Hintergrund und Studienziele: Versuche, die Diagnostik und Therapie der Helicobacter pylori (Hp)- Infektion zu standardisieren, haben zu vielfältigen Publikationen von Leitlinien verschiedener gastroenterologischer Fachgesellschaften in Europa und den USA geführt. Dennoch gibt es kaum Daten zur Anwendung dieser Leitlinien in der Routine bei Gastroenterologen und Hausärzten, oder zur Compliance und Akzeptanz der Patienten in der Routineversorgung. Mit der vorliegenden Arbeit ist deshalb 6-9 Monate nach der Erstveröffentlichung der Deutschen Leitlinie deren Umsetzung in der Praxis untersucht worden. Patienten und Methodik: Es handelt sich um eine retrospektive Auswertung aller ÖGD- Befunde, die innerhalb eines Monats in 11 teilnehmenden Zentren ( 2 Universitätskliniken; 9 gastroenterologische Fachpraxen) von insgesamt 24 Untersuchern erstellt wurden. Befunde mit der Indikation Sodbrennen, Diarrhoe, Tumor und Kontrolluntersuchung nach Eradikation, Gastrektomie, therapeutischer Gastroskopie ( z.B. Varizenbehandlung) wurden ausgeschlossen. Die Diagnosen und Behandlungsvorschläge der Gastroenterologen wurden festgehalten. Danach erhielten die zur Untersuchung überweisenden Hausärzte und im Anschluss die untersuchten H.p.-positiven Patienten jeweils einen Fragebogen über H.p.- Diagnose, Behandlungsindikation, Durchführung und Kontrolluntersuchungen. Ergebnisse: 772 Befunde wurden in die Studie eingeschlossen. 287 auswertbare Arztfragebögen (47%) und 265 auswertbare Patientenfragebögen (59%) wurden zurückgeschickt. Die Gastroenterologen empfahlen bei etwa 90% der Ulkuspatienten und bei 9% der Gastritis/ NUD- Patienten eine Eradikation. Die Hausärzte sahen nur in 82% der Ulcera eine absolute Indikation, in 6% bzw. 42% der NUD- Patienten eine absolute odder relative Indikation zur H.p.-Therapie. Bei den Patienten gaben 52% an, eine spezifische H.p.-Therapie erhalten zu haben. Ulzera waren laut Befunden in 22% aller Patienten vorhanden. Patients and Methods: A retrospective analysis was conducted of all outpatient upper gastrointestinal endoscopy reports for a one-month period in 11 different centers ( two university and nine private practice gastroenterology offices) with a total of 24 gastroenterologists. Endoscopy reports from patients with the indications of reflux, diarrhea, and tumors were excluded. Diagnoses and treatment recommendations given by gastroenterologists were recorded. Questionnaires concerning Hp diagnosis, treatment indications and performance, and follow-up were sent to referring physicians. Results: A total oof 772 endoscopy reports were included in the study; analyzable questionnaires were returned by 287 referring physicians(47%) nd 265 patients (59%). Gastroenterologists recommended Hp eradication in all ulcers and in 29% of non- ulcer dyspepsia (NUD) cases (14% strongly recommended and 15% to be considered). Referring physicians thought that 94% of ulcers should be treated by Hp eradication, which was also considered to be an absolute and relative indication in NUD by 15% and 53% of the referring physicians, repectively. Among the patients who replied, 61% had received Hp eradication regimens; ulcers were found in 22% of the total patient group. Check-up examinations after Hp therapy were considered necessary by 75% of the referring physicians, but only 22% of the responding patients actually underwent some form of check-up ( upper gastrointestinal endoscopy in91%) Conclusions: Gastroenterologists and ( to a somewhat lesser extent referring physicians appear to be following the current
