The well known Plotkin construction is, in the current paper, generalized and used to yield new families of Z2Z4-additive codes, whose length, dimension as well as minimum distance are studied. These new constructions enable us to obtain families of Z2Z4-additive codes such that, under the Gray map, the corresponding binary codes have the same parameters and properties as the usual binary linear Reed-Muller codes. New quaternary Plotkin constructions are given and are used to obtain new families of quaternary codes. The parameters of the obtained codes, such as the length, the dimension and the minimum distance are studied. Using these constructions new families of quaternary Reed-Muller codes are built with the peculiarity that after using the Gray map the obtained Z4-linear codes have the same parameters and fundamental properties as the codes in the usual binary linear Reed-Muller family. A code ${\cal C}$ is $\Z_2\Z_4$-additive if the set of coordinates can be partitioned into two subsets $X$ and $Y$ such that the punctured code of ${\cal C}$ by deleting the coordinates outside $X$ (respectively, $Y$) is a binary linear code (respectively, a quaternary linear code). In this paper $\Z_2\Z_4$-additive codes are studied. Their corresponding binary images, via the Gray map, are $\Z_2\Z_4$-linear codes, which seem to be a very distinguished class of binary group codes. As for binary and quaternary linear codes, for these codes the fundamental parameters are found and standard forms for generator and parity check matrices are given. For this, the appropriate inner product is deduced and the concept of duality for $\Z_2\Z_4$-additive codes is defined. Moreover, the parameters of the dual codes are computed. Finally, some conditions for self-duality of $\Z_2\Z_4$-additive codes are given., Comment: This paper will be submitted to IEEE Trans. on Inform. Theory

Cette étude précise le fonctionnement des articulations morpho‑historiques entre la via Domitia et un réseau régulier et géométrique de limites agraires, totalement dépendant de son parcours rectiligne entre les oppida d’Ambrussum et de Sextantio. De toute évidence, la voie a constitué le decumanus majeur d’une centuriation qui s’est développée à partir d’elle pour embrasser, dans un réticulé construit sur une périodicité de 20 actus, équivalant ici à 706 m, et incliné à 22°30’ à l’ouest du nord géographique, la totalité de la plaine de Lunel‑Mauguio, entre le Lez et le Vidourle. Les recherches archéologiques récentes permettent de révéler l’influence de cette « limitation », vraisemblablement césarienne, sur la distribution de l’habitat rural à partir du Haut‑Empire, sur l’urbanisme des agglomérations gallo‑romaines, et sur l’organisation du proche environnement rural des oppida comme ceux de Lattes et d’Ambrussum. L’objet de ce travail est de définir le réseau de communications routières entre les villes de Baetulo (Badalone) et Barcino (Barcelone), situées sur la Pla de Barcelona dans la zone centrale de la côte catalane. Ce type de communication est étudié conjointement à d’autres éléments constitutifs d’un territoire. L’étude de 300 sondages géologiques dans le delta du fleuve Besos a permis de déterminer les caractéristiques de la plate‑forme deltaïque, qui apparaît constituée de différents niveaux de limons organiques, de graviers et de sables. Ces éléments physiques sont à rattacher à l’occupation humaine du territoire, étudiée à partir de la documentation historique et archéologique. L’analyse complète de ces éléments nous a permis de mieux comprendre les processus de transformation de la structure du territoire et, parallèlement, de définir le tracé de la voie et la diffusion des implantations à l’époque romaine. Dans ce travail le tracé des voies De Italia in Hispanias y Ab Asturica Terragone (It. 387, 495, 4 – It. 448, 2‑452, 5) est analysé ainsi que sa relation avec les villes existantes ou récemment créées, situées tout le long de la vallée de l’Èbre. La disposition de ces villes‑stations y est détaillée par rapport à la voie, de même que l’implantation des cadastres détectés dans la zone. This study clarifies the function of the morphohistorical relationship between via Domitia and a network of regular and geometrical plots of land, completely dependent on its rectilinear course between the oppida of Ambrussum and Sextantio. From existing evidence, via Domitia has served as the main decumanus for a centuriation which extended because of the route to encompass, in a network consisting of regular plots measured out in 20 actus, here corresponding to 706 m, and oriented geographically 22° 39 northwest, in the entire Lunel‑Mauguio plain between the Lez and the Vidourle. The latest archaeological research shows the influence of these boundaries which were probably introduced under Caesar, with distribution of land from the Earlier Empire, on the Gallo‑Roman urbanisation and on the organisation of the nearby rural surroundings of the oppida as in the