Cancers are ranked second most common cause of mortality in our country. Consumption of tobacco products and obesity are among the primary risk factors for cancer progression. This study aimed to calculate the proportions of cancers that are attributable to these risk factors, and to evaluate the national risk map by assessing the geographical distributions of these cases. The prevalence of tobacco use and obesity that required for calculation of PAFs were obtained from the National Burden of Disease and Cost Effectiveness Study – 2003 report. Additionally, the estimated prevalence of tobacco smoking also derived from the incident cases according to Peto-Lopez method. The incidences of tobacco and obesity-associated cancers were obtained from the Turkish Cancer Statistics – 2016 report of the Turkish Ministry of Health, and Cancer Incidence in Five Continents – Volume XI report of the International Association for Cancer Research and International Association of Cancer Registries. The relative risks for tobacco and obesity on cancer progression were obtained from the United States Cancer Prevention Study – II data and World Cancer Research Fund reports. Using these different incidence and prevalence data, four different PAF models were calculated for tobacco-associated cancers, and two different PAF models were calculated for obesity-associated cancers, which were used for the Geographical Information System Analyses in ArcGIS Pro software (Esri Inc., USA). Areal interpolation and Anselin’s Local Moran I statistics was used in GIS analyses. The proportions of PAFs for smoking-associated cancers ranged between 54.2-61.9% in men and 23.2%-30.6% in women. The PAFs for obesity associated cancers ranged between 22%-22.4% in men and 23.7%-24.3% in women. Geographical distribution patterns revealed deceased PAFs in tobacco-associated cancers for esophageal and lung cancers in east and southeast, and oral cavity and pharynx cancers in east in men; esophageal cancers in east, lung cancers in west and northwest, gastric cancers in east, oral cavity and pharynx cancers in east in women. Decreased PAFs were found for obesity-associated cancers in east for colon cancer in men, and in east and northeast for postmenopausal breast cancer in women. Günümüzde, ülkemizdeki başlıca mortalite nedenleri arasında kanserler ikinci sırayı almaktadır. Tütün tüketimi ve obezite kanser gelişimi için öncelikli risk faktörleridir. Bu çalışmada, tütün ürünleri ve obezite ile ilişkili kanserlerde bu risk faktörlerine atfedilebilir fraksiyonların (PAF) hesaplanması ve coğrafi bilgi sistemleri (CBS) analiz yöntemleri kullanılarak dağılım ve kümelenme özelliklerinin değerlendirilmesi amaçlanmıştır. PAF hesaplamaları için risk faktörlerine ait prevalans verileri 2003 yılı Ulusal Hastalık Yükü ve Maliyet Etkililik Araştırması’ndan edinilmiş, tütün tüketimi için ayrıca Peto-Lopez yöntemine göre hesaplanan beklenen prevalanslar da analizlerde kullanılmıştır. Kanser insidansları ise T.C. Sağlık Bakanlığı tarafından yayınlanan 2016 yılı Türkiye Kanser İstatistiklerinden ve Uluslarası Kanser Araştırma Kurumu ile Uluslararası Kanser Kayıtçılığı Birliği tarafından yayınlanan Beş Kıtadaki Kanser İnsidansları serisinin XI. sayısından edinilmiştir. Tütün ve obezitenin kanser gelişimi için rölatif riskleri Birleşik Devletler Kanser Prevansiyon Kohort Çalışması-II verilerinden ve Dünya Kanser Araştırma Fonu raporlarından alınmıştır. Bu farklı insidans ve prevalans verileri kullanılarak tütünle ilişkili kanserlerde dört, obezite ilişkili kanserlerde iki farklı modele göre PAF hesaplamaları ve ArcGIS Pro (Esri Inc., ABD) yazılımı kullanılarak CBS analizleri yapılmıştır. CBS analizlerinde alansal interpolasyon ve Anselin Lokal Moran I yöntemi kullanılmıştır. Çalışmada 2019 yılı için tütün ile ilişkili kanserler grubundaki yeni tanı kanser olgularının erkeklerde %54,2-%61,9’unun, kadınlarda %23,2-%30,6’sının, obezite ile ilişkili kanserler grubundaki yeni tanı kanser olgularının ise erkeklerde %22-%22,4’'ünün, kadınlarda ise %23,7-%24,3’ünün atfedilebilir olgular oldukları hesaplanmıştır. Coğrafi dağılım özellikleri bakımından PAF değerlerinin tütünle ilişkili kanserlerde erkeklerde özefagus ve akciğer kanserlerinde doğu ve güneydoğuda, oral kavite ve farinks kanserlerinde doğuda, kadınlarda özefagus kanserlerinde doğuda, akciğer kanserlerinde batı ve kuzeybatıda, mide kanserlerinde doğuda, oral kavite ve farinks kanserlerinde doğuda, obezite ile ilişkili kanser grubunda ise erkeklerde kolon kanserinde, kadınlarda postmenopozal meme kanserinde doğu ve kuzeydoğuda daha düşük olduğu tespit edilmiştir.

