Rationale: Idiopathic pulmonary fibrosis (IPF) has a dismal prognosis. Mesenchymal stromal cells (MSCs) have shown benefit in other inflammatory diseases. Objectives: To evaluate the safety and feasibility of endobronchial administration of bone marrow autologous MSCs (BM-MSC) in patients with mild-to-moderate IPF. Methods: A phase I multicentre clinical trial (ClinicalTrials.gov NCT01919827) with a single endobronchial administration of autologous adult BM-MSCs in patients diagnosed with mild-to-moderate IPF. In a first escalating-dose phase, three patients were included sequentially in three dose cohorts (10x10(6), 50x10(6) and 100x10(6) cells). In a second phase, nine patients received the highest tolerated dose. Follow-up with pulmonary function testing, 6-min walk test and St George's Respiratory Questionnaire was done at 1, 2, 3, 6 and 12 months, and with computed tomography at 3, 6 and 12 months. Results: 21 bone marrow samples were obtained from 17 patients. Three patients were excluded from treatment due to chromosome aberrations detected in MSCs after culture, and one patient died before treatment. Finally, 13 patients received the BM-MSC infusion. No treatment-related severe adverse events were observed during follow-up. Compared to baseline, the mean forced vital capacity showed an initial decline of 8.1% at 3 months. Background. To describe the feasibility of the posterior arch of C2 accepting two crossing screws in the Spanish population. Methods. One hundred and fifty patients who underwent a routine neck CT scan for non-cervical pathology were enrolled. Submillimeter slices (thickness 0.7 mm) every 0.4 mm were performed with a 64 multi-detector CT scan, which allows isometric measurements. We measured the length and height of the cortical and cancellous (endomedullar) region of the lamina and the spinous process, inclination, maximal screw length and spinolaminar angle. Results. The average (standard deviation) measurements of the lamina were: width of the left cortical 7.2 (1.5) mm, right cortical 6.9 (1.3) mm, width of the cancellous part of the left lamina 4.8 (1.5) mm, right side 4.6 (1.4) mm. The mean left cortical height was 13.0 (1.5) mm and 13.1 (1.6) mm for the right. The mean height of the cancellous part was 9.0 mm for both sides. The average measurements of the spinous process were: cortical length 15.7 (3.5) mm, endomedullar length 12.5 (3.9) mm; cortical height 11.9 (2.2) mm, endomedullar height 8.4 (2.1) mm; spinolaminar angle 49º (4); the maximum screw length 3.18 cm, and the inclination angle 143º. Conclusion. A CT scan with submillimeter slices is necessary in order to avoid malpositioning of the screws. The outer cortical measurements are 2 to 4 mm bigger than the endomedullar ones. Taking into account the dimensions of the spinous process, 24% of the population would not be candidates for this crossing screw technique. Fundamento. Describir la capacidad del arco posterior de C2 en población española para colocar dos tornillos cruzados translaminares. Método. Se reclutaron 150 pacientes a los que se les realizó un escáner del cuello por patología no cervical. Para el estudio se utilizó un 64 multi-detector TAC realizando cortes submilimétricos (0,7 mm de grosor) cada 0,4 mm, permitiendo obtener medidas isométricas. Se midieron anchura y altura cortical y endomedular de la lámina y de la espinosa, inclinación de la lámina, máxima longitud de tornillo y ángulo espinolaminar. Resultados. Las media (desviación estándar) de las medidas de la lámina fueron: anchura cortical izquierda 7.2 (1,5) mm, cortical derecho 6,9 (1,3) mm, anchura endomedular izquierda 4,8 (1,5) mm, derecha 4,6 (1,4) mm La altura media cortical izquierda fue 13,0 (1,5) mm y de 13,1 (1,6) mm para la derecha. La altura media endomedular fue de 9,0 mm en ambos lados. Las medidas medias de la espinosa fueron: longitud media cortical 15,7 (3,5) mm, longitud endomedular 12,5 (3,9) mm; altura cortical 11,9 (2,2) mm, altura endomedular de 8,4 (2,1) mm; ángulo espinolaminar 49º (4); la longitud máxima de tornillo 3,18 cm; y el ángulo de inclinación 143º. Conclusiones. Para evitar la colocación errónea de los tornillos es necesario un estudio de TAC con cortes submilimétricos. La diferencia entre las medidas corticales y endomedulares oscila entre 2 y 4 mm. Teniendo en cuenta las dimensiones de la espinosa, un 24% de la población no sería candidato a esta técnica de tornillos cruzados translaminares.