case of Lattes and Ambrussum. The purpose of this work Is to define the road communications between the town of Baetulo (Badalona) and Barcinó (Barcelona), situated on the Pla de Barcelona, in the central area of the Catalan shore. That type of communication will be studied linked with other component elements of a territory. Studying three hundred geological drillings in the delta of the River Besos has enabled to establish the characteristics of the deltaic shelf which consists of different layers of organic alluvium and different types of gravel and sand. These physical elements have to be linked with the human occupation of the area, which has been studied from historico‑archeological documentation. The complete analysis of these differents elements has made us comprehend better the process of the changing of the organisation of the territory, and, at the same time, it has made us define the route of the road and the spreading of the various settlements in Roman times. In this work the trail of roads De Italia in Hispanias y Ab Asturia Terracone (lt.387, 495,4 – It.448,2‑452,5) is analysed with relation to the existent towns or towns recently created situated along the whole Valley of Ebre. The site of these « towns – stations » is here described in detail in comparison with the road and finally the implantation of the land registries found in the area. Aquest estudi precisa les relacions entre la via Domitia i una xarxa regular i geométrica de les parcel.les agràries, totalment dependent del seu trapat rectilini entre els oppida d’Ambrussum i de Sextantio. Amb tota evidència, la via ha constituit el decumanus major d’una centuriació que s’ha desenvolupat desde aquella per a estendre’s a tota la plana de Lunel‑Mauguio, entre el Lez i el Vidourle. Les centúries tenen un mòdul de 706 m i estan inclinades de 22°30’ cap a l’oest del nord geogràfic. Les recerques arqueològiques recents pernoten de revelar la influencia d’aquesta « limitació », versemblablement cesària, sobre la repartció de l’habitat rural des de l’alt Imperi, sobre I’urbanismo de les aglomeracions galio‑romanes, i sobre l’organització del medi envoltant rural proper dels oppida com els de Lattes i d’Ambrussum. El treball que presentera té com a objecte la definidó de la comunicadó viària entre les ciutats de Baetulo (Badalona) i Barcino (Barcelona), situades al Pía de Barcelona, al sector central de la costa catalana. La comunicadó viària no ha estat estudiada aïlladament, sinó en relació amb altres elemente que constitueixen un territori. L’estudi de 300 sondeigs geològics en el delta del riu Besòs ha permes de caracteritzar la plataforma deltaica, que apareix formada per diversos nivells de llims orgànics, graves i sorres. Aquesta elementa físics es relacionen amb l’ocupació humana del territori, estudiada a partir de la documentació històrico‑arqueològica. L’anàlisi integral de tots aquests elements ens permet d’entendre els processos de canvi en l’estructura del territori, i en relació amb aquests, de definir el traçat de la via i la distribució de l’assentament en época romana. En aquest treball s ’analitza el traçat de las vies De Italia in Híspanlas y Ab Asturica Terragone (It.387, 495,4 – It.448,2‑452,5), la seva relació amb les ciutats, existents o de nova creadó, situados al llarg de la Vall mitjana de l’Ebre. S’estudia la disposició d’aquestes ciutats‑mansions en relació amb la via i per fi la situació deis cadastres detectáis en la zona. Este estudio precisa las relaciones entre la via Domitia y una red regular y geométrica de las parcelas agrarias, totalmente dependente del su recorrido rectilíneo entre los oppida de Ambrussum y de Sextantio. El objetivo de este trabajo es la definición de la red de la comunicación viada entre las ciudades de Baetulo (Badalona) y Barcino (Barcelona), situadas en el Pía de Barcelona dentro de la zona central de la costa catalana. Este tipo de comunicación será estudiado con otros elementos constitutivos de un territorio. El estudio de 300 sondeos geológicos en el delta del río Besós ha permitido determinar las características de la plataforma deltaica, que aparece constituida de diferentes niveles de limos orgánicos, de grava y arenas. Estos elementos físicos tienen que ser relacionados con la ocupación humana del territorio, estudiada a partir de la documentación histórico‑arqueológica. El análisis completo de estos elementos ha permitido comprender mejor los procesos de transformación de la estructura del territorio y, paralelamente, precisar el trazado de la vía y la difusión de los establecimientos en la época romana. En este trabajo se analiza el trazado de las vías De Italia in Híspanias y Ab Asturica Terracone (It. 387, 495, 4 – It. 448, 2‑452, 5), su relación con las ciudades, existentes o de nueva creación, ubicadas a lo largo del Valle medio del Ebro. Se detalla la disposición de estas ciudades‑mansiones respecto a la vía y por último la ubicación de los catastros detectados en la zona.