Tobacco use is one of the most important causes of morbidity and mortality. As a consequence, quitting smoking is a fundamental aim of primary prevention of diseases. Diagnosing a smoker or his/her relative with cancer may result in increased motivation for quitting. This study aimed to evaluate the quitting wills and behaviors of first degree relatives of patients diagnosed with cancer, and also to assess whether the quit rates are different between primary caregivers and other first-degree relatives. A total of 176 relatives of patients diagnosed with cancer between July 2017-December 2017 at Hacettepe University Medical Oncology Outpatient Unit were recruited for the study. Mean age of the participants was 42.4±11.6 years, 54 were females (30.7%), and 122 were males (69.3%). The evaluations on the 30th day after first interview revealed that 134 (76.1%) continued smoking, and 42 (23.9%) have quitted. Comparisons of 30th day smoking status between groups revealed that quitting rates were higher in females (p=0.006). Quitting rates were higher in primary caregivers than others (27% vs 17%, p=0.08), but this difference was not statistically significant. Quitting rate was higher in relatives of lung cancer patients (40.4%) than those with other cancers (16.9%) (p=0.001). In conclusion, we have observed that smokers were more motivated for quitting if one of their relatives are diagnosed with a serious illness. Higher quit rates might be achieved if these individuals are supported adequately. Tütün kullanımı en önemli morbidite ve mortalite nedenlerindendir. Bu nedenle sigara bıraktırma, hastalıkların primer korunmasında temel hedeflerden biridir. Sigara içen kişiye veya bir yakınına kanser tanısı konması, sigarayı bırakma konusunda motivasyon artışına neden olabilmektedir. Bu çalışmada kanser tanısı alan hastaların birinci derece yakınlarının bu süreçte sigara bırakma niyet ve davranışları incelenmiş ve tanı sonrası sigara bırakma oranlarının hastaya asıl bakım veren yakını ile diğer birinci derece yakınları arasında farklılık gösterip göstermediğinin araştırılması planlanmıştır. Çalışmaya Temmuz 2017 - Aralık 2017 tarihleri arasında Hacettepe Üniversitesi Medikal Onkoloji Polikliniği’ne başvuran kanser hastalarının yakınlarından 176 kişi dahil edildi. Çalışmaya katılanların yaş ortalaması 42,4±11,6 yıl, 54’ü (%30,7) kadın, 122’si (%69,3) erkek cinsiyette idi. Katılımcıların başlangıç görüşmesinden sonraki 30. gün yapılan telefon görüşmelerinde 134’ünün (%76,1) sigara içmeye devam ettiği, 42’sinin (%23,9) ise sigarayı bıraktığı tespit edildi. Katılımcıların 30. gün sigara bırakma durumları demografik gruplar arasında karşılaştırıldığında kadınların sigara bırakma oranlarının anlamlı derecede daha yüksek olduğu (p=0,006) görüldü. Hastaya asıl bakım veren hasta yakınlarında sigarayı bırakma oranları, diğer hasta yakınlarından daha yüksek bulundu (%27 vs %17, p=0,08) ancak aradaki fark istatistiksel anlamlılığa ulaşmadı. Yakınlarına akciğer kanseri tanısı konan katılımcıların sigara bırakma oranlarının (%40,4) diğer kanser türlerine göre (%16,9) anlamlı derecede yüksek olduğu (p=0,001) tespit edildi. Sonuç olarak sigara içen kişilerin yakınlarında kanser gibi ciddi bir hastalık ortaya çıktığı durumlarda sigara bırakma konusunu daha motive oldukları görülmüştür. Bu kişilere gerekli destek sağlandığında daha yüksek sigara bırakma oranları sağlanabilir.

