Purpose . Gambling disorder has been recently reclassified under the category ‘‘substance-related and addictive disorders”. Recent studies performed through functional MRI (fMRI) have shown that the perseverance of some behaviors can alter brain activation [1,2]. In this work we aim at investigating functional connectivity changes in pathological gamblers (PGs) in comparison to healthy controls (HCs) by means of resting state functional Magnetic Resonance Imaging (rs-fMRI). Methods and materials. Thirteen HCs and fourteen PGs were recruited (all right handed males; drugs free; mean age 36 ± 10 yrs). All acquisitions were performed through a 1,5 T MRI scanner using a 8-channels phased-array head coil. Multi-session temporal concatenated Independent Component Analysis (concat-ICA) was carried out to achieve activations information on functionally linked brain regions. The resulted components were then matched and compared between groups. Correction for multiple comparisons across space was applied assuming an overall significance of p < temporal gyrus, right insula, right cerebellar hemisphere cortex and cerebellar vermis. Conclusion. We can conclude that an hyperconnectivity together with an overactivation of specific regions is observed in PGs. The persistent activation of specific functional networks during gambling tasks might represent the neurofunctional basis of PGs statereduced triggering threshold to gaming underlying the clinical features of gambling disorder. These preliminary results confirms the crucial role of fMRI studies to investigate brain networks and their changes in specific clinical functional disorders.

To evaluate the MRI findings in the various forms of pigmented villonodular synovitis (PVNS) of the foot.Seven hundred and fifty-three MR studies of the foot performed at our institute between June 1994 and April 2000 were retrospectively reviewed for the presence of PVNS. Spin echo (SE) T1W, Gradient echo (GE) T2*W, and fat suppression (Short Time Inversion Recovery: STIR) images were acquired with a 0.5 T superconductive unit (Vectra, GE Medical System, Milwaukee, WI, USA) provided with a dedicated transmitter/receiver coil. The site and type of lesions, the signal intensity patterns, and the presence of associated changes were evaluated.On the basis of the MR images and the above parameters, six patients (3 men, 3 women, age range: 35-48 years) with PVNS were selected. Three out of six PVNS were nodular, whereas the remaining three were diffuse. Of the three nodular forms, one was found in the sub-talar joint and the remaining two antero-medially to the talus. Instead, all of the diffuse lesions were located on the metatarsus. Perilesional oedema was seen in all cases, although more obvious in the nodular forms, whereas bone involvement (osteochondral erosion) was observed only in the diffuse metatarsal PVNS. Intra-articular bloody effusion was never observed. The MRI findings were confirmed by surgery in all cases.The high contrast resolution and multiplanar capabilities of MRI allow the complete evaluation of the structures of the foot affected by PVNS, and of the extent of soft tissue (bursae, synovial or nervous structures), bone and articular involvement. Although not specific, the presence of haemosiderin results in characteristic MR findings, due to the shortening of both T1 and T2 relaxation times. GET2* images are particularly well suited to thisFurthermore, in our experience, FIR images added better depiction of associated swelling.According to our results, MRI is now the most reliable technique for identifying and classifying PVNS, and allows correct treatment planning and effective monitoring.

