Abstract Background We hypothesized that postural instability observed in individuals with Parkinson's disease (PD) can be classified as distinct subtypes based on comprehensive analyses of various evaluated parameters obtained from time-series of center of pressure (CoP) data during quiet standing. The aim of this study was to characterize the postural control patterns in PD patients by performing an exploratory factor analysis and subsequent cluster analysis using CoP time-series data during quiet standing. Methods 127 PD patients, 47 aged 65 years or older healthy older adults, and 71 healthy young adults participated in this study. Subjects maintain quiet standing for 30 s on a force platform and 23 variables were calculated from the measured CoP time-series data. Exploratory factor analysis and cluster analysis with a Gaussian mixture model using factors were performed on each variable to classify subgroups based on differences in characteristics of postural instability in PD. Results The factor analysis identified five factors (magnitude of sway, medio-lateral frequency, anterio-posterior frequency, component of high frequency, and closed-loop control). Based on the five extracted factors, six distinct subtypes were identified, which can be considered as subtypes of distinct manifestations of postural disorders in PD patients. Factor loading scores for the clinical classifications (younger, older, and PD severity) overlapped, but the cluster classification scores were clearly separated. Conclusions The cluster categorization clearly identifies symptom-dependent differences in the characteristics of the CoP, suggesting that the detected clusters can be regarded as subtypes of distinct manifestations of postural disorders in patients with PD.

Objective: The aim of this study was to cross-sectionally investigate how static balance changes throughout the aging process in older women aged from their early sixties to their late eighties. Methods: Forty-six older women (aged 62-89 years) were requested to stand barefoot and quietly on a force platform for 30 s with their eyes either open or closed. During the trials, the position of the center of foot pressure (CoP) and the acceleration of the body's center of mass (ACC) were measured. The root mean square (RMS) of the CoP and ACC values was calculated to evaluate the amplitude of postural sway and the level of regulatory activity, respectively. The mean power frequency of the ACC was also calculated to represent the temporal characteristics of regulatory activity. Results: In the anterior-posterior direction, there was no significant relationship between the RMS of CoP and the participants' age, whereas the RMS of ACC significantly increased with increasing age. In the mediolateral direction, however, the RMS of CoP significantly increased with increasing age, whereas the RMS of ACC did not change with age. The mean power frequency of ACC did not exhibit any age-related change in either the anterior-posterior or the mediolateral direction. Conclusion: The results indicate that static balance in older women aged in their early sixties to their late eighties exhibits distinctly contrasting aging patterns between the anterior-posterior and mediolateral directions. Virtual reality (VR) paradigms have proved to be a valid method to challenge and perturb balance. There is little consensus in the literature on the best protocol design to perturb balance and induce postural sway. Current VR interventions still lack a well-defined standardized metric to generate a virtual environment that can perturb balance in an efficacious, lifelike, and repeatable manner. The objective of this study was to investigate different configurations of amplitude and frequency in an anterior–posterior translation VR environment, that is, lifelike and scaled. Thirteen young adults with no conditions affecting balance were recruited. Balance was challenged by anterior–posterior sinusoidal movement of the lab image within the VR headset. Four different amplitudes of the sinusoidal movement were tested: 1, 5, 10, and 20 cm, with each amplitude being presented at 2 test frequencies : 0.5 and 0.25 Hz. Mean center of pressure velocity was significantly greater than baseline at 0.5 Hz and amplitudes of 10 and 20 cm. Mean center of pressure at approximate entropy was greater than baseline at 0.5 Hz and amplitude of 20 cm. The results suggest that sinusoidal movement of a realistic VR environment produces altered balance compared with baseline quiet standing, but only under specific movement parameters. The aim of this study was to investigate the effects of backpack load carriage on quiet standing postural control and limits of stability (LOS) of older adults. Fourteen older adults (65 [6] y) performed quiet standing and a forward, right, and left LOS test in 3 conditions, unloaded, stable, and unstable backpack loads while activity of 4 leg muscles was recorded. Stable and unstable loads decreased postural sway (main effect ηp² = .84, stable P < .001, unstable P < .001), mediolateral (main effect ηp² = .49, stable P = .002, unstable P = .018) and anterior--posterior (main effect ηp² = .64, stable P < .001, unstable P = .001) fractal dimension, and LOS distance (main effect ηp² = .18, stable P = .011, unstable P = .046) compared with unloaded. Rectus femoris (main effect ηp² = .39, stable P = .001, unstable P = .010) and gastrocnemius (main effect ηp² = .30, unstable P = .027) activity increased in loaded conditions during LOS and quiet standing. Gastrocnemius--tibialis anterior coactivation was greater in unstable load than stable loaded quiet standing (main effect ηp² = .24, P = .040). These findings suggest older adults adopt conservative postural control strategies minimizing the need for postural corrections in loaded conditions. Reduced LOS may also increase fall risk when carrying a load. However, there was no difference between unstable and stable loads for postural control variables. [ABSTRACT FROM AUTHOR]

Accurate estimation of the center of mass is necessary for evaluating balance control during quiet standing. However, no practical center of mass estimation method exists because of problems with estimation accuracy and theoretical validity in previous studies that used force platforms or inertial sensors. This study aimed to develop a method for estimating the center of mass displacement and velocity based on equations of motion describing the standing human body. This method uses a force platform under the feet and an inertial sensor on the head and is applicable when the support surface moves horizontally. We compared the center of mass estimation accuracy of the proposed method with those of other methods in previous studies using estimates from the optical motion capture system as the true value. The results indicate that the present method has high accuracy in quiet standing, ankle motion, hip motion, and support surface swaying in anteroposterior and mediolateral directions. Exoskeleton devices can reduce metabolic cost, increase walking speed, and augment load-carrying capacity. However, little is known about the effects of powered assistance on the sensory information required to achieve these tasks. To learn how to use an assistive device, humans must integrate novel sensory information into their internal model. This process may be disrupted by challenges to the sensory systems used for posture. We investigated the exoskeleton-induced changes to balance performance and sensory integration during quiet standing. We asked 11 unimpaired adults to perform a virtual reality-based test of sensory integration in balance (VRSIB) on two days while wearing the exoskeleton either unpowered, using proportional myoelectric control, or with regular shoes. We measured postural biomechanics, muscle activity, equilibrium scores, postural control strategy, and sensory ratios. Results showed improvement in balance performance when wearing the exoskeleton on firm ground. The opposite occurred when standing on an unstable platform with eyes closed or when the visual information was non-veridical. The balance performance was equivalent when the exoskeleton was powered versus unpowered in all conditions except when both the support surface and the visual information were altered. We argue that in stable ground conditions, the passive stiffness of the device dominates the postural task. In contrast, when the ground becomes unstable the passive stiffness negatively affects balance performance. Furthermore, when the visual input to the user is non-veridical, exoskeleton assistance can magnify erroneous muscle inputs and negatively impact the user’s postural control.

