Simple Summary: As in humans, the treatment of skin wounds is a common clinical practice in veterinary medicine, especially in horses. This subject still requires great attention from researchers due to animal welfare concerns and economic losses resulting from the prolonged treatment of chronic wounds. This study has reviewed the scientific literature to identify which types of therapy have been described for equine wound healing. After selecting the publications using pre-established eligibility criteria, 81 manuscripts were included for data extraction on the characteristics of the articles, treatments, evaluation types, and results. Although the literature on equine wound management is very vast, there was observed a lack of evidence for the adoption of a treatment protocol, and many treatments with controversial results. Equine skin wound treatment continues to be a challenge for veterinarians. Despite being a frequent practice, it remains difficult to choose an evidence-based treatment protocol. This study aimed to comprehensively explore the literature and provide a scoping review of therapeutic strategies for equine skin wounds and identify knowledge gaps and opportunities for future research. This review was conducted using specific criteria to select literature that described methods to manage second intention wound healing. After removing duplicates and screening papers for suitability, 81 manuscripts were included for data extraction. Of these, 59 articles were experimental studies, 10 were case reports, 9 were case series, and 3 were clinical studies. The most frequent wound location was the distal limbs. Macroscopic assessment was the main tool used to evaluate treatment effectiveness. All of the case reports, case series, and clinical studies reported positive outcomes with regard to the treatment used, while only 36% of the experimental studies found significant healing improvement in treated wounds compared to control groups. It was found that there are many treatments that have exhibited controversial results, and there exists a lack of evidence for the adoption of specific treatment protocols. [ABSTRACT FROM AUTHOR]

6. Multimorbidity daily life activities and socio-economic classification in the Central Portugal primary health care setting: an observational study

Background. Multimorbidity (MM) is associated with decreased quality of life, mainly due to decreased functional capacity and increased use of health care. Objectives. Evaluate the prevalence of MM in older people of Central Portugal and understand the impact of MM on daily life activities (DLA) and how socioeconomic level influenced prevalence of MM. Material and methods. Observational study of data on age, sex, number of ICPC2 codes and Barthel (Bt) and Graffar (Gr) Indexes in clinical records of all aged between 65 to 99 years enrolled in the Health Centers of the Central Region of Portugal. The Bt was used to assess the dependence on DLA and the Gr the socioeconomic level. Results. Population of 190025, mean age of 80.16± 8.03 years. MM prevalence of 80% out of which, 52.9% were female and the majority was aged between 76-85 years (39.1%). Average number of health problems of 8.7 problems for males and 9.5 for females. The Bt and Gr Indexes filling for the studied sample were of, Barthel 7.4% and Graffar 4,9%. Older people without MM have higher dependency rates than those with MM (total dependence 7.4% vs 6.2% and severe dependence 20.1% vs 9.6%). MM in older people are mainly in the middle (55.3% vs 27.5%) and low (31.8% vs 19.6%) Graffar class, while the older people without MM are mainly at middle to upper (35.3%) and upper (17.6%) Graffar classes. Conclusions. Multimorbidity is mainly associated with lower social classes. Dependence for DLA appears unrelated to MM. Multimorbidity persons need special attention, based on socio-economic contexts.

8. Determining factors associated with inhaled therapy adherence on asthma and COPD: A systematic review and meta-analysis of the global literature

Background Adherence to therapy has been reported worldwide as a major problem, and that is particularly relevant on inhaled therapy for Asthma and Chronic Obstructive Pulmonary Disease (COPD), considering its barriers and features. We reviewed the global literature reporting the main determinants for adherence on these patients. Methods Searches were made using the Cochrane Library, MEDLINE, EMBASE and ISI Web of Science databases. Analytical, observational and epidemiological studies (cohort, case-control and cross-sectional studies) were included, reporting association between any type of determinant and the adherence for inhaler therapy on Asthma or COPD. Random-effects meta-analysis were used to summarise the numerical effect estimates. Results 47 studies were included, including a total of 54.765 participants. In meta-analyses, the significant determinants of adherence to inhaled therapy were: older age [RR = 1.07 (1.03–1.10); I2 = 94; p < 0.0001] good disease knowledge/literacy [RR = 1.37 (1.28–1.47); I2 = 14; p = 0.33]; obesity [RR = 1.30 (1.12–1.50); I2 = 0; p = 0.37]; good cognitive performance [RR = 1.28 (1.17–1.40); I2 = 0; p = 0.62]; higher income [RR = 1.63 (1.05–2.56); I2 = 0; p = 0.52]; being employed [RR = 0.87 (0.83–0.90); I2 = 0; p = 0.76] and using multiple drugs/inhalers [RR = 0.81 (0.79–0.84); I2 = 0; p = 0.80]. Overall, the strength of the underlying evidence was only low to moderate. Conclusions Many determinants may be associated to patient's adherence, and personalised interventions should be taken in clinical practice to address it by gaining an understanding of their individual features.

