Amaç: Kronik Obstrüktif Akciğer Hastalığı (KOAH) belirgin sağ lık ve ekonomik sonuçları olan ve sık görülen bir hastalıktır. Bu çalışmada, hastanemizde KOAH alevlenmesine bağlı yatış gerek tiren hastaların maliyet analizi yapılmıştır. Gereç ve Yöntemler: Hastanemiz göğüs servisine, bir yıl bo yunca, KOAH alevlenmesi nedeniyle yatışı yapılan 376 hastanın direk maliyet analizi değerlendirilmiştir. Bulgular: Çalışmaya alınan hastaların 311’i erkek, 65 tanesi kadındır. Hastaların median yatış süresi 8 (ortalama: 9.86) gündür ve ortalama yaşı 65.31’dir. Her atak başına düşen ilaç maliyeti 526.55 Türk Lirası (TL) ($392.94, €333.25), hastane bakım mali yeti 1271.94 TL ($949.20, €805.02) ve genel maliyet 1833.8537 TL’dır ($1368.54, €1160.66). Akciğer kanseri ve sekel akciğer tüberkülozu ek hastalığı olan hastalarda hastane yatış süresi ista tistiksel olarak daha uzundur. Antibiyotik kullanımı hastane yatış süresini ve maliyeti arttırmıştır. Yoğun bakımda tedavi görülmesi, hastane kökenli enfeksiyon varlığı ve uzun süreli oksijen ihtiyacı maliyeti ve hastane yatış süresini arttırmıştır. Sonuç: Bu çalışmada, hastane yatışı gerektiren KOAH alevlen- melerinde antibiyotik kullanımının, hastane kökenli enfeksiyon varlığının, uzun süreli oksijen tedavisinin ve yoğun bakımda te davi görülmesinin maliyeti arttırdığı saptanmıştır., OBJECTIVES: Chronic obstructive pulmonary disease (COPD) is a common disease that has severe health and economic conse- quences. in this study we estimated the economic cost of COPD exacerbations requiring hospital stay in our hospital. Material and Methods: the direct cost of 376 hospitaliza- tions during one year with the diagnosis of COPD exacerbations were evaluated retrospectively in our hospital‘s chest diseases department. Results: Three hundred and eleven of the patients enrolled in the study were men while only 65 were women. the mean age of patients was 65.31 and median hospitalization time was 8 (mean: 9.86) days. the average medication cost per exacerbation was 526.55 Turkish Lira (TL) ($392.94, €333.25), the average hospital care cost per exacerbation was 1271.94 TL ($949.20, €805.02) and the average total cost per exacerbation was 1833.8537 TL ($1368.54, €1160.66). the hospitalization time for patients with lung malignancy and sequelae of lung tuberculosis were found to be statistically longer. Antibiotic treatment increased the costs and hospitalization time. Treatment in intensive care units, noso- comial infections and necessity of long term oxygen treatment all increased the costs and hospital stay time. Conclusion: This study shows that the cost of intensive care unit treatment, long term oxygen treatment, treatment of hospital acquried infections and use of antibiotics were higher than usual in COPD exacerbations requiring hospitalisations.

The CAT is a short, simple eight-item questionnaire for assessing and monitoring COPD. It is not known how reliable the CAT scores are for COPD patients who are frequently exacerbated. The effectiveness of the CAT for assessing COPD severity and exacerbation rates was evaluated.This study enrolled 165 stable COPD patients who completed the CAT between April 2011 and February 2012.Patients had a mean forced expiratory volume in one second (FEV1) equal to 43.7% of the predicted value and a mean CAT score of 21.2 (± 7.56) units. There was a good association between the FEV1 (percentage of predicted value) and CAT scores (p0.0001). Frequent exacerbators had significantly higher CAT scores than infrequent exacerbators (24.8 ± 6.7 versus 17.5 ± 6.5, p0.0001). Also, as the frequency of the COPD exacerbations increased, CAT scores (p0.0001) significantly increased. There was a significant association between the frequency of hospitalization and the CAT scores (p = 0.001).We observed a good relation between the CAT, FEV 1, and disease severity in patients with COPD. We found that the baseline CAT scores are elevated in frequent exacerbators.

Background/Aim. Previous population-based studies found association between duration of sleep and cardiovascular and metabolic comorbidities. Our aim was to investigate the association between the duration of sleep and cardiovascular and metabolic comorbidities in OSAS.Patients and Methods. The study enrolled 312 patients, who had polysomnography (PSG) during 2006-2007 and responded to a telephone-administered questionnaire providing information on characteristics of sleep on average 12 months after PSG.Results. Of the patients, 90 were female (28.8%), 173 (58.5) received the diagnosis of OSAS, 150 (45%) had no comorbidities, 122 had hypertension (HT), 44 had diabetes mellitus (DM), and 38 had coronary heart disease (CHD). Mean ± SD of age in years was47.2±10.6,56.5±9.3,53.2±8.9, and59.9±9.0for the no comorbidity, HT, DM, and CHD groups, respectively. Reported duration of sleep was not associated with any of the comorbidities in the overall group. In the analysis restricted to OSAS patients, sleep duration ≤6 hours was significantly associated with CHD after the adjustment for age, gender, and other associated factors (OR: 5.8, 95% CI: 1.0–32.6).Conclusions. Confirmation of the association between shorter duration of sleep and CHD will provide prognostic information and help for the management of OSAS.

We present computerized tomography (CT) and magnetic resonance imaging (MRI) findings of a costal hydatid cyst (Echinococcus multilocularis) causing spinal cord compression. The hydatid disease was proved histologically. MRI was not only very useful for determining the spinal extension of the disease by its multiplanar imaging capability, but also gave important information about the texture of the cyst, thus aiding the preoperative diagnosis.
