Abstract Background Unlike SARS-CoV and MERS-C0V, SARS-CoV-2 has the potential to become a recurrent seasonal infection; hence, it is essential to compare the clinical spectrum of COVID-19 to the existent endemic coronaviruses. We conducted a retrospective cohort study of hospitalized patients with seasonal coronavirus (sCoV) infection and COVID-19 to compare their clinical characteristics and outcomes. Methods A total of 190 patients hospitalized with any documented respiratory tract infection and a positive respiratory viral panel for sCoV from January 1, 2011, to March 31, 2020, were included. Those patients were compared with 190 hospitalized adult patients with molecularly confirmed symptomatic COVID-19 admitted from March 1, 2020, to May 25, 2020. Results Among 190 patients with sCoV infection, the Human Coronavirus-OC43 was the most common coronavirus with 47.4% of the cases. When comparing demographics and baseline characteristics, both groups were of similar age (sCoV: 74 years vs. COVID-19: 69 years) and presented similar proportions of two or more comorbidities (sCoV: 85.8% vs. COVID-19: 81.6%). More patients with COVID-19 presented with severe disease (78.4% vs. 67.9%), sepsis (36.3% vs. 20.5%), and developed ARDS (15.8% vs. 2.6%) compared to patients with sCoV infection. Information and communication technology (ICT) tools such as radio, television, mobile phone, the internet, computers are gaining momentum in the development discourse of the agriculture sector in Nepal. In agriculture extension, ICT tools fill the void that traditional agriculture extension cannot address. So, this study aimed at assessing the paddy farmers' knowledge, perception, and satisfaction on ICT tools in Jhapa, Kapilbastu, and Kailai districts following a multistage purposive sampling method. A survey research design was used for the study. Pretested semi-structured interview schedule was employed to randomly selected 390 sample respondents. Descriptive statistics along with the appropriately developed scales were used in the data analysis. The findings revealed that respondents were moderately aware (0.44) of the significant roles of ICT tools. Respondents do have more knowledge on the radio (0.87), TV (0.85), and mobile phones (0.76), whereas the majority possess TV (94%), radio (93%), and mobile phones (88%) among ICT tools. Farmers from Bardiya were more aware of the roles of ICT as compared to other study districts. Likewise, Radio and TV were the primary ICT tools used for agriculture-related information. Respondents had high skills in using radio and TV for information but poor skills in using the computer in all study districts. Most of the respondents positively perceived (0.14) ICT tools and were satisfied (0.23) with them, but their use was limited to radio, TV, and mobile phones. In addition, farmers of Kapilbastu districts were less satisfied with the use of ICT tools as compared to other study districts.

Abstract Background Posterior reversible encephalopathy syndrome (PRES) is a clinical-radiologic entity characterized by headaches, altered mental status, seizures, visual loss, and a characteristic imaging pattern in brain magnetic resonance images. The exact etiology and pathogenesis of this condition are not yet fully elucidated. Case presentation A 72-year-old White man presented with 2 weeks of low-grade fever and chills, night sweats, fatigue, dysphagia, and new-onset rapidly increasing cervical lymphadenopathy. He had a history of chronic lymphocytic leukemia with transformation to diffuse large B-cell lymphoma for which he was started on dose-adjusted rituximab, etoposide, prednisone vincristine, cyclophosphamide, and doxorubicin (DA-R-EPOCH). Shortly after treatment initiation, the patient developed severe airway obstruction due to cervical lymphadenopathy that required emergency intubation. A few days later, the cervical lymphadenopathy and the status of the airway improved, and sedation was consequently weaned off to plan for extubation. However, the patient did not recover consciousness and developed generalized refractory seizures. Brain magnetic resonance imaging revealed edema in the cortical gray and subcortical white matter of the bilateral occipital and inferior temporal lobes, consistent with PRES. Conclusions Posterior reversible encephalopathy syndrome refers to a neurological disorder and imaging entity characterized by subcortical vasogenic edema in patients who develop acute neurological signs and symptoms of a usually reversible nature in different settings, including chemotherapy. Despite its name, PRES is not always fully reversible, and permanent sequelae can persist in some patients. Clinicians should be aware of the possible association between chemotherapy and PRES to ensure early recognition and timely treatment.

