14. Review of Key Performance Indicator in a Hematology Laboratory: A Preventable Source of Error.

Abstract

OBJECTIVE: To assess the key performance indicators and the laboratory errors in the hematology laboratory. METHODOLOGY: A cross-sectional study was conducted at the DDRRL Hematology laboratory from October 2021 to September 2022. Data from the blood bank and hematological laboratory were collected. The primary investigator managed every step of the data-gathering operation. All the tools required for the process were gathered and arranged on a tray or trolley to be safe, accessible, and readily visible. Sterile glass or plastic tubes with rubber covers were used for blood collection. The information acquired during the study was inserted into a computer using an Excel sheet. SPSS version 20 was used for the analysis. All samples from the hematology section of DDRRL were included in this study, while all error-free samples were excluded. RESULT: From July to December, 414,400 hematology request forms were collected from OPD, private wards, IPD and Emergency. Overall, 2376 (0.573%) hematology laboratory errors were detected, of which 419 were Pre-analytical errors, 122 were analytical, and 1834 were post-analytical errors. CONCLUSION: We can lower the errors' likelihood if we appropriately handle the pre-analytical variables. Abstract

BACKGROUND: Water-pipe smoking appears to be stimulated by unfolded assumptions of relatively safety compared to cigarettes, as well as the social nature of the activity. Commercial marketing, often with implicit or explicit safety related claims, may also be contributing to the spread of water-pipe smoking across the globe. MATERIAL & METHODS: Cross-sectional analytical study conducted among 13-19 years old students going to a private secondary school; were divided into two groups one who smoked water-pipe and the others who does not smoke. A structured and validated self-administered questionnaire was used to collect base-line data related to the knowledge, attitude and practice of water-pipe tobacco smoking. It was then followed by clinical dental examination using Plaque, gingival, modified papillary bleeding and clinical attachment loss. Data was entered and analyzed using Statistical Package for Social Sciences (SPSS). RESULTS: Among 320 subjects, 154 subjects were water pipe tobacco smokers while 166 subjects were non water pipe tobacco smoker. While, upon clinical examination of subjects only clinical attachment loss was found to be significant with the p-value of 0.000. CONCLUSION: Therefore, in aspect to periodontal health, water-pipe tobacco smoking does have an impact on the periodontal status. Abstract

Background: Donor notification of reactive status is important to prevent the spread of disease. Response of reactive donors to seek confirmation and treatment is a direct reflection of their knowledge and attitudes towards transfusion transmittable infections., Methods: A cross sectional observational study was conducted from August 2014 to July 2015 at the blood bank of a tertiary care hospital, Karachi, Pakistan. Reactive donors' notification and responses were noted with reasons of failure. A cross-sectional analytical survey with non-probability purposive sampling was done on 350 potential consenting blood donors using a pre-tested questionnaire to assess their knowledge and attitude about disease awareness, transmission routes, financial implications and disease sensitization., Results: Out of 16660 donations, 5.57% were rejected on positive screening tests. Repeat donors (69.5%) with primary to secondary qualifications constituted the bulk of reactive donors. Donor notification rate were 54.25% whereas 28.68% donors responded to blood bank in person. The survey showed limited awareness about transfusion transmitted infections. Respondents who were ignorant of disease spread through blood transfusion comprised of 48%. 96.6% donors did not know the financial impact of treatment and 69.7% were unable to afford it. Moreover, 94.9% donors were not protected against hepatitis B. Participants with secondary education had significantly less odds of being adequately knowledgeable (OR=0.372, 95% CI: 0.203-0.681, p-value <0.01) but more likely to have a positive attitude., Conclusions: There is need for structured pre-donation counselling to sensitize donors about transfusion related diseases in resource limited countries where treatment costs are high and out of reach for most donors..

Immunoglobulin A (IgA) pemphigus is a rare autoimmune blistering disease characterized by epidermal acantholysis and neutrophilic infiltrates, as well as intraepidermal IgA deposits. We report an unusual case of IgA pemphigus involving anal/ perianal skin and oral mucosa that demonstrated a successful response to dapsone treatment.

We report an unusual presentation of sinus histiocytosis with massive lymphadenopathy (SHML), also called Rosai Dorfman disease. The child presented with bilateral proptosis with massive cervical lymphadenopathy. The patient had fair response to steroid therapy but relapsed during the course of the disease.
