Introduction: Benign paroxysmal positional vertigo (BPPV) is a common peripheral vestibular disorder encountered in otolaryngology clinics. It is associated with a characteristic paroxysmal positional nystagmus, which can be diagnosed with specific positional manoeuvres, such as the Dix-Hallpike test. It is usually not recognised by general practitioners and as such is diagnosed late. There is a long delay between onset of symptoms and diagnosis, often with multiple healthcare contacts. Objective: to evaluate the relationship between age, gender and affected ear, in patients presenting with posterior canal benign paroxysmal positional vertigo. Materials and methods: This is a prospective observational study was conducted among patients attending the Department of ENT, Krishna Rajendra Hospital, Mysore Medical college and research institute, Mysore, for a period of one year from June 2020 to June 2021. A total of 50 patients were included and analysed for demographic data, age, gender, affected ear and efficacy of epley's manoeuvre in treating posterior canal BPPV. Results: females are more prone to BPPV seen frequently in middle age with right ear being more commonly seen. Resolution of symptoms were observed in maximum cases subjected to epley's manoeuvre within one week follow up. Conclusion: Dix Hallpike manoeuvre is the specific test to diagnose BPPV. Asthma is one of the most common respiratory diseases encountered in clinical practice. Although the vast majority of asthmatics can be adequately controlled with inhaled steroids and other preventer medications, a small proportion remain uncontrolled. Anti-IgE treatment with omalizumab has been proposed in patients as a preferred approach in step 5 asthma therapy according to GINA guidelines. Although therapy with this molecule is approved for patients with atopic asthma and pretreatment serum IgE levels of 30-1500 only, there have been a few reports of its efficacy in subjects outside this reference IgE range. We report the case of a middle-aged lady with severe corticosteroid-dependent asthma and low serum IgE levels who was successfully treated with 9 months of omalizumab therapy. She gained good asthma control and was tapered off steroid use by the fifth month of therapy with omalizumab. The case report stresses the need for further investigation into expanding the spectrum of omalizumab usage in asthma beyond the current IgE suitability range. [ABSTRACT FROM AUTHOR]

Polycystic Ovary Syndrome (PCOS) or hyperandrogenic anovulation is a common metabolic disorder in women which may leads to infertility. The condition determined with more than 10 small ovarian cysts of diameter ranging from 2-9mm. Excess gonadotropin secretion in PCOS women produce fluctuation in LH level which disturbs the LH: FSH ratio in body. Modulation of FSH receptors with plant phytoconstituents attenuate the production of FSH which gradually reduce the activity of LH and reduce the PCOS activity. Nisajeerakadichoornam; a novel ayurvedic formulation prepared with four plant parts; fruit of Foeniculumvulgare, bulb of Allilumsativum, seed of Macrotylomauniflorum and rhizomes of Curcuma longa. The plant parts are cleaned and dried properly. They are finely powdered separately, sieved, accurately weighed and then all mixed together. The powder pass through 80 mesh sieves. It should not adhere together or become moist. The finer powder has better therapeutic value and expected to be effective towards PCOS. In the present study, insilico evaluation of FSH receptor (4MQW) and insulin receptor (3BU3) by phytoconstituents present in the choornam were studied using PyRx, PyMOL, Open babel software. ADME parameters are evaluated with SWISS ADME software. The results show that all the phytoconstituents possess binding energy ranges from -5.2kcal/mol to -8.8kcal/mol with the receptor. Genetic variability among thirty chickpea genotypes with three replications utilising 19 yield, drought tolerance and mechanical harvestable traits was studied under rainfed and irrigated conditions at Regional Agricultural Research Station, Nandyal, Andhra Pradesh, India. The analysis of variance carried out for 19 yield, yield attributing and drought tolerant traits revealed highly significant differences among the genotypes for all characters under rainfed as well as irrigated conditions. Variability study revealed high heritability accompanied with high to moderate genetic advance for drought tolerant traits like SLA, SCMR and proline. Traits related to mechanical harvesting viz., height of the first pod and plant height exhibited moderate variability, high heritability and high genetic advance both under rainfed and irrigated conditions. Seed yield, harvest index, 100 seed weight and days to 50 per cent flowering exhibited moderate to high genetic variability, high heritability coupled with high genetic gain under rainfed and irrigated conditions. The classification of recurrence and non recurrence of Hepato Cellular carcinoma (HCC) outcome after Radio Frequency Ablation therapy is a critical task. Multiple time series clinical liver cancer dataset is collected from different