Recent advancements in medical technology have propelled the field of arterial and cerebral blood pressure management into a new era of precision and foresight. This discourse delves into contemporary methodologies and technologies utilized in the diagnosis and prognosis of arterial and cerebral blood pressure conditions, alongside their potential implications for future health prognoses. Notably, the discourse underscores the merits of cutting-edge technologies, including machine learning, artificial intelligence, genomic sequencing, and nanotechnology, in enhancing diagnostic precision, early detection of pathological markers, and tailoring personalized treatment regimens. Moreover, ethical considerations and confidentiality concerns pertinent to the adoption of these technologies are carefully scrutinized. To this end, an exhaustive review of contemporary literature pertaining to medical diagnostic technologies, encompassing machine learning, artificial intelligence, genomic sequencing, and nanotechnology, was conducted. The collaborative efforts of interdisciplinary teams comprising medical professionals, engineers, computer scientists, and ethicists are advocated as pivotal for realizing the full potential of modern diagnostic technologies in clinical settings. Los avances recientes en la tecnología médica han impulsado el campo del manejo de la presión arterial y cerebral hacia una nueva era de precisión y previsión. Este discurso profundiza en las metodologías y tecnologías contemporâneas utilizadas en el diagnóstico y pronóstico de las condiciones de presión arterial y cerebral, junto con sus posibles implicaciones para los pronósticos de salud futuros. En particular, el discurso subraya los méritos de las tecnologías de vanguardia, incluido el aprendizaje automático, la inteligencia artificial, la secuenciación genómica y la nanotecnologia, para mejorar la precisión del diagnóstico, la detección temprana de marcadores patológicos y la adaptación de regímenes de tratamien- to personalizados. Además, se examinan cuidadosamente las consideraciones éticas y las cuestiones de confidencialidad pertinentes a la adopción de estas tecnologías. Para ello, se realizó una revisión exhaustiva de la literatura contemporánea relacionada con las tecnologias de diagnóstico médico, que abarca el aprendizaje automático, la inteligencia artificial, la secuenciación genómica y la nanotecnologia. - Abstract
Introduction: It is unknown whether the selection of individualized pharmacological treatment according to arterial hypertension (AH) subtype leads to greater blood pressure (BP) control. Objective: To evaluate the association between the selection of pharmacological treatment based on AH subtype and BP control. Materials and method: This was a real-world cohort study that included subjects with a de novo diagnosis of AH who received anti-hypertensive pharmacological treatment following current guidelines. Retrospectively, patients were classified by AH subtype according to the pattern of systolic or diastolic predominance of AH. Also, appropriateness of the pharmacological treatment was assessed based on AH subtype and the underlying hemodynamics of each subtype. BP control at the follow-up, within the first 12 weeks after diagnosis, was defined as < 140/90 mmHg. Results: Among the subjects included in the study (n = 1397), the mean age was 52.4 ± 13.2. Initiation of appropriate anti-hypertensive pharmacological treatment was associated with greater BP control (OR 2.17; CI: 1.49-3.15; p < 0.001). The patients who did not reach BP control presented a higher frequency of divergent systolic-diastolic hypertension subtype and greater hemodynamic alterations. Conclusions: Appropriate selection of pharmacological treatment based on AH subtype is associated with better BP control in patients with newly diagnosed AH. [ABSTRACT FROM AUTHOR]

16. Perfil clínico de pacientes con insuficiencia cardiaca en los servicios de Medicina Interna de un hospital terciario.

- Abstract
OBJECTIVE: To describe the clinical profile of patients with heart failure upon hospital admission. MATERIALS AND METHODS: Cross-sectional observational study of patients hospitalized in the Internal Medicine service of a tertiary hospital in Lima, Peru, with a diagnosis of heart failure from January 2022 to February 2023. RESULTS: One hundred and three patients were included, with an average age of 75.57 ± 12.79 years, 52% were women. Among the clinical characteristics, dyspnea (94.2%), pulmonary rales (74.2%), having 3 or more comorbidities (23.3%) stood out; the main decompensating factor was of infectious etiology (36.9%). A functional class III was found in 64.1% of cases; and left ventricular ejection fraction preserved in 48.5% of them. A significant relationship was found between advanced functional class and systolic and diastolic blood pressure levels at the lower limit with 30-day mortality (p = 0.01, 0.028 and 0.022, respectively). CONCLUSIONS: The clinical profile revealed that 80% of patients were over 60 years of age, with a predominance of the female sex, with dyspnea as the predominant symptom, a preserved left ventricular ejection fraction, advanced functional class and blood pressure levels in the lower limit of normality. 19. Impacto de un programa de ejercicios isotónicos en la reducción de la presión arterial en adultos con hipertensión