According to the Turkish Health Ministry 2010 Statistics, Non-Hodgkin lymphoma (NHL) incidence is 5.2/100000 in women 7.7 in men. Although diffuse large B-cell lymphoma (DLBCL) accounts for majority of cases; little known about real incidence, clinical and demographical data and prognosis of this prevalent NHL subtype. The aim of this study is to summarize the demographics of DLBCL patients of past 16 years and to investigate histopathologic characteristics from the pathology department of a tertiary reference hospital of Turkey. A total of 939 cases who were diagnosed in or referred to University of Hacettepe between 1/2000 and 11/2015 were included. Mean age of patients was 54±17; 56.2% of patients were males and 43.8% were females. It was found that 15.8% of patients were diagnosed at stage I; 23.4% patients Stage II; 21.1% were in stage III and 39.7% were diagnosed in stage IV. There was no differences in groups of stages when compared according to gender. When categorized according to the organs of involvement, it was found that they were diagnosed most frequently from the lymph nodes (39.9%) followed by the gastrointestinal system (17.3%). When gender differences was examined according to extranodal involvement, no difference was found (p = 0,18). GIS, CNS and bone involvement were the most common sites of EN involvement. There were no gender predominance in these involvement areas (0,33). Bone marrow involvement was not related to DBBHL subgroup according to Hans criteria (p = 0.76). Median overall survival is 94±12.3 months. At 5 years, 93.2% of patients were alive and at 10 years 85.2% of patients were alive. DLBCL is the most common subtype of NHL in our center with distinctive features. Although the presented results of this study was valid for single center experience, the pathology department of Hacettepe University is a tertiary reference center who get consultation around Turkey. Hence, these results might be extrapolated to whole Turkey. Diffüz büyük B-hücreli lenfoma (DBBHL) vakaların çoğunu oluştursa da gerçek insidans, klinik ve demografik veriler ile prognoz hakkında bilgi kısıtlıdır. Bu çalışmanın amacı, son 16 yılda Türkiye'deki bir referans hastanenin patoloji anabilim dalına olan başvuruların retrospektif olarak incelenmesi ile DBBHL hastalarının demografik ve histopatolojik özellikleri araştırmaktır. Bu amaçla çalışmaya 1/2000 ve 12/2015 arasında patoloji bilim dalında tanı alan 939 vaka dahil edilmiştir. Yaş ortalaması 54±17 olarak saptandı. Olguların %43,8'i kadın (n=411); %56,2'si (n=528) erkekti. Ann-Arbor evrelerine göre sınıflandırıldığında %15,8 hasta Evre I; %23,4 hasta Evre II; %21,1 hasta Evre III ve %39,7 hasta Evre IV ile tanı aldıkları tespit edildi. Bu evre grupları arasında cinsiyet açısından farklılık saptanmadı. Vakaların en sık lenf nodundan tanı aldıkları (%39,9), bunu gastrointestinal (GI) sistemin (%17,3) takip ettiği tespit edilmiştir. Ekstranodal tutulumu olanlarda kadın/erkek oranı bakımından fark olmadığı saptandı (p=0,18). En sık EN tutulum alanı olan GI sistem, santral sinir sistemi ile kemik tutulumlu vakaların dağılımı incelendiğinde cinsiyet açısından farklılık olmadığı görüldü (p=0,33). Kemik iliği tutulumunun DBBHL subgrubu ile ilişkili olmadığı sonucu tespit edildi (p=0,76).Medyan genel sağkalım süresi 94 ay (Standart hata: 12,3; %95 GA 69,9-118,1) idi. DBBHL merkezimizde tanı alan en sık lenfoma türüdür ve farklı histomorfolojik özelliklere sahiptir. HÜTF Patoloji AD tek bir merkez olsa da tüm Türkiye'den hasta konsülte edilen tersiyer bir ünite olması; vaka sayısı, dağılımı ve heterojenitesi nedeniyle bu araştırmanın sonuçları ülke genelini yansıtmakta önem taşımaktadır.