Objetivos: Descrever o perfil sociodemográfico/clínico e avaliar a qualidade de vida das pessoas diabéticas tipo 2 com 75 ou mais anos, seguidas numa Unidade de Saúde Familiar, e analisar as variáveis que com ela interferem. Metodologia: Estudo transversal, descritivo, com componente analítica. Os dados foram recolhidos no período compreendido entre dezembro de 2009 e março de 2010, utilizando-se as versões portuguesas do EASYcare e da Escala de Satisfação Com a Vida, e um questionário complementar. Resultados: A amostra incluiu 83 pessoas idosas diabéticas. A maioria são mulheres, com uma média etária de 80,9 anos (DP=4,3), viúvas ou casadas, com escolaridade baixa, que vivem acompanhadas em zona urbana, e com rendimentos baixos/médios. Clinicamente, a amostra apresenta bom controlo glicémico, cumpre medicação antidiabética oral, tem em média 11,2 anos de doença (DP=10,1), apresenta complicações da diabetes, excesso de peso, tensão arterial e ficha lipídica não controladas. De uma forma geral, os participantes apresentaram uma avaliação positiva para a qualidade de vida. Contudo, percecionam de pior forma as áreas referentes à vida em comunidade, saúde mental e bem-estar, e funcionamento mnésico. Das variáveis sociodemográficas é o género e a capacidade financeira que mais interferem com os domínios da qualidade de vida. As variáveis clínicas estudadas interferem muito modestamente com a qualidade de vida. A satisfação com a vida é influenciada negativamente pela baixa capacidade financeira, pela presença de complicações da diabetes, e pelo agravamento da qualidade de vida. Conclusões: No idoso diabético a qualidade de vida é um constructo multidimensional envolvendo outros fatores para além da saúde.

