Background/Aims: This study aimed to evaluate the real-life efficacy and tolerability of direct-acting antiviral treatments for patients with chronic hepatitis C (CHC) with/without cirrhosis in the Turkish population. Material and Methods: A total of 4,352 patients with CHC from 36 different institutions in Turkey were enrolled. They received ledipasvir (LDV) and sofosbuvir (SOF)+/- ribavirin (RBV) ombitasvir/paritaprevir/ritonavir +/- dasabuvir (PrOD)+/- RBV for 12 or 24 weeks. Sustained virologic response (SVR) rates, factors affecting SVR, safety profile, and hepatocellular cancer (HCC) occurrence were analyzed. Results: SVR12 was achieved in 92.8% of the patients (4,040/4,352) according to intention-to-treat and in 98.3% of the patients (4,040/4,108) according to per-protocol analysis. The SVR12 rates were similar between the treatment regimens (97.2%-100%) and genotypes (95.6%-100%). Patients achieving SVR showed a significant decrease in the mean serum alanine transaminase (ALT) levels (50.90 +/- 54.60 U/L to 17.00 +/- 14.50 U/L) and model for end-stage liver disease (MELD) scores (7.51 +/- 4.54 to 7.32 +/- 3.40) (p, Turkish Association for the Study of The Liver (TASL), The present study was supported by The Turkish Association for the Study of The Liver (TASL).

Anjiyodisplazi, gastrointestinal kanal mukoza ve submukozasında belirgin olarak dilate izlenen damarsal lezyonlardır. Yaşla birlikte görülme sıklığı artmakta ve sistemik hastalıklara eşlik edebilmektedir. Endoskopik olarak genellikle sağ kolonda, multiple, kırmızı, düz, 2-10 mm boyutlarında, oval yada yıldızvari-örümcek şeklinde izlenir. Tedavide skleroterapi, heater probe, argon lazer fotokoagülasyon, anjiyografik embolizasyon kullanılmakta olup yanıt vermeyen olgularda cerrahi tedavi uygulanabilir. Burada akut alt gastrointestinal sistem kanaması ile başvuran ve kolonoskopide anyjiyodisplazi saptanıp argon plazma koagülasyon uygulanan iki olgu sunulmaktadır., Angiodysplasia is known as vascular dilatation of submucosal veins. These lesions increase with age and may accompany systemic diseases. Colonoscopy shows angiodysplasia as red, usually multiple, less than 10 mm in size, spider- like lesions especially in the right colon. Endoscopic therapies such as sclerotherapy, heater probe and argon plasma coagulation can control acute bleeding from colonic angiodysplasias in most patients. Furthermore, selective embolization with angiography during bleeding episodes from angiodysplasia may control the bleeding. Herein, we report two patients with colonic angiodysplasia that presented with acute bleeding; argon plasma coagulation was applied in both patients.

Benign and malignant tumors of the small bowel are rarely seen and account for a small part of all gastrointestinal system tumors. Most of the small bowel tumors are malignant and adenocarcinomas, and are usually detected in the duodenum. Similarly, benign tumors of the small bowel are rare and are seen in the duodenum. The risk of malignant transformation is high, especially in villous adenomas. We report a patient with tubulovillous adenoma in the duodenum that was detected as a result of the investigation of anemia., İnce barsak, gastrointestinal sistem tümörlerinin nadir görülen yerleşim bölgesidir. İnce barsağın en sık malignitesi adenokarsinomlardır ve sıklıkla duodenumda görülmektedir. Benign ince barsak tümörleri de oldukça seyrek olup çoğu duodenum yerleşimlidir. Adenomalar daha çok duodenum 2. kısımda yerleşimli olarak saptanırlar. Villöz özellik gösteren adenomlarda malign transformasyon olasılığı daha yüksektir. Burada anemi etyolojısınin tetkikinde saptanmış olan periampuller yerleşimli dev tubulovilloz adenom olgusu sunulmaktadır.

