Las enfermedades trofoblásticas gestacionales forman un conjunto heterogéneo de patologías infrecuentes. Agrupan patologías benignas (principalmente las molas hidatiformes) y los tumores trofoblásticos gestacionales. Las molas son el resultado de una anomalía de fecundación que origina un exceso de cromosomas paternos. Los tumores trofoblásticos gestacionales son el resultado de una transformación neoplásica de células del trofoblasto de un embarazo, independientemente de su naturaleza: embarazo molar (lo más frecuente), aborto o embarazo normal. El reto del tratamiento de las enfermedades trofoblásticas debe responder a varios problemas: garantizar la mejor supervivencia preservando la fertilidad de estas mujeres en edad de procrear.

5. The use of optimal estimation techniques in the analysis of gas turbines

Abstract

This thesis discusses several methods that can be used to analyse gas turbines, based on an optimal estimation algorithm called the Kalman Filter. These techniques overcome the difficulties of more 'traditional' analysis methods, which can give misleading results because they do not explicitly consider the possibility of measurement error. An enhancement to the Kalman Filter (the 'Concentrator') is presented, which overcomes the Kalman Filter's tendency to 'smear' the effects of genuine changes in a small number of component changes and/or sensor biasses over the whole set of changes and biasses being considered. To complement this, methods of optimising some of the statistical inputs to the Kalman Filter in order to improve the ability of the 'Concentrator' to carry out the required analysis are discussed. These are based on analytical methods developed to determine the sensitivity of the Kalman Filter to its inputs. Techniques are also presented for determining the gas-path measurements in a gas turbine that are needed to enable the required analysis of component changes and/or sensor biasses to be performed, including determination of both possible measurement redundancy and the ability of a set of measurements to successfully differentiate between all the component changes and sensor biasses being sought. A recursive algorithm for time series analysis (the Smoothing/Trending Algorithm) is also presented. This produces, for each point in a time series, best estimates of the underlying levels and trends (rates of change of level) of the process generating the observations. A method of combining the 'Concentrator' and the Smoothing/Trending Algorithm is also presented, which reduces the effects of sensor noise on the analysis of component changes and sensor biasses from time series data. Many types of prime movers and process plant could be effectively analysed using the methods described in this thesis.

Abstract

Background. Data on durability of first-line antiretroviral therapy (ART) in children with human immunodeficiency virus (HIV) are limited. We assessed time to switch to second-line therapy in 16 European countries and Thailand.Methods. Children aged = 2 nucleoside reverse transcriptase inhibitors p[NRTIs] plus nonnucleoside reverse transcriptase inhibitor p[NNRTI] or boosted protease inhibitor p[PI]) were included. Switch to second-line was defined as (i) change across drug class (PI to NNRTI or vice versa) or within PI class plus change of >= 1 NRTI; (ii) change from single to dual PI; or (iii) addition of a new drug class. Cumulative incidence of switch was calculated with death and loss to follow-up as competing risks.Results. Of 3668 children included, median age at ART initiation was 6.1 (interquartile range (IQR), 1.7-10.5) years. Initial regimens were 32% PI based, 34% nevirapine (NVP) based, and 33% efavirenz based. Median duration of follow-up was 5.4 (IQR, 2.9-8.3) years. Cumulative incidence of switch at 5 years was 21% (95% confidence interval, 20%-23%), with significant regional variations. Median time to switch was 30 (IQR, 16-58) months; two-thirds of switches were related to treatment failure. In multivariable analysis, older age, severe immunosuppression and higher viral load (VL) at ART start, and NVP-based initial regimens were associated with increased risk of switch.Conclusions. One in 5 children switched to a second-line regimen by 5 years of ART, with two-thirds failure related. Advanced HIV, older age, and NVP-based regimens were associated with increased risk of switch.