Previous studies have noted possible associations between periodontitis and the risk of various cancers, particularly head and neck cancers, esophageal cancer, lung cancer, pancreatic cancer, breast cancer and non-Hodgkin lymphoma. However, the differences in the design of such studies with respect to the definition of periodontal disease (self-reported data by the patients vs. dentist diagnosed), adjustment for confounding factors, and constitution of study groups lead to variations in the results. We herein assessed cancer risk in a cohort of patients with severe periodontitis and we compared the age- and gender-standardized incidence rates with corresponding figures from Turkish National Cancer Registry (TNCR). Patients diagnosed with severe periodontitis by an experienced periodontist team between 2002 and 2009 were identified from the hospital registry. Data on the diagnosis of any cancer after periodontitis were collected from patient files and oncology hospital registries where available. Patients younger than 35 years of age and those with a prior cancer diagnosis were excluded. TNCR 2013 data were used for age- and gender-specific incidence rates. The standardized incidence ratio (SIR) was calculated by dividing the number of observed cases by the number of expected cases. A total of 280 patients with severe periodontitis were included. Median age was 49.6 and 54% of the cases were females. Median follow-up was 12 years. Twenty-five new cancer cases were observed on follow-up. The most common cancer was breast cancer in females (5/13 patients) and prostate, lung and hematological cancers (each in 3/12 patients) in males. Patients with periodontitis had 77% increased risk of cancer compared with the expected counts at the corresponding age and sex group. (SIR 1.77, 95% CI 1.17-2.58, p=0.004). Women with periodontitis had significantly higher risk of breast cancer (SIR 2.4, 95% CI 0.88-5.33) and men with periodontitis had significantly higher risk of prostate cancer (SIR 3.75, 95% CI 0.95-10.21) and hematological cancers (SIR 6.97, 95% CI 1.77-18.98). Although showing a casual association necessitates further investigation, our results support that severe periodontitis increased the risk of hematological cancer, breast cancer, and prostate cancer. Bu kanserlerin başında baş-boyun kanserleri, özefagus kanseri, akciğer, pankreas ve meme kanserleri ve non-Hodgkin lenfoma gelmektedir. Ancak bu konuda yapılmış çalışmaların sonuçları çelişkilidir çünkü çalışmalardaki periodontal hastalık tanı kriterleri (hastanın kendi beyanı ya da diş hekimi tanılı periodontal hastalık), eşlik eden diğer faktörlerin analizi gibi metodolojik farklılıklar sonuçlardaki bu çelişkilere neden olabilmektedir. Bu çalışmada ileri derece periodontitisi olan hastalarda kanser sıklığı belirlendi ve elde edilen veriler Türk Kanser Kayıt Veri Tabanı 2013 yılı verileri ile karşılaştırılarak bu hastaların kanser riskinin genel topluma kıyasla artmış olup olmadığı araştırıldı. Çalışmamıza 2002-2009 yılları arasında Hacettepe Üniversitesi Diş Hekimliği Fakültesi Periodontoloji Anabilim Dalı’nda, tecrübeli bir periodontist tarafından orta-ileri derece periodontitis tanısı konmuş olan hastalar dahil edildi. Periodontitis tanısı aldıktan sonra takip sürecinde hastanın kanser tanısı alıp almadığı hasta dosyasından ve hastane otomasyon sisteminden araştırıldı. Periodontitis tanısından önce kanser tanısı almış olanlar ve 35 yaşın altındaki hastalar çalışmaya dahil edilmedi. Yaş ve cinsiyete spesifik kanser insidansları Türk Kanser Kayıt Veritabanı 2013 yılı verilerinden elde edildi. Standardize insidans oranı (SIR), belirli bir yaş ve cinsiyet grubunda gözlenen kanserli olgu