A ossificação das cartilagens colaterais da falange distal é um achado muito frequente em exames radiográficos nos equinos. Entre os objectivos deste estudo estão a determinação da prevalência deste processo numa população constituída por 100 exemplares da raça Puro Sangue Lusitano, com idades compreendidas entre os 4 e 9 anos de idade, recorrendo, para o efeito, à avaliação radiográfica dos membros anteriores. Proceder-se-á, ainda, à caracterização da mesma população quanto à extensão, simetria, e causas predisponentes ao processo de ossificação e, de igual modo, à determinação da influência da extensão e simetria do mesmo na ocorrência e gravidade de claudicação. Foi realizado exame clínico direccionado para a pesquisa e diagnóstico de claudicação seguido por projecção radiográfica dorsopalmar da porção distal dos membros anteriores, e posterior análise e classificação do grau de ossificação, quando presente. A incidência de ossificação das cartilagens colaterais da falange distal na referida população é de 93%, com 41% dos equinos a apresentarem algum grau de claudicação. A população estudada demonstra tendência para uma simetria mediolateral e entre membros anteriores esquerdo e direito, mas com a cartilagem medial a apresentar por norma menor grau de ossificação. Não se verificou influência do género ou idade na ocorrência do processo, o mesmo não se tendo verificado relativamente à influência do grau de ossificação na ocorrência de claudicação, onde foi encontrada uma correlação positiva., Ossification of the collateral cartilages of the distal phalanx is a very common radiographic finding in horses. As goals for the present study we sought to define the processes' prevalence in a population of 100 Lusitano individuals, with ages from 4 to 9 years old. For that a radiographic evaluation of the forelimbs was performed to characterize the same population on the extension, symmetry, and predisposing factors of the ossification process, and to determine the influence of ossification’s extension and symmetry on the presence and severity of lameness. A clinical exam was performed to search and diagnose lameness, followed by a dorsopalmar radiographic projection of the distal forelimbs, and posterior analysis and grading of the ossification when present. The incidence of ossification of the distal cartilages of the distal phalanx in this population is 93%, with 41% presenting some level of lameness. The population showed a tendency for mediolateral and forelimb symmetry, but usually the medial cartilage presented less ossification. Cólica é um síndrome de elevada importância na medicina e cirurgia de equinos. Cólicas que são referenciadas para correcção cirúrgica apresentam um impacto maior na taxa de sobrevivência. O presente trabalho pretende determinar o valor da taxa de sobrevivência, bem como, descrever as possíveis complicações e recorrências a longo prazo de uma população de 26 equinos que foram submetidos a cirurgia de cólica. Para a elaboração deste estudo foram revistos todos os casos clínicos de equinos submetidos a cirurgia de cólica, no período decorrente entre 1 de Janeiro de 2008 e 31 de Dezembro de 2010, na Clínica Medicina e Cirurgia de Equinos, LDA e foram elaborados questionários que foram endereçados via Web para o correio electrónico dos proprietários. Neste trabalho estão contemplados todos os cavalos com dor abdominal aguda, aos quais foram identificados parâmetros indicativos de iniciação de um protocolo de cirurgia, e excluídos, todos os que foram eutanasiados após o diagnóstico de cólica e os que foram vendidos até ao momento do questionário. A taxa de sobrevivência neste estudo foi de 86,7%, valor que se enquadra na bibliografia revista. Após a cirurgia de cólica 80% dos cavalos voltaram a praticar a actividade física. Todas as recidivas de cólica foram tratadas cirurgicamente não se verificando a influencia negativa da segunda laparotomia exploratória sobre a taxa de sobrevivência na nossa amostra. Dissertação de Mestrado Integrado em Medicina Veterinária A Osteoartrite (OA) é a principal causa de claudicação no cavalo de desporto e lazer, sendo uma afecção que tem grandes repercussões económicas. Este trabalho descreve algumas das características importantes da estrutura articular, bem como da sua fisiologia. Define a OA e todas as estruturas envolvidas no seu processo. Os mecanismos fisiopatológicos põem em evidência os factores de risco em causa e que determinam todo o processo de degradação articular. São descritos os vários meios de diagnóstico utilizados, quer os convencionais, quer as recentes técnicas imagiológicas (ressonância magnética, tomografia computadorizada e cintigrafia nuclear) quer ainda os biomarcadores do metabolismo da cartilagem articular e osso. A abordagem terapêutica é diversa incluindo os anti-inflamatórios não esteróides, os corticosteróides, os glicosaminoglicanos polisulfatados, o hialuronato de sódio, o pentosan polisulfato e o ácido tiludrónico, com o objectivo de travar a progressão da doença articular. Também são abordadas novas formas alternativas de tratamento da OA, como a terapia por meio de ondas de choque, a utilização de células estaminais mesenquimatosas e a terapia genética. Este trabalho é complementado pela descrição de dois casos clínicos, onde são abordados o diagnóstico e respectivo tratamento. ABSTRACT - The Osteoarthritis (OA) is the main cause of lameness in the sport and leisure horse, being an affection with great economic evolvement. This work describes the main characteristics of the joint structure, as well as its physiology. It defines the OA and all the involved structures in its process. It’s physiopathology puts in evidence all the risk factors and all the process of joint degradation associated. Diagnosis methods are mentioned, not only the conventional ones, but also the recent imaging techniques (magnetic resonance imaging, computed tomography and nuclear scintigraphy) and the use of the bone and cartilage biomarkers. Diverse types of treatment are used, as it is the case of non-steroidal anti-inflammatory, corticosteroids, polysulfated glycosaminoglycan, hyaluronan, pentosan polysulfate and acid tiludronic, drugs, achieving to decrease the progression of the joint disease. Also new forms of treatment of the OA are referred as the therapy using shock waves, mesenchymal stem cells and the gene therapy. Two clinical cases are described, where the diagnosis and treatment are boarded.