The mechanism by which postural threat induced by standing at a high height causes a decrease in the amplitude and an increase in the frequency of postural sway might involve voluntary control (VC) to avoid swaying, rather than conscious balance processing, in which postural threat directs conscious balance processing. This study aimed to clarify the differences between VC and conscious balance processing during quiet standing. Twenty-seven healthy young adults were instructed to stand with their feet placed together and keep their eyes open. The standing task was performed under three standing conditions: relaxed, VC, and high-conscious movement processing (high-CMP). The center of pressure in the anteroposterior (AP) and mediolateral (ML) directions was measured using a stabilometer to assess differences in postural control. The results indicated that the mean power frequency (MPF) ML and high frequency (HF) ML were higher in the VC condition than in the high-CMP condition. In the VC and high-CMP conditions, compared with the relaxed condition, MPF AP was higher, whereas the root mean square AP and low frequency AP were lower. These results show that the sway amplitude is lower, and the frequency is higher in both the VC and high-CMP conditions compared with those in the relaxed condition; however, the frequency is higher in the VC condition than in the high-CMP condition, suggesting that the VC condition is similar to postural control under the postural threat condition. The quiet standing test is used to detect diseases of the postural control system. The descriptive statistics of the center of pressure (COP) of older people during the test tend to be larger than those of healthy young people, but they cannot indicate structural problems in postural control. COP trajectories can be mathematically modeled with structural parameters such as viscosity, stiffness, and stochastic terms; however, the classification accuracy of older and fall-experienced people using such parameters has not been sufficiently verified. In this study, six structural parameters of a mass–spring–damper (MSD) model were estimated using two datasets, in which a total of 212 subjects performed quiet standing tests under four conditions. The estimated parameters were used for classification with a random forest algorithm to examine the differences in classification accuracy compared to seven conventional descriptive statistics methods. For the classification of older subjects, the classification accuracy of the MSD parameter method was the highest in foam condition, with positive likelihood ratios approximately 8.0. For the classification of fall-experienced subjects, the positive likelihood ratio of the MSD parameter method was 5.0, which is better than conventional descriptive statistics. Various MSD parameters revealed that aging and changing the floor surface and visual conditions cause oscillations in the COP behavior. While the MSD parameters were confirmed to help classify older subjects more accurately than the conventional descriptive statistics, there was room for further improvement in the classification accuracy of fall-experienced subjects.

Postural control impairments following a stroke have an impact on mobility, reduce independence, and increase the risk of falls. Assessing these impairments during tasks representative of real-life situations, such as quiet standing (QS) and voluntary stepping response (VSR), will enhance our understanding of how the postural control system is impaired in individuals post-stroke (IPS). It will also inform the development of a more targeted and effective rehabilitation to prevent falls in IPS. Identify the postural control impairments encountered by IPS during QS and VSR. Twenty IPS and 16 healthy controls were recruited to perform QS and VSR tasks, while ground reaction forces and whole-body motion were measured. Displacement and speed variation of the COM, center of pressure (COP) displacement and spatiotemporal data were calculated and compared between groups. During QS, IPS exhibited greater maximal COP displacement in mediolateral direction, COM displacement in vertical direction and COM speed excursions compared to controls. During VSR, IPS exhibited smaller step length, braking force, posterior foot placement in relation to the pelvis and COM anteroposterior excursion compared to controls. IPS presented less static and dynamic postural stability compared to controls. Greater postural sway during QS, smaller anteroposterior COM displacement before losing balance and altered voluntary recovering steps during VSR could place IPS at more risk of falling when they face a postural challenge in the community. Background: This study explored useful indices of potential fall risk in healthy older adults based on comprehensive clinical and simple quantitative posturographic measurements., Methods: A total of 64 community-dwelling older adults aged ≥65 years were classified into fallers and non-fallers based on previous fall history. After excluding two participants due to missing data, 16 and 46 participants were included in the faller and non-faller groups, respectively. We conducted major clinical measurements, including timed up and go test, 10-m walk test, functional reach test, one-leg stand test, isometric muscle strength tests, open-close stepping test, and sit-to-stand test. For quantitative posturographic measurements, participants performed standing tasks with their eyes open and closed. In the standing tasks, the time series of the center of pressure in the anteroposterior and mediolateral directions were measured and decomposed into rambling and trembling components to evaluate postural control in detail, separately. The mean velocity and root mean square of the center of pressure, rambling, and trembling were calculated and compared between fallers and non-fallers., Findings: A significant fall-related difference was found only in the mean velocity of the rambling in the anteroposterior direction; fallers displayed greater values than non-fallers, especially while standing with their eyes closed., Interpretation: The clinical measures failed to distinguish differences in fall risk, whereas the rambling component in the anteroposterior direction could detect substantial differences. Parkinson's disease (PD) is a neurodegenerative disease that affects motor abilities with increasing severity as the disease progresses. Traditional methods for diagnosing PD include a section where a trained specialist scores qualitative symptoms using the motor subscale of the Unified Parkinson's Disease Rating Scale (UPDRS-III). The aim of this feasibility study was twofold. First, to evaluate quiet standing as an additional, out-of-clinic, objective feature to predict UPDRS-III subscores related to motor symptom severity; and second, to use quiet standing to detect the presence of motor symptoms. Force plate data were collected from 42 PD patients and 43 healthy controls during quiet standing (a task involving standing still with eyes open and closed) as a feasible task in clinics. Predicting each subscore of the UPDRS-III could aid in identifying progression of PD and provide specialists additional tools to make an informed diagnosis. Random Forest feature importance indicated that features correlated with range of center of pressure (i.e., the medial-lateral and anterior-posterior sway) were most useful in the prediction of the top PD prediction subscores of postural stability (r = 0.599; p = 0.014), hand tremor of the left hand (r = 0.650; p = 0.015), and tremor at rest of the left upper extremity (r = 0.703; p = 0.016). Quiet standing can detect body bradykinesia (AUC-ROC = 0.924) and postural stability (AUC-ROC = 0.967) with high predictability. Although there are limited data, these results should be used as a feasibility study that evaluates the predictability of individual UPDRS-III subscores using quiet standing data. Background: The stochastic resonance (SR) phenomenon has been used to improve postural control through the application of imperceptible noise to the somatosensory system. White noise signals have been applied in numerous SR studies on postural control. However, because the SR effect depends on the noise structure, the stimulation effects of signals