The role of factor VIII (FVIII) consists in activating factor X, which directly participates in the generation of thrombin, which is able to produce stable fibrin, which in turn forms blood clots. There are divergent opinions regarding the significance of high levels of FVIII in stroke pathogenesis.Aim: The aim of our study was to evaluate FVIII activity in individuals with cryptogenic stroke in order to determine a potential relationship between it and cerebral ischemia.Material and methods: Nine patients suffering with stroke were used in this study: six women and three men aged 49–63 years. In all of the patients, the presence of known and potential risk factors for stroke had been excluded during previous diagnostic procedures. These patients accounted for 1.2% of the 719 people who suffered a stroke and were hospitalized in 2011 at the Stroke Unit. FVIII activity was examined in each of the nine qualified subjects within 1–2 months of the occurrence of stroke (the first test) and repeated (the second test) in five patients with abnormal results obtained from the first examination.Results: Increased activity of FVIII was found in 5 out of 9 patients. In patients with abnormal results, elevated FVIII was found in follow-up examinations in the 8th–10th month following stroke. Hemodynamic abnormalities in carotid or cerebral artery (presence of thrombus) were found in 3 of the 5 patients with increased FVIII levels. In the first 24 hours following stroke the neurological state of patients with abnormal FVIII was worse than individuals with normal FVIII activity. The patients with abnormal FVIII levels were found to be more disabled in the examination of self-dependence on the 90th day after stroke.Conclusion: When searching for the causes of stroke, it is worth examining the coagulation system, including FVIII concentration, the abnormality of which may play a significant part in brain ischemia. More research is needed to determine the relationship between abnormal FVIII activity and stroke.Keywords: factor VIII, cryptogenic stroke, hemostasis, risk factor

Abstract

The article reveals the organiza­tional features of medical and psychological assistance in cases of perinatal losses in modern conditions. The paper presents the peculiarities of perinatal loss, psychological condition of women’s health during the onset of subsequent pregnancy as well as experiences of the women who have lost a child at different stages of pregnancy. The article notes that the majority of pregnant women with a history of perinatal loss are in a state of chronic stress, which can lead to the emergence and development of various medical and psychological complications. The peculiarities of the experience of grief in the situation of perinatal loss, as a serious mental trauma for the woman's psyche were analyzed. It has been substantiated that the experience of perinatal loss is a non-normative crisis in marital relations, and the process of providing psychological assistance should be directed not only to the woman, but also to the married couple as a whole, which will reduce the risk of rupture of relations, promote their harmonization, which can be considered as an effective resource for a constructive experience of the situation of loss. The main tasks of psychologists and medical staff during the organization of medical and psychological assistance for women and married couples experiencing perinatal loss are also revealed. It is crucial to comprehend the feelings women experience in such cases, which makes medical and psychological care more effective in the process of working with those who are through child loss or have had such an experience previously. The conclusion notes the priority of organizing and providing comprehensive medical and psychological assistance not only to women, but also to married couples in a situation of perinatal loss; the need for special training both for medical psychologists, obstetricians and gynecologists to provide complex assistance to women experiencing perinatal losses.

4. Ways of Assessing Students' Progress in School Learning by Language Teachers during the Lockdown and Remote Learning in Poland

Abstract

The aim of the study was to check how language teachers employed in Polish schools assessed students' progress in learning during the period of remote learning. This study used the original questionnaire, which consisted of 15 questions regarding the implementation of a specific research goal. 110 language teachers employed in state primary, secondary and vocational schools located in the central and northern part of Poland participated in the study. In assessing students' progress in school learning during the remote learning period, language teachers most often used: homework assigned to students; various types of oral and written assignments (essays, tests, oral class assignments); semester tests; short tests commencing the lesson; and partial grades to assess the class performance. Research shows that language teachers adopted various forms and methods of assessing students' progress in school learning during the distance learning period, but the dominant ones were the forms and methods of assessment that required the least amount of time from them.

Abstract

Стаття присвячена висвітленню однієї із актуальних проблем психології здоров’я – проблеми ставлення молоді до здоров’я. Розкрито поняття ставлення до здоров’я та проаналізовано чинники, що визначають процес його формування, а також розглянуто поняття стану психоемоційного напруження, що характеризується різним ступенем мобілізації нервово-психологічних функцій організму та на високому рівні може обумовлювати негативні зміни у когнітивному, емоційному, поведінковому чи ціннісно-мотиваційному компонентах ставлення до здоров’я. Здійснено порівняльний аналіз результатів констатуючого експерименту про те, що у молоді з різним рівнем психоемоційного напруження є певні відмінності у ставленні до власного здоров’я, зокрема молодь з високим рівнем психоемоційного напруження характеризується неадекватним ставленням до здоров’я.

