Implant dental therapy is a clinical procedure used for treating patients with tooth loss with known clinical success. This clinical study aimed to evaluate the long-term clinical outcomes of dental implants in partially and totally edentulous patients. A total of 544 Microdent (Microdent SU, Implant Microdent System®, Santa Eulàlia de Ronçana Barcelona, Spain) screw implants were placed in 111 patients using a two-stage surgical technique and a conventional loading protocol (lasting 3 months). Implant and prosthetic clinical findings were evaluated during a 15-year follow-up. A total of 6 implants were lost during the healing period, and 124 prostheses were placed over the 538 implants that remained: 20 single crowns, 52 partially fixed bridges, 45 full-arch fixed restorations, and 7 overdentures. A total of 20 of these were lost during the follow-up period. The cumulative survival rate for all implants was 96.4%. The data underwent statistical analysis (significance level: p < 0.05). The mean marginal bone loss was 1.82 ± 0.54 mm, ranging from 1.2 to 3.1 mm. The most frequent complications were mechanical prosthodontic complications (16.2%). In all, 11.8% of implants showed periimplantitis as the primary biological complication. Dental implants inserted in both the maxillary and mandibular areas produce long-term favorable outcomes and stable tissue conditions when a delayed loading protocol is followed.

Mandibular angle augmentation is a procedure that, despite its obvious aesthetic and reconstructive indications, is not routinely performed, even by experienced surgeons. We present a case series of 10 patients treated with custom-made mandibular or inferior border prostheses for aesthetic concerns and sequelae following trauma or orthognathic surgery. Customized implants were designed with CAD/CAM technology, milled from Polyetheretherketone (PEEK) and 3D-printed implant placement guides were used. Ten patients (6 men and 4 women) were included; median age was 36.5 years (28-45 years). Indications for surgery were aesthetics (5 cases) and postoperative sequelae (4 orthognathic surgery and 1 mentoplasty). No significant early complications occurred, except edema and small bruises. Three patients developed wound dehiscence and prosthesis exposure, which closed spontaneously after 2-3 weeks, except one case that required a slight reduction of the upper edge of the prosthesis. No prostheses required removal. After the initial healing period there were no exposures or any late-onset infections or prosthesis intolerance. The use of customized PEEK prostheses together with tooth-supported guides provides treatment that is safe, easier surgically and more reliable. More experience is needed to define bone beauty standards to achieve the desired aesthetic results

Many of these situations can be surgically treated with the patient's own autologous tissues. However, in some patients reconstruction is complex and the use of autologous tissues is unadvisable whereas reconstruction utilizing alloplastic materials may be an appropriate alternative. The following report describes 4 clinical cases in which autologous grafts or Christensen joint prosthesis are employed in temporomandibular joint reconstruction

La apnea del sueño es probablemente el trastorno del sueño más frecuente, y la obstructiva es la más común. El síndrome de apnea obstructiva del sueño (SAOS) se caracteriza por presentar episodios de obstrucción parcial o total de las vías aéreas superiores que provocan una ausencia o disminución del flujo de aire a los pulmones. Estos episodios duran de 10 a 30 segundos ocasionando ronquidos, descenso de la aturación de oxígeno, somnolencia diurna y fatiga crónica. Este complejo síndrome se asocia con enfermedades sistémicas como la hipertensión arterial y pulmonar, insuficiencia cardíaca, arritmias cardíacas nocturnas, infarto de miocardio y con accidentes de tráfico. Los pacientes con SAOS suelen tratarse con métodos conductuales, incluidos la pérdida de peso, el tratamiento postural y el abandono del hábito tabáquico, aunque habitualmente es necesario efectuar un tratamiento adicional en los pacientes con más si.ntomatología. La presión positiva continua en la vía aérea e el tratamiento básico y, a pesar de ser muy efectivo, el cumpli.n1iento por parte del paciente es bajo. Otras opciones de tratamiento son las férulas de avance mandibular y la cirugía de la lengua, los maxilares y las partes blandas. El propósito de este artículo es proporcionar la información necesaria para diagnosticar y tratar a los pacientes que presentan esta enfermedad, cada vez más frecuente en la población general, desde el punto de vista odontológico y multidiscipJinario.

Los trastornos respiratorios del sueño, como el ronquido o el síndrome de apneas hipoapneas obstructivas del sueño, se pueden tratar eficazmente con una férula de avance mandibular, pero puede provocar algunos efectos adversos. El odontoestomatólogo debe conocerlos y explicárselos al paciente para obtener el consentimiento informado. Se ha realizado una revisión bibliográfica de los posibles efectos adversos a nivel del sistema masticatorio del uso de las férulas de avance mandibular en el tratamiento de los trastornos respiratorios del sueño. Prácticamente la mitad de los pacientes refieren algún efecto adverso, pero en general de carácter leve y temporal. Los efectos adversos más frecuentes son dolor o sensibilidad dental (27%), xerostomía (24%), excesiva salivación (190/4), dolor o disconfort muscular o mandibular (18%), cambios oclusales (18%), dolor articular (9%), irritación gingival y ruidos articulares. El porcentaje de abandono suele estar entre el 5% y el 24%, siendo las principales causas el dolor articular, los cambios oclusales y la poca efectividad

Presentamos un caso clínico que sirve de base para la discusión de los conceptos protésicos que se relacionan con la restauración y rehabilitación de pacientes que han sufrido amplias resecciones maxilares y cuya extensión va más allá de los límites bucales. Se expone la realización de una prótesis labial, obturadora maxilar y dental que realizadas consecutivamente dan lugar a la recuperación (limitada) de funciones del sistema masticatorio.

The purpose of this study is to investigate the orthodontic and orthopaedic real effects of the Klammt's Elastic Open Activator (EOA) in 25 Class II Division 1 patients in growing period. We wanted to determine statistically the cephalometrics changes produced in the patients, comparing the lateral cranium teleradiographies we took for the diagnosis with the ones we took at the end of treatment. At the end of this study we confirm that by using the EOA we obtained the desired effects, especially reducing the molar relation 2.53 mm and the overjet 2.56 mm. The EOA corrected the inclination and protrusion of incisors, although we cannot avoid the use of fixed appliances to round off. The reduction of 2.48 mm of facial convexity stands out as the most important skeletal effect; the facial depth angle increases 0.8 degree, and the maxillary depth decreases 1.16 degrees. The length of the mandibular corpus also increases 6.7 mm, although this change is mainly due to the growth of the patient. The changes in the aesthetic profile do not stand out.

La descompresión de los grandes quistes maxilares mediante el drenaje y la irrigación diaria de su interior favorece la disminución de la presión osmótica en el interior del mismo y una limpieza antiséptica permitiendo una regeneración ósea periférica y una enucleación posterior más sencilla. Para lograr esta descompresión se puede construir un tubo rígido o semirrígido que puede ser retenido en su lugar mediante una prótesis y damos una serie de instrucciones al paciente para que irrigue diariamente el interior del quiste. Tiene la desventaja de tener una fase protésica para la construcción del tubo de descompresión, su posicionamiento y la retención del mismo. La ventaja principal de la descompresión es que evitaremos las complicaciones de un procedimiento quirúrgico extenso asociado a una enucleación completa y a las secuelas quirúrgicas que la acompañan como comunicaciones buconasosinuales, lesiones vásculonerviosas, fractura de los maxilares, etc. Es muy importante tener un diagnóstico clínico e histopatológico preliminar de la lesión para asegurar el éxito de la descompresión.