- Abstract
Background: Aspiration community-acquired pneumonia (ACAP) and community-acquired pneumonia (CAP) in patients with aspiration risk factors (AspRFs) are infections associated with anaerobes, but limited evidence suggests their pathogenic role. Research Question: What are the aspiration risk factors, microbiology patterns, and empiric anti-anaerobic use in patients hospitalized with CAP? Study Design and Methods: This is a secondary analysis of GLIMP, an international, multicenter, point-prevalence study of adults hospitalized with CAP. Patients were stratified into three groups: (1) ACAP, (2) CAP/AspRF+ (CAP with AspRF), and (3) CAP/AspRF- (CAP without AspRF). Data on demographics, comorbidities, microbiological results, and anti-anaerobic antibiotics were analyzed in all groups. Patients were further stratified in severe and nonsevere CAP groups. Results: We enrolled 2,606 patients with CAP, of which 193 (7.4%) had ACAP. Risk factors independently associated with ACAP were male, bedridden, underweight, a nursing home resident, and having a history of stroke, dementia, mental illness, and enteral tube feeding. Among non-ACAP patients, 1,709 (70.8%) had CAP/AspRF+ and 704 (29.2%) had CAP/AspRF-. Microbiology patterns including anaerobes were similar between CAP/AspRF-, CAP/AspRF+ and ACAP (0.0% vs 1.03% vs 1.64%). Patients with severe ACAP had higher rates of total gram-negative bacteria (64.3% vs 44.3% vs 33.3%, P =.021) and lower rates of total gram-positive bacteria (7.1% vs 38.1% vs 50.0%, P <.001) when compared with patients with severe CAP/AspRF+ and severe CAP/AspRF-, respectively. Most patients (>50% in all groups) independent of AspRFs or ACAP received specific or broad-spectrum anti-anaerobic coverage antibiotics. Interpretation: Hospitalized patients with ACAP or CAP/AspRF+ had similar anaerobic flora compared with patients without aspiration risk factors. - Abstract
Background and objective: Enterobacteriaceae (EB) spp. family is known to include potentially multidrug-resistant (MDR) microorganisms, and remains as an important cause of community-acquired pneumonia (CAP) associated with high mortality. The aim of this study was to determine the prevalence and specific risk factors associated with EB and MDR-EB in a cohort of hospitalized adults with CAP. Methods: We performed a multinational, point-prevalence study of adult patients hospitalized with CAP. MDR-EB was defined when ≥3 antimicrobial classes were identified as non-susceptible. Risk factors assessment was also performed for patients with EB and MDR-EB infection. Results: Of the 3193 patients enrolled with CAP, 197 (6%) had a positive culture with EB. Fifty-one percent (n = 100) of EB were resistant to at least one antibiotic and 19% (n = 38) had MDR-EB. The most commonly EB identified were Klebsiella pneumoniae (n = 111, 56%) and Escherichia coli (n = 56, 28%). The risk factors that were independently associated with EB CAP were male gender, severe CAP, underweight (body mass index (BMI) < 18.5) and prior extended-spectrum beta-lactamase (ESBL) infection. Additionally, prior ESBL infection, being underweight, cardiovascular diseases and hospitalization in the last 12 months were independently associated with MDR-EB CAP. Conclusion: This study of adults hospitalized with CAP found a prevalence of EB of 6% and MDR-EB of 1.2%, respectively. The presence of specific risk factors, such as prior ESBL infection and being underweight, should raise the clinical suspicion for EB and MDR-EB in patients hospitalized with CAP.