The aim of the present study was to assess the accuracy of magnetic resonance imaging (MRI) in the diagnosis of bucket-handle tears of the knee menisci, determining the sensitivity and specificity of the imaging for each of the signs typical of these tears.MR examinations of 495 patients suffering from knee traumas were assessed retrospectively. Forty-eight patients had arthroscopically-proven bucket-handle tears of the meniscus. MR examinations were performed using a 0.5-T superconducting magnet, with T1-weighted spin-echo (SE) and gradient-echo (GE) T2*-weighted sequences. Imaging findings used for the diagnosis were: a) double posterior cruciate ligament sign; b) flipped meniscus sign; c) presence of a displaced fragment of the meniscus in the intercondylar notch, visible in coronal and axial images; d) truncated triangular shape of the peripheral non-displaced portion of the meniscus, visible in coronal images.A total of 43 out of 48 bucket-handle tears of the meniscus were correctly diagnosed at MR, thus overall MR accuracy was 98%. In 12 (28%) cases three signs were present simultaneously the double posterior cruciate ligament (sensitivity 28%, specificity 99%, accuracy 93%) + the displaced fragment in the intercondylar notch (sensitivity 69.8%, specificity 98.7%, accuracy 96.2%) + the truncated triangular shape of the peripheral portion of the meniscus (sensitivity 74.4%, specificity 98%, accuracy 96%). In 18 (42%) cases two signs were present together the displaced fragment of the meniscus + the truncated triangular shape of the peripheral portion of the meniscus. In 13 (30%) cases only one sign was presenting two cases the truncated triangular shape of the peripheral portion of the meniscus and in 11 cases the flipped meniscus sign (sensitivity 25.6%, specificity 93.4%, accuracy 87.5%). No statistically significant differences were found comparing the results for tears of the medial meniscus with those for the lateral meniscus.MR imaging is highly accurate in diagnosing bucket-handle tears of the menisci due to its ability to identify a displaced fragment of the meniscus in the intercondylar notch or flipped over the anterior horn of the meniscus of origin. We speculate that bucket-handle tears not found by MR imaging are cases where the meniscus was displaced after MR examination.

Swallowing disorders can be secondary to different types of diseases in which, at least initially, patients succeed in establishing voluntary or involuntary compensatory mechanisms that enable them to maintain a sufficient nutritional state. When the compensatory mechanisms become insufficient massive food aspiration into the airways can occur and suffocation may prove to be the main pathology. It has been calculated that in the USA about 8,000-10,000 people die each year due to suffocation. The dynamic radiological examination of swallowing is considered important not only for diagnosis, but also for planning a rehabilitation therapy and type of nutrition for the patient and for verifying the results of the therapy. The aim of this study is to analyse the results of our experience in the use of the digital cineradiography system to evaluate patients with normal and pathological swallowing.We reviewed the digital cineradiography of 220 patients that at no time had undergone surgery and presented no organic pharyngeal or oesophageal disease (excluding hiatus hernia). All the exams followed a standard protocol that included the dynamic evaluation of the larynx, soft palate, pharynx, and gastro-oesophageal junction with a cineradiographic sequence of 12 frames/second with a 512x1024 matrix. There was also an archive of the film in a post-processing console. The patients received single photograms (printed on laserfilm), videotape recordings or CD-ROM of the dynamic exam.137 (62%) of the patients did not present swallowing alterations although only 7 patients had a negative examination. In 35 cases hiatus hernia was appreciable while in 69 cases the hernia was associated with gastro-oesophageal reflux. In 23 cases aspecific functional disorders of the oesophagus were demonstrated and in 3 cases achalasia. The remaining 83 patients (38%) (37 males and 46 females, average age 57.02 yrs) presented alterations of the oral and/or pharyngeal stages of swallowing: reduction in soft-palate motility (2 cases), unilateral paralysis of the vocal chords (1 case), incontinence of the bolus during the oral stage (8 cases), lingual movement anomalies (4 cases), subepiglottic penetration (62 cases), asymmetric epiglottic tilt, aspiration of the contrast medium in the airway (17 cases), reduction of laryngeal and hyoid bone movement (9 cases), bolus retained in the valleculae and pyriform sinus (13 cases), cricopharyngeal spasm (6 cases), pharyngeal paralysis (1 case); hiatus hernia was also evident in 20 cases and gastro-oesophageal reflux was associated in 13 of them. Overall, 36% of the cases presented an isolated form while 64% of the cases presented a complex dysfunction with several simultaneous alterations.The videofluorographic swallow study is an important step in the diagnostic evaluation of a dysphagic patient not only as regards the analysis of the main alteration and its capacity to confirm the presence or absence of contrast medium aspiration in the airway, but also because it provides important information on rehabilitation and nutritional orientation (oral/no oral), as well as on the results of the therapy. The recent diffusion of the digital X-ray equipment has made possible its use for the study of the organic and functional diseases of the upper alimentary tract. Currently a standard protocol for the study of swallowing with digital fluorography is not available. The technique we applied, already verified in a significant number of dysphagic patients, has allowed us to distinguish patients with normal swallowing from those with disorders of the oral and pharyngeal stage, and thus to identify disturbance and establish an appropriate rehabilitation treatment.