Проаналізовано особливості професійного становлення майбутніх психологів на етапі професійного навчання, розкрито поняття комунікативної компетентності, висвітлено особливості прояву комунікативної компетентності у професійній діяльності психологів, визначено умови формування комунікативної компетентності у майбутніх психологів під час професійного становлення. Проведено емпіричне дослідження особливостей розвитку комунікативної компетентності у майбутніх психологів. Встановлено, що студенти-психологи старших курсів мають більш високий рівень комунікативної компетентності у порівнянні зі студентами молодших курсів. Abstract

We describe PNUTS, a massively parallel and geographically distributed database system for Yahoo!'s web applications. PNUTS provides data storage organized as hashed or ordered tables, low latency for large numbers of concurrent requests including updates and queries, and novel per-record consistency guarantees. It is a hosted, centrally managed, and geographically distributed service, and utilizes automated load-balancing and failover to reduce operational complexity. The first version of the system is currently serving in production. We describe the motivation for PNUTS and the design and implementation of its table storage and replication layers, and then present experimental results.

Based on the literature, present study analyzes such forms of violation of maternal behavior as mother's refusal from a baby; mother's cruel treatment of a baby; frequent abortions; maternity in the early reproductive age; conscious maternity postponement for a later reproductive age. Also the factors that contribute to various manifestations of deviant motherhood are described.

25. Metabolism of Direct-acting Antiviral Agents (DAAs) in Hepatitis C Therapy: A Review of the Literature

Abstract

Background: Hepatitis C virus (HCV) infection is still one of the leading causes of chronic liver disease, with chronically infected making up approximately 1% of the global population. Of those infected, 70% (55-85%) will develop chronic HCV infection. Chronic HCV infection causes substantial morbidity and mortality, with complications including cirrhosis, end-stage liver disease, hepatocellular carcinoma, and eventually death. Objective: Therapeutic options for chronic HCV infection have evolved dramatically since 2014, with a translation from pegylated interferon and ribavirin (associated with suboptimal cure and high treatment-related toxicity) to oral direct-acting antiviral treatment. There are four classes of direct-acting antivirals which differ by their mechanism of action and therapeutic target. They are all pointed to proteins that form the cytoplasmic viral replication complex. Multiple studies have demonstrated that direct-acting antiviral therapy is extremely well tolerated, highly efficacious, with few side effects. Methods: We performed an indexed MEDLINE search with keywords regarding specific direct-acting antiviral regimes and their pharmacokinetics, drug-drug interactions, and metabolism in specific settings of pregnancy, lactation, liver cirrhosis, liver transplantation and HCC risk, kidney failure and kidney transplantation. Results: We present a comprehensive overview of specific direct-acting antiviral metabolism and drug-drug interaction issues in different settings. Conclusion: Despite its complex pharmacokinetics and the possibility of drug-drug interactions, direct-acting antivirals are highly efficacious in providing viral clearance, which is an obvious advantage compared to possible interactions or side effects. They should be administered cautiously in patients with other comorbidities, and with tight control of immunosuppressive therapy.

Abstract

Acute cerebral ischemia triggers local and systemic immune response. The aims of this project was to assess the blood serum concentration of the markers of inflammation and markers of the blood brain barrier damage on the first day of ischemic stroke, and the mutual correlations between these marker levels. Patients with first-in-life stroke were analyzed according to: plasma concentration of the following markers on the first day of stroke: interleukin 2 (IL-2) and interleukin 6 (IL-6), S100B, tumor necrosis factor-a (TNF-a), progranulin (GRN), neuron specific enolase (NSE), urokinase-type plasminogen activator (uPA), vascular endothelial growth factor (VEGF), brain-derived neurotrophic factor (BDNF), C-reactive protein (CRP), leucocyte and thrombocyte counts; their neurological status on the first day of stroke (NIHSS) and their functional status at 30 days following stroke (mRS). The study included 138 patients with mean age: 73.11 ± 11.48. Patients with a higher score on the NIHSS showed significantly higher concentrations of TNFa, white blood cells (WBC), CRP, NSE, IL-6 and S100B. Patients with a higher score on the modified Rankin Score (mRS) showed significantly higher concentrations of WBC, CRP, GRN, IL-6, S100B. Factors with an independent influence on the neurological status on the first day of stroke were: sex, WBC, total blood platelet (PLT) count, CRP, S100B and IL-6 levels. Atrial fibrillation, leukocyte count, CRP, NSA, uPA, IL-6 and S100B showed an independent impact on the functional status on the 30th day of stroke. Patients with symptomatic atherosclerosis, as compared to others, were older (P = 0.003) and had higher levels of CRP, IL-6, and S100B. In each case, the differences were statistically significant. We conclude that the concentration of Il-6 and S100B on the first day of stroke are important for both the neurological status and the functional status in the acute period of the disease. Increased CRP and leukocyte count are associated with a worse prognosis regarding the course of acute stroke. The expression of pro-inflammatory agents and markers of blood-brain barrier damage in the acute phase of stroke seem to be more prominent in patients with symptomatic atherosclerosis than in patients with no clinical features of atherosclerosis. The expression of inflammatory parameters may indicate the importance of the inflammatory process starting during the early days of ischemic stroke, for the post-stroke neurological deficit. 31. Risk factor profile in patients with stroke at a young age.

