Maxillonasal dysplasia, known as Binder's syndrome, is a rare congenital deformity characterised by distinctive facial features, including a malformed midface and nose, as well as abnormal positions of the nasal bones. In addition, maxillary hypoplasia, or a shorter upper jaw, and nasal abnormalities may manifest as a flattened nose. Many cases of this condition are associated with other malocclusions, and those affected can be easily identified. Individuals with Binder's syndrome typically present with an undeveloped upper jaw, a projecting lower jaw, a smaller nose, a flat nasal bridge, and midfacial hypoplasia. This is the first case report describing a physiotherapy method for maxillonasal dysplasia. Hereby, the authors present a case report of a six-month-old male child with Binder's syndrome who exhibited delayed developmental milestones and physical anomalies. The child's mother had a complicated pregnancy, culminating in an emergency caesarean section due to preterm premature rupture of membranes. The infant, born with Binder's syndrome and a cleft palate, displayed poor head control, an inability to roll, and reluctance to engage in sensory interactions. Examination revealed bilateral cortical thumbs, hip joint abnormalities, and heightened sensitivity to touch. Magnetic Resonance Imaging (MRI) findings indicated semilobar Holoprosencephaly (HPE) and dysgenesis of the corpus callosum. Physiotherapeutic interventions focused on parent education and home exercise programs targeting developmental milestones. Over the course of two months, the infant showed significant progress in head control and rolling. Furthermore, after four months of training, the child gained sitting control. Hydrocephalus Ex-vacuo refers to an elevated volume with no increasing Cerebrospinal Fluid (CSF) pressure, particularly in instances of diminished brain tissue. Since there is no increased intracranial pressure in Hydrocephalus Ex-vacuo, few general symptoms such as irritability, drowsiness, and even larger head presentation can be absent compared to congenital hydrocephalus. The current case study presents a one-year-old male child exhibiting delays in developmental milestones, including poor head and trunk control, associated with a history of prenatal bleeding and subsequent birth complications. Despite a seemingly normal birth, the child experienced postnatal complications such as vomiting, seizures, and abdominal distension, leading to a 15-day Neonatal Intensive Care Unit (NICU) stay. Persistent delays prompted medical consultations, culminating in a referral for physiotherapy following Magnetic Resonance Imaging (MRI) findings indicative of hydrocephalus ex-vacuo. Clinical assessments revealed a restricted range of motion, muscle tightness in the lower extremities, and poor muscle control and reflex persistence. Imaging confirmed ventricular enlargement consistent with Hydrocephalus Ex-vacuo, necessitating targeted physiotherapy interventions. Therapeutic strategies focused on promoting head and trunk control through prone positioning, bolster support, and dynamic exercises utilising physio balls. Progressive interventions facilitated significant improvements within six months, highlighting the efficacy of early rehabilitation and parental involvement in achieving motor milestones. The present case underscores the importance of timely diagnosis, multidisciplinary collaboration, and rehabilitative interventions in optimising outcomes for children with hydrocephalus ex-vacuo. Background: To test the functional implications of impaired white matter (WM) connectivity among patients with schizophrenia and their relatives, we examined the heritability of fractional anisotropy (FA) measured on diffusion tensor imaging data acquired in Pittsburgh and Philadelphia, and its association with cognitive performance in a unique sample of 175 multigenerational non-psychotic relatives of 23 multiplex schizophrenia families and 240 unrelated controls (total = 438). Methods: We examined polygenic inheritance (h2r) of FA in 24 WM tracts bilaterally, and also pleiotropy to test whether heritability of FA in multiple WM tracts is secondary to genetic correlation among tracts using the Sequential Oligogenic Linkage Analysis Routines. Partial correlation tests examined the correlation of FA with performance on eight cognitive domains on the Penn Computerized Neurocognitive Battery, controlling for age, sex, site and mother's education, followed by multiple comparison corrections. Results: Significant total additive genetic heritability of FA was observed in all three-categories of WM tracts (association, commissural and projection fibers), in total 33/48 tracts. There were significant genetic correlations in 40% of tracts. Diagnostic group main effects were observed only in tracts with significantly heritable FA. Correlation of FA with neurocognitive impairments was observed mainly in heritable tracts. Conclusions: Our data show significant heritability of all three-types of tracts among relatives of schizophrenia. Significant heritability of FA of multiple tracts was not entirely due to genetic correlations among the tracts. Diagnostic group main effect and correlation with neurocognitive performance were mainly restricted to tracts with heritable FA suggesting shared genetic effects on these traits. The impact of slip on micropolar fluid through inclined tapering stenosed artery having permeable walls is studied. To compute the phenomena of Nanoparticle and Temperature profiles, Homotopy Perturbation Method (HPM) is considered. The analysis with respect to different flow parameters on flow impedance (λ) and shear stress (τh) are anticipated by deriving equations for the flow characteristics and solutions are obtained. Introduction After infection, herpes simplex virus-1 (HSV1) becomes latent in the trigeminal ganglion and can enter the brain via retrograde axonal transport. Recurrent reactivation of HSV1 may lead to neurodegeneration and Alzheimer’s disease (AD) pathology. HSV1 (oral herpes) and HSV2 (genital herpes) can trigger amyloid beta-protein (Aβ) aggregation and HSV1 DNA is common in amyloid plaques. Anti-HSV drugs reduce Aβ and phosphorylated tau accumulation in cell-culture models. Cognitive impairment is greater in patients with HSV seropositive, and antiviral drugs show robust efficacy against peripheral HSV infection. Recent studies of electronic health records databases demonstrate that HSV infections increase dementia risk, and that antiviral medication treatment reduces this risk. The generic antiviral drug valacyclovir was superior to placebo in improving memory in a schizophrenia pilot trial but has not been tested in AD. Methods and analysis In patients with mild AD who test positive for HSV1 or HSV2 serum antibodies, valacyclovir, repurposed as an anti-AD drug, will be compared with placebo (lactose pills) in 130 patients (65 valacyclovir and 65 placebo) in a randomised, double-blind, 78-week phase II proof-of- concept trial. Patients on valacyclovir, dose-titrated from 2 g to a targeted oral dose of 4 g daily, compared with placebo, are hypothesised to show smaller cognitive and functional decline, and, using 18F-Florbetapir positron emission tomography (PET) and 18F-MK- 6240 PET imaging, to show less amyloid and tau accumulation, respectively. In the lumbar puncture subsample, cerebrospinal fluid acyclovir will be assayed to assess central nervous system valacyclovir penetration.
