4. Molecular Characterization of Genital and Extragenital Lesions With the PlexPCR VHS Assay in Patients Diagnosed With Syphilis.

Abstract

Background Syphilis diagnosis relies on immunologic markers and clinical protocols. However, syphilitic lesions can be confused with other genital ulcer diseases. Methods Using a PlexPCR VHS assay, we analyzed lesion DNA samples from 87 individuals who were clinically diagnosed with early syphilis infection and had at least 1 positive serologic test result. DNA was detected by the PlexPCR VHS multiplex assay and β-globin genes. Results Among the participants, 99% (86/87) had a positive rapid treponemal test result. DNA was successfully detected in 91% (79/87) of the lesion samples. PlexPCR VHS identified 5 herpes simplex virus (HSV)/ Treponema pallidum coinfections (2 HSV-1 and 3 HSV-2), only T pallidum DNA in 62% (49/79), and only HSV-2 in 12.7% (10/79). While 19% (15/79) were negative for all pathogens, none were varicella zoster virus positive. The PlexPCR VHS had 68.4% agreement with the clinical diagnosis. Conclusions Since the PlexPCR VHS detects multiple organisms simultaneously, it can help to confirm actual syphilis and identify other pathogen coinfections or the pathogen causing the ulcer. Abstract

Background The increasing prevalence of drug-resistant Neisseria gonorrhoeae(NG) infections has caused great concern. Ciprofloxacin remains the empiric antimicrobial recommended to treat NG infections in Peru disregarding the susceptibility profile of circulating NG strains. We report the prevalence of individuals infected with NG strains presenting mutations in the gyrAgene that confers ciprofloxacin resistance.Methods We conducted a descriptive study assessing extragenital swab samples collected from a cohort of men who have sex with men and transgender women in Lima, Peru. Anal and pharyngeal NG positive swabs for Aptima Combo 2 assay (Hologic Inc., USA) were used for DNA extraction. We performed TaqMan real time PCR assays to detect a point mutation at codon Ser91 of the gyrase A (gyrA) gene.Results From 156 individuals who had at least one positive sample for NG reported by the Aptima assay, 80 individuals had at least one amplified DNA for the gyrAgene. We found that 67 of them (84.0%) were infected with a gyrA-mutated NG strain at the Ser91 codon.Conclusions We report a high prevalence of gyrAmutation conferring ciprofloxacin resistance among individuals with extragenital NG infection. Empirical treatment of NG needs to be urgently updated in Peru in concordance with international guidelines.

11. P380: Laboratory Evaluation of Oral Fluid for Syphilis Screening, Lima, Peru.

Abstract

Background: Syphilis remains a curable bacterial sexually transmitted infection (STI), with a global burden of 7.1 million new cases during 2020. Serological tests for syphilis require equipment unavailable in many health centers; however, point-of-care testing has allowed rapid screening using finger-prick whole blood samples. A further improvement could be the use of oral fluid. We evaluated the performance of treponemal antibody point-of-care testing for syphilis screening using oral fluid samples. Methods: Individuals who routinely attended STI clinics in Lima, Peru, were recruited. We collected oral fluid using the OraSure collection device (OraSure Technologies Inc., USA) according to manufacturer's instructions and serum from each participant. Oral fluid and serum were analyzed using the SD Bioline Syphilis 3.0 rapid test (Standard Diagnostics Inc., Korea). Oral fluid was processed using 30µL + 2 drops of sample diluent, while serum was processed using 10µL + 4 drops of sample diluent; serum were also tested using RPR (RPR slide test, Wiener Laboratorios SAIC, Argentina) and TPPA (Serodia, Fujirebio Diagnostics Inc., Japan). We assessed oral fluid rapid test overall percent agreement, sensitivity, and specificity against the following comparators: (1) serum SD Bioline Syphilis, (2) serum TPPA, and (3) both TPPA and RPR reactive or both TPPA and RPR non-reactive. Results: Among 324 participants, median age was 31.5 years (IQR: 11.5) and 71.6% were male. Nearly half (51.3%) reported prior syphilis and 30.6% were living with HIV. The overall percent agreement, sensitivity, and specificity of oral fluid was 71.0% (95% CI: 65.7% -- 75.9%), 78.2% (71.3% -- 83.8%), and 63.5% (55.8% -- 70.6%), respectively, versus serum SD Bioline Syphilis; similar results were obtained versus serum TPPA. The overall percent agreement and sensitivity increased when using the third comparator (74.6% [68.8% -- 79.8%] and 86.6% [78.9% -- 92.3%], respectively). When limiting the sample to TPPA-reactive with RPR titer ≥ 1:8, the sensitivity increased to 88.1% (75.0% -- 94.8%), but the percent agreement decreased to 70.4% (63.3% -- 76.9%). Conclusions: We observed a good performance of the rapid treponemal test using oral fluid. Further investigations are needed to improve the specificity of oral fluid as a potential sample for accurate syphilis screening. [ABSTRACT FROM AUTHOR]

12. P281: Trends and Predictors of Antenatal HIV Testing: Analysis of National Demographic Health Surveys in Peru.

Abstract

Background: Pregnant women should be tested for HIV at least once to prevent mother-to-child transmission (MTCT) of HIV. In addition, there are unknown disparities in access to HIV services that result in HIV non-testing, leading to a high risk of MTCT. This study aimed to analyze trends of antenatal HIV testing coverage in Peru over the last 12 years and to perform a machine learning-based model to identify the most suitable predictors that increase the probability that a woman not being tested for HIV during pregnancy. Methods: Data compiled from annual Demographic and Health Surveys (2010 - 2021) were analyzed. Overall, 37526 women aged 15-45 years met the inclusion criteria. Evolution of HIV testing coverage was assessed according to education level and wealth index. Regarding machine learning models, three algorithms were included: XGBoost, Decision Tree, and LightGBM. Data from 2010 to 2019 was split into 80% (training and validation) and 20% (testing), further validation was conducted using 2021 data. Model performance was evaluated according to an f1 score, sensitivity, and specificity. Variable importance of the best model was estimated according to the Gain metric. All analyses were carried out using R software. Results: HIV testing coverage increased (from 73.3% to 90.5%) between 2010 and 2019, although it decreased (59.7%) in 2020, and increased moderately (83.8%) in 2021. This coverage varied by education level (47.8% of women with none/preschool education were tested for HIV, while 91.9% of women with higher education were tested), and wealth index (61.1% of poorest women were tested for HIV, while 94.5% of richest women were tested). The XGBoost model showed the best performance with an f1 score of 0.883, sensitivity of 68.7%, and specificity of 82.5%. Top five predictors for not getting tested for HIV were residing in an urban area, not knowing any sexually transmitted disease (STD), the poorest wealth index, not knowing any STD symptoms, and none/preschool education level. Conclusion: Over the last 12 years, antenatal HIV testing has reached high coverage; however, it is still below the UNAIDS goal of 95%. Efforts should be concentrated on pregnant women with lower levels of wealth, education, and knowledge about HIV/STD. [ABSTRACT FROM AUTHOR]

Abstract

HIV incidence estimation is critical for monitoring the HIV epidemic dynamics and the effectiveness of public health prevention interventions. We aimed to identify sexual and gender minorities (SGM) with recent HIV infections, factors associated with recent HIV infection, and to estimate annualised HIV incidence rates.
