Abstract

Aims The aim of this study was to determine the contemporary use of reperfusion therapy in the European Society of Cardiology (ESC) member and affiliated countries and adherence to ESC clinical practice guidelines in patients with ST-elevation myocardial infarction (STEMI). Methods and results Prospective cohort (EURObservational Research Programme STEMI Registry) of hospitalized STEMI patients with symptom onset Conclusions The use of reperfusion therapy for STEMI in the ESC member and affiliated countries was high. Primary PCI was the most frequently used treatment and associated total in-hospital mortality was below 5%. However, there was geographic variation in the use of primary PCI, which was associated with differences in in-hospital mortality.

Abstract

Two cultivars of Matthiola incana (L.) R. Br. ('Cheerful White' and 'Frolic Carmine') were grown in greenhouse sand cultures to determine the effect of salt stress on growth, ion relations, and flower quality. Two types of irrigation waters, differing in ion composition, were prepared to simulate saline wastewaters commonly present in two inland valley locations in California. Solution ICV was typical of saline tailwaters frequently found in the Imperial and Coachella Valleys and contained Cl-, Na+, SO42-, Mg2+, Ca2+, predominating in that order. Solution SJV was dominated by Na+ and SO42- and simulated saline drainage effluents often present in the San Joaquin Valley. Five treatments of each salinity type were imposed; each was replicated three times. Electrical conductivities of the irrigation waters (ECi) were 2.5, 5, 8, 11, and 14 dS⋅m-1. Plant heights were determined weekly. Seedlings were sampled for ion analysis 9 weeks after planting. Flowering stems were harvested when about 50% of the florets in the inflorescence were open. Total stem length, weight and diameter, numbers of florets and buds, and inflorescence length were measured at final harvest. All plants remained healthy throughout the experimental period with no visible signs of ion toxicity or deficiency. Although length of the flowering stems decreased with increasing salinity, stems were of marketable quality even at the highest salinity level. Mineral ion composition of the vegetative tissues generally reflected ion concentrations in the irrigation waters. Shoot Mg2+ and Cl- were higher and shoot Na+ lower in seedlings irrigated with ICV waters than with SJV waters. Shoot P was reduced over control levels once salinity exceeded 11 dS⋅m-1. Both cultivars were highly selective for K+ over Na+ and selectivity coefficients (SK,Na) increase about 60% as salinity increased from 2.5 to 14 dS⋅m-1. Abstract

To explore the possibility that saline wastewaters may be used to grow commercially acceptable floriculture crops, a study was initiated to determine the effects of salinity on two statice cultivars. Limonium perezii (Stapf) F. T. Hubb. 'Blue Seas' and L. sinuatum (L.) Mill 'American Beauty' were grown in greenhouse sand cultures irrigated with waters prepared to simulate saline drainage waters typically present in the western San Joaquin Valley (SJV) of California. Seven salinity treatments were imposed on 3-week-old seedlings. Electrical conductivities of the irrigation waters (EC) were 2.5 (control), 7, 11,15, 20, 25, and 30 dS·m-1 Vegetative shoots were sampled for biomass production and ion analysis ten weeks after application of stress. Flower stem numbers, length, and weight were determined at harvest. Stem length of L. perezii was significantly reduced when irrigation water salinity exceeded a threshold of 2.5 dS·m-1. Salt tolerance threshold based on stem length for L. sinuatum was 7 dS m-1. The species exhibited significant differences in shoot-ion relations which appear to be related to differences in salt tolerance. Sodium, K+, Mg2+, and total-P were more strongly accumulated in the leaves of L. sinuatum than L. perezii. Both species accumulated K+ in preference to Na+, but selectivity for K+ over Na+ was significantly higher in L. sinuatum than in the more salt-sensitive L. perezii. Chloride concentration in L. sinuatum leaves increased significantly as salinity increased, whereas the 20-fold increase in substrate-Cl had no effect on leaf-Cl in L. perezii. Both Limonium species completed their life cycles at salt concentrations exceeding 30 dS·m-1, a character associated with halophytic plants. Abstract

Elephant grass (Pennisetum purpureum Schum.) is a new fast-growing alternative forage crop. However, salinity is a major concern for its production in the arid south-western United States. This study was conducted in the arid Imperial Valley of Southern California to evaluate salt tolerance of elephant grass. Salinity treatments were created in field plots irrigated with water possessing an electrical conductivity (ECiw) of 1·5, 5, 10, 15, 20, and 25 dS m−1, respectively. Canopy spectral reflectance, temperature, plant height, leaf area index (LAI), chlorophyll-SPAD meter readings, and dry weights were measured over time. Results indicated that canopy reflectance in the near-infrared spectral region was reduced incrementally with increasing levels of salt stress. Canopy temperature increased with increasing salinity, especially at longer times after salinity treatment. Plant height and LAI were reduced with increasing salinity. Biomass accumulation was reduced incrementally with increasing salinity. About 50% yield reduction was found when ECiw increased from 5 to 25 dS m−1. The study shows that elephant grass is sensitive to salt stress, and relatively low salinity must be maintained to achieve a high rate of growth and biomass production. [Copyright &y& Elsevier]