Sea water represents more than three quarters of the whole world. The radioactive fall out caused by nuclear experiments, radioactive drosses from nuclear power plants and other pollution sources, are able to contaminate the world ecosystems, and especially the sea water. The aim of this work is the evaluation of a possible radioactive pollution in the Gulf of Palermo.The Gulf of Palermo was divided into 5 homogeneous hydrological parts, in which fish and water were collected. The dosage of 131I e del 137Cs was mainly assessed because they have a long half-life and their presence in the water can be due to nuclear accidents but also to a wrong removal of the waste materials of nuclear medicine procedures; the other radioisotopes assessed (144Ce, 106Rh, 140La) are due only to nuclear fall-out. Gamma spectrometry technique was used for the evaluation of the radioisotopes' concentration after an adequate treatment of the samples.The concentration of the examined radionuclides was below the instrumental capability apart from the 137Cs, whose dose was increased, and can be due to nuclear accidents and explosions. The radionuclide levels detected do not determine a higher risk than normal for the population's health even regard the eatablity of ittical species.

To assess the outcome of Magnetic Resonance Imaging (MRI) in the diagnosis of sportsman hindfoot pain.Fortythree professional athletes (31 men, 12 women, age range: 17-37 years) affected by hindfoot pain underwent MRI. Spin echo (SE) T1W, Gradient echo (GE) T2*W, and fat suppression (Short Time Inversion Recovery: STIR) images were acquired with a 0.5 T superconductive unit (Vectra, GE Medical System, Milwaukee, WI, USA). A dedicated extremities transmitter/receiver coil was used. The lesion site, the presence of anatomic variants (os trigonum, Haglund's deformity), and signal intensity changes were evaluated.In all cases MRI allowed the identification of the cause of the hindfoot pain, in relation to soft tissue (bursae, synovial or nervous structures), bone and articular diseases. Particularly, as regards soft tissue diseases, tendinous abnormalities and inflammatory bursal involvement were frequently found (77% of cases). Bone diseases (22% of cases with posterior talalgia alone), mostly involved the heel (60%), whereas cartilagineous diseases were present in 9% of cases. In 60% of cases an intra-articular osseous or cartilagineous displaced fragment coexisted, determinating joint locking during foot flexo-extension movements. In 38% of cases contemporary involvement of different articular structures was observed.Both MRI high contrast resolution and multiplanar capabilities allow the complete evaluation of hindfoot region. In our experience sagittal and axial planes were particularly well suited for the diagnosis and the assessment of disease extension. Furthermore, T2W (GET2*) and fat suppression (STIR) images allow high sensitivity even in early disease detection, when hyperemia or fluid collection occur.According to our results, it seems possible to state that nowadays MRI is the most reliable technique for identifying the causes of hindfoot pain, in order to provide a correct and effective pre-therapeutic planning.