El presente estudio tuvo como objetivo evaluar la efectividad de un programa de ejercicios isotónicos en la reducción de la presión arterial en adultos hipertensos. La muestra estuvo compuesta por 30 participantes que completaron un programa de seis semanas de ejercicios isotónicos, tres veces por semana. Los resultados mostraron una disminución significativa en la presión arterial sistólica (PAS) y diastólica (PAD). La PAS se redujo en un promedio de 19 mmHg y la PAD en 11 mmHg, siendo estos resultados estadísticamente significativos (P < 0.05). Además, se observó una mejora en la composición corporal, con una reducción promedio del índice de masa corporal (IMC) entre 1.0 y 1.8 unidades. Estos hallazgos sugieren que los ejercicios isotónicos son una intervención efectiva no solo para reducir la presión arterial, sino también para mejorar la salud metabólica de los participantes. Los ejercicios isotónicos, al requerir recursos mínimos, son una opción accesible para poblaciones con acceso limitado a tratamientos farmacológicos. El análisis estadístico, utilizando la prueba t de Student, validó la significancia de las diferencias observadas, demostrando que las mejoras no fueron producto del azar. En conclusión, este estudio respalda el uso de ejercicios isotónicos como parte de las intervenciones no farmacológicas para el manejo de la hipertensión arterial y la mejora de la salud general.

20. Relación de la calidad y la duración del sueño en población colombiana con hipertensión arterial

Introducción. Las alteraciones en la calidad y la duración del sueño son factores de riesgo para el desarrollo de hipertensión arterial sistémica en los países orientales. Sin embargo, hay pocos estudios de los países de Latinoamérica para investigar esta asociación. Objetivo. Analizar la asociación entre la calidad y la duración del sueño, y la incidencia de hipertensión arterial sistémica en población colombiana. Materiales y métodos. Se llevó a cabo un estudio observacional, longitudinal, prospectivo y analítico, anidado en la cohorte de base poblacional INEFAC, desarrollado con participantes mayores de 18 años de Bucaramanga (Colombia). El sueño se evaluó mediante el índice de calidad del sueño de Pittsburgh y, su duración, mediante preguntas estandarizadas. Se realizó un análisis multivariado con modelos de regresión logística ajustados por las posibles variables de confusión. Resultados. Se incluyeron 1.306 participantes no hipertensos con edad media de 40 ± 12 años. El 92,8 % de la población presentaba algún problema del sueño, el 45,15 % dormía 6 horas o menos y el 28,6 % dormía 8 horas o más. El análisis multivariado mostró un mayor riesgo de hipertensión en los participantes con diabetes (OR = 5,27) (IC95 %: 2,27-12,26), obesidad (OR = 2,81) (IC95 %: 1,11-7,13), tabaquismo activo (OR = 2,02) (IC95 %: 1,01-4,04) y mayor estrato socioeconómico (OR = 4,94) (IC95 %: 1,59-15,38 para estrato 4), pero no se encontró un mayor riesgo en los participantes con mala calidad o poca duración del sueño. Conclusiones. No se demostró asociación alguna entre la duración o la calidad del sueño y la incidencia de hipertensión arterial sistémica en población colombiana. Se requieren más estudios en esta población para llegar a conclusiones definitivas.