Dissertação de Mestrado Integrado em Medicina Veterinária Apesar dos vários recursos e metodologias utilizados atualmente em estudos relativos à motilidade intestinal equina (MIE), o conhecimento relativo à motilidade gastrointestinal equina é ainda inferior ao desejável. O presente estudo pretendeu averiguar se a MIE é passível de ser avaliada por ecografia de forma objetiva. Recorreu-se a uma amostra de onze equinos, avaliados física e ecograficamente no momento da admissão e a cada 24 horas, durante o período de hospitalização na Clinica de Sto. Estevão. A avaliação ecográfica incluiu a pesquisa de ansas de intestino delgado em quatro regiões: ventral, inguinal esquerda, inguinal direita e duodenal. Foram obtidos 184 filmes ecográficos, dos quais se selecionaram 60, que foram pontuados por um painel de dez avaliadores com base numa escala pré-estabelecida. Verificou-se uma concordância elevada entre os avaliadores na atribuição do Score de Motilidade Intestinal por ecografia (ICC=0.915; p˂0.001). A análise revelou a existência de uma relação entre a pontuação da motilidade e as seguintes variáveis: jejum, dor abdominal, grau de dor abdominal, frequência cardíaca, auscultação abdominal, refluxo nasogástrico e fluidoterapia. A ecografia revelou-se um meio objetivo de avaliação da MIE, tornando válida a sua utilização, complementar a outros meios, no diagnóstico e monitorização de afeções relacionadas com dismotilidade intestinal. ABSTRACT - Despite the several resources and methodologies being currently used in equine intestinal motility (EIM) studies, the knowledge about the motility of the equine gastrointestinal tract is still lower than desirable. The purpose of this study is to determine if the EIM can be assessed by ultrasound accurately. Eleven horses were submitted to both physical and sonographic exam during admission and each 24 hours, during the hospitalization period at the St Estêvão Clinic. EIM was assessed in four different areas: ventral; left inguinal; right inguinal; and, duodenum. A number of 184 ultrasound movies was obtained, from which 60 were selected and scored by ten evaluators based on a predetermined scale. A general agreement was noticed between evaluators in respect to scoring EIM (ICC=0.915; p˂0.001). Data revealed a relation between motility scoring and different variables such as fasting, abdominal pain, degree of abdominal pain, heart rate, abdominal auscultation, nasogastric reflux and fluid therapy: EIM can be accurately assessed by ultrasonography, supporting the use of this method in the diagnosis and monitoring intestinal dismotility conditions. 25. The Best Protocol to Treat Equine Skin Wounds by Second Intention Healing: A Scoping Review of the Literature.

Equine skin wound treatment continues to be a challenge for veterinarians. Despite being a frequent practice, it remains difficult to choose an evidence-based treatment protocol. This study aimed to comprehensively explore the literature and provide a scoping review of therapeutic strategies for equine skin wounds and identify knowledge gaps and opportunities for future research. This review was conducted using specific criteria to select literature that described methods to manage second intention wound healing. After removing duplicates and screening papers for suitability, 81 manuscripts were included for data extraction. Of these, 59 articles were experimental studies, 10 were case reports, 9 were case series, and 3 were clinical studies. The most frequent wound location was the distal limbs. Macroscopic assessment was the main tool used to evaluate treatment effectiveness. All of the case reports, case series, and clinical studies reported positive outcomes with regard to the treatment used, while only 36% of the experimental studies found significant healing improvement in treated wounds compared to control groups. It was found that there are many treatments that have exhibited controversial results, and there exists a lack of evidence for the adoption of specific treatment protocols.

- Abstract
The objective of this study is to analyze the perinatal results of the prophylactic measures adopted in the prevention of the vertical transmission of HIV. The study was carried out using the medical records of 168 women with HIV and their newborns in a public maternity unit of Fortaleza, in the state of Ceará, from 2005 to 2009. The data was obtained between the months of March and September 2010 and was analyzed using the program SPSS 10.0. The women diagnosed before pregnancy received timely care that permitted them an effective antiretroviral therapy and a greater number of check-ups (p<0,05). Antiretroviral therapy was administered to 97.6% of the pregnant women, and triple therapy in 88.7%. AZT was administered intrapartum in 95.2% of the women and oral AZT in syrup to 100% of the newborns. Birth by cesarean section was predominant (92.8%) and was associated to the intrapartum use of AZT when compared to vaginal birth (p<0.001). Cesarean section favored intrapartum administration of AZT.