Healthcare telematics, or telemedicine, is a new methodology that applies the potentialities of telecommunications technologies to the needs of medicine, thereby greatly contributing to improving the management of clinical data and medical information for the benefit of the individual patient and the community at large. The fields of application of Telemedicine are becoming increasingly vast, and this gives rise to technical problems (interconnections) as well as professional, ethical, medico-legal and legal problems. The dissemination of Telemedicine will require changes to be made to the organisation and delivery of the medical/administrative services connected to the management of patient data, the remote provision of care, and the impact of Telemedicine itself (e.g. need to standardise the nomenclature for telemedicine services). In addition, it will also call for a careful analysis of costs and benefits for both healthcare providers and patients. One of the most interesting experiences in terms of impact is Teleradiology. This is neither a new discipline nor a (sub)specialty: the practice of Teleradiology must comply with the rules regulating any radiological medical act, the primary aim of which is to contribute to establishing - rapidly and accurately - a diagnosis that will affect treatment strategies. It may be anticipated that in some situations Teleradiology will significantly change the working practices of Radiology Specialists and Radiology Technicians. Because it is better to anticipate problems rather than wait for them to arise, our Scientific Society, which is always sensitive to emerging issues, intends to propose the following recommendations/guidelines for the use of Teleradiology in the common interest of the community and healthcare workers. The invitation to take part in the initiative proposed by the Italian Society of Medical Radiology (SIRM), the Italian Association of Nuclear Medicine (AIMN) and the Italian Association of Neuroradiology (AINR) was received with great satisfaction and is proof that we are working towards common goals. As on other occasions, this was reflected in the excellent collaboration between the National Radiologists' Union (SNR), the Italian Association of Medical Physics (AIFM) and the National Federation of Radiology Technicians (F.N.C.TSRM) in drafting the document. The document aims to highlight the potentialities and limitations in the use of Teleradiology and to provide a set of recommendations/guidelines, which are not, however, to be intended as strict, absolute rules. Because this field is continually evolving both in structural and regulatory terms, and because it is very difficult to establish universal criteria to rigidly define behavioural models for implementing and managing Teleradiology-related activities (which in any case fall into the category of radiological medical acts), the recommendations/guidelines proposed necessarily have an informative rather than prescriptive nature. The document starts by defining the meaning of the following currently used terms: Teleconsultation; Telediagnosis; Teledidactics. It then goes on to analyse the following aspects of Teleradiology: Technological requirements; Qualifications and training of Medical Personnel; Qualifications, training and competences of Radiology Technicians. Based on the regulations in force in Italy, these recommendations are structured in terms of rationale and possible professional issues arising from the use of Teleradiology. A section is devoted to data security and confidentiality, including legal implications, an area which is currently evolving and being studied in Italy and abroad. Finally, the professional liabilities of all the healthcare providers involved in Teleradiology (imprudence, incompetence, negligence) are outlined, as well as the responsibilities related to the necessary maintenance of equipment. The aim of the document is to propose recommendations/guidelines for the correct use and validation of Teleradiology. The Scientific Societies SIRM-AIMN-AINR and the Academic Institutions, together with the SNR, AIFM and the F.N.C.TSRM, the Regional Bodies and the Italian National Health Trusts are required to make a positive and constructive commitment to this new field, which has become the object of considerable attention and interests. While confirming that, if used correctly, this methodology will no doubt increase the overall efficiency of Diagnostic Imaging - in the primary interest of the patient's right to health - it is felt that attention should also be paid to the working practices of the Radiology Specialists and Radiology Technicians and to the training of both. It is necessary to standardise training and practice, and to make an integrated and coherent use of resources with respect to the healthcare goals we wish to attain. Many have described the undisputed advantages of Teleradiology, but there have been relatively few reports on the related risks and recommendations for its use. It is certain that an improper use of the method (forcedinterpretation of the bill on complementary diagnostic activities, operational misunderstandings between Radiology Specialists and Radiology Technicians, rarefaction of the relationship between Radiologist and Patient, obligation to use teleconsultation in all situations) will lead to considerable disadvantages for all those working in the field of Radiology, specialists and radiology technicians alike. Therefore, by adopting this document, which is by no means against Teleradiology but for a rational use of the method and full awareness of what it actually implies, the Scientific Society intends on the one hand to participate actively and concretely in the process of regulating all those aspects of Teleradiology for which it is competent - rather than waiting for regulations to be imposed from above - and, on the other, to provide, through its highly qualified Study Group (Specialists in Radiology, Nuclear Medicine, Legal Medicine, Medical Physics, and Radiology Technicians), Radiologists, Specialists in Nuclear Medicine, Neuroradiologists and Radiology Technicians with practical recommendations for use/guidelines for the correct and rational performance of their (tele)radiological acts. These indications and recommendations are also being submitted to the Italian Government. Teleradiology has attracted so much attention that, even as a result of pressure from Industry, an international consensus conference is needed to regulate the rational and informed use of this new methodology.

