Wstęp. Współcześnie odnotowuje się wyraźny wzrost częstości występowania zaburzeń czynnościowych układu stomatognatycznego. Ma to zapewne związek ze wrastająca potrzebą dbałości o swoje zdrowie, ale także może wynikać z rzeczywistego wzrostu częstości występowania tej patologii. Duża aktualność problematyki skłoniła autorów do podjęcia badań własnych. Cel pracy. Celem badań była ocena częstości występowanie objawów bruksizmu wśród studentów krakowskich uczelni wyż-szych. Materiał i metody. Grupę badaną stanowiło 277 osób (172 kobie-ty i 105 mężczyzn) w wieku 19 - 29 lat, studentów krakowskich uczelni. Wykorzystano autorską ankietę oraz przeprowadzono krótkie badanie fizykalne. Wyniki i wnioski. Najczęściej odczuwaną dolegliwością wśród badanych był ból głowy. Zgłaszało go 80% respondentów, następnie zaburzony tor ruchu odwodzenia żuchwy, który rejestrowano u 78% badanych, trzaski w stawie skroniowo- żuchwowym, które podawało 50% badanych: obecność starć zębów i brak symetrii napięcia mięśni żwaczy , które stwierdzano z jednakową częstością u 38% badanych. Szumy uszne dotyczyły 22,74% badanych. Zaburzony tor odwodzenia żuchwy występował u 77,98% chorych z dysfuncją narządu żucia . Wnioski. Jednym z wiodących , choć niecharakterystycznych objawów dysfuncji narządu żucia jest ból. Chorym z zaburzeniami układu stomatognatycznego towarzyszyć mogą różnorodne objawy otologiczne występujące w dużym przedziale częstości. Introduction. Nowadays, there is a clear increase in the incidence of functional disorders of the stomatognathic system. This is probably related to the growing need to care for your health, but it also may result from a real increase in the incidence of this pathology. High relevance of the issues, prompted the authors to undertake their own research. Aim of the study. The aim of the study was to assess the frequency of the occurrence of bruxism symptoms among students of Krakow’s universities. Material and methods. The study group consisted of 277 people (172 women and 105 men) aged 19 – 29 who were students of Krakow’s universities. An original questionnaire was used and a short physical examination was carried out. Results and conclusions. The most frequently felt ailment among the subjects was headache. It was reported by 80% of respondents, followed by a disturbed mandibular movement pathway, which was recorded in 78% of respondents, cracks in the temporo-mandibular joint, which were given by 50% of subjects: presence of tooth clashes and lack of symmetry of masseter muscles, which were found at the same frequency in 38 % of respondents. Tinnitus related to 22.74% of the subjects. A disturbed mandibu-lar disorder pathway occurred in 77.98% of patients with masticatory system dysfunction.

Wstęp. Do istotnych elementów pracy personelu ZRM należy zarówno sporządzanie elektronicznej dokumentacji medycznej (składającej się na elektroniczny rekord pacjenta), jak i udostępnianie zawartych w niej danych, co możliwe jest dzięki wdrożeniu rozwiązań telemedycznych, takich jak system teletransmisji EKG. Ponieważ opinia pracowników może warunkować wykorzystywanie wdrożonych systemów, duże znaczenie mają poglądy personelu nt. przydatności opracowanych rozwiązań oraz wynikających z nich korzyści. Cel pracy. Celem pracy było ustalenie opinii członków ZRM (P oraz S) dotyczącej funkcjonowania systemu teletransmisji EKG. Materiał i metoda. Na potrzeby badania (przeprowadzonego metodą sondażu diagnostycznego) przygotowano kwestionariusz ankiety składający się z 18 pytań. W badaniu udział wzięło 115 członków ZRM pracujących w KPR (w tym 82 ratowników medycznych, 21 pielęgniarek oraz 12 lekarzy). Wyniki. Wykonywanie teletransmisji EKG zadeklarowało 93,91% (n=108) respondentów. Ze stwierdzeniem, że bezpośredni transport pacjenta do OKI znacznie skraca czas rozpoczęcia inwazyjnego leczenia w porównaniu do przewiezienia chorego do najbliższego SOR zdecydowanie lub raczej zgadza się 99,13% ankietowanych. 