En el presente trabajo se va a realizar el diseño de un asistente de dirección para bicicleta. El proyecto se basa en una necesidad de aumentar la seguridad en el manejo de la bicicleta debido a su auge como medio de transporte con el consiguiente aumento en el número de accidentes. Se estudiarán una serie de alternativas de asistentes de dirección y finalmente se realizará el diseño de la solución adoptada. En primer lugar, se llevará a cabo un diseño empleando herramientas de CAD para, a continuación, realizar el correspondiente prototipado mediante impresión 3D. Se realizarán los cálculos correspondientes para dimensionar y seleccionar todos los componentes del sistema de dirección asistida propuesto, así como análisis de tensiones y deformaciones de las partes más críticas. Por último, se desarrollará una simulación del sistema de control del modelo y se realizará una evaluación del mismo, así como una propuesta de mejoras y futuros avances de este proyecto. The purpose of the present project is to design a bicycle steer assistant. This project will be based on the necessity of improving bicycle riding safety as its use has been increasing in the last decades and, therefore, the number of accidents while riding it. Several steer assistant alternatives will be studied, and one will be selected for it being designed. A CAD design will be made for its later prototyped with 3D printers. The corresponding calculations and analysis for the dimensioning and selection of the components of the system will be done, as well as stress and deformation studies of the most critical parts of the design. Last, but not least, a simulation of the balancing control of the bicycle will be performed and an evaluation of the whole design with improvements proposals and future work of the project will be shared.

N.J.S. is partially funded by the Department of Veterans Affairs, Quality Enhancement Research Initiative (QUERI) Partnered Evaluation Initiative Grant (HX002263-01A1). Background and objective : Enterobacteriaceae (EB) spp. family is known to include potentially multidrug-resistant (MDR) microorganisms, and remains as an important cause of community-acquired pneumonia (CAP) associated with high mortality. The aim of this study was to determine the prevalence and specific risk factors associated with EB and MDR-EB in a cohort of hospitalized adults with CAP. Methods : We performed a multinational, point-prevalence study of adult patients hospitalized with CAP. MDR-EB was defined when ≥3 antimicrobial classes were identified as non-susceptible. Risk factors assessment was also performed for patients with EB and MDR-EB infection. Results : Of the 3193 patients enrolled with CAP, 197 (6%) had a positive culture with EB. Fifty-one percent (n = 100) of EB were resistant to at least one antibiotic and 19% (n = 38) had MDR-EB. The most commonly EB identified were Klebsiella pneumoniae (n = 111, 56%) and Escherichia coli (n = 56, 28%). INTRODUCTION: Many drug trials for chronic pain fail because of high placebo response rates in primary endpoints. Neurophysiological measures can help identify pain-linked pathophysiology and treatment mechanisms. They can also help guide early stop/go decisions, particularly if they respond to verum treatment but not placebo. The neurologic pain signature (NPS), an fMRI-based measure that tracks evoked pain in 40 published samples and is insensitive to placebo in healthy adults, provides a potentially useful neurophysiological measure linked to nociceptive pain. OBJECTIVES: This study aims to validate the NPS in knee osteoarthritis (OA) patients and test the effects of naproxen on this signature. METHODS: In 2 studies (50 patients, 64.6 years, 75% females), we (1) test the NPS and other control signatures related to negative emotion in knee OA pain patients; (2) test the effect of placebo treatments; and (3) test the effect of naproxen, a routinely prescribed nonsteroidal anti-inflammatory drug in OA. RESULTS: The NPS was activated during knee pain in OA (d = 1.51, P < 0.001) and did not respond to placebo (d = 0.12, P = 0.23). A single dose of naproxen reduced NPS responses (vs placebo, NPS d = 0.34, P = 0.03 and pronociceptive NPS component d = 0.38, P = 0.02). Naproxen effects were specific for the NPS and did not appear in other control signatures. CONCLUSION: This study provides preliminary evidence that fMRI-based measures, validated for nociceptive pain, respond to acute OA pain, do not appear sensitive to placebo, and are mild-to-moderately sensitive to naproxen.

