Abstract During the treatment of 89 pediatric patients with Acute Myeloid Leukemia (AML) at the Hematology Department of Kunming Medical University’s Children’s Hospital from 2020 to 2023, three patients were identified to co-express the NUP98-NSD1, FLT3-ITD, and WT1 gene mutations. The bone marrow of these three patients was screened for high-risk genetic mutations using NGS and qPCR at the time of diagnosis. The treatment was administered following the China Children’s Leukemia Group (CCLG)-AML-2019 protocol. All three patients exhibited a fusion of the NUP98 exon 12 with the NSD1 exon 6 and co-expressed the FLT3-ITD and WT1 mutations; two of the patients displayed normal karyotypes, while one presented chromosomal abnormalities. During the induction phase of the CCLG-AML-2019 treatment protocol, the DAH (Daunorubicin, Cytarabine, and Homoharringtonine) and IAH (Idarubicin, Cytarabine, and Homoharringtonine) regimens, in conjunction with targeted drug therapy, did not achieve remission. Subsequently, the patients were shifted to the relapsed/refractory chemotherapy regimen C + HAG (Cladribine, Homoharringtonine, Cytarabine, and G-CSF) for two cycles, which also failed to induce remission. One patient underwent Haploidentical Hematopoietic Stem Cell Transplantation (Haplo-HSCT) and achieved complete molecular remission during a 12-month follow-up period. Regrettably, the other two patients, who did not receive transplantation, passed away. The therapeutic conclusion is that pediatric AML patients with the aforementioned co-expression do not respond to chemotherapy. Non-remission transplantation, supplemented with tailor-made pre- and post-transplant strategies, may enhance treatment outcomes.

Abstract Background GeneXpert MTB/RIF (Xpert) assay was applied widely to detect Mycobacterium tuberculosis (MTB) and rifampicin resistance. Methods Retrospectively investigated the association among treatment histories, phenotypic drug susceptibility testing (pDST) results, and clinical outcomes of patients infected with probe A absent mutation isolate confirmed by Xpert. Results 63 patients with only probe A absent mutation and 40 with additional pDST results were analyzed. 24 (60.0%) patients had molecular-phenotypic discordant rifampicin (RIF) susceptibility testing results, including 12 (12/13, 92.3%) new tuberculosis (TB) patients and 12 (12/27, 44.4%) retreated ones. 28 (28/39, 71.8%) retreated patients received first-line treatment regime within two years with failed outcomes. New patients had better treatment outcomes than retreated ones (successful: 83.3% VS. 53.8%; P value = 0.02). The clinical results of RIF-susceptible TB confirmed by pDST were not better than RIF-resistant TB (successful: 62.5% VS. 50.0%; P value = 0.43). INH-resistant TB and INH-susceptible TB had similar treatment outcomes too (successful: 61.5% VS. 50.0%; P value = 0.48). 11 (11/12, 91.7%) new patients treated with the short treatment regimen (STR) had successful outcomes. Conclusions More than half of mono probe A absent isolates had RIF molecular-phenotypic discordance results, especially in new patients. Probe A mutations were significantly associated with unsuccessful clinical outcomes, whether the pDST results were RIF susceptible or not. STR was the best choice for new patients. For networking of big data applications, an essential issue is how to represent networks in vector space for further mining and analysis tasks, e.g., node classification, clustering, link prediction, and visualization. Most existing studies on this subject mainly concentrate on monoplex networks considering a single type of relation among nodes. However, numerous real-world networks are naturally composed of multiple layers with different relation types; such a network is called a multiplex network. The majority of existing multiplex network embedding methods either overlook node attributes, resort to node labels for training, or underutilize underlying information shared across multiple layers. In this paper, we propose Multiplex Network Infomax (MNI), an unsupervised embedding framework to represent information of multiple layers into a unified embedding space. To be more specific, we aim to maximize the mutual information between the unified embedding and node embeddings of each layer. On the basis of this framework, we present an unsupervised network embedding method for attributed multiplex networks. Experimental results show that our method achieves competitive performance on not only node-related tasks, such as node classification, clustering, and similarity search, but also a typical edge-related task, i.e., link prediction, at times even outperforming relevant supervised methods, despite that MNI is fully unsupervised.

