studies showed that workers and community health suffers negative impacts from such practices in high-, middle- and low-income countries. In the Brazilian context, the institutional and social vulnerability intensifies these problems. To conclude, we argue for the necessity of increasing the institutional capacity of health and environmental agencies in Brazil, through staff training and better infra-structure. Additionally, we also propose a return of the debate about political and ethical aspects of industrial waste trade.

The 1st National Environmental Health Conference, in December 2009, presents countless challenges to the field of Public Health. It debates key concepts as development, sustainability, production and consumption processes, democracy and public policies; advocating for innovative, interdisciplinary and intersectorial aspects of Environmental Health. The Conference recovers and articulates important themes for the Public Health, and also indicates the need of reflecting the socio-environmental determinants of health at the present time, in order to provide progresses in the construction of guidelines and actions to health surveillance and promotion. This article discusses the characteristics of the Brazilian model of development, its impacts and conflicts within social, environmental and health fields. We use theoretical and empirical contributions from the fields of Ecological Economy and Political Ecology, as well as, experiences of cooperation with the Brazilian Network on Environmental Justice and several social movements. Two cases are discussed in more detail: the first related to agribusiness and the use of pesticides, and the other about the expansion of the iron and steel industry in Brazil. We conclude proposing some elements that could be incorporated by a research agenda committed to the debate about the 'socioenvironmental crisis'.

This paper discusses the negative externalities associated with the intensive use of pesticides in the Brazilian savannah. These externalities are mainly related to impacts on the environment and on human health (rural workers and families, consumers), the costs of which end up being socialized. The externality considered in the present paper is of soil and water contamination by pesticides. The data source is the questionnaire of the Basic Municipal Information Research applied in 2003. Maps are used in order to associate contaminated areas with agricultural activity. Some risk factors associated with soil and water contamination by pesticides such as seasonal crop area, air pollution by burning and weed proliferation, were obtained through a logistic regression. The study concludes that the results can be helpful to formulate policies and aid in the design of regulating instruments and the definition of priority areas where preventive actions should be implemented.

This article proposes to focus contributions from political ecology and ecological economics to the field of collective health with a view towards integrating the discussions around health promotion, socio-environmental sustainability, and development. Ecological economics is a recent interdisciplinary field that combines economists and other professionals from the social, human, and life sciences. The field has developed new concepts and methodologies that seek to grasp the relationship between the economy and ecological and social processes such as social metabolism and metabolic profile, thereby interrelating economic, material, and energy flows and producing indicators and indexes for (un)sustainability. Meanwhile, political ecology approaches ecological issues and socio-environmental conflicts based on the economic and power dynamics characterizing modern societies. Collective health and the discussions on health promotion can expand our understanding of territory, communities, and the role of science and institutions based on the contributions of political ecology and ecological economics in analyzing development models and the distributive and socio-environmental conflicts generated by them.

This article presents the results of a study on the lives, work, and health conditions of garbage pickers in the largest metropolitan landfill in Rio de Janeiro, Brazil. Using a semi-structured questionnaire with open-ended and closed questions, the study interviewed these individuals and developed a discussion of their daily lives, work, and health conditions. According to a quantitative-qualitative analysis, the garbage pickers identified garbage as a source of survival and defined health simply as the ability to work. They thus tended to neglect the relationship between work and health. However, the risks and reported morbidity highlighted the hazardous nature of this activity, aggravated by their living and housing conditions. Finally, the article emphasizes the importance of establishing public policies that integrate different dimensions of the problem, such as social inclusion, environmental preservation, public health, and the dignity of these workers.

Chemical safety is acknowledged by Agenda 21 as one of the most serious problems worldwide, involving governance at the national and international levels. In Brazil, chemical safety problems have increased in intensity and extent, far beyond the capacity to deal with them. The problems are all the more serious in Brazil because issues of democracy, security, sustainability, and equity, all fundamental to governance, are still incipient and still far from being solved. New societal arrangements and a new, contextualized and more participatory science form the basis for developing and expanding strategies for governance to deal with the problem of chemical safety.