We evaluated the cost-effectiveness of pressure-redistribution foam mattresses on ED stretchers and beds for early prevention of pressure ulcers in elderly admitted ED patients. METHODS: Using a Markov model, we evaluated the incremental effectiveness (quality-adjusted life-days) and incremental cost (hospital and home care costs) between early prevention and current practice (with standard hospital mattresses) from a health care payer perspective during a 1-year time horizon. RESULTS: The projected incidence of ED-acquired pressure ulcers was 1.90% with current practice and 1.48% with early prevention, corresponding to a number needed to treat of 238 patients. The average upgrading cost from standard to pressure-redistribution mattresses was $0.30 per patient. Compared with current practice, early prevention was more effective, with 0.0015 quality-adjusted life-days gained, and less costly, with a mean cost saving of $32 per patient. If decisionmakers are willing to pay $50,000 per quality-adjusted life-year gained, early prevention was cost-effective even for short ED stay (ie, 1 hour), low hospital-acquired pressure ulcer risk (1% prevalence), and high unit price of pressure-redistribution mattresses ($3,775). Taking input uncertainty into account, early prevention was 81% likely to be cost-effective. Expected value-of-information estimates supported additional randomized controlled trials of pressure-redistribution mattresses to eliminate the remaining decision uncertainty. CONCLUSION: The economic evidence supports early prevention with pressure-redistribution foam mattresses in the ED. Abstract

Background: COVID-19 and resulting health system and policy decisions led to significant changes in healthcare use by nursing homes (NH) residents. It is unclear whether healthcare outcomes were similarly affected among older adults in assisted living (AL). This study compared hospitalization events in AL and NHs during COVID-19 pandemic waves 1 through 4, relative to historical periods., Methods: This was a population-based, repeated cross-sectional study using linked clinical and health administrative databases (January 2018 to December 2021) for residents of all publicly subsidized AL and NH settings in Alberta, Canada. Setting-specific monthly cohorts were derived for pandemic (starting March 1, 2020) and comparable historical (2018/2019 combined) periods. Monthly rates (per 100 person-days) of all-cause hospitalization, hospitalization with delayed discharge, and hospitalization with death were plotted and rate ratios (RR) estimated for period (pandemic wave vs historical comparison), setting (AL vs NH) and period-setting interactions, using Poisson regression with generalized estimating equations, adjusting for resident and home characteristics., Results: On March 1, 2020, there were 9,485 AL and 14,319 NH residents, comparable in age (mean 81 years), sex (>60% female) and dementia prevalence (58-62%). All-cause hospitalization rates declined in both settings during waves 1 (AL: adjusted RR 0.60, 95%CI 0.51-0.71; NH: 0.74, 0.64-0.85) and 4 (AL: 0.76, 0.66-0.88; NH: 0.65, 0.56-0.75) but unlike NHs, AL rates were not significantly lower during wave 2 (and increased 27% vs NH, January 2021). Hospitalization with delayed discharge increased in NHs only (during and immediately after wave 1). 45. Identifying Factors Associated with Bullying Roles Using the interRAI Child and Youth Mental Health (ChYMH) Suite of Instruments.

Abstract

Bullying is a common problem amongst school-aged children and youth and is a significant concern for caregivers and teachers. interRAI is an international not-for-profit network of roughly 150 researchers and clinicians from over 35 countries. The main goal of interRAI is to develop and support standardized assessment systems for vulnerable individuals to support care planning, evidence-based clinical decision making, outcome measurement and quality assurance. This study aimed to examine factors associated with bullying roles in a large clinical sample (n = 26,069) using interRAI Child and Youth Mental Health assessments. Findings revealed children who both bullied peers and were victims of bullying (compared to those who were solely bullies, victims, or neither) were more likely to experience interpersonal traumas including witnessing domestic violence, physical and sexual assault; increased risk of self-harm and suicide, depression; more behavioural/externalizing problems; conflict within the school and home contexts; and higher levels of financial, familial, and living instability. The potential causes and implications of these distinctions are discussed. Abstract

Nursing homes were profoundly affected by the COVID-19 pandemic, influencing work outcomes of care aides who provide the most direct care. We compared care aides' quality of work life by conducting a repeated cross-sectional analysis of data collected in February 2020 and December 2021 from a stratified random sample of urban nursing homes in two Canadian provinces. We used two-level random-intercept repeated-measures regression models, adjusting for demographics and nursing home characteristics. 2348 and 1116 care aides completed the survey in February 2020 and December 2021, respectively. The 2021 sample had higher odds of reporting worked short-staffed daily to weekly in the previous month than the 2020 sample. The 2021 sample also had a small but significant drop in professional efficacy and mental health. Abstract