Abstract Ischemic stroke stands as a prevalent neurological ailment, where a paucity of methodologies exists for the assessment of functional outcomes post-stroke. Our objective was the development of a WeChat applet for the evaluation of muscle strength and the subsequent evaluation of its validity in ischemic stroke patients experiencing motor dysfunction. The assessment encompassed Lovett and NIHSS, followed by muscle strength values (MSV) and muscle strength ratios (MSR). These metrics were then scrutinized in relation to NIHSS and Lovett, followed by examination of their interrelationships. We enrolled a cohort of 157 patients, with an average age of 65 years, consisting of 96 males and 61 females. Lovett scores in the range of 2–4 and NIHSS scores spanning from 0 to 3 were found to correspond to specific values of MSV and MSR, respectively. Upon conducting correlation analysis, we noted that both MSV and MSR exhibited significant positive correlations with Lovett scores and NIHSS. Remarkably, the correlation of MSR with Lovett scores or NIHSS surpassed that of MSV. The WeChat applet offers a means of digitization and visualization of muscle strength. It correlates well with Lovett score and NIHSS, especially MSR. This bears potential significance in guiding the rehabilitation of stroke patients.

Quantum-cutting luminescent solar concentrators (QC-LSCs) have great potential to serve as large-area solar windows. These QC nanocrystals can realize a photoluminescence quantum yield (PLQY) of as high as 200% with virtually zero self-absorption loss. Based on our previous work, we have constructed a Monte Carlo simulation model that is suitable to simulate the performance of the QC-LSCs, which can take into account the band-edge emissions and near-infrared emissions of the QC-materials. Under ideal PLQY conditions, CsPbClxBr3−x:Yb3+-based LSCs can reach 12% of the size-independent external quantum efficiency (ηext). Even if LSCs have a certain scattering factor, the CsPbClxBr3−x:Yb3+-based LSCs can still obtain an ηext exceeding 6% in the window size (>1 m2). The flux gain (FG) of the CsPbClxBr3−x:Yb3+-based LSC-PV system can reach 14 in the window size, which is a very encouraging result.

PurposeThe development of tuberculosis and inflammatory status are closely related. The aim of this study was to investigate the prognostic value of inflammatory biomarkers in patients with rifampicin/multidrug-resistant tuberculosis (RR/MDR-TB).Patients and methodsThis study recruited 504 patients with RR/MDR-TB from Wuhan Jinyintan Hospital. A total of 348 RR/MDR patients from January 2017 to December 2019 were defined as training set, the rest of patients as validation set. The patients were divided into three-risk degrees according to the levels of inflammatory biomarkers (median, 85th percentile). Kaplan-Meier curve and log-rank test were used to assess survival differences among the groups. Cox proportion risk regression was used to identify risk factors for RR/MDR-TB mortality.ResultsIn training set, cox proportion risk regression analysis showed that high age (≥60 years) [OR (95%CI):1.053(1.03188-1.077)], smoking [OR (95%CI):2.206(1.191-4.085)], and bronchiectasia [OR (95%CI):2.867(1.548-5.311)] were prognostic factors for RR/MDR-TB patients. In addition, lower survival rates were observed in high CAR group [OR (95%CI):1.464(1.275-1.681)], high CPR group[OR (95%CI):1.268(1.101-1.459)], high CLR group[OR (95%CI):1.004(1.002-1.005)], high NLR group[OR (95%CI):1.103(1.069-1.139)], high PLR group[OR (95%CI):1.003(1.002-1.004)], and high MLR group[OR (95%CI):3.471(2.188-5.508)].Furthermore, AUCs of age, smoking, bronchiectasia, CAR, CPR, CLR, NLR, PLR, and MLR for predicting mortality in RR/MDR-TB patients were 0.697(95%CI:0.618-0.775), 0.603(95%CI:0.512-0.695), 0.629(95%CI:0.538-0.721), 0.748(95%CI:0.675-0.821, P

Objective To investigate if the addition of bedaquiline and clofazimine to a treatment regimen for multidrug-resistant tuberculosis (MDR-TB) could improve patient outcomes. Methods A prospective, randomized, controlled study was conducted in patients with MDR-TB. Treatment was for 18 months. Patients in the experimental group received bedaquiline and clofazimine in addition to their regular treatment regimen whereas patients in the control group did not. Results 68 patients with MDR-TB were randomised to treatment, 34 to each group. At the end of treatment, cure rates were statistically significantly greater for the experimental group compared with the control group (82% vs. 56%). There was no difference between groups in the number of severe adverse events (3[9%]) in both groups and none were skin-related. Conclusions The addition of bedaquiline and clofazimine to the treatment regimen significantly improves outcomes for patients with MDR-TB. Clinicians should be aware of the clinical benefits of this addition but be mindful of contraindications and adverse effects.