with different structures, such as pink noise and chaotic signals, on postural control, must be determined to achieve better clinical applications of SR technology.Research Question: During quiet standing, how is postural control affected by subthreshold electrical stimulation to the knee joints when signals with different structures (white noise, pink noise, and chaotic signals) are used?Methods: Sixteen healthy young adults stood quietly for 40 s with their eyes closed. To evaluate postural sway, we calculated the mean velocity, root mean square (CoPRMS), and range (CoPRange) values for the center of pressure (CoP) in the anteroposterior direction. The standing task was conducted under subthreshold electrical stimulation with white noise, pink noise, and chaotic signals based on the Lorenz system, in addition to the no-stimulation condition. The four stimulation conditions were randomized within each set and repeated seven times.Results: Significant effects of stimulation were observed in the CoPRMS and CoPRange values. The CoPRMS value under the pink noise signal was significantly lower than that under the no-stimulation condition. The CoPRange value also tended to decrease under the pink noise signal compared with the no-stimulation condition; however, the differences were not statistically significant. No significant changes were found with the white noise and chaotic signals compared with the no-stimulation condition.Significance: We demonstrated that the pink noise signal was more effective in reducing postural sway than the white noise and chaotic signals based on the Lorenz system during quiet standing. BACKGROUND: Knee osteoarthritis (OA) impairs postural control and may affect how the lower limb joints are used for postural control. OBJECTIVE: To investigate how individuals with knee OA use lower limb joints for static postural control. METHODS: Ten patients with knee OA and thirteen healthy controls performed quiet standing for 30 s. The standard deviation of the center of mass (COM) and lower limb joint motions in the anterior-posterior (AP) and medial-lateral (ML) planes were calculated from three-dimensional marker trajectories. Pearson's correlation analysis and independent t -tests were conducted to investigate the relationship between COM and lower limb joint motion and to compare group difference, respectively. RESULTS: The AP hip angular velocity alone in the knee OA group and the AP hip and knee angular velocity in the control group were significantly correlated with the AP COM velocity. The ML hip angular velocity was significantly correlated with the ML COM velocity in both groups. The knee OA group exhibited a significantly larger standard deviation of AP COM velocity than the control group. CONCLUSIONS: Individuals with knee OA depended solely on the contribution of the hip to the AP COM velocity, which could not be successfully controlled by the knee. [ABSTRACT FROM AUTHOR]

Background: Theoretically, balance is affected by the height of center of mass (COM) during quiet standing. However, no one examined this in humans with variables derived from the center of pressure (COP). Objects: We have conducted balance experiment to measure COP data during quiet standing, in order to examine how the COP measures were affected by the height of COM, vision, floor conditions, and gender. Methods: Twenty individuals stood still with feet together and arms at sides for 30 seconds on a force plate. Trials were acquired with three COM heights: 1% increased or decreased, and not changed, with two vision conditions: eyes closed (EC) and eyes open (EO), and with two floor conditions: unstable (foam pad) and stable (force plate) floor. Outcome variables included the mean distance, root mean square distance, total excursion, mean velocity, and 95% confidence circle area. Results: All outcome variables were associated with the COM height (p < 0.0005), vision (p < 0.0005), and floor condition (p < 0.003). The mean velocity and 95% confidence circle area were 5.7% and 21.8% greater, respectively, in raised COM than in lowered COM (24.6 versus 23.2 mm/s; 1,013.4 versus 832.3 mm2). However, there were no interactions between the COM height and vision condition (p > 0.096), and between the COM height and floor condition (p > 0.183) for all outcome variables. Furthermore, there was no gender difference in all outcome variables (p > 0.186). Conclusion: Balance was affected by the change of COM height induced by a weight belt in human. However, the effect was not affected by vision or floor condition. Our results should inform the design of balance exercise program to improve the outcome of the balance training.

Unperceivable electrical noise stimulation has been applied to improve postural control through the enhancement of somatosensory feedback. It has been observed that stimulation with a pink noise (1/ f) structure is more effective than stimulation with other noise structures. In addition, the 1/ f structure embedded in the postural control system may have a superior effect on postural control stabilization. However, the direct relationship between the long-range correlations of the pink-noise signal applied to somatosensory receptors and those of the postural control system has not been elucidated. Thus, we aimed to explore a common long-range correlation factor shared in the time series of the provided noise and foot center of pressure (CoP) during quiet standing. Sixteen young adults stood quietly on the force platform for 65 s. Four noise conditions (no stimulation and stimulation of knee joints with white-, pink-, and red-noise-like signals) were employed during the standing trials. The detrending moving-average cross-correlation analysis revealed that in each of the anteroposterior and mediolateral directions, the CoP velocity time series displayed significant long-range cross-correlations with the white and pink noise signals provided at that time, whereas such an effect was not observed in the red noise signal. This result indicates that pink and white noise signals would alter the temporal behavior of the CoP during quiet standing, although the mechanism remains to be elucidated. [ABSTRACT FROM AUTHOR]

Abstract Background Gymnastics training enhances the development of postural control in children and adolescents. In competitive gymnastics, the training regimen is specific to the given gymnastic discipline and is usually followed from the early years of practicing. This study aimed to determine whether postural steadiness differed between young gymnasts practicing two distinct disciplines, and whether it was related to the duration of their training experience, age, and their anthropometric characteristics. Methods Thirty 10–13-year-old females ̶ ten artistic gymnasts, ten acrobatic gymnasts (training as “tops”), and ten non-athletes ̶ were examined during 60-s quiet standing trials on a force platform with the eyes open and closed. Their postural sway was represented by directional components of centre of foot pressure mean velocity. Mann-Whitney U, Wilcoxon and Spearman’s ρ tests were used for analyses. Results Anterior-posterior and medial-lateral centre of foot pressure mean velocities were not different between the artistic and acrobatic gymnasts (p > 0.05). In the artistic gymnasts, the duration of training experience, age, body height, body mass, and maturity offset were negatively correlated with the anterior-posterior centre of foot pressure mean velocity under eyes-open conditions. The acrobatic gymnasts’ body mass and BMI percentiles were negatively correlated with their anterior-posterior and medial-lateral centre of foot pressure mean velocities under both visual conditions (r ranged from − 0.64 to − 0.93; p 0.05). Conclusions The artistic gymnasts’ longer training experience, greater age, body height, body mass, and biological maturity were associated with better anterior-posterior postural steadiness when vision was available; the acrobatic gymnasts’ greater body mass and BMI percentiles were associated with better overall postural steadiness regardless of visual conditions. Relationships were observed between postural steadiness and discipline-specific training experience and anthropometric characteristics; however, causes and effects were not proven.

