2. The Potential Role of Neutrophil Reactivity Intensity and Reactive Lymphocytes as Extended Hematological Parameters to Detect Active Systemic Lupus Erythematosus.

Abstract

Objectives: Systemic Lupus Erythematosus (SLE) is a chronic autoimmune disease characterized by the presence of autoantibodies against nuclear antigens and immune complex deposition. The pathogenesis of SLE is not fully understood; however, there are alterations in neutrophils and lymphocytes. Recent parameters assessing both neutrophil activations (Neut‐RI, Neut‐GI, IG) and activated lymphocytes (Re‐Lymp, As‐Lymp) can be used to assess the activation of immune cells. The aim of this study was to explore the potential role of this parameter in detecting active SLE. Methods: Cross‐sectional study using secondary data from Hasan Sadikin Lupus Registry. Parameters were examined using Sysmex XN‐1500. Lupus activity was determined according to SLEDAI‐2K. Results: This study included 30 active (SLEDAI 2K ≥ 4) and 30 inactive SLE patients. Compared to inactive SLE, active SLE showed significantly higher Neut‐RI (51.5 vs. 49.6 FL, p = 0.004), and lower Re‐Lymph (0.09 vs. 0.14 × 103/mm3, p = 0.024). There was no significant difference in Neut‐GI (150 vs. 151.6 SI, p = 0.359), As‐Lymp (0.02 vs. 0.01 × 103/mm3, p = 0.621), and IG (0.11 vs. 0.06 × 103/mm3, p = 0.384) between active and inactive SLE. Neut‐RI/Re‐Lymp ratio cutoff > 0.47 could distinguish between active and inactive SLE with AUC 0.689, 66.7% sensitivity, and 70.0% specificity. Conclusion: In active SLE, there is an elevation in Neut‐RI and a reduction in Re‐Lymp. Neut‐RI/Re‐Lymp ratio exceeding 0.47 could be used to detect active SLE. [ABSTRACT FROM AUTHOR]

Consumptive thrombocytopenia is also related to platelet activation. Elevation of sP-selectin and thrombocytopenia are related to COVID-19 complication and often correlated with severity of COVID-19.Purpose: Assess the correlation between sP-selectin and platelet in COVID-19 patients at intensive care and non-intensive care.Patients and Methods: The study population was hospitalized COVID-19 patients confirmed by Real-Time PCR that underwent platelet examination within 48 hours upon admission, divided into intensive care and non-intensive care group. sP-selectin examination using ELISA methods. Platelet cell count and sP-selectin divided based on normal reference range.Results: The subjects consist of 24 were in intensive care, 25 were in non-intensive care group. A 66.7% of subject in intensive care group has an elevation in sP-selection (> 44.0 ng/mL). Thrombocytopenia was significantly correlated with intensive group (r =− 0.32, p< 0.05). The combination of platelet count < 150.000/mm3 and sP-selectin > 44.0ng/mL was not correlated with the intensive and non-intensive group. Platelet and sP-selectin were not correlated with each other.Conclusion: Thrombocytopenia is able to induce the expression of sP-selectin.Keywords: COVID-19, sP-selectin, platelet, thrombocytopenia

Abstract

Thalassemia major is a red cell disorder caused by alpha or beta gene mutation. Those with thalassemia major need blood transfusion regularly in their whole life. In Indonesia, thalassemia major ranks fifth as a catastrophic disease in the National Health Insurance. Therefore, thalassemia major should be prevented by among others not marrying individual with thalassemia carrier when the partner also has a thalassemia carrier status. The aim of this activity was to raise awareness to the community in preventing thalassemia major by education, followed by screening thalassemia carrier free of charge. The method was by making the flyers viral through social media to the community about blood donation and thalassemia carrier screening. While waiting for blood donation, video about thalassemia was played. For those who were interested in thalassemia status screening, were further explained individually and the informed consent form was signed prior complete blood examination was conducted. In case of suspect thalassemia carrier, Hb-electrophoresis examination was performed. In total, 112 respondents came to Eijkman Building Bandung in 27 May 2021. There were two groups of respondents; community cluster and family cluster who had a family member with thalassemia major. Result showed that the proportion of thalassemia carrier among community and family clusters were 6.8% and 52.3%, respectively. To conclude, awareness about thalassemia needs to be further raised especially for those in the premarital period, therefore, blood examination for thalassemia carrier status needs to be well considered. Moreover, blood donation activity needs to be largely announced for good intentions in helping others, especially for thalassemia major. [ABSTRACT FROM AUTHOR]