Abstract

Flash steam heating efficiently pasteurizes beef frankfurter surfaces, with little change in the color or weight of the final product. The process is a high temperature-short time process, executed at 136 degrees Celsius for 32 seconds. Subsequent long-term storage at 6 and 19 degrees Celsius did not reveal any elevation in bacterial levels.

Abstract

Background Once-daily dosing of abacavir and lamivudine has been approved for adults, but paediatric data are insufficient. We conducted a pharmacokinetic study of once-daily and twice-daily abacavir and lamivudine in children aged 3–Methods Children with stable HIV type-1 (HIV-1) RNA levels after 12 weeks treatment with twice-daily abacavir (8 mg/kg) with or without lamivudine (4 mg/kg) underwent plasma pharmacokinetic sampling. Children then switched to once-daily abacavir (16 mg/kg) with or without lamivudine (8 mg/kg), and sampling was repeated 4 weeks later. The area under the plasma concentration– time curve over 24 h (AUC0–24) and the maximum concentration (Cmax) were compared using geometric mean ratios (GMRs); 90% confidence intervals (CIs) within the range of 0.80–1.25 were considered bioequivalent. Results A total of 18 children (4, 6 and 8 in the 3–0–24, once-daily versus twice-daily, was 1.07 (90% CI 0.92–1.23) for abacavir and 0.91 (90% CI 0.79–1.06) for lamivudine. Cmax almost doubled on once-daily versus twice-daily dosing: abacavir and lamivudine GMRs were 2.04 (90% CI 1.73–2.42) and 1.78 (90% CI 1.52–2.09), respectively. At baseline, 12, 24 and 48 weeks, 89%, 94%, 100% and 89% of children had HIV-1 RNAConclusions Bioequivalence was demonstrated on AUC0–24 between twice-daily and once-daily abacavir; very similar AUC0–24 values were seen for twice-daily and once-daily lamivudine. Given that viral load suppression rates were maintained, these data suggest that once-daily abacavir and lamivudine might be an option for children aged 3–

Abstract

Pharmacists are in an ideal position to assess, monitor and treat adherence-related problems that can adversely affect patients' health outcomes. To accomplish these goals, pharmacists must accept the responsibilities and challenges of a primary care provider. They also must assume an interdisciplinary role in collaborative drug therapy management. Strategies to monitor and improve adherence are key components of pharmaceutical care plans, especially for patients with chronic diseases, such as hypertension, diabetes mellitus and atherosclerotic heart disease. This article gives an overview of guidelines, recommendations, current practices and related issues in the management of patients with diabetes mellitus. It also reviews the behavioral and social factors that influence adherence to therapeutic and lifestyle regimens, and highlights special needs in selected high-risk populations. Finally, best practice strategies that could serve as appropriate models for pharmaceutical care services are discussed. The overall goal is to enhance pharmacists' professional abilities to coordinate pharmaceutical care services targeted for major modifiable behavioral and biological risk factors. Pharmacists can overcome their apprehension about undertaking a primary care role in diabetes management through adequate preparation (including training and certification). The primary care functions that have been evaluated to date in the care of patients with diabetes mellitus by pharmacists show successful patient outcomes in terms of cost, quality of life and reduction of complications. Abstract

ABSTRACTServices for elders with severe mental illness (SMI) have major deficiencies, among them a lack of adequate psychosocial services. Some analysts have attributed this situation to ?double stigmatization? targeting both ageing and mental illness in our societies. Using qualitative methods (23 semi-directed interviews, theme-based content analysis), our exploratory research aims to understand better the perceptions of psychosocial practitioners working in community and institutional settings about the elderly with SMI and their living situations. Our informants evoke living situations marked by a lack of support (isolation), of resources (financial precariousness/poverty) and of power (learned passivity), traits that are related not only to mental illness per se, but also to long term psychiatric institutionalization. For them, the current situation of elders with SMI is the end product of biographies in which life-course, illness-course and life in services and/or institutions join and, sometimes, become indistinguishable. Implications for psychosocial practices are discussed.