Setting: The shorter treatment regimen (STR) for multidrug- or rifampicin-resistant tuberculosis (MDR/RR-TB) has achieved successful outcomes in many countries. However, there are few studies on high-dose gatifloxacin-based STR with adverse drug reactions (ADRs) and management. Design: A prospective observational study was conducted with MDR/RR-TB patients who were treated with a standardized 9 or 12 - month regimen: including gatifloxacin (Gfx), clofazimine (Cfz), ethambutol (EMB), and pyrazinamide (PZA), and supplemented by amikacin (Am), isoniazid (INH), and prothionamide (Pto) during an intensive phase of 4 or 6 - month. Monitored ADRs monthly until treatment completion and then followed up every three months for one year. Results: Among the 42 eligible patients, 35 (83.3%) completed treatment successfully, 1 (2.4%) lost to follow-up (LTFU), and 6 (14.3%) failed due to ADRs, with no death. The most important ADR was drug-induced liver damage, which occurred in 24 out of 42 (57.1%) patients and resulted in 4 (9.5%) failed treatments and 4 (9.5%) adjusted treatments. QT interval prolongation occurred in 17 out of 42 (40.5%) patients, 9 (21.4%) of them with the corrected QT interval according to Fridericia (QTcF) > 500 ms resulting in 7 (16.7%) adjusted treatments. Conclusions: This study confirmed the effectiveness of the high-dose gatifloxacin-based STR but severe ADRs, especially hepatotoxicity and QT interval prolongation should never be ignored.

The current study aimed to examine the association between serum selenium levels and lipids and explore whether the association was modified by diabetic status. A total of 4132 adults from the National Health and Nutrition Examination Survey (2011–2016) were included in this study. Multiple linear regression models were used to estimate the association between serum selenium and lipids. Higher serum selenium levels were significantly associated with increased total cholesterol (TC) (p < 0.001), triglyceride (TG) (p = 0.003), and low-density lipoprotein cholesterol (LDL-C) (p = 0.003) in the overall population. The Nanmushu Zn–Pb deposit is a large-scale and representative deposit in the Mayuan ore field on the northern margin of the Yangtze Block. This study investigates the trace element geochemistry of sphalerite from this deposit using laser ablation inductively coupled plasma mass spectrometry (LA-ICP-MS). The results show that the main trace elements in sphalerite include various trace elements, such as Mn, Fe, Cu, Ga, Ge, Ag, Cd, Pb, Co, Hg, Tl, In, Sn, and Sb. Among them, Ag, Ge, Cd, and Cu are valuable components that may be recovered during mineral processing or smelting techniques. The histograms, LA-ICP-MS time-resolved depth profiles, and linear scan profiles indicated that most trace elements occur in sphalerite as isomorphs, while partial Pb, Fe, and Ag occur as tiny mineral inclusions. The correlation diagrams of trace elements revealed that Fe2+, Mn2+, Pb2+, and Tl3+ can substitute Zn2+ in sphalerite through isomorphism. In sphalerite, Cd2+ and Hg2+ together or Mn2+, Pb2+, and Tl3+ together can replace Zn2+, i.e., ((3Mn, 3Pb, 2Tl)6+, 3(Cd, Hg)2+) ↔ 3Zn2+. Moreover, there is a mechanism of Ge4+ with Cu+ or Ga3+ with Cu+ replacing Zn2+ in the Nanmushu deposit, i.e., Ge4+ + 2Cu+ ↔ 3Zn2+ or 2Ga3+ + 2Cu+ ↔ 4Zn2+. A DC instrument transformer test set (ITTS), as an error measurement device for DC instrument transformers, needs to be calibrated regularly to ensure its accuracy. Existing calibration technology can only achieve DC ITTS error calibrations within a zero analog standard error. This paper proposes a wide-range digital-analog mixed calibration scheme for a DC ITTS based on multistage serial proportional micro-difference technology and high-speed high-precision sampling and coding technology, achieving a digital-analog standard error of ±1% that can be fine-tuned in 0.001% steps. A prototype is developed, and the prototype test results show that the expanded uncertainty of the prototype does not exceed $1\times 10^{-4}$ , the maximum error of the standard error does not exceed $\pm \,\,3\times 10^{-5}$ , and the output frequency of the digital protocol message is 4 kHz and 12.8 kHz. This scheme can achieve the error calibration of the analog and digital channels for the DC ITTS within a ±1% standard error and solve the error calibration problem of wide-range digital channels for DC ITTSs.

