Abstract

The beginning of the narrative "Payalpur's Recital 3", a text similar to the "Dowry recital", particularly relevant to women and women's afflictions. The shaman recites each line, which is then repeated by his disciples. The full transcription is published in NSOTII 513-528. The present transcription, covering lines 1-60 (of 404), differs very slightly from the published version (NSOTII 513-515), mainly in that a few extra repetitions and asides omitted from the published version have been restored. As in the published version, the regular repetitions by the disciples are not transcribed, although they occasionally differ in minor details from the guru's recital. Leading syllables, often a simple "e" or "ya he" are not transcribed.

Abstract

The beginning of the narrative myth "The Nine Good Skillful Ones", recited by the shaman Haribān Kāmī of Bobang (d. 2003 aged 57) at the request of Gregory Maskarinec. The text relates the origin of the first shaman and the first witches. The text presented here corresponds to lines 1-166 (pp. 33-39) of the full recitation (1139 lines) published in NSOTII:33-75. It differs slightly in line numbering and in content from the published source in that (1) some lines have been re-divided to correspond to breath groups in the recording and (2) some temporizing lines, omitted from the published version, have been restored. Before the recitation presented here, the shaman made a series of false starts consisting of such temporizing lines; these are transcribed NSOTII:634n154.

Each line is recited by the shaman Ginde Thāpā and repeated by his disciples, the shamans Tam Bahādur Adaī and Dal Bahādur Adai. The recital is accompanied by regular drumming. The full transcription and translation is published in NSOTII:513-528. This digitalization ends with Ginde's recital of line 175 (p. 520) of the published text; the remainder is on a second minicassette Bhuji2000#4A. The digitalization is an extract from the digitalization of the cassette Bhuji2000#9A, from 11m17s to 25m06s, where the recording appears to have been accidentally interrupted. Some of the recording (corresponding to lines 167-270 of the published text) is lost before it resumes on cassettes #9B and #10A. A minidisk recording, made at the same time, is complete but is marred by parasite clicks. The present track begins with some false starts, transcribed in NSOTII:634n154, followed by the beginning of the myth (NSOTII:33-39, lines 1-166). 23. Propofol, Triglycerides, and Acute Pancreatitis: A Multi-Center Epidemiologic Analysis.

Abstract

Rationale: Propofol is one of the first-line sedative-hypnotic agents for critically ill adults requiring mechanical ventilation. While propofol can elevate triglyceride levels, and the latter is a risk factor for pancreatitis, the association between propofol and acute pancreatitis is unclear., Objectives: To determine the clinical impact and potential associations between propofol infusion, hypertriglyceridemia and acute pancreatitis., Methods: This is an observational multicenter study of adults (≥18 years old) admitted to an intensive care unit. requiring mechanical ventilation and receiving continuous propofol infusion for at least 24 hours. The primary outcomes were the frequency of hypertriglyceridemia and acute pancreatitis. Further analyses were done to determine the clinical impact of elevated triglyceride levels (i.e. sedation changes) and risk factors for pancreatitis development., Measurements and Main Results: Of 11,828 patients included, 33.2% (N=3922) had triglyceride levels measured, of whom 21.7% (N=851) had hypertriglyceridemia at 4.5 (SD 6.8) days after propofol initiation. Of those still requiring sedation, 70.4% (N=576/818) received alternative sedatives following hypertriglyceridemia. Pancreatitis occurred in 1.2% (N=47/3922) and was more frequent in those with hypertriglyceridemia (3.2%, 27/851 versus 0.7%, 20/3071; P<0.001). Following adjustment for potential confounding variables, each 100 mg/dL increase in triglyceride levels was associated with an 11% increase in risk of pancreatitis. Propofol dose was not associated with pancreatitis development., Conclusions: Acute pancreatitis is uncommon in patients receiving propofol infusion, and occurs over a wide range of triglyceride levels, indicating a multifactorial pathophysiology. Hypertriglyceridemia frequently prompts the use of alternative sedatives. Further study is needed to determine how to best monitor and treat hypertriglyceridemia in critically ill patients receiving propofol infusion.