Chemical safety is recognized in Agenda 21(UNCED--Rio 92), as one of the most serious problems to be faced worldwide being a problem not just of governability, restricted more to the role of states and governments, but of governance at the national and international levels. It poses greater challenges for countries like Brazil where the issues of democracy, security, sustainability and equity, which are fundamental to governance, are merely incipient and still far from solved. Taking as references the analysis of four cases in the Brazilian context, we illustrate the situation from less densely populated areas (as in the cases of mercury use by gold miners in the Amazon and pesticides in agricultural producing areas), through medium-sized towns (as in the accident with cesium 137 in Goiânia), to the major urban and industrial centers (as in the cases of lead from battery renovators and benzene in the oil and steel industries and oil refineries). What can be seen is a situation where problems connected with chemical safety have grown in intensity and extent far more than the capacity to deal with them. In industrializing countries, the formulation of chemical safety policies must take into account not only the complexity and uncertainties involved in understanding the problems, but also the aspects relating to the different modes and levels of vulnerability. This is necessary in order to attempt to build better contextualized and more participatory knowledge and decision-making processes at the local and global levels, which we take as basic prerequisites for governance.

The offshore oil industry is characterized by complex systems in relation to technology and organization of work. Working conditions are hazardous, resulting in accidents and even occasional full-scale catastrophes. This article is the result of a study on work-related accidents in the offshore platforms in the Campos Basin, Rio de Janeiro State. The primary objective was to provide technical back-up for both workers' representative organizations and public authorities. As a methodology, we attempt to go beyond the immediate causes of accidents and emphasize underlying causes related to organizational and managerial aspects. The sources were used in such a way as to permit classification in relation to the type of incident, technological system, operation, and immediate and underlying causes. The results show the aggravation of safety conditions and the immediate need for public authorities and the offshore oil industry in Brazil to change the methods used to investigate accidents in order to identify the main causes in the organizational and managerial structure of companies.

The objective of this paper is to show the development program about health and environment in an institution of academic research and teaching, technological development and services. We analyse data from the register of projects in health and environment in the development process as the thematic axis for this program. We show the sectors which the projects are related, the key words of research and teaching, the construction of thematic axis that should guide and the different approach of interdisciplinary. At the end we show the challenges and perspectives for an interdisciplinar work about health and environment in an academic research and teaching institution.

The objective of this paper is to identify theoretical questions and challenges for the public health field, especially the field of workers' health, raised by technological and environmental risks related to the workplace but going beyond it. Using industrial chemical risks as an example, the authors show the vulnerability of contemporary societies in handling such risks and the worsening situation in peripheral countries. The scope of risk analysis is presented according to the engineering, toxicological, and epidemiological approaches as well as the critique raised by the social sciences. Science alone cannot solve such complex problems as technological and environmental risks. To overcome these limits, some theoretical and methodological proposals have been developed by the risk analysis and workers' health fields. The authors emphasize the discussions on the interdisciplinary, systemic, and participant approaches, incorporating the interaction between knowledge and dialogue as fundamental conditions for the prevention and control of technological and environmental risks.

Accidents in the chemical industry, such as those that took place in Seveso (1976) and Bhopal (1984), may kill or injure thousands of people, cause serious health hazards and irreversible environmental damage. The aim of this paper is to examine the ever-increasing risk of similar accidents becoming a frequent ocurrence in the so-called industrializing countries. Using figures from some of the worst chemical accidents in the last decades, data on the Bhopal disaster, and Brazil's social and institutional characteristics, we put forward the hypothesis that present social, political and economic structures in industrializing countries make these countries much more vulnerable to such accidents and create the type of setting where--if and when these accidents occur--they will have even more catastrophic consequences. The authors argue that only the transformation of local structures, and stronger technical cooperation between international organizations, industrialized and industrializing countries could reduce this vulnerability.
