Abstract

The cytokine human interleukin for Da cells/leukemia inhibitory factor (HILDA/LIF) exerts multiple biological effects in vitro. In mice, high circulating levels of HILDA/LIF induce a wide range of pathophysiological events, some of them closely involved with immunological and inflammatory responses. Using a sandwich ELISA recognizing the natural human HILDA/LIF molecule with a threshold of 50 pg/ml in urine and 150 pg/ml in plasma, we monitored the urine and plasma HILDA/LIF levels of 22 patients in their first year after a kidney transplant. HILDA/LIF urine excretion is increased during acute rejection, and infections also trigger heavy HILDA/LIF plasma concentrations or urine excretion. In addition, this study raises the question of HILDA/LIF involvement in post-kidney-transplant phenomena such as hypercalcemia, osteoporosis, or the reversal of anemia.

Abstract

In eight patients remaining acidotic after more than 1 year of bicarbonate haemodialysis, we studied the effect of correcting the chronic metabolic acidosis using acetate-free biofiltration for 4 months on the course of secondary hyperparathyroidism. An AN69 capillary membrane was employed with a bicarbonate infusion rate initially set at 1.8 l/h in all patients and then adjusted in each one to obtain a predialysis bicarbonate of ≥23 mmol/l. Standard blood chemistry parameters were determined every 2 weeks. Measurements of PTH, calcifediol and calcitriol, as well as calcium-PTH curves were determined at the beginning and end of the study. While acetate-free biofiltration appears to be an adequate technique for the correction of chronic metabolic acidosis when bicarbonate dialysis fails, this study indicates that it does not influence secondary hyperparathyroidism in haemodialysed patients. The level of intact PTH did not vary significantly and the calcium-PTH curves at 0 and 4 months were superimposable with no significant differences in the set point and the slope of the curves. 39. Pharmacokinetics of atenolol in patients with terminal renal failure and influence of haemodialysis.

Abstract

1 The pharmacokinetics of atenolol, after 200 mg orally, were studied in 18 patients with terminal renal insufficiency (creatinine clearance less than 5 ml/min), of whom twelve were being treated by chronic dialysis. 2 The peak plasma level, 1.59 +/- 0.43 mg/l, was reached in 4.7 +/- 2.1 h. 3 Without dialysis treatment, the apparent plasma half- life of atenolol was greatly increased (73.4 +/- 28.8 /). During dialysis, it dropped to 7.5 +/- 3.7 h but returned to 51.2 +/- 17.3 h after dialysis. The plasma atenolol plot was a rising slope for a few hours after the end of dialysis. 4 Renal clearance of atenolol was very low (4.6 +/- 1.5 ml/min). 5 Plasma clearance during dialysis was 42.6 +/- 21.3 ml/min for a mean blood flow-rate of 236 +/- 25 ml/min through a cuprophane membrane dialyser. 6 These results suggest that dosage should be modified for these patients. Abstract

30 second renal transplants from cadaver donors were performed. 93% of patients survived for 1 year and 89% for 2 years. 66% of grafts survived for 2 year and 57% for 2 years. Best results can be expected with patients less than 40 years old whose first transplant failed through chronic rejection, whether or not cytotoxic antibodies were present. The best histocompatibility in the HLA A and B system should be sought.

Abstract

Combined kidney and hepatic transplantation which may treat the renal insufficiency, and correct the alanine glyoxalate aminotransferase deficiency responsible for the disease, seems to be the treatment of choice in type I primary hyperoxaluria. We report an original case of combined kidney and auxiliary partial orthotopic liver transplantation in this indication.

44. Sympathetic reflex algodystrophy following kidney transplantation

Abstract

Between January 1985 and December 1987, 5 out of 109 cadaver‐kidney transplant recipients developed an algodystrophy involving knees and ankles. Pain hindering movements occurred during the 3rd month following transplantation. Radiographic findings demonstrated epiphysical demineralization and bone‐scanning revealed increased uptake of a radioisotope in the affected joints. Calcium and parathormone levels were either normal or subnormal. Recovery was always achieved after calcitonin or griseofulvin therapy. Different factors possibly promote the occurrence of this pathology: preexistent renal osteodystrophy with secondary hyperparathyroïdism and pelvis surgery might have played underlying but not determinant roles with regard to the 500 patients who underwent kidney transplantation before January 1985 and who never experienced algodystrophy. The only difference between the two groups of patients was the introduction of cyclosporin‐A therapy after 1985. We suggest that this drug might contribute to the onset of algodystrophy by its vasoconstrictive action on the peripheral vessels, by its neurotoxicity and/or by an increased bone‐remodelling effect.

45. Effect of cyclosporin and the prostacydin analogue iloprost on human glomerular vasoreactivity.

Abstract

Cyclosporin is known to cause a decrease in renal blood flow and glomerular filtration rate in both humans and animals. These acute modifications are reversible if the drug is withdrawn, and seem to be caused by a local hormonal imbalance between vasoconstricting and vasodilating substances. We studied human glomeruli incubated with either CsA alone, iloprost alone, or CsA + iloprost. Photomicrographs of glomeruli were taken at t0, t1, t2 and t5 mm and glomerular areas were measured with a videoanalyser linked to a computer. Results show a significant vasoconstriction with CsA alone, and no significant modification of glomerular areas with iloprost or CsA + iloprost. We conclude that iloprost prevents CsA-induced vasoconstriction in human glomeruli in vitro. [ABSTRACT FROM PUBLISHER]