Previous research has reported mixed findings regarding age-related changes in dynamic postural stability, quantified by margin of stability (MOS), during gait. However, age-related changes in MOS may be better elicited by tasks imposing greater challenges to the postural control system. Older adults' MOS during obstacle crossing, a destabilizing task, has previously been characterized, although studies comparing MOS during this task between younger and older adults remain sparse. This study investigated age-related changes in dynamic postural stability during quiet standing, gait, and obstacle crossing. Participants aged 20–30 (n = 20), 60–69 (n = 18), 70–79 (n = 15), and 80+ (n = 7; not analyzed statistically) years old performed these tasks while whole-body motion was tracked using motion capture. MOS in each direction was estimated throughout each trial, and integrals, transient ranges, and trial minima were extracted (as applicable). MOS time series were also ensemble averaged across age groups. No age-related differences were identified for quiet standing or gait. However, obstacle crossing metrics revealed greater stability (i.e., more positive MOS) and less instability (i.e., less negative MOS) in older adults, and reduced ranges during transients. These findings potentially arise from shorter step lengths, which may be the result of age-related physical declines; or may reflect a cautious strategy in older adults, which maximizes postural stability in the direction with the greatest consequences for foot-obstacle contact, as it changes throughout the task. This study supports the use of tasks imposing physical challenges and/or voluntary perturbations to study age-related changes in dynamic postural stability. Findings also contribute to our theoretical understanding of the time course of dynamic postural stability during functional tasks in relation to periods of transition in the base of support, and task-specific strategies adopted for obstacle crossing by older adults to maintain dynamic postural stability and mitigate fall risk. • Age-related effects on dynamic postural stability were examined. • Features of margin of stability signals were compared between age groups. • Age-related differences primarily manifested during obstacle crossing. • Older adults showed greater stability and less instability than younger adults. • Potential strategies for postural stability control in older adults were revealed. Stance stability in individuals with bilateral spastic cerebral palsy (BSCP) in various standing postures including the quiet standing (QS) and limits of stability (LoS) has been widely studied. However, the relationships between the QS and LoS remain unclear. This study aimed to determine the relationships between the positions and postural sway in the QS and anteroposterior LoS in individuals with BSCP. It included 27 adolescents and young adults with BSCP (BSCP group) and 27 adolescents and young adults without disability (control group). The position of center of pressure in the anteroposterior direction (CoPy position) and the path length of center of pressure (CoP path length) during the QS and the anterior and posterior LoS (A-LoS and P-LoS, respectively) were measured using a force platform. The CoPy positions in the A-LoS and P-LoS in the BSCP group were limited compared with those in the control group. In the BSCP group, the more anterior the CoPy position in the QS, the more anterior (i.e., limited) it was in the P-LoS. Although the CoP path length in the QS was larger in the BSCP group, those in the A-LoS and P-LoS were larger in the control group. The BSCP group also showed that the more anterior the CoPy position or the longer the CoP path length in the QS, the more decreased the anteroposterior LoS range was. Force plate analyses of various activities sometimes require the average (WPav) and standard deviation (WPsd) of force across the Weighing Phase (i.e., quiet period) to calculate kinetic, temporal and kinematic metrics. Yet, the influence of weighing phase duration on these analyses has been scarcely investigated. This study investigated the effects of weighing phase duration on the agreement between vertical force-time variables and the repeatability of WPav and WPsd. Durations of 0.5, 1.0 and 1.5 s were compared to 2.0 s. Limits of agreement (LOA) for system weight, onset threshold, onset time, net impulse, take-off velocity and take-off displacement were calculated for 137 counter-movement, squat and single leg jumps. Repeatability coefficients for WPav and WPsd estimated the consistency between repeated trials. Shorter weighing phase durations produced small differences in WPav (LOA < ±0.25%), which accumulated during integration, affecting net impulse, take-off velocity (LOA ±2%) and take-off displacement (LOA ±23%). Differences were substantial using 5xWPsd as the onset threshold (LOA approximately ±25% to ±72%), consequently influencing onset time (LOA approximately ±6% to ±18%). WPav repeatability was high but the within-trial differences could augment with integration, requiring weighing phases longer than 2 s. Clinicians frequently assess and intervene on postural alignment; however, notions of what constitutes good postural alignment are variable. Furthermore, the majority of current evidence appeals either to population norms or defines good postural alignment as the negation of what has been observed to correlate with pathology. The purpose of this study was to identify affirmative indicators of good postural alignment in reference to motor control theory. Electromyography (anterior leg, posterior leg, and trunk muscles) and motion capture data were acquired from 13 participants during 4 min bipedal standing trials in 4 conditions: control, − 10%, + 30%, and + 60% of subject-specific anterior limits of stability. Synergistic kinematic coordination was quantified via the uncontrolled manifold framework, and correlated neural drive was quantified in posture-relevant muscle groups (anterior, posterior, and trunk) via intermuscular coherence. Multilevel models assessed the effects of sagittal plane alignment on both outcomes. We observed a within-subjects fixed effect in which kinematic synergistic coordination decreased as subjects became more misaligned. We also observed within-subjects fixed effects for middle- and high-frequency intermuscular coherence in the posterior group (increased coherence with increased misalignment) and for trunk intermuscular coherence across all frequency bands (decreased coherence with increased misalignment). Our findings indicate that it may be possible to describe healthy postural alignment in light of referent control theory. Greater misalignment with respect to vertical is associated with compromises in synergistic control of posture and increased corticospinal drive to specific muscle groups. A force plate is one of the most popular tools for postural stability assessment. It decomposes the ground reaction force (GRF) applied to the human body into multiple force components to determine the center of pressure (COP), which is the point at which the GRF is applied. Many COP measures have been proposed to characterize postural stability. Despite postural stability being closely related to GRF components, the interactions between them have rarely been studied. By studying the correlation structure of these GRF components, we developed a set of features to assess postural stability. We determined the correlation matrix of these GRF components and subsequently solved the corresponding eigenvalue problem; we used the resulting eigenvalues to characterize postural stability. The effectiveness of the proposed features was demonstrated by using them to differentiate between individuals in two age groups: 18–24 and 65–73 years. Statistical test results showed that the correlation matrix eigenvalues of the two age groups differed significantly. The classification results demonstrated that most of the correlation matrix eigenvalues were more sensitive to age variations than one of the most reliable and accurate conventional COP features. Furthermore, by reducing the force sensing requirement from three-dimensional to one-dimensional by considering only the vertical GRF components, a simplified version of the proposed approach can be obtained, which could reduce the cost of the force plate system substantially.