Oily sludge is recognized as hazardous waste. To reduce the potential danger and harmful factors of oily sludge, it is very important to analyze its environmental risk. In this paper, the characterization of oily sludge from Shengli Oilfield in China was tested experimentally, including the composition content, particle size, microscopic morphology, heavy metal content, organic composition, inorganic composition, and thermogravimetric analysis, which were used to analyze environmental risks. The results show that the oil content of oily sludge is as high as 10.3%, which will cause serious pollution. It is calculated that China can recover 772.5 million liters of oil and reduce 553.9 million kg of carbon emissions compared with incineration in one year, if the oily sludge can be managed effectively. The content of heavy metals such as Ba, Zn, Cr, As, Ni, Se, Be, and Hg in oily sludge exceeds the standard. It will restrain the self-healing ability of soil, pollute groundwater, and endanger animals and plants. The organic matter of oily sludge is concentrated in C11 to C29. It contains a large amount of benzene series and polycyclic benzene hydrocarbons, which can lead to cancer in the human body. Inorganic substances in oily sludge are mixed with some additives, which can not only reduce the toxicity of heavy metals, but also be used as building materials. The median particle size D50 of oily sludge is 0.91 μm, and it spreads all over the narrow pores. Generally, it needs to be treated under high temperature conditions, which will cause secondary pollution to the environment. The research content of this paper provides a theoretical reference for the management of oily sludge.

Aiming at the problems of pipeline blockage and equipment wear caused by large sand production in shallow gas hydrate mining on the seabed, based on the solid-state fluidization mining method, the idea of in situ separation of natural gas hydrate is proposed, and the downhole design is based on the physical parameters of hydrate mixed slurry. For the in situ helical separator, the CFD-Fluent commercial software was used to establish an analysis model and optimize the response surface of the model. The effects of the three-stage variable-pitch helix and blades on the performance of the separation device were investigated. The simulation results and response surface optimization were conducted through experiments to verify the accuracy. The results show that the third-stage pitch has the greatest impact on the separation efficiency and pressure drop, while the first-stage pitch has the least impact. The pressure drop and separation efficiency are fully considered. After the response surface optimization, the optimal three-stage pitch is the first-stage pitch x1 = 72.227 mm, the pitch of the second stage x2 = 105 mm, and the pitch of the third stage x3 = 124.817 mm. The separation efficiency of the optimal structure is verified by experiments. Compared with the previously used fixed-pitch downhole cyclone separator, the three-stage variable pitch cyclone, the separator improves the separation efficiency from 88.29% to 97.16% while keeping the pressure drop unchanged.

Deep water shallow natural gas hydrate (NGH) is a kind of clean energy and has entered the commercial exploitation stage. However, it produces a lot of seabed sediment in the process of large-scale mining, which not only easily causes undersea natural hazards, but also leads to pipeline equipment blockage and high energy consumption in the mining process. A downhole solid–liquid separator can effectively separate natural gas hydrate from sand and backfill sand in situ, which can effectively solve this problem. In this paper, the safety of a downhole solid–liquid separator desander under torsion conditions is determined by a test method. A numerical simulation method was used to simulate the tension and pressure of the downhole solid–liquid separator, and a modal simulation analysis and erosion analysis of the downhole solid–liquid separator were carried out. The experiments showed that the downhole solid–liquid separator could withstand 30 KN/m of torque, and a numerical simulation analysis showed that it could withstand 30 MPa of pressure and 50 KN of tension. The results show that the maximum stress is 116.56 MPa, and the maximum allowable stress is 235 MPa. The modal analysis showed that the downhole solid–liquid separator produces resonance at a frequency of about 93 Hz, resulting in large deformation, which should be avoided as far as possible. Through the erosion analysis, the life of the downhole solid–liquid separator was determined to be about 2.3 years. Numerical simulation and experimental results show that the designed downhole solid–liquid separator for natural gas hydrate can ensure safety.