87,83% (n=101) respondentów jest zdecydowanie lub raczej zadowolonych z możliwości wykonywania teletransmisji EKG. Wdrożony system oceniony został jako zdecydowanie lub raczej przydatny przez 93,91% członków ZRM. Wśród respondentów 91,30% osób jest zdecydowanie lub raczej pozytywnie nastawionych wobec wdrażania oraz rozwijania w systemie PRM rozwiązań telemedycznych. Wnioski. Ratownicy medyczni i pielęgniarki znamiennie statystycznie częściej deklarują wykonywanie teletransmisji ECG podczas pracy zawodowej w porównaniu do lekarzy. Ratownicy medyczni i pielęgniarki istotnie statystycznie częściej postrzegają system teletransmisji ECG jako przydatny w opiece nad pacjentem z podejrzeniem Ostrego Zespołu Wieńcowego w porównaniu do lekarzy. Ratownicy medyczni istotnie statystycznie częściej uważają, że wdrożenie systemu teletransmisji ECG wpływa na poprawę jakości pracy ZRM w porównaniu do lekarzy. Ratownicy medyczni znamiennie statystycznie częściej są zadowoleni z możliwości wykonywania teletransmisji ECG w porównaniu do lekarz. Introduction. Filling in the electronic medical documentation (which the electronic patient record is comprising of) and sharing data (which are included in documents) are important elements of Medical Rescue Team (MRT) staff’s work. That’s possible due to implementation telemedicine solutions like ECG teletransmission. The personnel’s opinions can affect the use of systems. That is the reason, the staff’s opinion on usefulness and benefits of systems is very important. The purpose of the study. The aim of the study is to establish the MRT (Basic: BMRT and Specialist: SMRT) staff’s opinion on the subject of functioning of ECG teletransmission system in Emergency Medical Service (EMS) in Cracow. Material and method. The survey questionnaire (which contains 18 questions) was prepared for the needs of the study (carried out by a method of diagnostic survey). 115 MRT employees of EMS in Cracow (82 paramedics, 21 nurses and 12 doctors) took part in the research. Results. 93,91% respondents declared making an ECG teletransmission. 99,13% participants definitely or rather agree with the statement that direct patient transport to the interventional cardiology unit significantly shortens the time of invasive treatment. 87,83% MRT members are definitely or rather satisfied with the possibility of using ECG teletransmission system. 93,91% MRT employees evaluate the implemented system as definitely or ra-ther useful. 91,30% respondents are definitely or rather positive about the implementing and developing the telemedicine in EMS in Poland. Conclusions. Medical rescuers and nurses significantly more often declare Electrocardiograpfy teletransmissions during their professional work than doctors. Medical rescuers and nurses significantly more often perceive the ECG teletransmission system as useful in the care of a patient with suspected Acute Coronary Syndrome compared to doctors. Medical rescuers are statistically signifi-cantly more likely to believe that the implementation of the ECG teletransmission system improves the quality of work of the Medical Rescue Team as compared to physicians. Medical rescuers are statistically significantly more often satisfied with the possibility of ECG teletransmission in comparison to physicians.

Testicular macrophages (TMs) are located in the interstitial tissue of male gonad. These phagocytic cells take part in forming the organ-specific functional blood-testis barrier and participate in the regulation of the local hormonal balance. In the present study, we isolated TMs from testicular tissues using previously described methods - mechanical (M-TMs) or enzymatic, by treatment with collagenase (E-TMs) and then we studied production by these cells of several cytokines and reactive oxygen intermediates (ROI’s). Similarly treated oil-induced peritoneal macrophages (PMs) were used as control cells. PMs had a higher baseline level of production of TNF-α, IL-6, IL-10 and IL-12 than M-TMs and collagenase treatment increased the production of these cytokines (except IL-12) by both cell populations. This effect was significantly more expressed in TMs. In contrast to PMs, TMs produced little ROI’s when stimulated by zymosan. We conclude that in the case of local inflammation in the testis, ROI-negative TMs do not contribute to the tissue damage and instead may direct the local immune response into humoral pathway.