44. Post COVID-19 Condition in Children and Adolescents: An Emerging Problem

The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection became a pandemic in 2020 and by March 2022 had caused more than 479 million infections and 6 million deaths worldwide. Several acute and long-term symptoms have been reported in infected adults, but it remains unclear whether children/adolescents also experience persistent sequelae. Hence, we conducted a review of symptoms and pathophysiology associated with post-coronavirus disease 2019 (post-COVID-19) condition in children and adolescents. We reviewed the scientific literature for reports on persistent COVID-19 symptoms after SARS-CoV-2 infection in both children/adolescents and adults from 1 January 2020 to 31 March 2022 (based on their originality and relevance to the broad scope of this review, 26 reports were included, 8 focused on adults and 18 on children/adolescents). Persistent sequelae of COVID-19 are less common in children/adolescents than in adults, possibly owing to a lower frequency of SARS-CoV-2 infection and to the lower impact of the infection itself in this age group. However, cumulative evidence has shown prolonged COVID-19 to be a clinical entity, with few pathophysiological associations at present. The most common post-COVID-19 symptoms in children/adolescents are fatigue, lack of concentration, and muscle pain. In addition, we found evidence of pathophysiology associated with fatigue and/or headache, persistent loss of smell and cough, and neurological and/or cardiovascular symptoms. This review highlights the importance of unraveling why SARS-CoV-2 infection may cause post-COVID-19 condition and how persistent symptoms might affect the physical, social, and psychological well-being of young people in the future.

- Abstract
Background: Concurrent chemotherapy and thoracic radiotherapy followed by prophylactic cranial irradiation (PCI) is the standard treatment in limited-disease small-cell lung cancer (ID-SCLC), with 5-year overall survival (OS) of only 25% to 33%. Patients and methods: STIMULI is a 1:1 randomised phase II trial aiming to demonstrate superiority of consolidation combination immunotherapy versus observation after chemo-radiotherapy plus PCI (protocol amendment-1). Consolidation immunotherapy consisted of four cycles of nivolumab [1 mg/kg, every three weeks (Q3W)] plus ipilimumab (3 mg/kg, Q3W), followed by nivolumab monotherapy (240 mg, Q2W) for up to 12 months. Patient recruitment dosed prematurely due to slow accrual and the statistical analyses plan was updated to address progression-free survival (PFS) as the only primary endpoint. Results: Of the 222 patients enrolled, 153 were randomised (78: experimental; 75: observation). Among the randomised patients, median age was 62 years, 60% males, 34%/65% current/former smokers, 31%/66% performance status (PS) 0/1. Up to 25 May 2020 (median follow-up 22.4 months), 40 PFS events were observed in the experimental arm, with median PFS 10.7 months [95% confidence interval (CI) 7.0-not estimable (NE)] versus 42 events and median 14.5 months (8.2-NE) in the observation, hazard ratio (HR) = 1.02 (0.66-1.58), two-sided P = 0.93. With updated follow-up (03 June 2021; median: 35 months), median OS was not reached in the experimental arm, while it was 32.1 months (26.1-NE) in observation, with HR = 0.95 (0.59-1.52), P = 0.82. In the experimental arm, median time-to-treatment-discontinuation was only 1.7 months. CTCAE v4 grade >= 3 adverse events were experienced by 62% of patients in the experimental and 25% in the observation arm, with 4 and 1 fatal, respectively. Conclusions: The STIMULI trial did not meet its primary endpoint of improving PFS with nivolumab-ipilimumab consolidation after chemo-radiotherapy in LD-SCLC. A short period on active treatment related to toxicity and treatment discontinuation likely affected the efficacy results.