Background/aim: Diffuse antral gastritis (DAG) is a duodenal ulcer phenotype of chronic Helicobacter pylori gastritis, while multifocal atrophic gastritis (MAG) is a gastric cancer phenotype. The aim of this study was to investigate ascorbic acid (vitamin C) levels in the gastric juice in patients with DAG and MAG, to compare the gastric epithelial cell proliferation rates between the groups and also to examine any correlation between ascorbic acid levels in gastric juice and the cell proliferation rates. Materials and methods: Thirty-seven patients with chronic H. pylori gastritis (27 MAG, 10 DAG) were included in the study (male/female: 15/22, range: 19-74, mean age: 43.6 yrs). Ascorbic acid concentrations in fasting plasma and gastric juice samples of the patients were measured. Loss of maturation in foveolar epithelium due to cell proliferation was shown immunohistochemically using cytokeratin-20 antibody in antral and corporal regions of both patient groups. Results: There was no significant difference in plasma ascorbic acid levels between the two groups. No statistically significant difference could be demonstrated in gastric juice ascorbic acid concentrations between patients with DAG and MAG (0.93±0.97 mg/dl and 0.96±1.12 mg/dl, respectively; p>0.05). Immunohistochemical examination revealed that staining for cytokeratin- 20 was significantly less in MAG patients than in those with DAG (74.7% vs 90.9%, respectively; p, Giriş ve amaç: Diffüz antral gastrit (DAG), kronik Helicobacter pylori gastritinin duodenal ülser fenotipini, multifokal atrofik gastrit (MAG) ise gastrik kanser fenotipini oluşturmaktadır. Bu çalışmada DAG ve MAG’lı olgularda gastrik sıvı askorbik asit düzeylerinin incelenmesi, grupların mide epitel hücre proliferasyon oranları açısından karşılaştırılması ve mide sıvısı askorbik asit (C vitamini) düzeyleriyle hücre proliferasyon oranları arasındaki olası korelasyonların araştırılması amaçlanmıştır. Gereç ve yöntem: H. pylori’ye bağlı 27 MAG, 10 DAG saptanan 37 hasta çalışmaya alındı (22 kadın, 15 erkek, 19-74, yaş ort: 43,6). Hastaların plazma ve gastrik sıvı örneklerinde askorbik asit düzeyleri ölçüldü. Olguların antrum ve korpuslarında hücre proliferasyonundan dolayı foveolar epiteldeki maturasyon kaybının varlığı, sitokeratin-20 (CK-20) antikoru kullanılarak immunohistokimyasal yöntemle gösterildi. Bulgular: Her iki grubun plazma askorbik asit düzeyleri arasında anlamlı farklılık yoktu. Gruplar, mide sıvısı askorbik asit düzeyleri açısından karşılaştırıldığında istatistiksel anlamlı bir farklılık gözlenmedi (DAG ve MAG için sırasıyla; 0.93±0.97, 0.96±1.12mg/dl, p > 0.05) MAG grubunda CK-20 için boyanma yüzdesi DAG grubundakilere göre anlamlı derecede düşüktü (%74.7 ve %90.9, p < 0.01). Boyanma yüzdeleriyle mide sıvısı askorbik asit değerleri arasında bir korelasyon saptanmadı. Sonuç: Bu çalışmada diffüz antral gastrit ve multifokal atrofik gastritli hastalar arasında mide sıvısı askorbik asit düzeyleri açısından anlamlı farklılık bulunmamıştır. Ancak, multifokal atrofik gastritin intestinal tip gastrik karsinogenez sürecinin proksimal aşamalarında yer aldığını destekleyecek şekilde, bu gastrit formunda epitel hücre proliferasyonunun artmış olduğu gösterilmiştir.

Bezoars are masses that arise in the gastrointestinal system from indigestible material such as food, hair, carpet fiber, or clothing. They are usually seen in patients with abnormal gastrointestinal system anatomy due to surgery, impairment of gastric emptying or mental retardation. Endoscopic therapy with fragmentation or enzymatic dissolution has been reported with varying degrees of success. Surgery is indicated when conservative therapy with chemical dissolution or endoscopic therapy is unsuccessful. Herein, we report two patients with gastric bezoar who were treated with surgery., Bezoarlar gastrointestinal sistemde bulunan, hazmedilememiş gıda, lif veya saç örnekleri nedeniyle anatomi oluşan kitlelerdir. Özellıkle mental retarde veya gastrointestinal sistem anotomisinde bozukluk gelişen hastalarda daha sık görülür. Enzimatik eritme veya endoskopik parçalama gibi konservatif işlemlerin yanı sıra, başarısız olunan olgularda gecikmeksizin cerrahi tedavi uygulanmalıdır. Burada midesinde bezoar saptanmış ve cerrahi tedavi uygulanan iki olgu sunulmaktadır.