sayısının, beklenen sayıya bölünmesiyle hesaplandı. Orta ileri derece periodontitisi olan 280 hasta çalışmaya dahil edildi. Hastaların %54’ü kadındı ve ortanca yaşı 49,6 idi. Ortanca takip süresi 12 yıldı. İzlemde 25 yeni kanser olgusu tespit edildi. En sık gözlenen kanserler kadınlarda meme kanseri (13 olgunun 5’i), erkeklerde ise prostat, akciğer ve hematolojik kanserler (her bir kanser 12 olgunun 3’ünde) olarak bulundu. Periodontitisi olan hastaların kanser riski benzer yaş ve cinsiyet gruplarında kıyaslandığında genel toplum verilerine kıyasla %77 daha yüksek bulundu (SIR 1,77, 95% CI 1,17-2,58, p=0,004). Periodontitisi olan kadınlarda meme kanseri riski anlamlı olarak daha yüksek bulundu (SIR 2,4, 95% CI 0,88-5,33). Yine erkeklerde prostat kanseri (SIR 3.75, 95% CI 0,95-10,21) ve hematolojik kanser riskleri (SIR 6,97, 95% CI 1,77-18,98) benzer yaş ve cinsiyet gruplarındaki genel toplum verilerine kıyasla daha yüksek bulundu. WOS: 000388274800006, The polyamines are essential for rapidly growing cells. Polyamines is synthesized by two reactions which begin from arginine. The aim of this study is to examine the serum levels of the enzymes located in the putrescine synthesis pathway and their relationship with lung cancer. In addition, the study also aim determine the oxidant-antioxidant status and the oxidative stress index. This study was performed on 36 lung cancer patients and 36 healthy controls. The arginase activity and the ornithine level were determined spectrophotometrically while the arginine decarboxylase, ornithine decarboxylase and agmatinase levels were determined by using Enzyme Linked Immunosorbent Assay (Elisa kit). The total antioxidant (TAS) and total oxidant status (TOS) were determined using a commercial kit auto analyser, oxidative stress index (OSI) was subsequently estimated. We observed an increase in ornithine decarboxylase, agmatinase and arginase levels in cancer patients (p< 0.05). however, no significant difference was observed between the two groups with regards to orithine and arginase decarboxylase levels (p> 0.05). In the patient group, the TAS values decreased (p< 0.05), while the TOS values increased (p< 0.05). In addition, the OSI values increased (p< 0.05). Reduced agmatine level can cause an increase in polyamine synthesis and cell proliferation. WOS: 000319980200084, PubMed ID: 23534777, Background: In this study we aimed to determine the rate and habitual patterns of smoking, intentions of cessation, dependence levels and sociodemographic characteristics of relatives of patients with a diagnosis of cancer. Materials and Methods: This study was designed by the Turkish Oncology Group, Epidemiology and Prevention Subgroup. The relatives of cancer patients were asked to fill a questionnaire and Fagerstrom test of nicotine dependence. Results: The median ages of those with lower and higher Fagerstrom scores were 40 years and 42 years, respectively. We found no evidence of variation between the two groups for the remaining sociodemographic variables, including the subject's medical status, gender, living in the same house with the patient, their educational status, their family income, closeness to their cancer patients or spending time with them or getting any help or wanting to get some help. Only 2% of the subjects started smoking after cancer was diagnosed in their loved ones and almost 20% of subjects had quit smoking during the previous year. Conclusions: The Fagerstrom score is helpful in determining who would be the most likely to benefit from a cigarette smoking cessation program. Identification of these people with proper screening methods might help us to pinpoint who would benefit most from these programs.