Introducción. La vitamina B12 incidiría sobre la presión arterial mediante mecanismos hormonales y metabólicos. Objetivo. Analizar la relación entre las concentraciones de vitamina B12 y la presión arterial en adultos. Materiales y Métodos. Estudio observacional, analítico, retrospectivo y transversal, proveniente de una base de datos abierta conformada por 4154 personas de una comunidad mexicana atendida en un establecimiento de salud. Las variables fueron: sexo, vitamina B12 y presión arterial sistólica. Se utilizó las pruebas Ji-cuadrado y prueba t de student. Resultados. los promedios de vitamina B12 fueron menores en presencia de presión arterial igual o mayor a 130 mmHg (hombres: 217,12 pg/ml; mujeres 204,51 pg/ml) respecto a cuándo se comparó con niveles de presión arterial sistólica entre 90 a 129 mmHg (hombres; 266,98 pg/ml; mujeres: 205,18 og/ml). Introduction: Low-dose ketamine infusions have shown analgesic effectiveness for the management of postoperative pain. The impact of low-dose ketamine infusions on cardiovascular response is dose-dependent and requires a better knowledge about its effects on this population. Objective: To conduct a systematic review to describe changes in systolic, diastolic and mean arterial pressure, and heart rate 24, 48 and 72 hours after surgery. Methods: Randomized, controlled trials were reviewed in the Cochrane Library, PubMed, EMBASE, SciELO, Lilacs and grey literature on low-dose ketamine infusions for the study variables. The quality of the studies was assessed using the Cochrane's risk of bias tool. Results: Six randomized, controlled trials with 641 patients were included. Low-quality evidence was found suggestive of a lack of certainty of any significant differences in the systolic blood pressure variables at 24 hours (mean standard deviation -1.00, 95 % CI: -7.27 to 5.27). A statistically significant higher mean heart rate at 24 hours was identified in the low-dose ketamine infusion group, (mean standard deviation 1.64 95 % CI: 0.38 to 2.90) which did not reach clinical significance. A lower pain level and less use of opioids was identified in the low-dose ketamine infusion group. Conclusions: Low quality evidence was found, suggesting that low-dose ketamine infusions are not associated with significant changes in blood pressure or heart rate 24 -- 48 hours after surgery. Introducción: la hipertensión arterial es una enfermedad crónica que se caracteriza por un aumento continuo y mantenido de las cifras de la tensión arterial en las arterias. Representa por sí misma una enfermedad y un factor de riesgo modificable para diversas patologías. Se posiciona como la principal patología relacionada a la mortalidad en todo el mundo. Objetivo: caracterizar la hipertensión arterial en las edades pediátricas. Método: se realizó una revisión bibliográfica narrativa con relación al tema con un total de 25 referencias bibliográficas. Se utilizaron artículos científicos en inglés, español y portugués disponibles en bases de datos como: Scielo, PubMed, Elsevier y en el motor de búsqueda de Google Académico. Desarrollo: la mayoría de los casos de hipertensión arterial en los pacientes pediátricos son de causa secundaria: problemas renales, endocrinos, cardíacos, neurológicos, psicológicos y exógenos, abuso de drogas, exposición a sustancias nocivas, hábitos alimenticios incorrectos y sedentarismo. Al presentarse en ocasiones de forma asintomática, se requiere un correcto seguimiento a los pacientes con factores de riesgo. Conclusiones: la hipertensión arterial es una patología que se presenta cada vez con una mayor incidencia en la infancia y adolescencia. Las cifras elevadas de presión arterial en estas etapas representan una predisposición a padecer de hipertensión arterial en la vida adulta. Un oportuno diagnóstico y un manejo adecuado garantizan la reducción del riesgo de desarrollar disímiles patologías.

Primary hyperaldosteronism (PAH) is an important cause of secondary hypertension (HTN). The study of the same requires a high clinical suspicion in addition to a hormonal study that confirms hormonal hypersecretion. It is important to start the appropriate treatment once the diagnosis is confirmed, and for this is necessary to demonstrate whether the hormonal hypersecretion is unilateral (patients who could be candidates for surgical treatment) or bilateral (patients who are candidates for pharmacological treatment only).At the Hospital del Mar since 2016 there has been a multidisciplinary work team in which Nephrologists, Endocrinologists, Radiologists and Surgeons participate to evaluate cases with suspected hyperaldosteronism and agree on the best diagnostic-therapeutic approach for these patients, including the need for adrenal vein sampling, which is a technique that in recent years has become the gold standard for the study of PAH.In the present study we collect the experience of our centre in performing AVC and its usefulness for the management of these patients. Resumen: El hiperaldosteronismo primario (HAP) es una causa importante de hipertensión arterial (HTA) secundaria. El estudio del mismo precisa de una alta sospecha clínica además de un estudio hormonal que confirme la hipersecreción hormonal. Es importante iniciar el tratamiento adecuado una vez se confirma el diagnóstico y para ello es preciso demostrar si la hipersecreción hormonal es unilateral (pacientes que podrían ser tributarios a tratamiento quirúrgico) o bilateral (pacientes que son tributarios a tratamiento únicamente farmacológico).En el Hospital del Mar desde el año 2016 existe un equipo de trabajo multidisciplinar en el que participan Nefrólogos, Endocrinólogos, Radiólogos y Cirujanos para evaluar los casos con sospecha de hiperaldosteronismo y consensuar el mejor abordaje diagnóstico-terapéutico de estos pacientes, incluyendo la necesidad de cateterismo venoso adrenal que es una técnica que en los últimos años se ha erigido como gold standard para el estudio de HAP.En el presente estudio recogemos la experiencia de nuestro centro en la realización de CVA y en la utilidad de este para el manejo de estos pacientes.