The use of low frequency probes allows to overcome the resistance of the skull and evaluate Willis's circle by B-mode and Trans-Cranial Color-Doppler (TCCD) to obtain morphological and functional information related to brain circulation during pathologic conditions. With the new software available today modern technology allow us to measure the transient scattering produced by the rupture of the contrast medium microbubbles and estimate the presence of the contrast medium both in macro- and microcircle. In this way it is possible to appraise parenchymal perfusion. This study aims to assess the intracranial micro- and macrocircle using TCCD with contrast medium (Levovist) and to compare the results with the patients' clinical signs.We studied 21 subjects aged 45-73 years (mean 68 years) with atheromatous uncomplicated plaques in the internal carotid artery producing varying degrees of stenosis and 10 healthy controls. The examinations were performed using an ATL HDI 3000 ultrasound machine with a Phased Array 3.25 MHz probe. The mechanic index was calibrated to high values to obtain rupture of the microbubbles under insonation. Intensity/time curves of transient scattering were extrapolated for both the cerebral macrocircle and the parenchymal microcircle in the region of interest.The curves were compared with the clinical presentation of the different classes of patients and the results obtained were consistent in showing a clinical pattern of perfusional deficit in subjects with symptoms of chronic brain ischemia. In particular, it was possible to compare the morphologic data relative to the contrast medium decay curves with the patient's clinical condition, confirming the suspicion of cerebral microcircle pathology.Thanks to improvements in the software and to the definition of effective algorithms, contrast-enhanced TCCD will be able to provide information on brain perfusion in a simple, inexpensive and relatively non-invasive manner.

To evaluate the CT findings of intracerebral hemorrhage in patients undergoing thrombolytic therapy for acute myocardial infarction and to correlate the type of intracerebral hemorrhage with clinical outcome.We retrospectively reviewed the clinical records and CT scans of intracerebral hemorrhage on a total of 302 patients who underwent thrombolytic therapy for acute myocardial infarction at our institution from January 1996 to September 1999. In each patient we evaluated: the number, sites and size of hemorrhage, and the presence and severity of mass effect. The site of the hemorrhage was classified as intraparenchymal, intraventricular, subdural and subarachnoid.Six patients (2%, mean age 74, range 66-80) developed intracerebral hemorrhage. There was a total of 22 hemorrhages: 1 subdural hemorrhage, 6 subarachnoid, 11 intraparenchymal and 4 intraventricular. Excluding intraventricular hemorrhage, 14/18 hemorr-hages were located supratentorially. In five patients there was a fluid-blood level. Three patients had severe mass effect with midline shift. Symptoms presented within 24 hours from the administration of thrombolytic therapy in all patients. All the patients who died had a large hematoma with a severe mass effect and a severe midline shift at CT scan. In the remaining patients, the hematoma was of medium size and no mass effect was seen.The most common site of hemorrhage was supratentorial and intraparenchymal. Large volume intracerebral hemorrhage, multiple hemorrhages and mass effect with midline shift were associated with increased mortality. The most commonly observed finding was a fluid-blood level hematoma.

To compare US results with those of surgery in the assessment of the subscapularis tendon.From January 1995 to December 1998 1500 patients underwent US of the shoulder. 12 of these patients had an injured subscapularis tendon. US results of these patients were evaluated retrospectively and compared with those of surgery. US examinations of the subscapularis tendon were performed with two US units (AU4 Idea and HDI 3000), using linear arrays 10-13 MHz transducers. The following features of the tendon were evaluated: morphology, thickness and echotexture.A) US findings. US showed: - chronic degenerative tendon changes with diffusely inhomogeneous echotexture (5 cases); - focal tears with small hypoechoic ill-defined areas or gross hypo-anechoic areas involving the full thickness of the tendon (4 cases); - complete tendon tear with disappearance on US of the subscapularis tendon or with severe thickening of the tendon (2 cases); - in 1 case of recent anterior dislocation of the shoulder a bulky hematoma was seen with coexisting thickening and subtotal tear of the tendon. B) Surgical findings. US findings were confirmerd at surgery in 10 of the 12 patients. In the patient with anterior dislocation of shoulder the diagnosis hematoma was confirmed, but the subscapularis tendon was undamaged. Moreover, in 1 out of the 5 patients with US diagnosis of chronic degenerative tendon changes, a small focal tear of the tendon was found at surgery.US is a first level investigation which is able to demonstrate early changes of the subscapularis tendon and nearby structures. Awareness of pitfalls might decrease the use of second level investigations such as CT arthrography and MR imaging, which on the other hand allow a better definition and assessment of the extent of the injury.