Aortic aneurysms (AA) are characterized by structural deterioration leading to progressive dilation. During the development of AA, two key structural changes are pronounced, one being degradation of extracellular matrix and the other loss of smooth muscle cells (SMCs) through apoptosis. Reactive oxygen species (ROS) are produced above physiological levels in dilated (aneurismal) part of the aorta compared to the nondilated part and they are known to be associated with both the extracellular matrix degradation and the loss of SMCs. In this study, we hypothesized that aneurismal SMCs are more prone to apoptosis and that at least some cells undergo apoptosis due to elevated ROS in the aortic wall. To test this hypothesis, we first isolated SMCs from thoracic aneurismal tissue and compared their apoptotic tendency with normal SMCs in response to H(2)O(2), oxidized sterol, or UV treatment. Exposed cells exhibited morphological changes characteristic of apoptosis, such as cell shrinkage, membrane blebbing, chromatin condensation, and DNA fragmentation. Terminal deoxynucleotidyl transferased UTP nick end labeling (TUNEL) further confirmed the fragmentation of nuclear DNA in these cells. Vascular SMCs were analyzed for their micronuclei (MN) and binucleate (BN) frequency as indicators of genomic abnormality. These data were then compared to patient parameters, including age, gender, hypertension, or aortic diameter for existing correlations. While the tendency for apoptosis was not significantly different compared to normal cells, both the %MN and %BN were higher in aneurismal SMCs. The data suggest that there is increased DNA damage in TAA samples, which might play a pivotal role in disease development.

Background: Current understanding of hemostatic systems enables us to better explore the enigmatic pathobiology of tamoxifen (TAM)-induced thrombotic diathesis. We have therefore aimed to assess the hemostatic changes in breast cancer patients receiving TAM on an adjuvant basis. Patients and methods: The study population consisted of 43 female patients with hormone receptor-positive breast cancer who received TAM 20 mg/day as part of their adjuvant treatment. Mean age was 52 ± 12 years (range 25 – 74). Twenty-one patients (49%) were premenopausal. Plasma samples were collected prior to and following 6 months of TAM therapy and were assayed for total tissue factor pathway inhibitor (TFPI), free TFPI, lipid-bound TFPI, thrombomodulin, D dimer, activated protein C resistance (APC res), factors VIIa, II, V, VII and X, and global fibrinolytic capacity (GFC). Results: Median total TFPI decreased significantly from 48.5 ng/ml to 36.2 ng/ml (P = 0.001), free TFPI from 10 to 7.6 ng/ml (P = 0.001) and lipid-bound TFPI from 39.1 to 28.7 ng/ml (P = 0.001). There were significant decreases in the levels of factor II (P = 0.03), factor V (P = 0.001), factor VII (P = 0.06), thrombomodulin (P = 0.01) and D dimer (P = 0.001). However, APC res times were significantly prolonged (P = 0.04). The remaining parameters that we have studied were not significantly affected. Conclusion: Our findings suggest that TAM tends to activate the coagulation pathway by counteracting major molecules involved in coagulation inhibition, namely TFPI and TM. As reflected by unchanged GFC, the drug appears to impair the expected compensatory activation of the fibrinolytic system, which removes fibrin polymers resulting from coagulation activation.