Elderly and disabled people frequently experience falls that may require early assessment and training. The quiet standing test provides descriptive statistics such as the center of pressure (COP) sway data, which can be used to analyze the decline in balance ability. Generally, higher descriptive statistics indicate lower balance ability. Stochastic models can model COP trajectories, and such equation parameters were previously used to assess the balance of patients. However, the model equation is a hypothesis and should be verified. In this study, we evaluated whether stochastic models can predict the descriptive statistics of observed COP trajectories. We estimated the model parameters by fitting postural sway data from 49 individuals in four linear stochastic models, and the prediction accuracy was verified by comparing the observed descriptive statistics with the predicted COP trajectories. We observed that the prediction accuracy of the models with stiffness was much higher than those with viscosity only, and the models with the center of mass (COM) had higher prediction accuracy than COP-only models. Therefore, we identified that coefficients from Langevin-based models contained descriptive statistics about a subject’s COP trajectory, which suggested that these coefficients can be used to effectively assess balance issues.

The purpose of this study was to examine the effects of occasional and habitual wearing of high-heeled shoes on static balance in young women. Groups of habitual high-heel wearers and non-wearers (n = 7 in both groups) were asked to stand quietly on a force platform without shoes (WS condition) or with high heels (heel area 1 cm2, heel height 7 cm) (HH condition). During the trials, the center-of-pressure (CoP) position in the anterior-posterior direction was measured, and its root mean square (as a measure of postural sway magnitude, CoPRMS) and mean velocity (as a measure of regulatory activity, CoPMV) were calculated. To further examine the effect of high-heel wearing on the temporal aspects of slow and fast processes in static balance, the CoP sway was decomposed into low- (below 0.5 Hz) and high- (above 0.5 Hz) frequency components, and then spectral analysis was performed. Results showed that the CoPRMS was not significantly different between the groups or between the shoe conditions, indicating that wearing high heels with a heel height of 7 cm did not increase the magnitude of postural sway, irrespective of high-heel experience. The CoPMV was significantly larger in the HH condition than in the WS condition, whereas it was not significantly different between the groups. This result indicates that wearing high heels increased the amount of regulatory activity in both habitual wearers and non-wearers. The spectral analysis further showed that habitual high-heel wearers showed significantly decreased rate of regulatory activity than non-wearers, both while standing with and without high heels. These results suggest that use-dependent changes in static balance control are evident in both high-heeled and without shoes conditions.

The quiet standing test is used to detect diseases of the postural control system. The descriptive statistics of the center of pressure (COP) of older people during the test tend to be larger than those of healthy young people, but they cannot indicate structural problems in postural control. COP trajectories can be mathematically modeled with structural parameters such as viscosity, stiffness, and stochastic terms; however, the classification accuracy of older and fall-experienced people using such parameters has not been sufficiently verified. In this study, six structural parameters of a mass–spring–damper (MSD) model were estimated using two datasets, in which a total of 212 subjects performed quiet standing tests under four conditions. The estimated parameters were used for classification with a random forest algorithm to examine the differences in classification accuracy compared to seven conventional descriptive statistics methods. For the classification of older subjects, the classification accuracy of the MSD parameter method was the highest in foam condition, with positive likelihood ratios approximately 8.0. For the classification of fall-experienced subjects, the positive likelihood ratio of the MSD parameter method was 5.0, which is better than conventional descriptive statistics. Various MSD parameters revealed that aging and changing the floor surface and visual conditions cause oscillations in the COP behavior. While the MSD parameters were confirmed to help classify older subjects more accurately than the conventional descriptive statistics, there was room for further improvement in the classification accuracy of fall-experienced subjects. Coordinated movements of both lower limbs may be a clinically important indicator of motor control during quiet standing. From a neurological point of view, it is known that extensive coupling of muscles must be coordinated an upright posture. However, movement coordination between the lower limbs is the final motor output, is unknown. In this study, we focussed on the ground reaction force (GRF) vector and clarified the time and frequency characteristics of the force vectors of both lower limbs. A total of 16 healthy young adults and 18 healthy older adults participated and placed each bare foot on one of two force plates to measure the GRF vectors (i.e., anteroposterior, mediolateral, and vertical) of each lower limb and determine the centre of mass (COM) acceleration in the anteroposterior direction (COMacc). Characteristics of the coordination of both lower limbs during movements were analysed using coherence analysis and cross-correlation function analysis (CCF). The coherence levels of the force vectors of both lower limbs were higher in all three directions and significantly increased in the older adults. CCF analysis showed that the force vectors of both lower limbs were negatively correlated at the zero-time lag. Moreover, a weak correlation was observed between COMacc and coherence values. The assessment of bilateral lower limb connectivity using force vectors can be used as an evaluation method to reflect changes in the ability to control bipedal standing during ageing. Background: In this study, three detecting approaches have been proposed and evaluated for online detection of balance situations during quiet standing. The applied methods were based on electromyography of the gastrocnemius muscles adopting the hidden Markov models. Methods: The levels of postural stability during quiet standing were regarded as the hidden states of the Markov models while the zones in which the center of pressure lies within determines the level of stability. The Markov models were trained by using the well-known Baum-Welch algorithm. The performance of a single hidden Markov model, the multiple hidden Markov model, and the multiple hidden Markov model alongside an adaptive neuro-fuzzy inference system (ANFIS), were compared as three different detecting methods. Results: The obtained results show the better and more promising performance of the method designed based on a combination of the hidden Markov models and optimized neuro-fuzzy system. Conclusion: According to the results, using the combined detecting method yielded promising results.