To investigate if the use of a contrast agent (Levovist) improves the specificity of US in the diagnosis of prostate carcinoma, having the results of prostate biopsy as the gold standard.Thirty patients with physical findings suspicious for prostate carcinoma and PSA ranging 5 ng/mL to 15 ng/mL were examined with transrectal US (TRUS) integrated with the color Doppler mode and contrast agent administration (4 g injected at 4 mL/min: 300 mL concentration).Based on bioptic and surgical results, 14 prostate carcinomas were found, all of them1.2 cm. Conventional US recognized the suspected nodule in 11 cases, with 78% sensitivity and 93% specificity. Color Doppler showed color signals in 8/14 cases, which were peripheral to the lesion or intranodular, but did not add any important finding to those of gray-scale US. In contrast, contrast enhanced studies showed 13/14 carcinomas, which improved sensitivity significantly (92%). Particularly, 11/14 lesions had a typical avascular pattern within the strongly enhanced peripheral gland, while 2 small lesions only exhibited intranodular vessels.We compared our results with the Microscopic Angiogenesis Grading System (MAGS) index and found it exceeded 30 in the 11 avascular lesions value indicates microneoangiogenesis. In the other two cases a value30 was correlated with a different type of tumor vascularization typical of macroangiogenesis.

BACKGROUND AND PURPOSE: Several techniques have been used to image the nasolacrimal system, providing functional (dacryoscintigraphy) or morphologic (dacryocystography, CT dacryocystography [CTD]) information. Using gadopentetate dimeglumine–diluted solution injected into the lacrimal canaliculus or instilled into the conjunctival sac, we compared the sensitivity of MR dacryocystography (MRD) with that of CTD. METHODS: Eleven healthy volunteers and 25 patients affected by primary epiphora (21 patients) or postsurgical recurrent epiphora (four patients) underwent MRD after the topical administration of contrast media or cannulation of the lacrimal canaliculus. The MR imaging findings were compared with irrigation and CTD data. All patients underwent surgical treatment (dacryocystorhinostomy), which served as a standard of reference for confirming the MRD findings. RESULTS: The topical administration of contrast-enhanced saline solution and the injection of contrast-enhanced saline solution after cannulation were always well tolerated. In healthy volunteers, outflow of contrast media was always revealed by MRD. Eight (32%) of 25 patients with epiphora had stenosis proximal to the lacrimal sac revealed by MRD, whereas 17 (68%) of 25 showed a dilated lacrimal sac and nasolacrimal duct stenosis, as confirmed by surgical findings. The findings of MRD after the topical administration of contrast medium and MRD after cannulation of the lacrimal canaliculus were comparable with irrigation or CTD data for all patients except one. CONCLUSION: In patients with epiphora, MR imaging performed after the topical administration of diluted contrast material can reveal stenosis of the lacrimal apparatus and can be added to the standard orbital imaging protocol when lacrimal system involvement is suspected.

The os trigonum tarsi is an accessory bone of the foot localized posterolateral to the lateral tubercle of talus. It is usually an asymptomatic condition. However, particular activities such as ballet, soccer, or football may cause repeated stress and chronic microtraumas to the hindfoot, resulting in the os trigonum syndrome. Pain is typically localized anterior to the Achilles tendon; nevertheless, diagnosis may be very difficult because other conditions may show the same symptoms. Radiography can only demonstrate the os trigonum and its morphostructural changes, while MR imaging can also depict associated soft tissue damage. We report on 9 cases of os trigonum tarsi syndrome studied with MR imaging.Nine patients with the os trigonum tarsi syndrome were submitted to MRI. All the examinations were performed with the patients in supine recumbency with the injured foot in neutral position and then in forced plantar flexion. Axial and sagittal T1 SE, T2* GE and FIR images were acquired. We evaluated os trigonum location and shape, signal intensity of bone, cartilages and adjacent soft tissues, and possible associated tendon injuries.No changes were found in the os trigonum location and shape. Signal intensity changes were seen in 2/9 cases. Particularly, a small area of very high signal intensity, due to necrosis, was depicted on the talar aspect in 1 case; a subchondral spot of slightly increased signal intensity, with a low-signal outline, was seen on the calcanear aspect in another case. Disruption of the cartilaginous synchondrosis between the accessory navicular bone and the posterior tibial aspect was observed in 7/9 patients. Tenosynovitis of the flexor hallucis longus was associated in 6/9 patients. Pseudoarthrosis with irregular bone margins and high-signal spots within the cartilage was found in 3 cases. Finally, fluid effusion surrounding the os trigonum and adjacent soft tissues was always detected.The os trigonum syndrome may result from chronic microtraumas. Indeed, forced plantar flexion may cause os trigonum compression between the posterior aspect of the tibial malleolus and the calcaneus, with disruption of the synchondrosis with the lateral tubercle of talus. Joint inflammation may be associated with possible development of pseudoarthrosis. Other possible complications are related to vascular changes which may lead to bone necrosis. Furthermore, the particular anatomical site of the os trigonum may sometimes cause compression to the flexor hallucis longus tendon, resulting in severe tenosynovitis. MR imaging allows complete morphostructural assessment because it depicts the margins and the signal intensity of bone and ligaments on the 3 spatial planes. Particularly, sagittal T2 images best demonstrate the cartilage changes indicating synchondrosis disruption. This condition may cause abnormal mobility of the accessory bone with possible impingement with the posterior aspect of the tibia, or hypomobility due to pseudoarthrosis. Forced plantar flexion acquisitions are particularly useful in this condition because they can demonstrate the mechanism of injury.