dataset and time interval. A merging algorithm is used to merge all attributes collected from different sources in multiple time periods. In order to preserve the originality of information, statistical measures of each attribute is calculated and considered them as additional attributes for accurate prediction. However the merged dataset is unbalanced, in which, the number of samples from HCC recurrence class is much smaller than from HCC non recurrence. The feature weighting scheme select optimal features and parameter of classifiers are sequentially obtained from multiple iterations which causes higher computation time. In this paper, an efficient sampling approach is proposed using Inverse Random under Sampling (IRUS) to overcome class imbalance issue. IRUS under sample the majority class which creates a number of distinct partitions with a boundary separated minority and majority class samples. Additionally an optimization approach is proposed using Artificial Plant Optimization (APO) algorithm to select optimal features and parameters of classifiers to improve the effectiveness and efficiency of classification. The optimization approach reduces the number of iteration and computation time for feature selection and parameter selection for classifiers which classify the recurrence and non recurrence of HCC. Classify patients with HCC and without HCC based on optimal features and parameters by Support Vector Machine (SVM) and Random Forest(RF) classifiers. Finally, the experimental results are conducted to prove the effectiveness of the proposed method over existing method in terms of accuracy, specificity, sensitivity and balanced accuracy. The purpose of this study is to explore the long-run relationships and short-run dynamic interactions between environmental degradation (proxied by carbon dioxide, CO emissions) and the independent variables of consumption (proxied by income level or gross domestic product, GDP per capita) and energy use in India over the period 1975 to 2015, using time-series analysis. The multivariate cointegration methodology is applied in this study to establish the possible causal relations between the variables concerned. The cointegration test and the vector error correction model display the evidence of a positive long-run relationship between consumption and environmental degradation, while energy use is negatively related to environmental degradation. The long-term elasticity coefficients of the exploratory variables on environmental degradation display relationships that are theoretically grounded. This study was undertaken to assess the antioxidant capacity of extract of five different vegetables namely Brassica oleracea var. capitata (White cabbage), Brassica oleracea var. capitata.f.rubra (Red cabbage), Brassica oleracea var. botrytis (Cauliflower), Brassica rapa var.rapa (Turnip), Brassica oleracea var. gongylodes (Knoll khol). The freshly prepared extract of Brassica (Turnip, Knol khol, Cauliflower, Red cabbage and White cabbage) vegetables was taken for the present study. The fresh vegetables were purchased from local markets and the analyses were carried. From the above said vegetables 500gms were taken and about 200ml of fresh juice was used for analysis. Red cabbage was found to have the highest antioxidant activity. Compared to other vegetables, Brassica vegetables have higher antioxidant potential which makes them very interesting crops from the consumer's point of view. Here, we will provide an overview of the role of Total phenols, Carotenoids, Proline, Protein present in Brassica vegetables in relation to antioxidant properties and human health. As conclusion Brassica vegetables contain bioactive substances with a potential for reducing the physiological as well as oxidative stress and this could explain the suggested cancer preventive effect of these plants as well as protective role on other major diseases. Among the pulses, bengal gram (Chickpea) has been considered for the present study in the scenario of uncertain monsoon behaviour where it is an excellent crop benefiting the farmers since its ability to withstand drought condition with lesser rainfall. The present study was under taken intentionally to assess the trends in area, production, productivity and Efficient cropping zones (ECZ) of the emerging pulse crop (Bengal gram) of Tamil Nadu, to identify and analyse the factors influencing the area, production and productivity of bengal gram as well as to key note the available niches over the state (Tamil Nadu). The zones were identified for latest 10 year average (2005-2014). The trend analysis was carried out for 1950-2014, 1950-1979 and 1980-2009 and at decadal scale for state level. The results showed that the trend in area, production and productivity were increasing with the new technologies that could further extend the crop over the efficient cropping zones. The compound growth rate of 2.44 per cent per annum over Tamil Nadu during the time period of 1950-2014 could be achieved as a whole. Coimbatore, Tiruppur, Dindigul and Dharmapuri districts of Tamil Nadu during the latest ten years, 2005 to 2014 were found