Background: Inadequate pain management persists in nursing homes. Nursing assistants provide the most direct care in nursing homes, and significantly improving the quality of care requires their adoption of best care practices informed by the best available evidence. We assessed the association between nursing assistants' use of best practices and residents' pain levels., Methods: We performed a cross-sectional analysis of data collected between September 2019 and February 2020 from a stratified random sample of 87 urban nursing homes in western Canada. We linked administrative data (the Resident Assessment Instrument-Minimum Data Set [RAI-MDS], 2.0) for 10,093 residents and survey data for 3547 nursing assistants (response rate: 74.2%) at the care unit level. Outcome of interest was residents' pain level, measured by the pain scale derived from RAI-MDS, 2.0. The exposure variable was nursing assistants' use of best practices, measured with validated self-report scales and aggregated to the unit level. Two-level random-intercept multinomial logistic regression accounted for the clustering effect of residents within care units. Covariates included resident demographics and clinical characteristics and characteristics of nursing assistants, unit, and nursing home., Results: Of the residents, 3305 (30.3%) were identified as having pain. On resident care units with higher levels of best practice use among nursing assistants, residents had 32% higher odds of having mild pain (odds ratio, 1.32; 95% confidence interval, 1.01-1.71; p = 0.040), compared with residents on care units with lower levels of best practice use among nursing assistants. The care units did not differ in reported moderate or severe pain among residents., Conclusions: We observed that higher unit-level best practice use among nursing assistants was associated with mild resident pain. Abstract

Background: In Canada, publicly-funded home care programs enable older adults to remain and be cared for in their home for as long as possible but they often differ in types of services offered, and the way services are delivered. This paper examines whether these differing approaches to care shape the pathway that home care clients will take. Older adult client pathways refer to trajectories within, and out of, the home care system (e.g., improvement, long term care (LTC) placement, death)., Methods: A retrospective analysis of home care assessment data (RAI-HC was linked with health administrative data, long-term care admissions and vital statistics in Nova Scotia Health (NSH) and Winnipeg Regional Health Authority (WRHA). The study cohort consists of clients age 60 + years, admitted to home care between January 1, 2011 to December 31, 2013 and up to four years from baseline. Differences in home care service use, client characteristics and their pathways were tested across the two jurisdictions overall, and among the four discharge streams within jurisdictions using t-tests and chi-square tests of significance., Results: NS and WHRA clients were similar in age, sex, and marital status. NS clients had higher levels of need (ADL, cognitive impairment, CHESS) at base line and were more likely discharged to LTC (43% compared to 38% in WRHA). Caregiver distress was a factor correlated with being discharged to LTC. While a third remained as home care clients after 4 years; more than half were no longer in the community - either discharged to LTC placement or death. Such discharges occurred on average at around two years, a relatively short time period., Conclusions: By following older clients over 4 years, we provide enhanced evidence of client pathways, the characteristics that influence these paths, as well as the length of time to the outcomes. This evidence is central to identification of clients at risk in the community and aids in planning for future home care servicing needs that will allow more older adults to remain living in the community., (© 2023. 50. The association of facility ownership with COVID-19 outbreaks in long-term care homes in British Columbia, Canada: a retrospective cohort study.

Abstract

Background: Long-term care (LTC) in Canada is delivered by a mix of government-, for-profit- and nonprofit-owned facilities that receive public funding to provide care, and were sites of major outbreaks during the early stages of the COVID-19 pandemic. We sought to assess whether facility ownership was associated with COVID-19 outbreaks among LTC facilities in British Columbia, Canada., Methods: We conducted a retrospective observational study in which we linked LTC facility data, collected annually by the Office of the Seniors Advocate BC, with public health data on outbreaks. A facility outbreak was recorded when 1 or more residents tested positive for SARS-CoV-2 between Mar. 1, 2020, and Jan. 31, 2021. We used the Cox proportional hazards method to calculate the adjusted hazard ratio (HR) of the association between risk of COVID-19 outbreak and facility ownership, controlling for community incidence of COVID-19 and other facility characteristics., Results: Overall, 94 outbreaks involved residents in 80 of 293 facilities. Compared with health authority-owned facilities, for-profit and nonprofit facilities had higher risks of COVID-19 outbreaks (adjusted HR 1.99, 95% confidence interval [CI] 1.12-3.52 and adjusted HR 1.84, 95% CI 1.00-3.36, respectively). The model adjusted for community incidence of infection (adjusted HR 1.12, 95% CI 1.07-1.17), total nursing hours per resident-day (adjusted HR 0.84, 95% CI 0.33-2.14), facility age (adjusted HR 1.01, 95% CI 1.00-1.02), number of facility beds (adjusted HR 1.20, 95% CI 1.12-1.30) and facilities with beds in shared rooms (adjusted HR 1.16, 95% CI 0.73-1.85)., Interpretation: Findings suggest that ownership of LTC facilities by health authorities in BC offered some protection against COVID-19 outbreaks.