Abstract

Background The distribution of risk factors for stroke is related to gender and age. The main objective of study was to determine the type and frequency of stroke risk factors in patients with first-in-life stroke before the age of 50. Methods Our study included patients under the age of 50 with first-in-life stroke. The following data obtained from all participants were analyzed: a neuroimaging of the head, wide panel of blood tests, electrocardiogram, transesophageal echocardiography, ultrasound of the carotid/cerebral arteries. 24-hour ECG monitoring and angio-MRI of head were performed in most patients. Results One hundred and eighty-four patients were qualified into the study; mean age - 39 years (female 45%). The female gender was significantly more often associated with diabetes, carotid/cerebral artery stenosis, myocardial infarction, cardiomyopathy and risk factors associated with lifestyle. More than half of the patients presented at least two risk stroke factors. The most frequent categories of risk factors were related to arteries and metabolic diseases. Vascular risk factors were among the most frequent in individual age categories. Hereditary stroke-burden was significant in the patient before the age of 20.15% of the patients haven’t got known stroke risk factor. Conclusions Angiopathy is the most common risk factor for stroke at a young age. More than half of the patients with stroke at a young age have at least two independent risk factors for stroke. In the first two decades of life, the most frequent risk factors for stroke are heart diseases; arterial diseases play a dominant role in the consecutive three decades. [ABSTRACT FROM AUTHOR]

Abstract

The results of available studies on assessment of neurodegenerative lesions in multiple sclerosis (MS) patients using different approaches have not been conclusive. Currently, clinical assessment is the most commonly used (involving primarily mobility assessment), along with magnetic resonance imaging and electrophysiological testing. In this review we describe available clinical, neuroimaging, electrophysiological and laboratory tests used to assess the neurodegeneration in MS. Laboratory markers to determine the risk of disease, its conversion and prognosis in MS patients are being constantly sought. Cerebrospinal fluid (CSF) sample collection is invasive and constitutes a burden to a patient, so serum biomarkers are being investigated. Optimistic preliminary results of studies assessing neurofilament light chains (NFL) in serum of MS patients, encourage further research. The possibility to use such marker (or a group of markers) would significantly facilitate clinical decisions at the stage of diagnosis and treatment. Currently used treatments have limited efficacy and are associated with numerous adverse effects. Additional information from available clinical, imaging, electrophysiological or laboratory biomarker or a group of biomarkers, which predict the course and prognosis, will facilitate choosing optimal treatment and its escalation at the relevant stage. Conclusion: Using the diagnostic panel consisting of imaging, neurophysiology and serology testing along with clinical and neuropsychological assessment may improve the reliability of diagnostic instruments evaluating cerebral atrophy in MS patients. Abstract

Background Transcranial sonography may be applied to assess the basal ganglia nuclei and brain atrophy by the measurement of the width of the third ventricle. The aim of this study was to assess usefulness of transcranial sonography (TCS) in patients with multiple sclerosis ( MS) by examining the echogenicity of subcortical structures and the width of the third ventricle. Method Transcranial sonography evaluation of substantia nigra, brain stem raphe nuclei, diameter of the third ventricle, width of the anterior horn of the lateral ventricle, thalamus, lenticular nucleus, and head of the caudate nucleus in 41 patients with relapsing-remitting MS ( RRMS), 23 with secondary progressive MS ( SPMS), and 20 healthy controls was compared. A potential link between the patients' age, sex, Expanded Disability Status Scale ( EDSS) score, relapse index, and ultrasound parameters was assessed. Results The following were found in patients with MS, as compared to the control group: a greater area of the substantia nigra, a longer diameter of the third ventricle and wider frontal horns of the lateral ventricles, hypo-echogenicity of the brain stem raphe, and hyperechogenicity of the lenticular nucleus. The study group was found to have a significant correlation between the area of the substantia nigra, and the age of patients, the duration of the illness, EDSS score, and the number of relapses. There was a significant correlation between the diameter of the third ventricle and the age of patients and EDSS score. Conclusions Patients with MS reveal ultrasound features of subcortical structure atrophy. Selected TCS findings show a correlation with disease progression and activity. [ABSTRACT FROM AUTHOR]