The goal of this study was to examine the effects of 3 factors and their interactions on posturography: a period of time to become accustomed to the force platform before the initiation of data collection, presence of a visual fixation point, and participant talking during testing. The postural stability of 30 young adults and 30 older adults was evaluated to determine whether any observed effects were confounded with age. Analysis of variance techniques were used to test all possible combinations of the 3 factors. We hypothesized that all 3 factors would significantly affect postural stability. For both participant groups, the results suggest that a period of time to become accustomed to the force platform before the initiation of data collection and a visual fixation point significantly affect postural control measures, while brief participant talking does not. Despite this, no significant interactions existed suggesting that the effects of these factors, which may occur in clinical testing, do not depend on each other. Our results suggest that inconsistencies in posturography testing methods have the potential to significantly affect the results of posturography, underscoring the importance of developing a standardized testing methodology. Two experiments examined the roles of the dimensions of movement-specific reinvestment (movement selfconsciousness and conscious motor processing) on performance under demanding conditions. In Experiment 1, novice golfers practiced a golf putting task and were tested under low- and high-anxiety conditions. Conscious motor processing was not associated with putting proficiency or movement variability; however, movement self-consciousness was positively associated with putting proficiency and appeared to be negatively associated with variability of impact velocity in low-anxiety conditions, but not in high-anxiety conditions. Increased anxiety and effort possibly left few attention resources for movement self-consciousness under high anxiety. In Experiment 2, participants performed a quiet standing task in single- and dual-task conditions. Movement self-consciousness was positively associated with performance when attention demands were low (single task) but not when attention demands were high (dual task). The study of human balance recovery strategies is important for human balance rehabilitation and humanoid robot balance control. To date, many efforts have been made to improve balance during quiet standing and walking motions. Arm usage (arm strategy) has been proposed to control the balance during walking motion in the literature. However, limited research exists on the contributions of the arm strategy for balance recovery during quiet standing along with ankle and hip strategy. Therefore, in this study, we built a simplified model with arms and proposed a controller based on nonlinear model predictive control to achieve human-like balance control. Three arm states of the model, namely, active arms, passive arms, and fixed arms, were considered to discuss the contributions of arm usage to human balance recovery during quiet standing. Furthermore, various indexes such as root mean square deviation of joint angles and recovery energy consumption were verified to reveal the mechanism behind arm strategy employment. In this study, we demonstrate to computationally reproduce human-like balance recovery with and without arm rotation during quiet standing while applying different magnitudes of perturbing forces on the upper body. Individuals with motor incomplete spinal cord injuries (iSCI) often have impaired abilities to maintain upright balance. For ablebodied (AB) individuals, the ankle and hip joint accelerations are in antiphase to minimize the postural sway during quiet standing. Here we investigated how interjoint coordination between the ankle and hip joints was affected in individuals with iSCI, leading to their larger postural sway during quiet standing. Data from 16 individuals with iSCI, 14 age- and sex-matched AB individuals, and 13 young AB individuals were analyzed. The participants performed quiet standing during which kinematic and kinetic data were recorded. Postural sway was quantified using center-of-pressure velocity and center-of-mass acceleration. Individual ankle and hip joint kinematics were quantified, and the interjoint coordination was assessed using the cancellation index (CI), goal-equivalent variance (GEV), nongoal-equivalent variance (NGEV), and uncontrolled manifold (UCM) ratio. Individuals with iSCI displayed greater postural sway compared with AB individuals. The contribution of ankle angular acceleration toward one's sway was significantly greater for those with iSCI compared with AB groups. CI and the UCM ratios were not statistically different between the groups, while GEV and NGEV were significantly greater for the iSCI group compared with the AB groups. We demonstrated that individuals with iSCI show larger postural sway compared with the AB individuals during quiet standing, primarily due to larger ankle joint acceleration. We also demonstrated that the interjoint coordination between ankle and hip joint is not affected in individuals with iSCI, which is not successfully able to reduce the large COM acceleration. This study characterized the center of pressure planar displacement by palindromic strings. The objective is to test if the center of pressure pathway of able-bodied girls and those with a moderate and severe scoliosis displayed similar palindromic tendencies. The center of pressure excursions of 21 able-bodied girls were compared to 14 girls with a moderate scoliosis and 14 girls with severe one. Each girl was asked to stand upright on a force platform for 64 s. A crisscross grid of nine areas was centered around the mean center of pressure position (G) to define three other zones to use the MATLAB built-in nucleotide sequence analysis function. These were the antero-posterior extremities A, the coronal extremities C and the tilted or the four corners of the crisscross grid, T. The center of pressure positions were associated to any of the 4 zones using the GATC acronym. For all groups center of pressure pattern in decreasing order was A, G, T and C. Able-bodied girls favored the A zones. Girls with moderate scoliosis displaced their center of pressure mostly in the A zones with shifts in the T sections (P ≤ 0.001). Girls with severe scoliosis, additionally displaced their center of pressure in the C zones (P ≤ 0.001). An ankle modality characterized able-bodied girl's standing balance. Girls with a moderate scoliosis privilege the palindromic zones in the antero-posterior extremities with excursions in the corners of the base of support, girls with severe scoliosis further relied on the medio-lateral zones, suggesting a wobbling standing balance. • Center of pressure displacements were measured for 21 able-bodied and 28 scoliotic girls. • MATLAB nucleotide analysis function allowed the center of pressure palindromic sequencing. • The palindromic strings revealed that scoliotic subjects adopted a wobbling standing balance. • Standing balance control follows organized pathways in different zones of the base of support. Abstract Background Falling is the most common accident of daily living and the second most prevalent cause of accidental death in the world. The complex nature of risk factors associated with falling makes those at risk amongst the elderly population difficult to identify. Commonly used clinical tests have limitations when it comes to reliably detecting the risk of falling, but existing laboratory tests, such as force platform measurements, represent one method of overcoming this lack of a test. Despite their widespread