Purpose: Although subjective knowledge about the prognosis of an advanced disease is extremely important for coping and treatment planning, the concept of prognostic awareness (PA) remains inconsistently defined. The aims of the scoping review were to synthesize a definition of PA from the most recent literature, describe preconditions, correlates and consequences, and suggest a conceptual model. Methods: By using scoping review methodology, we searched the Web of Science and PubMed databases, and included publications, reviews, meta-analyses or guidelines on all physical diagnoses, as well as publications offering a conceptual or an operational definition of PA. The data were analyzed by means of content analysis techniques. Results: Of the 24 included publications, 21 referred exclusively to cancer, one to patients with hip fractures and two to palliative care in general. The deduced definition of PA comprised the following facets: adequate estimation of chances for recovery, knowledge of limited time to live, adequate estimation of life expectancy, knowledge of therapy goals, and knowledge of the course of the disease. Further content analysis results were mapped graphically and in a detailed table. Conclusion: There appears to be a lack of theoretical embedding of PA that in turn influences the methods used for empirical investigation. Einführung: Der Nutzen einer frühzeitigen Integration von Palliative Care in die onkologische Standardversorgung (Integrated Palliative Cancer Care; Abk. IPCC) ist für Tumorpatienten und ihre Angehörigen belegt und wird von internationalen Organisationen parallel zu einer tumorspezifischen Behandlung empfohlen. Best- Practice-Modelle von IPCC im Verlauf der Behandlung zielen darauf ab, Patienten und Angehörige bei der Erfüllung ihrer individuellen Bedürfnisse und Linderung von spezifischen Symptomen einer Krebserkrankung zu unterstützen. Bisher weiß man wenig darüber, ob IPCC krankheitsphasenspezifisch oder patienten- bzw. angehörigenzentriert angeboten werden sollte. Diese Arbeit untersucht Bedürfnisse von Patienten und Angehörigen unter Berücksichtigung individueller Herausforderungen, Behandlungspräferenzen und Informationsbedürfnissen im gesamten Krankheitsverlauf. Methodik. Angelehnt an die Grounded Theory wurden neun onkologische Patienten in verschiedenen Erkrankungsphasen, 16 Angehörige und acht klinische Behandler mittels explorativer, leitfadengestützter Einzelinterviews (Patienten und Angehörige) und Fokusgruppeninterviews (klinische Behandler und Angehörige) qualitativ untersucht. Die Auswertungsprozesse wurden in einem iterativen Prozess mit Experten diskutiert und die Ergebnisse in einer kommunikativen Validierung mit den Teilnehmern überprüft. Für die deskriptive Stichprobenbeschreibung wurden ein soziodemografischer und ein krankheitsbezogener Fragebogen verwendet. Ergebnisse. Die Bedürfnisse von Patienten und Angehörigen sind stärker von individuellen Herausforderungen und Krankheitsverläufen abhängig, als von klinischen Behandlungsphasen. Wichtige Bedürfnisse zeigten sich in Bezug auf Erkrankung, Behandlung, Krankheitsverarbeitung, Behandlungspräferenz, Prozesse und Strukturen im ambulanten und stationären Kliniksetting. Angehörige hatten eine bedeutende Rolle in der Versorgung von Tumorpatienten inne. Trotz dieser bedeutenden Rolle, wurden die Belastungen von den Behandlern unterschätzt und die vorhandenen Ressourcen überschätzt. Die Angehörigen fühlten sich ungenügend auf ihre Rolle vorbereitet und beschrieben unzureichende Unterstützung in der Vorbereitung auf die Pflege und Unterstützung des Patienten nach Entlassung aus Klinik. Schlussfolgerung. Die Ergebnisse dieser Studie unterstreichen die Notwendigkeit, Bedürfnisse von Tumorpatienten und ihren Angehörigen über den gesamten Krankheitsverlauf systematisch zu erfassen. Eine solche Erfassung kann die Grundlage für eine effektive, integrierte Palliative Care in der onkologischen Standardversorgung sein. Weiterhin ist es notwendig, die strukturellen Versorgungsbedingungen zu verbessern und mehr Transparenz in Bezug auf vorhandene Ressourcen, Abläufe und Verantwortlichkeiten im jeweiligen Versorgungssetting zu schaffen. Dabei kommt den onkologischen Primärbehandlern eine bedeutende Schlüsselrolle für eine frühzeitige IPCC zu. Darüber hinaus könnte eine aktive Einbindung von Angehörigen in die Tumorbehandlung, deren Belastungen verringern und die Selbstwirksamkeit stärken. Hierfür sollte das Angebot an Schulungen und auf das Versorgungssetting bezogene Informationen ausgeweitet werden. Außerdem sollte die besondere Rolle der Angehörigen, die diese in der onkologischen Behandlung für die Krebspatienten, aber auch selbst als Betroffene haben, auf gesundheitspolitischer und gesellschaftlicher Ebene mehr als bisher thematisiert werden, um eine Verbesserung ihrer Situation zu erreichen., Purpose: Early integration of Palliative Care into standard oncological treatment (Integrated Palliative Cancer Care; IPCC) is beneficial for cancer patients and their caregivers. Multiple international organizations recommend offering this care simultaneously with tumor-specific treatment. Best practice models of IPCC in the course of cancer treatment aim to help patients and their caregivers meeting their individual needs and relief specific symptoms of cancer. So far, there is only limited knowledge about whether IPCC should be offered phase-specific or patient- and caregiver-centered. This study investigated patients’ and caregivers’ needs, considering individual challenges, treatment preferences and information needs throughout the disease trajectory. Methods: Using a modified grounded theory approach, nine oncological patients at different stages of the disease, 16 caregivers and eight clinical practitioners in oncology participated in a qualitative study employing explorative, semi-structured interviews (patients and caregivers) and focus group interviews (clinical practitioners and caregivers). The evaluation processes were discussed in an iterative process with experts and the results were confirmed in a communicative validation process. Descriptive data were collected via a socio-demographic questionnaire and a disease-related questionnaire. Results: The needs of the patients and their caregivers were more dependent on individual challenges and disease trajectories than on clinical treatment phases. Important needs were identified in terms of illness, treatment, disease progression, treatment preference, and processes and structures in outpatient and inpatient settings. While caregivers played an important role in cancer care, their experienced burdens were underestimated by the practitioners and the available resources were overestimated. The caregivers felt inadequately prepared for their supporting role and described insufficient support in preparation for the accompaniment and care of the patient after transitioning from the hospital. Conclusions: The results of this study highlighted the need for continuous and systematic assessment of the needs of cancer patients and their cargivers throughout the disease trajectory. Such assessment can be the basis for an effective integrated palliative care into standard oncology care. For this purpose, it is important to improve structural conditions more generally and, to create more transparency regarding existing resources, procedures and responsibilities in the clinic setting, in particular. Oncological primary care providers play an important key role in initiating IPCC early. In addition, active involvement of caregivers in the cancer treatment could reduce their burden and increase self-efficacy. Likewise, the special role of the caregivers should be addressed more than ever before at the health and social level in order to improve their situation.