The efficacy and safety of thrombolytic therapy in stroke depend on multiple factors. The aim of this study was to evaluate the significance of atrial fibrillation the prognosis in terms of the functional status in patients with stroke treated with intravenous thrombolysis. An additional aim was also to assess the potential significance of reduced ejection fraction (EF) and enlarged left atrium (LA) of the heart for the prognosis in patients with stroke who underwent thrombolytic therapy.

Abstract

Recent results of plasma electrolytic oxidation modification of the surfaces of Ti as well as NiTi alloy obtained by a joint effort of Institute of Chemistry FEB RAS, Institute of Strength Physics and Materials Science SB RAS, and Russian Ilizarov Scientific Center for Restorative Traumatology and Orthopedics are demonstrated in comparison with the literature ones. Conditions of coating synthesis determine their functional designation. It is presented that purposeful selection of the electrolyte composition used for coating formation has allowed to obtain layers containing Ca and P, whose bioactivity is confirmed both in vitro and in vivo. Prospects for the application of the plasma electrolytic oxidation method for the formation of the composite bioinert coatings at the titanium nickelide surface improving its morphological structure and electrochemical properties and significantly reducing the quantity of nickel ions releasing into the organism are demonstrated. Abstract

The formation of protective multifunctional coatings on magnesium alloy MA8 using plasma electrolytic oxidation (PEO) in an electrolytic system containing nanosized particles of titanium nitride was investigated. Electrochemical and mechanical properties of the obtained layers were examined. It was established that microhardness of the coating with the nanoparticle concentration of 3 g l −1 increased twofold (4.2 ± 0.5 GPa), while wear resistance decreased (4.97 × 10 −6  mm 3  N −1  m −1 ), as compared to respective values for the PEO-coating formed in the electrolyte without nanoparticles (2.1 ± 0.3 GPa, 1.12 × 10 −5  mm 3  N −1  m −1 ). Abstract

Carotid artery stenosis can result in the brain tissue injury related to the intracranial aterial flow disturbances as well as microembolic complications. The choice of the proper therapy in patients with carotid artery stenosis, especially asymptomatic, remains still a significant clinical problem. The study aim was an assessment of the cerebral embolism and brain vascular reserve parameters in patients with carotid artery stenosis regarding the occurrence of the clinical symptoms, the degree of stenosis as well as plaque morphology.

Abstract

Metallic implants have been successfully used in medicine for the past 60–70 years. Historically, implants were designed only as mechanical devices, whereas the biological aspects of their application were beyond the researchers’ interest. The improvement of living conditions and the increase of the average life span have changed the situation. The clinical requirements for medical implants rise up substantially. Presently, it seems impossible to imagine the use of metallic implants in the human body without preliminary surface modification to modulate the interaction between the surrounding biological environment and the implant. The review highlights the most recent advances in the field of functional coatings formed on implants by the plasma electrolytic oxidation technology. Special attention is dedicated to the principles of surface modification of the commercially pure titanium, titanium nickelide, and Mg-Mn-Ce magnesium alloy. The advantages and disadvantages of the method and the characteristics of these materials are discussed from this point of view. Some aspects of this review are aimed at corrosion protection of implants with application of polymer materials.

Abstract

Multifunctional corrosion resistant and bioactive coatings containing hydroxyapatite Ca10(PO4)6(OH)2 and magnesium oxide MgO were obtained on Mg-Mn-Ce alloy by plasma electrolytic oxidation. The phase and elemental composition, morphology and anticorrosion properties of the coatings were investigated. The PEO layers were post-treated using Forum® superdispersed polytetrafluoroethylene powder, which caused a considerable reduction of the magnesium alloy corrosion rate (>4 orders of magnitude). In particular, the polarisation resistance has increased, as a result of such duplex treatment, by five orders of magnitude. The composite coatings are considered promising to induce bioactivity and control corrosion degradation of resorbable Mg implants. [ABSTRACT FROM AUTHOR]