Staphylococcal Toxic Shock Syndrome (TSS) is characterized by rapid onset of fever, rash, hypotension, and multiorgan system involvement. Clinical manifestations of staphylococcal TSS include fever, chills, hypotension, and a diffuse macular erythroderma followed by desquamation one to two weeks later. The disease came to public attention in the 1980s with the occurrence of a series of menstrual-associated cases. However, the relative incidence of staphylococcal TSS not associated with menstruation has increased, and still, it remains an overlooked cause of septic shock. We present the case of a healthy 19-year-old male that presented with fever, chills, malaise, near-syncope, and a non-fluctuant, mobile nodule in the left armpit. The patient developed septic shock requiring critical care. He underwent extensive investigations resulting negative except for PCR for the detection of MRSA, raising the suspicion for STSS. For that reason, antibiotics for staphylococcal coverage were started, after which he started to improve. Ultimately, the mobile nodule evolved to fluctuant access. Incision and drainage was performed, and cultures confirmed the presence of Staphylococcus aureus.

BACKGROUND:Improving quality of intrapartum care will reduce intrapartum stillbirth and neonatal mortality, especially in resource-poor settings. Basic neonatal resuscitation can reduce intrapartum stillbirth and early neonatal mortality, if delivered in a high-quality health system, but there is a dearth of evidence on how to scale up such evidence-based interventions. We evaluated the scaling up of a quality improvement (QI) package for neonatal resuscitation on intrapartum-related mortality (intrapartum stillbirth and first day mortality) at hospitals in Nepal. METHODS AND FINDINGS:We conducted a stepped-wedge cluster randomized controlled trial in 12 hospitals over a period of 18 months from April 14, 2017, to October 17, 2018. The hospitals were assigned to one of four wedges through random allocation. The QI package was implemented in a stepped-wedge manner with a delay of three months for each step. The QI package included improving hospital leadership on intrapartum care, building health workers' competency on neonatal resuscitation, and continuous facilitated QI processes in clinical units. An independent data collection system was set up at each hospital to gather data on mortality through patient case note review and demographic characteristics of women using semi-structured exit interviews. The generalized linear mixed model (GLMM) and multivariate logistic regression were used for analyses. During this study period, a total of 89,014 women-infant pairs were enrolled. The mean age of the mother in the study period was 24.0 ± 4.3 years, with 54.9% from disadvantaged ethnic groups and 4.0% of them illiterate. Of the total birth cohort, 54.4% were boys, 16.7% had gestational age less than 37 weeks, and 17.1% had birth weight less than 2,500 grams. The incidence of intrapartum-related mortality was 11.0 per 1,000 births during the control period and 8.0 per 1,000 births during the intervention period (adjusted odds ratio [aOR], 0.79; 95% CI, 0.69-0.92; p = 0.002; intra-cluster correlation coefficient [ICC], 0.0286). The right to food is the right to life. Ensuring food security for all the citizens and their food sovereignty is the responsibility of the State. Currently, the need for food security, especially for marginalized and oppressed sections of society, including women in Nepal, is inadequately addressed. In this context, the main objective of this paper is to examine the gender dimensions in food policies and programs in Nepal. The paper explores five dimensions of food security, the right to food and food sovereignty, and analyzes gender inclusivity in food policies and governance in particular, since the advent of the sixth periodic plan (1980-1984) that included gender issues for the first time in the planning history. The paper, employing qualitative methods, recognizes that ensuring food governance is not only essential for equitable food security and the right to food, but also to the process of transforming discriminatory cultural norms and values into equitable ones and strengthening the psychological well-being of women. This paper argues that exclusion of women from decision-making processes leads to their psychological disempowerment. Women's participation in socio-cultural, economic, and political spheres directly impacts processes to identify and recognize their needs, preferences and priorities in food policies and programs. The article concludes, that since food security and the right to food impact women and men differently, a transformational process must respond to gender-differentiated interests, choices, preferences and entitlements. Background: Currently, tumor control probability (TCP) can be determined through experiments and observations, which enable deriving its values and construction of phenomenological and mechanistic TCP models. However, there are few experiments and observations extent; for this reason the development of virtual simulations is an interesting option for the determination of TCP. Method: Probabilistic descriptions were made of three kinds of cells, and the effects of radiation interaction with each tumor cell was investigated. For a homogeneously irradiated tumor with a dose d, the “TumorRBEf” simulator determines the TCP using simulations of a fractioned treatment, based on the tumor cell ratio sensitivities (cell kill and cell sublethal damage), tumor volume, cell density, and number of fractions. Results: In this study, a simulator was developed to estimate the TCP using a new methodology. The TCP was calculated from a fractioned treatment as a ratio of simulations, with 100% of killed cells and total of simulations. This technique may be useful for determining optimal treatment schedules, and as a teaching tool. Conclusions: Despite that the first version of this simulator neglected tumor cellular processes, such as cell repair and cell repopulation, the current version provides hypothesis-generating results. The simulator has whole probabilistic foundations, using probabilities, such as the probability of meeting a killed cell, of killing an undamaged cell, and of killing a sublethal damage cell. Chronic myelomonocytic leukemia (CMML) is a relatively rare clonal hematologic disorder with features of myelodysplastic syndrome and myeloproliferative disease. Extramedullary leukemic involvement is rarely a presenting feature of CMML. As there are no clear guidelines in regard to the treatment of patients with extramedullary manifestations, its management is challenging. In this report, we discuss the management of our patient who presented with submandibular lymphadenopathy and gingivitis and was diagnosed with CMML.

