Abstract

All newborn infants of HIV-positive women should be tested for HIV and p24 antigen to determine if they have the rapidly progressive form of HIV infection. Researchers used data from the French Pediatric HIV Cohort, which is a network of 62 hospitals that report the results of clinical and laboratory tests on all infants born to HIV-positive mothers. A total of 1,386 children were included in the study, including 267 who had become infected. Sixty-three of the infected children developed a rapidly progressive form of the disease, which is characterized by opportunistic infections, recurrent bacterial infections, cancer and wasting syndrome within the first 12 months. Sixty-one percent of the children who developed the rapidly progressive form had been HIV positive at birth compared to 31% of those who did not. Twenty-five percent of the former group had p24 antigen in their blood compared to 5% of the latter group. Enlarged lymph nodes, liver and spleen and low CD4+ counts were also associated with the rapidly progressive form., Objective.--to identify clinical and laboratory parameters at birth that are associated with the rapidly progressive form of human immunodeficiency virus type 1 HIV-1) disease in children born to infected mothers. Abstract

The authors discuss their study on epidemiology of HIV-infection in Tunisia. They report that subtype B dominates in Tunisia, and suggest the probability that HIV-infection origin might have been imported and the importance of further investigation of potential diversification of HIV-1 isolates in Tunisia.

Abstract

Virus load in pregnancy and its relation to mother-to-child human immunodeficiency virus (HIV) transmission were studied prospectively. From 1989 to 1994, 320 HIV-infected women from 18 centers had plasma samples stored. Among women not receiving antiretroviral therapy, the polymerase chain reaction RNA level was 3.6 log at delivery, and 15% of women had levels below the detection limit. There was no variation during pregnancy. Women born in sub-Saharan Africa had lower RNA levels, although their CD4 cell distribution did not differ from that in other women. Among 236 evaluable children, 19% ± 5% were infected. Transmission occurred in 12% of cases (confidence interval, 5%–22%) with < 1000 copies/mL versus 29% ± 10% of those with >10,000 copies/mL (P < .02). Abstract

The heterogeneity of the envelope 2 (E2) gene of the hepatitis C virus (HCV) was involved in the sensitivity of HCV to interferon-alpha (IFN-alpha). To assess the factors leading to virus eradication by IFN-alpha, patients whose first treatment by IFN-alpha failed and who had virus eradication after a second treatment were studied. These patients were paired with subjects in whom both treatments failed. The phosphorylation homology domain of the E2 gene (E2-PHD) had no sequence variation between the two stages in both groups of patients. Therefore, this region has no clinical predictive value within a specific genotype. The hypervariable region 1 (HVR1) was analyzed by cloning and sequencing 20 clones per sample. Comparison of samples showed that the change in quasispecies induced by the first IFN-alpha therapy could be associated with virus elimination obtained after a second treatment. The greater proportion of nonsynonymous mutations that was noted before the second treatment in responders suggest that pretherapeutic immune response is a major factor determining virus elimination and that the immune status of these patients changed between the first and the second treatment., (Copyright 2002 Wiley-Liss, Inc.)