Previous studies have proposed higher requirements for the transient characteristics of a DC transformer used in a flexible high-voltage direct current (HVDC) system to achieve faster sampling speed and meet wider bandwidth requirements of the control and protection signal, and to eventually suppress the large transient fault current. In this study, a transient characteristics verification method is proposed for transient characteristics verification of a DC transformer used in a flexible HVDC system based on resampling technology and LabVIEW measurement technology after analyzing the key technology for transient characteristics verification of a DC transformer. A laboratory experiment for the transient characteristics of a full-fiber electronic DC transformer is conducted, and experimental results show that such verification method can be employed for frequency response and step response verification of a DC transformer at 10% of the rated voltage and current, and can eventually improve the screening of a DC transformer. Background: Anhydrous ethanol, for its part, has been successfully used to treat renal cyst, hepatocellular carcinoma and ovarian chocolate cyst et al. However, in spite of the high frequency of tuberculous purulent pleural effusion, we found that only a few very early studies that attempted to assess the use of intrapleural anhydrous ethanol in tuberculous effusions with signs of empyema. We report a patient who was injected anhydrous ethanol into pleural cavity to treat chronic tuberculous empyema. Case presentation: A 23-year old male was admitted in the hospital because of chronic tuberculous empyema. Ultra-sonography guided thoracentesis and thoracic close drainages were done, but had no effect. However, the patient refused Video-assisted Thoracoscopic Surgery (VATS) and traditional thoracotomy. Therefore, we injected anhydrous ethanol into the pleural cavity after getting the patient's consent. Pyothorax was quickly controlled and the patient finally recovered fully. Conclusion: Surgical operation is the main treatment of chronic tuberculous empyema, which has a high cost and large injury, and many patients do not accept this treatment. In this study, intrapleural injection of anhydrous ethanol could achieve the purpose of eliminating the pus cavity, which is particularly suitable for patients who cannot tolerate surgery or are unwilling to undergo surgical treatment. Community detection and structural hole spanner (the node bridging different communities) identification, revealing the mesoscopic and microscopic structural properties of complex networks, have drawn much attention in recent years. As the determinant of mesoscopic structure, communities and structural hole spanners discover the clustering and hierarchy of networks, which has a key impact on transmission phenomena such as epidemic transmission, information diffusion, etc. However, most existing studies address the two tasks independently, which ignores the structural correlation between mesoscale and microscale and suffers from high computational costs. In this article, we propose an algorithm for simultaneously detecting communities and structural hole spanners via hyperbolic embedding (SDHE). Specifically, we first embed networks into a hyperbolic plane, in which, the angular distribution of the nodes reveals community structures of the embedded network. Then, we analyze the critical gap to detect communities and the angular region where structural hole spanners may exist. Finally, we identify structural hole spanners via two-step connectivity. Experimental results on synthetic networks and real networks demonstrate the effectiveness of our proposed algorithm compared with several state-of-the-art methods.

In civil aviation ground deicing system, the traditional reciprocating pump is often used as the power supply device to ensure the physical and chemical properties of high viscosity deicing fluid. The traditional reciprocating pump exist the problem of flow fluctuation. A new horizontal space multiphase crankshaft pump is proposed to suppress the flow fluctuation. The new type of pump adopts segmented crankshaft: each crank pin on the crankshaft drives two connecting rods simultaneously. The connecting rod drives the pistons distributed on both sides of the crankshaft to achieve linear reciprocating motion. Three sections of the crank pin are arranged by 120° phase space circular, and this kind of crankshaft implements multiple phase coupling of piston movement. The hydraulic cylinder adopts the horizontal layout, so the machine structure presents spatial symmetry. A mathematical model of two connecting rods driven by crankpin is established for the new type of pump. The kinematics equation of the piston is solved by the analytical method and then the flow rate of the pump is calculated. In contrast with the experimental data, the new pump can effectively reduce the flow fluctuation.
