Appropriate use criteria (AUC) for the performance of outpatient transthoracic echocardiography are based on history, physical examination and electrocardiograms. Telehealth provides access to clinical care in the absence of a physical examination. We assessed application of AUC for performance of echocardiography following a telehealth visit. In this single center study, we retrospectively reviewed the charts of pediatric patients whose originally scheduled in person visits were converted to telehealth visits between March and May 2020. Patients were referred for echocardiogram by the telehealth cardiologist. Echocardiograms requested during the telehealth visit were retrospectively classified as appropriate [A], may be appropriate [M], or rarely appropriate [R] by two independent reviewers. Findings from the echocardiogram were classified as normal, abnormal, or abnormal and motivating treatment by the same two independent reviewers based upon results from the echocardiogram report and physician office visit note. Of the 441 patients seen by telehealth, interquartile range of 1.9-15.3 years, 114 (26%) patients were referred for an echocardiogram. Of the patients for whom echocardiograms were requested, 56/114 (49%) were new to cardiology. Echocardiograms were rated: A-85 (75%), M-5 (4%), and R-24 (21%). Of the 108 completed studies, 61 (56%) were normal, 44 (41%) were abnormal, and three (3%) were abnormal motivating treatment. Abnormal TTE findings were not detected in the R encounters compared with abnormal findings in 46/82 (56%) of the A completed studies. Seventy-one percent (15/21) of the R-rated studies were performed for murmur evaluation.

Background: Approximately 25% of Philadelphia's sexually transmitted infections (STI) cases are among teens aged 15-19. Furthermore, there are increased rates of teen pregnancy in Philadelphia compared to other Pennsylvania counties. Lack of comprehensive sexual health education can leave youth vulnerable to various STIs and teen pregnancy. Proud Teens of Philly (PTOP) is a comprehensive sexual health education program for youth aged 12-19 to promote healthy choices and ultimately reduce sexual health disparities in Philadelphia. Methods: Participants were recruited from a network of partner sites such as public and charter schools, churches, and community-based organizations to attend eight 1-hour virtual synchronous sessions. An evidence-based comprehensive curriculum, Making Proud Choices (MPC), which aims to reduce risky behaviors was utilized. Pre- and post-surveys that examine intention to have sex, perceived susceptibility to STIs/HIV, and attitudes toward sex, condom use, and STIs were administered to 246 youth. Paired samples t-tests were used to evaluate changes before and after the program. The World Health Organization declared the novel coronavirus, or COVID-19, a pandemic in March 2020. Given the severity of COVID-19, appropriate use criteria have been implemented for fetal echocardiography. Screening low risk pregnancies for critical congenital heart disease has typically been a shared responsibility by pediatric cardiologists, obstetricians, and maternal fetal medicine (MFM). Currently, many of the fetal echocardiograms for low risk pregnancies for critical congenital heart disease have been deferred or cancelled with the emphasis on suspected abnormalities by MFMs and obstetricians. In this review, we discuss the literature that has been the basis of screening of low risk pregnancies by pediatric cardiologists. A new approach to more widespread usage of fetal tele-echocardiography may play a large part during COVID-19 and may continue after the pandemic., Highlights • Appropriate use criteria for fetal echocardiography have been implemented during the COVID-19 pandemic. • Pediatric cardiologists have deferred fetal echo for low risk pregnancies, emphasizing those with suspected abnormalities. • Current fetal echo guidelines highlight maternal, familial, and fetal risk factors, and the associated incidence of CHD. • Fetal tele-echocardiography and telehealth consultation may enhance the ability to provide care during and beyond COVID-19.