Wstęp. Powszechnie podkreśla się, ze ryzyko rozwoju odleżyn jest związane z synergią wielu czynników. Jednym z nich jest na pewno choroba podstawowa będąca przyczyną hospitalizacji. Panuje powszechna zgoda, że chorzy hospitalizowani na oddziałach ortopedycznych stanowią grupę narażoną na ryzyko rozwoju odleżyn. To przekonanie skłoniło autorów do podjęcia badań własnych, których celem była ocena ryzyka i działań profilaktycznych zapobiegających powstaniu odleżyn u chorych hospitalizowanych na oddziale ortopedycznym. Materiał i metody. Przebadano 20 pielęgniarek pracujących na od-dziale ortopedyczno-traumatologicznym jednego ze szpitali warszawskich. Metodą badawczą był sondaż diagnostyczny wykonany techniką ankiety. Jako narzędzia badawczego użyto kwestionariusz ankiety. Przebadano także dokumentację pielęgniarską 40 pacjentów w zakresie ryzyka występowania odleżyn według skali Weterlow. Wyniki i wnioski. Na oddziale ortopedycznym istnieje bardzo wysokie zagrożenie występowania odleżyn sięgające 25%. Oddział jest przygotowany do pielęgnacji chorych. Wprowadzono standard profilaktyki odleżyn, prowadzona jest dokumentacja pielęgniarska, występowanie odleżyn odnotowane jest w karcie profilaktyki odleżyn oraz w raporcie pielęgniarskim. Prowadzona jest obserwacja ryzyka wy-stępowania odleżyn (skala Weterlow-15 odpowiedzi). Wyposażenie oddziału w sprzęt ułatwiający pielęgnację chorych jest niewystarczający. Za mało jest łóżek z automatyczną regulacją, zbyt mało materacy przeciwodleżynowych. Czynności, jakie podejmują pielęgniarki w celu zapobiegania odleżynom to: wczesne rozpoznanie pacjentów z grupy wysokiego ryzyka, stosowanie materacy przeciwodleżynowych, nacieranie spirytusem, oklepywanie i masaż, natłuszczanie skóry chorego, utrzymanie całego ciała w czystości, stosowanie udogodnień, prześcielanie łóżka. Introduction. It is generally underlined that the risk of bedsores is related to the synergy of many factors. One of them is certainly the basic illness constituting the cause of the hospitalization. It is a commonly acknowledged truth that the patients of orthopaedic wards are exposed to potential bedsores. This fact encouraged the authors to undertake their own research, the purpose of which was to assess the risk and preventive measures against bedsores of the orthopaedic patients. Materials and methods. A group of 20 nurses working at the orthopaedic-and-traumatological ward in one of the hospitals in Warsaw was interviewed. The research tool was a diagnostic poll in the form of a survey. Moreover, the nursing documentation of 40 patients was examined with the relation to bedsores (Waterlow score was utilized). Results and conclusion. Orthopaedic wards pose a significant risk of bedsores of 25%. The ward is prepared for admitting patients. The standards of bedsore preventive measures were introduced, nursing record is kept and any bedsores are recorded in the bedsore prevention chart as well as in the nursing report. The bedsore risk is constantly under observation (Waterlow score – 15 answers). The ward is insufficiently equipped as far as the handling of patients is concerned. Too few beds are fixed with autoadjustment mechanism; there is not enough antipressure sore mattresses. The measures taken against bedsores by the nurses are: early diagnosis of a highrisk patient, the use of anti-pressure sore mattress, rubbing in spirit, percussion and massages, lubrication the patient’s skin, keeping the patient’s body clean, the use of convenient measures and using new bedclothes regularly.

Przedstawili cechy charakterystyczne odleżyn, rolę czynników sprzyjających ich powstawaniu, oraz sposoby oceny odleżyn oraz leczenia. The authors discuss basic anatomical and physiological features of skin and subcutaneous tissue. They present the characteristic features of bedsores, the significance of the factors that contribute to them and the ways to assess and treat bedsores.