use, however, Center of Pressure (COP) signal analysis techniques vary and there is currently no consensus on which features should be used diagnostically. Our objective is to identify, through a systematic review and meta-analysis, the COP characteristics of older adults (≥ 60 years old) during quiet bipedal stance which will allow fallers to be distinguished from non-fallers. Methods The systematic review will include both prospective and retrospective articles. Five databases will be searched: PubMed, Cochrane CENTRAL, EMBASE, and ScienceDirect. In addition, a search of gray literature will be performed using Google Scholar and ClinicalTrials.gov. Searches will be circumscribed to include only older adults (aged over 60 years) who underwent a bipedal quiet standing measure of their balance and for whom the number of falls was reported. Two authors will independently assess the risk of bias for each included article using a 26-item checklist. Funnel plots will be drawn to attest of possible publication biases for each COP parameters. The results will be synthesized descriptively and a meta-analysis will be undertaken. When trial methodological heterogeneity is too great for pooling of the data into a meta-analysis, evidence strength will be evaluated using best evidence analysis. Discussion Despite the numerous advantages of posturography, the diversity of studies exploring balance in older fallers has led to uncertainty regarding the method’s ability to reliably identify fall-prone older adults. It is expected that the findings from this systematic review will help clinicians use bipedal quiet standing measures as a diagnostic test and allow researchers to explore COP characteristics to create better models for fall prevention care. We investigated the patterns of coordination between the left and right legs that support the task of maintaining an upright standing posture. We used cross-wavelet analyses to assess coordination between the centers of pressure under the left and right feet. We recruited participants with a lateralized functional preference for their right leg, and we manipulated whether these participants stood with symmetric/asymmetric stances and whether their eyes were open or closed. Our hypotheses were derived from the Haken-Kelso-Bunz (HKB) model of interlimb coordination dynamics. Consistent with HKB model predictions, we observed (1) coordination taking the form of metastable, transient epochs of stable phase relations, (2) preferences for in-phase and anti-phase coordination patterns, and (3) changes in pattern stability and phase leads associated with both stance asymmetry and right-side lateral preference. The form and stability of observed coordination patterns were mediated by the availability of visual information. Our findings confirm the existence of a metastable coordination dynamic associated with the task of maintaining upright stance. We discuss the implications of these findings in the context of evaluating the utility of the HKB model for understanding the functional organization of the posture system. [ABSTRACT FROM AUTHOR]

Freezing of gait (FoG) is one of the main reasons for movement initiation disorders and abnormal coupling of posture and gait in Parkinson's disease (PD). Patients with FoG have poor postural control when compared to patients without FoG. However, the nature of the interrelationship between FoG and domains of postural control remains unknown. The aim of this study was to estimate the association between different domains of postural control and severity of FoG in patients with moderate‐to‐severe PD. Thirty patients with idiopathic PD with FoG (age range 45–80 years, Hoehn & Yahr stages 3 and 4) participated in the study. We evaluated objective (FoG‐ratio during turning task) and subjective (New Freezing of Gait Questionnaire, NFoG‐Q) measures of FoG severity, reactive postural adjustments in response to an external perturbation, first step anticipatory adjustment for step initiation and quiet standing stability. In the multiple regression analysis, step initiation was the strongest significant correlation of the NFoG‐Q score explaining 23% of the variance of the assessment. For the objective FoG measure, mediolateral CoP amplitude in quiet standing and mediolateral CoP amplitude in step initiation explained 39% of the variance of the FoG‐ratio. As main conclusions, this study identified the association between objective and subjective measure for FoG severity and postural control domains. Background: Gymnastics training enhances the development of postural control in children and adolescents. In competitive gymnastics, the training regimen is specific to the given gymnastic discipline and is usually followed from the early years of practicing. This study aimed to determine whether postural steadiness differed between young gymnasts practicing two distinct disciplines, and whether it was related to the duration of their training experience, age, and their anthropometric characteristics. Methods: Thirty 10–13-year-old females ̶ ten artistic gymnasts, ten acrobatic gymnasts (training as "tops"), and ten non-athletes ̶ were examined during 60-s quiet standing trials on a force platform with the eyes open and closed. Their postural sway was represented by directional components of centre of foot pressure mean velocity. Mann-Whitney U, Wilcoxon and Spearman's ρ tests were used for analyses. Results: Anterior-posterior and medial-lateral centre of foot pressure mean velocities were not different between the artistic and acrobatic gymnasts (p > 0.05). In the artistic gymnasts, the duration of training experience, age, body height, body mass, and maturity offset were negatively correlated with the anterior-posterior centre of foot pressure mean velocity under eyes-open conditions. The acrobatic gymnasts' body mass and BMI percentiles were negatively correlated with their anterior-posterior and medial-lateral centre of foot pressure mean velocities under both visual conditions (r ranged from − 0.64 to − 0.93; p < 0.05). The non-athletes' centre of foot pressure mean velocities were non-significantly correlated with their age and anthropometric measures under both visual conditions (p > 0.05). Conclusions: The artistic gymnasts' longer training experience, greater age, body height, body mass, and biological maturity were associated with better anterior-posterior postural steadiness when vision was available; the acrobatic gymnasts' greater body mass and BMI percentiles were associated with better overall postural steadiness regardless of visual conditions. Relationships were observed between postural steadiness and discipline-specific training experience and anthropometric characteristics; however, causes and effects were not proven. Objective: The aim of this study was to cross-sectionally investigate how static balance changes throughout the aging process in older women aged from their early sixties to their late eighties., Methods: Forty-six older women (aged 62-89 years) were requested to stand barefoot and quietly on a force platform for 30 s with their eyes either open or closed. During the trials, the position of the center of foot pressure (CoP) and the acceleration of the body's center of mass (ACC) were measured. The root mean square (RMS) of the CoP and ACC values was calculated to evaluate the amplitude of postural sway and the level of regulatory activity, respectively. This study analyzed the development of postural control and the cost of the cognitive task on postural control in the bipedal standing position during childhood. Sixty-six normally developed children divided into four groups by age participated in this study. Single (ST) and dual-tasks (DT) were conducted