In clinical practice, evaluation of clinical efficacy of treatment planning stems from the radiation oncologist's experience in accurately targeting tumors, while keeping minimal toxicity to various organs at risk (OAR) involved. A more objective, quantitative method may be raised by using radiobiological models. The purpose of this work is to evaluate the potential correlation of OAR-related toxicities to its radiobiologically estimated parameters in simultaneously integrated boost (SIB) intensity modulated radiation therapy (IMRT) plans of patients with head and neck tumors at two institutions. Lyman model for normal tissue complication probability (NTCP) and the Poisson model for tumor control probability (TCP) models were used in the Histogram Analysis in Radiation Therapy (HART) analysis. In this study, 33 patients with oropharyngeal primaries in the head and neck region were used to establish the correlation between NTCP values of (a) bilateral parotids with clinically observed rates of xerostomia, (b) esophagus with dysphagia, and (c) larynx with dysphagia. The results of the study indicated a strong correlation between the severity of xerostomia and dysphagia with Lyman NTCP of bilateral parotids and esophagus, respectively, but not with the larynx. In patients without complications, NTCP values of these organs were negligible. Using appropriate radiobiological models, the presence of a moderate to strong correlation between the severities of complications with NTCP of selected OARs suggested that the clinical outcome could be estimated prior to treatment.

To encourage the use of the NTCP0 for evaluating safety as a new alternative of assessing the S-Es of the radiation oncology treatments; and the use of the 'NTCP0cal' methodology that calculates/estimates NTCP0. Revisions of studies related to use of the NTCP in the evaluations of S-Es. The 'NTCP0cal' Matlab application developed in this study has three calculation options. Two of the options provide additional graphical information about the distributions. The NTCP0 is a new radiobiological concept, its introduction let to correct some current P + and UTCP formulations, and will allow evaluating S-Es in whatever activity involving ionizing radiation, like radiation treatments; and its phenomenological model function of dose prescribed (D = n*d) will allow calculating values of NTCP0 for a range of dose per fraction (d) in a treatment with a determined number of fractions (n), or for range of n for a constant d. The DVH is irrelevant for this model. For whatever radiation treatment given to a population of similar patients under similar circumstances, the NTCP0 is calculated as ratio of the number of patients without acute/late complications and total of them.
