Abstract

Background and Purpose- Pilot trials suggest that glyceryl trinitrate (GTN; nitroglycerin) may improve outcome when administered early after stroke onset. Methods- We undertook a multicentre, paramedic-delivered, ambulance-based, prospective randomized, sham-controlled, blinded-end point trial in adults with presumed stroke within 4 hours of ictus. Participants received transdermal GTN (5 mg) or a sham dressing (1:1) in the ambulance and then daily for three days in hospital. The primary outcome was the 7-level modified Rankin Scale at 90 days assessed by central telephone treatment-blinded follow-up. This prespecified subgroup analysis focuses on participants with an intracerebral hemorrhage as their index event. Analyses are intention-to-treat. Results- Of 1149 participants with presumed stroke, 145 (13%; GTN, 74; sham, 71) had an intracerebral hemorrhage: time from onset to randomization median, 74 minutes (interquartile range, 45-110). By admission to hospital, blood pressure tended to be lower with GTN as compared with sham: mean, 4.4/3.5 mm Hg. The modified Rankin Scale score at 90 days was nonsignificantly higher in the GTN group: adjusted common odds ratio for poor outcome, 1.87 (95% CI, 0.98-3.57). A prespecified global analysis of 5 clinical outcomes (dependency, disability, cognition, quality of life, and mood) was worse with GTN; Mann-Whitney difference, 0.18 (95% CI, 0.01-0.35; Wei-Lachin test). GTN was associated with larger hematoma and growth, and more mass effect and midline shift on neuroimaging, and altered use of hospital resources. Death in hospital but not at day 90 was increased with GTN. There were no significant between-group differences in serious adverse events. Conclusions- Prehospital treatment with GTN worsened outcomes in patients with intracerebral hemorrhage. Since these results could relate to the play of chance, confounding, or a true effect of GTN, further randomized evidence on the use of vasodilators in ultra-acute intracerebral hemorrhage is needed. 18. An Accelerated Dual Proximal Gradient Method for Applications in Viscoplasticity

Abstract

We present a very simple and fast algorithm for the numerical solution of viscoplastic flow problems without prior regularisation. Compared to the widespread alternating direction method of multipliers (ADMM / ALG2), the new method features three key advantages: firstly, it accelerates the worst-case convergence rate from $O(1/\sqrt{k})$ to $O(1/k)$, where $k$ is the iteration counter. Secondly, even for nonlinear constitutive models like those of Casson or Herschel-Bulkley, no nonlinear systems of equations have to be solved in the subproblems of the algorithm. Thirdly, there is no need to augment the Lagrangian, which eliminates the difficulty of choosing a penalty parameter heuristically. In this paper, we transform the usual velocity-based formulation of viscoplastic flow problems to a dual formulation in terms of the stress. For the numerical solution of this dual problem we apply FISTA, an accelerated first-order optimisation algorithm from the class of so-called proximal gradient methods. Finally, we conduct a series of numerical experiments, focussing on stationary flow in two-dimensional square cavities. Our results confirm that Algorithm FISTA*, the new dual-based FISTA, outperforms state-of-the-art algorithms such as ADMM / ALG2 by several orders of magnitude. We demonstrate how this speedup can be exploited to identify the free boundary between yielded and unyielded regions with previously unknown accuracy. Since the accelerated algorithm relies solely on Stokes-type subproblems and nonlinear function evaluations, existing code based on augmented Lagrangians would require only few minor adaptations to obtain an implementation of FISTA*.

Abstract

We present a new approach to the problem of stationary viscoplastic duct flow as modelled by the Herschel-Bulkley model, with Bingham fluids included as a special case. While the mathematical formulation of this problem is conventionally based on a variational inequality, or equivalently, on a nonsmooth minimisation problem for the flow velocity, we suggest an alternative approach. Considering the Lagrangian dual in terms of the stress, rather than the velocity, turns out to be advantageous in numerous ways. The objective functional possesses higher regularity, which ensures applicability of second order methods. Our numerical experiments with a trust-region SQP algorithm also demonstrate clearly superior performance compared to the widely used augmented Lagrangian method, although no artificial regularisation is introduced into the problem. Hence, besides providing a new theoretical angle to a classical problem, our results also pave the way for an entirely new class of numerical approaches to simulating flows of viscoplastic fluids.

46. A Single Case Study of the Impact of Policy Changes on Identification for Gifted Programs

Abstract

The annals of gifted education research contain few educational policy studies and even fewer studies on the impacts of changes in policy. To partially fill this gap, the authors performed an ABA study investigating the impact of accountability legislation on the number of students reported gifted in Texas public schools. Data were collected from the 1999-2000 school year to 2012-2013 school year from a public educational database. Data analysis showed that when accountability legislation was in effect, more students were identified as gifted in Texas public schools. In addition, the percentage of campuses with few or no gifted students was lower when accountability systems were in place. Therefore, it seems likely that accountability legislation is an important component for gifted programs in public schools. The article concludes with a discussion of the limitations of this single case study.