in the bipedal standing position with eyes open. DT included carrying out a cognitive task consisting of a modified Stroop Test. A Wii Balance Board was used to measure postural sway data. The results showed effects of age and condition on postural control (p < 0.01). The interaction of age x condition showed that in DT, 4-year-old children presented higher ellipse area than 5, 6, 7-year-olds (p < 0.01). Moreover, an effect of age on cognitive performance was found (p < 0.001). In conclusion, four-year old children present lower postural control than 5, 6, 7-year-olds. The center of pressure (COP), which is defined as the point at which the resultant ground reaction force (GRF) is applied on a body, provides valuable information for postural stability assessment. This is because the fundamental goal of balance control is to regulate the center of mass (COM) of the human body by adaptively changing the position of the COP. By using Newtonian mechanics to develop two equations that relate the two-dimensional COP coordinates to the GRF components, one can easily determine the location of the COP using a force plate. An important property of these two equations is that for a given COP position, there exists an infinite number of GRF component combinations that can satisfy these two equations. However, the manner in which a postural control system deals with such redundancy is still unclear. To address this redundancy problem, we introduce four postural stability features by quantifying the coupling strengths between the COP coordinates and their GRF components. Experiments involving younger (18–24 years old) and older (65–73 years) participants were conducted. The efficacy of the proposed features was demonstrated by comparing the differences between variants of each feature for each age group (18–24 and 65–73 years). The results demonstrated that the coupling strengths between the anterior–posterior (AP) direction coordinate of the COP and its GRF components for the older group were significantly higher than those of the younger group. These experimental results suggest that (1) the balance control system of the older group is more constrained than that of the younger group in coordinating the GRF components and (2) the proposed features are more sensitive to age variations than one of the most reliable and accurate conventional COP features. The best testing classification accuracy achieved by the proposed features was 0.883, whereas the testing classification accuracy achieved by the most accurate conventional COP feature was 0.777. Finally, by investigating the interactions between the COP and its GRF components using the proposed features, we found that that the AP component of the GRF of younger people plays a more active role in balance control than that of the GRF of older people. Based on these findings, it is believed that the proposed features can be used as a set of stability measures to assess the effects on posture stability from various health-related conditions such as aging and fall risk. To assess how postural sway deficits during eyes open and closed relate to the integrity of cerebellar peduncles in individuals with multiple sclerosis (MS). Cross-sectional study. Laboratory based setting. Twenty-nine adults with MS (Expanded Disability Status Scale: 2-4) and 15 adults without MS were recruited (N=44). Inclusion criteria for all participants were ability to maintain balance independently by standing on toes for 3 seconds, and no known biomechanical conditions affecting balance. Not applicable. Postural sway using body-worn, inertial sensors during quiet standing, integrity of cerebellar peduncles quantified using diffusion-tensor imaging and clinical assessment scales for ataxia and balance. Radial diffusivity of the inferior cerebellar peduncle was related to postural sway measures during both eyes open and closed. In contrast, radial diffusivity of the superior cerebellar peduncle was related to postural sway only in stance with eyes open. The inferior cerebellar peduncle, which carries somatosensory information to the cerebellum, contributes to control of standing balance with or without visual inputs, consistent with the high dependence on somatosensory information for posture. The superior cerebellar peduncle, which carries cortical information to the cerebellum, contributes to control of standing posture only when vision is available. Radial diffusivity of the inferior cerebellar peduncle was related to reactive balance control, whereas radial diffusivity of the superior cerebellar peduncle was related to the kinetic component of the ataxia rating scale. This paper proposes a new intermittent control model during human quiet standing, which is consisted of postulated "regular intervention" and "imminent intervention". The regular intervention is within the main control loop, and its trigger condition is equivalent to the switching frequency of center of pressure (COP) data calculated by wavelet transform. The imminent intervention will only be triggered after the postural sway angle exceeds a certain threshold. In order to prove the effectiveness of the new model, the simulation results of the new model and the model proposed by Asai et al. (2009) are compared with the experimental data. The setting parameters of both models are retrieved by Bayesian regression from the experimental data. The results show that the new model not only could exhibit two power law scaling regimes of power spectral density (PSD) of COP, but also show that indices of the probability density function distance, root mean square (RMS), Total Sway Path, displacement Range, 50% power frequency of center of mass (COP) between the simulation results and the experimental data are closer compared to the existing model. Moreover, the limit cycle oscillations (LCOs) obtained from the simulation results of the new model have a higher degree of matching with those retrieved from the experimental data. Purpose: Predicting body Center of Mass (COM) acceleration is carried out with more accuracy based on the acceleration of three joints of lower limb compared to only accounting joints of hip and ankle. Given that trunk movement during quite standing is noticeable, calculating trunk acceleration in model might increase prediction accuracy of COM acceleration. Moreover, in previous research studies, dominant and nondominant limb was neglected as influencing variables in prediction accuracy. Therefore, this study aimed to investigate the accuracy of predicting COM acceleration based on the accelerations of lower limb joints and trunk, with emphasis on weight distribution on legs. Methods: The relevant kinematic data were collected using motion analysis systems. In this regard, visual 3D software was used to create a 14-segment model for each subject and estimate the positions of body COM. A force plate was used to assess the body weight distribution between legs. Calculation of the summation of joints angular accelerations was done using algebraic addition of time series data. Results: Prediction of COM acceleration, based on the individual acceleration of lower limb joints and trunk, revealed that just acceleration of hip joint in both dominant and nondominant lower limbs was an appropriate variable for predicting COM acceleration (R2adj =0.40). However, during prediction based on the summation of trunk and joints accelerations, its accuracy showed a significant increase (R2adj=0.90). Conclusion: The summation of angular accelerations of trunk and lower limb joints is the most accurate predictor of COM acceleration during quiet standing balance control. Simultaneous changes of lower limb joints and trunk accelerations control COM acceleration.