to be the efficient cropping zones for bengal gram from this study. Background: Governments have imposed lockdowns in the wake of the COVID-19 pandemic. Hospitals have restricted outpatient clinics and elective services meant for non-COVID illnesses. This has led to patients facing unprecedented challenges and uncertainties. This study was carried out to assess patients' concerns and apprehensions about the effect of the lockdown on their treatments. Materials and Methods: An ambispective, observational cross-sectional single centre study was conducted. Patients were contacted telephonically and requested to answer a structured questionnaire. Their responses were documented and summarized as frequency and proportions. Results: A total of 727 patients were interviewed. Epilepsy (32%) was the most common neurological illness in our cohort followed by stroke (18%). About half the patients and/or their caregivers reported health-related concerns during the lockdown. The primary concern was how to connect with their treating neurologist if need arose. Forty-seven patients (6.4%) had drug default. Among patients on immunomodulatory treatments, only eight patients had drug default. High compliance rates were also observed in the stroke and epilepsy cohorts. Of the 71 patients who required emergency care during the lockdown, 24 could reach our hospital emergency. Fourteen patients either had a delay or could not seek emergency care. Two-thirds of our patients found the telemedicine experience satisfactory. Conclusion: The ongoing pandemic will continue to pose challenges to both physicians and patients. Patients in follow-up may need to be contacted regularly and counselled regarding the importance of maintaining drug compliance. BACKGROUND: Hypertension is highly prevalent in India but frequency of resistant hypertension has not been well studied. These patients are prone to high risk of hypertensive complications. The objective of our study was to evaluate the prevalence of resistant hypertension patients among the hypertensive population. METHODS: The study was carried out among the hypertensive patients in tertiary care hospital in India. Patient data, characteristics and class of drugs taking was recorded using a pre-coded questionnaire. Morisky questionnaire were used to assess the self medication adherence. Chi-square test for association was used to analyse the statistical difference between the hypertensive and resistant hypertension patients and related variables. RESULT: A total of 200 patients were included in the study, In the 35% (n=70) of patients have essential hypertension and 65% (n=130) of patients have resistant hypertension. Resistant hypertension was more significantly associated with patients ≥40 years of age, who are all have normal BMI, not doing their physical activity, non compliant to recommended diet and not taking their antihypertensive medication regularly. Prevalence of comorbid condition, including diabetes mellitus (p=0.000), hyperlipidemia (p=0.000), cardiovascular disease(p=0.000),chronic kidney disease (p=0.003) was statistically significant between the hypertensive and resistant hypertension patients. Only 3% patients (n=4) have high medical adherence. CONCLUSION: Resistant hypertension is a prevalent clinical condition, and its prevalence is on the rise (n=130, 65%). With our ageing (≥40 years), normal BMI population, lack of physical activity, personal habits (smoking,alcohol), noncompliance with recommended diets, lack of physical activity, and failure to take antihypertensive medication on a regular basis, the associated increasing incidence of diabetes, hyperlipidemia, coronary artery disease and chronic kidney disease, resistant hypertension will be even more prevalent. Background: Cellulitis, especially of the lower limbs is found to be more common now-a-days in both the sexes. Ascending cellulitis was found to pose both short and long term morbidities. Objective and Methodology: The following study carried out is at the general medicine ward of a government hospital where about 90 patients were taken for study with their cellulitis condition. Demographic details and their predisposing causes towards the condition were noted and risk factors were identified. Results: Diabetes, infections due to streptococci, infections from tinea pedis etc, were the main etiological conditions among others contributing to increased cellulitis cases. Risk factors in both ipsilateral and contralateral limbs were recorded and studied upon. Long term morbidity of most of the patients was identified for which a proper follow-up along with serological examination for critical diagnosis could be carried out. Culturing of the wound for identifying the causative organism is a prior issue which would reduce the risk of antibiotic resistance. Prophylaxis with Penicillins and its derviatives, both natural and synthetic could be suggested for long term conditions and avoiding recurrent episodes. Common therapy was with Cephalosporin (23), Ciprofloxacin (29) etc. both natural and synthetic penicillins were also found to be given in a good number. [ABSTRACT FROM AUTHOR]