Introduction: due to the catabolic characteristics of hemodialysis (HD), patients should consume foods or supplements during this treatment to meet their energy requirements and maintain a neutral nitrogen balance; however, there are some outcomes in which the effect of intradialytic oral nutrition (ION) is scarcely known. Objectives: this study aims to evaluate the effect of two types of ION (liquid and solid) on quality of Life (qoL), appetite, and safety in HD patients. Methods: a pilot randomized, crossover clinical trial was performed in 18 patients on chronic HD. One group received ION for 18 HD sessions, after the crossover continued for 18 more sessions in the control group, and vice versa. We recorded qoL, appetite, systolic blood pressure (SBP), and intradialytic hypotension (IH) events. Results: clinical improvement was observed for most qoL components. Regardless of the consistency of supplementation, SBP increased to 4.10 mmHg. Both study groups reported a "very good-to-good" appetite. Conclusion: favorable clinical changes were observed in qoL scores during the study. Five of six IH events were reported for patients in the ION group, and SBP increased within the safe range (= 10 mmHg); appetite remained stable in both groups. Therefore, we concluded that this strategy, regardless of implementation consistency, is safe to be used in stable patients. [ABSTRACT FROM AUTHOR]

Circadian blood pressure variations are classified into different phenotypes that provide additional knowledge about patients’ morbidity and prognosis of patients. Ambulatory blood pressure monitoring (ABPM) is the ideal tool to obtain a record of these variations. The etiology of non-dipper hypertension is multifactorial, including dysregulations in the autonomic nervous system, neuroendocrine axes, and mineral turnover system, with complex pathophysiological behavior. There are many associations between altered circadian phenotypes (such as the non dipper phenotype) and adverse cardiovascular outcomes such as cardiovascular mortality, coronary and cerebrovascular disease, chronic kidney disease, and heart failure. La presión arterial media es un indicador indirecto del grado de perfusión orgánica, la cual podría variar en pacientes con y sin diabetes mellitus-2. El objetivo fue determinar las diferencias y riesgo de alteraciones en la presión arterial media de adultos con y sin diabetes mellitus-2 de la población peruana según datos de la encuesta demográfica de salud familiar. Se realizó un estudio observacional, analítico, retrospectivo y transversal; Los datos provinieron de la encuesta demográfica y de salud familiar-2022 (ENDES-2022), se incluyó adultos diabéticos y no diabéticos. Las variables fueron: presión arterial media, diabetes mellitus-2, sexo y se incluyeron las variables: edad, consumo de alcohol, nivel educativo e índice de masa corporal para el análisis multivariado. Se realizó la prueba de Chi-cuadrado de Pearson, la prueba T student para muestras independientes, Odds Ratio crudo y ajustado mediante regresión logística binaria. El promedio de presión arterial media en diabéticos fue de 94,81 y 93,52 mmHg en la primera y segunda medición y en no diabéticos de 89,18 y 88,02 mmHg. La presión arterial media alta fue más frecuente en diabéticos que en no diabéticos; En el modelo logístico, incluyendo las variables sexo y edad, los diabéticos tuvieron un riesgo 2 veces mayor que los no diabéticos de alteraciones en la presión arterial media tanto en la primera como en la segunda medición. En conclusión, la diabetes mellitus-2 incrementa la presión arterial media y representa un factor de riesgo para la elevación de la presión arterial media en la población peruana.

Introduction: Acute pulmonary edema is a frequent disease among patients in emergency services. Objective: To characterize patients with acute pulmonary edema who attended the emergency services. Methods: A descriptive and cross-sectional study of 37 patients with acute pulmonary edema who attended the emergency services of Dr. Joaquín Castillo Duany Teaching Clinical Surgical Hospital in Santiago de Cuba was carried out, from January to November 2019. The studied variables were age, sex, alive or dead when discharged, noninvasive ventilation usage. Results: There were no differences between the sexes; there was a prevalence of patients older than 60 years of age, who suffered from hypertension. It was demonstrated low noninvasive or invasive ventilation usage. Conclusions: Acute pulmonary edema is more frequent among patients older than 60 years of age in both sexes. Most of the patients suffered from hypertension and there was a deficit in the implementation of noninvasive ventilation in these cases.

Foundation: excess body fat is marked by a permanent imbalance between food consumption and energy expenditure of human beings that generate various health risks, including increased blood pressure. Objective: determine the relationship between the percentage of body fat and blood pressure in adults from rural and urban areas of the municipality of Carmen de Chucurí, Colombia. Method: a descriptive and correlational study with a quantitative approach was carried out on a randomly chosen sample, composed of 406 subjects for the inhabitants of the rural sector and 409 of the urban sector between 18 and 40 years of age from the Carmen Chucurí municipality. Body mass was evaluated, with the use of the tool called electrical impedance and the OMRON HBF-514C brand equipment, and blood pressure and the OMRON HEM-4030 digital blood pressure monitor was used. Results: the main finding of this research was the positive and significant association between the percentage of body fat and diastolic blood pressure in rural men and women in the Carmen de Chucurí municipality. Conclusions: the percentage of body fat is positively and significantly associated with diastolic blood pressure in rural men and women of the Carmen de Chucuri municipality; for the other parameters analyzed, a significant correlation was not evident.