Conclusions: The endobronchial infusion of BM-MSCs did not cause immediate serious adverse events in IPF patients, but a relevant proportion of patients suffered clinical and/or functional progression. Genomic instability of BM-MSCs during culture found in three patients may be troublesome for the use of autologous MSCs in IPF patients.

Identification of risk factors for lung cancer can help in selecting patients who may benefit the most from smoking cessation interventions, early detection, or chemoprevention. OBJECTIVE: To evaluate whether the presence of emphysema on low-radiation-dose CT (LDCT) of the chest is an independent risk factor for lung cancer. METHODS: The study used data from a prospective cohort of 1,166 former and current smokers participating in a lung cancer screening study. All individuals underwent a baseline LDCT and spirometry followed by yearly repeat LDCT studies. The incidence density of lung cancer among patients with and without emphysema on LDCT was estimated. Stratified and multiple regression analyses were used to assess whether emphysema is an independent risk factor for lung cancer after adjusting for age, gender, smoking history, and the presence of airway obstruction on spirometry. RESULTS: On univariate analysis, the incidence density of lung cancer among individuals with and without emphysema on LDCT was 25.0 per 1,000 person-years and 7.5 per 1,000 person-years, respectively (risk ratio [RR], 3.33; 95% confidence interval [CI], 1.41 to 7.85). Emphysema was also associated with increased risk of lung cancer when the analysis was limited to individuals without airway obstruction on spirometry (RR, 4.33; 95% CI, 1.04 to 18.16). Multivariate analysis showed that the presence of emphysema (RR, 2.51; 95% CI, 1.01 to 6.23) on LDCT but not airway obstruction (RR, 2.10; 95% CI, 0.79 to 5.58) was associated with increased risk of lung cancer after adjusting for potential cofounders. CONCLUSIONS: Results suggest that the presence of emphysema on LDCT is an independent risk factor for lung cancer.

La tomografía computarizada multicorte (TCMC) permite estudiar las arterias coronarias de forma no invasiva. A pesar de la elevada resolución espacial y temporal de los equipos de sesenta y cuatro cortes (TCMC- 64) esta técnica no se encuentra exenta de limitaciones. La tomografía computarizada de doble fuente (TCDF) permite estudiar las arterias coronarias con gran calidad diagnóstica en todos los sujetos independien- temente de su frecuencia cardiaca sin necesidad, por tanto, de emplear betabloqueantes. En este trabajo se describen los estudios por TCDF de tres pacientes con elevada frecuencia cardiaca y gran variabilidad del ritmo. Se demuestra la utilidad de esta técnica para obtener estudios de excelente calidad diagnóstica en casos en que la coronariografía por TCMC-64 convencional presenta limitaciones. INGLÉS: Multidetector computed tomography (MDCT) has been demonstrated to be a very useful technique to non-invasively study coronary arteries. Despite the high spatial and temporal resolution of 64-slice MDCT scanners, this technique has several limitations. Dual-source computed tomography (DSCT) allows to study coronary arteries with excellent diagnostic quality in all subjects independent of the heart rate, thus avoiding the use of beta-blockers. In this article DSCT studies from three subjects with elevated heart rate and irregular heart rhythm are described. Usefulness of this technique to obtain studies of excellent quality in cases in which conventional 64-row-MDCT might present limitations is emphasized.