We have previously reported that T lymphocytes proliferating in vitro to the hapten trinitrochlorobenzene (TNCB) exhibit a very restricted V beta gene usage and response to TNCB is limited to T-cell receptors (TCR) composed of V beta 8.2 in combination with V alpha 3.2, V alpha 8 and V alpha 10. This paper investigates the role played by T lymphocytes expressing the V beta 8.2 gene segment in the contact sensitivity (CS) reaction to TNCB in the intact mouse and in its passive transfer into naive recipient mice. Mice injected with monoclonal antibodies to V beta 8 are unable to develop CS upon immunization with TNCB and 4-day TNCB-immune lymph node cells from mice that had been depleted in vivo or in vitro of V beta 8+ T lymphocytes fail to transfer CS. However, when separated V beta 8+ and V beta 8- cells were used for passive transfer, it was found that V beta 8+ T lymphocytes failed to transfer CS when given alone to recipient mice and a V beta 8- population was absolutely required. Further analysis revealed that within the V beta 8- population, T lymphocytes expressing the gamma delta TCR were fundamental to allow transfer of the CS reaction. These gamma delta cells were found to be antigen non-specific, genetically unrestricted and to rearrange the V gamma 3 gene segment. This indicates that transfer of the CS reaction requires cross-talk between V beta 8+ and gamma delta+ T lymphocytes, thus confirming our previous results obtained using TNCB-specific T-cell lines. Time-course experiments showed that V beta 8+ lymphocytes taken 4-24 days after immunization with TNCB were able to proliferate and produce interleukin-2 (IL-2) in response to the specific antigen in vitro. Similar time-course experiments were then undertaken using the passive transfer of the CS reaction system. The results obtained confirm that TNCB-specific V beta 8+ T lymphocytes are present in the lymph nodes of immunized mice from day 4 to day 24, and reveal that gamma delta+ T lymphocytes are active for a very short period of time, i.e. days 4 and 5 after immunization. In fact, TNCB-specific V beta 8+ cells are able to transfer CS when taken 4-24 days after immunization, providing the accompanying V beta 8- or gamma delta+ T lymphocyte are obtained 4 days after immunization. In contrast, injection of V beta 8+ T lymphocytes together with V beta 8- or gamma delta+ T lymphocytes that had been taken 2 or 6 days after immunization, failed to transfer significant CS into recipient mice. Taken together, our results confirm that cross-talk between V beta 8+ and gamma delta+ T lymphocytes is necessary for full development of the CS reaction and may explain why the CS reaction in the intact mouse lasts up to 21 days after immunization while the ability of immune lymph node cells to transfer CS is limited to days 4 and 5 after immunization.

C3H/HeN mice were immunized to induce contrasuppressor T-cell (Tcs) activity, splenic T cells from these mice were fused with the BW5147 thymoma, and the resulting hybridomas were tested for their ability to produce a contrasuppressor T-cell factor (TcsF). Nine TcsF-producing hybridomas were preliminarily identified by their ability to inhibit the effect of antigen-specific suppressor T-cell factor (TsF) on the adoptive transfer of contact sensitivity. One of these hybrids, AF5.C6, was cloned, the production of a contrasuppressor factor confirmed, and the high-titred TcsF produced by this cloned hybrid characterized. Hybridoma-derived TcsF is antigen-specific and specifically binds its antigen, but does not bear immunoglobulin (Ig) determinants. Thus, hybridoma-derived TcsF is serologically and functionally identical to an antigen-specific contrasuppressor factor for contact sensitivity, whose production from splenocyte cell cultures has previously been described. The generation of a hybridoma secreting a contrasuppressor factor identical to that produced by spleen cells significantly strengthens the hypothesis that the phenomenon of T-cell contrasuppression is mediated by a specific subset of cells whose activity is contrasuppressive. The further advantages of employing T-cell hybridomas for functional, biochemical and molecular genetic analyses of contrasuppression are also discussed.