Introducción. La falta del cumplimiento terapéutico es un problema en el control de la hipertensión arterial. Objetivo. Caracterizar los pacientes no adherentes al tratamiento de hipertensión arterial en pobladores de la V Región Sanitaria del Paraguay. Material y Métodos: Estudio transversal que incluyó pacientes hipertensos conocidos que se encontraban en tratamiento para la hipertensión arterial. Se utilizó la Escala de adherencia a la medicación de Morisky 8 ítems que mide comportamientos de adherencia específicos asociados a la ingesta de medicamentos. Resultados. Se incluyeron 163 pacientes, todos considerados no adherentes al tratamiento según la Escala Morinsky MMAS-8. El 52,2% tenía más de 40 años, 66,3% del sexo femenino, 40,5% de nivel universitario, 57,1% casado y el 87,7% vive con la familia. El 66,9% informó trabajar hasta 8 horas por día y el 58,3% con ingresos mayores a 2 salarios mínimos. En cuanto al conocimiento el 53,9% fue alto. El 50% de los pacientes declaró utilizar plantas medicinales para su tratamiento, el 7,4% con depresión frecuentemente o casi siempre y el 22,1% ansiedad frecuentemente o casi siempre. En cuanto a las preguntas de no adherencia, el 53,4% olvidó tomar alguna vez su medicación o dejar de tomarla cuando siente que está bajo control (53,3%), el 44,2% deja de tomar su medicación porque se siente peor con ella y el 46,5% la olvida cuando viaja. Conclusión. La no adherencia en los pacientes de la V Región fue alarmante, por tanto, se deben de realizar medidas de acción respecto a las mismas.

Background: Posttraumatic stress disorder (PTSD) and depression are associated with increased risk for cardiovascular disease (CVD), which is the leading cause of death and disability worldwide. Epidemiological studies have revealed these illnesses to be highly comorbid, particularly among women. In the current study, we explored associations between indices of cardiovascular health, PTSD, and depression among a sample of trauma-exposed individuals assigned female at birth. Methods: Participants were N = 49 individuals without CVD who reported lifetime Criterion A trauma exposure. Blood pressure (BP), heart rate (HR), and high-frequency heart rate variability (HF-HRV) were collected during a 5-minute resting period. Symptoms of CVD (e.g. extremity pain and swelling, shortness of breath), PTSD, and depression were assessed, along with an exploratory measure of anhedonia. Results: Trauma exposure was positively correlated with systolic BP (r =.32, p =.029) and diastolic BP (r =.30, p =.040). The number of reported CVD symptoms was positively correlated with symptoms of PTSD (r =.41, p =.004), depression (r =.40, p =.005) and anhedonia (r =.38, p =.007). CVD symptoms were also significantly associated with PTSD (β =.41, t = 2.43, p =.023), depression (β =.40, t = 2.76, p =.009), and anhedonia (β =.38, t = 2.51, p =.017) after controlling for age and trauma exposure. These associations were not moderated by HF-HRV in our sample. Conclusions: Our results support the association between PTSD and depressive symptoms and worse cardiovascular functioning among an often-overlooked population that is particularly vulnerable to these illnesses. Future studies should investigate residual impacts of PTSD and depression treatment on CVD risk among trauma-exposed individuals, particularly women. Trauma exposure and PTSD are associated with depression and cardiovascular disease (CVD) risk. We explored cardiovascular health, PTSD, and depression among 49 trauma-exposed individuals assigned female at birth. Trauma exposure positively correlated with blood pressure. CVD symptoms were positively correlated with PTSD, depression, and anhedonia. Associations were not moderated by heart rate variability. [ABSTRACT FROM AUTHOR]

The article presents the results of a study conducted in Spanish community pharmacies during May 2019, with the aim of understanding the situation of blood pressure in the Spanish population and estimating the prevalence of masked hypertension. It was found that almost one in four participants had elevated blood pressure values, and the most related risk factors were overweight, diabetes, and age. Additionally, it was confirmed that 40% of suspected cases of masked hypertension were confirmed through self-measurement of blood pressure. The importance of regular blood pressure measurement and the need to improve the management of hypertension in the population are highlighted. [Extracted from the article]