Chronic inflammation in rheumatoid arthritis usually involves articular synovia and extends to other joint components such as bursae, tendons and sheaths. Conventional US with high frequency transducers is an accurate tool for assessing abnormal changes in evolutive rheumatoid arthritis. We investigated the role of color and power Doppler imaging in staging extra-articular involvement, monitoring local inflammatory changes and drug treatment response.We used a color Doppler unit with a 5-10 Mhz transducer and automatic power Doppler switch to examine 23 patients with tenosynovitis of the flexor (4/23) and extensor (8/23) tendons of the hand, Achilles (6/23) and posterior tibial (3/23) tendons, and long head of biceps (2/23) in acute rheumatoid arthritis. Only minimal pressure was exerted with the probe on the patients' skin to avoid compression and collapse of blood vessels. Ten normal volunteers were also examined as a control group.In all 23 acute rheumatoid arthritis patients, conventional US showed tendon involvement with intra-articular fluid, thickened tendons with partial tear and markedly hypoechoic thickened sheaths. Color signals were shown in all patients. Mid-caliber vessels were visualized coursing straight from the sheath deep into the tendon. Intra-articular signals were seen in 10/12 patients only. Spectral analysis showed arterial flow, with RI ranging .40-.70. Power Doppler added no important information, but improved vessel depiction relative to color Doppler thanks to its higher accuracy in detecting flow signals. There were no color signals in the tendons of the 10 healthy volunteers in the control group. No color signals were seen in both joints and tendons in 12 patients submitted to medical treatment.Color and power Doppler can be a necessary and useful integration to high resolution US for flow mapping in rheumatoid arthritis patients with tendon and extra-articular involvement. These modes depict local circulation changes related to disease stage and treatment response.

We investigated possible alterations in the enzyme activity of catalase and isozyme MB-creatine kinase induced by prolonged exposure of laboratory rodents to a static magnetic field generated by a .5 T Magnetic Resonance unit.Thirty Wistar albino mice were divided into two groups of 15 mice, one to be exposed to the static magnetic field for 12 hours and the other to be kept in the same environmental conditions as a control group. Immediately after the exposure a peripheral venous blood sample was collected, the cardiac muscle was removed from the mice and the enzyme activity of catalase and MB-creatine kinase were assayed using the spectrophotometric analysis.No statistically significant variation was detected between the enzyme activity of catalase and MB-creatine kinase in the serum and cardiac muscle of the exposed versus the control mice. In the mice exposed to the static magnetic field the enzyme activity of serum and cardiac muscle catalase were respectively .2154 U/L and .0707 U/L after 10 minutes; they were; .2699 U/L and .0946 U/L after 160 minutes. In the control mice the enzyme activity of serum and cardiac muscle catalase were respectively .1941 U/L and .0707 U/L after 10 minutes; they were .2061 U/L and .1068 U/L after 160 minutes. The enzyme activity of MB-creatine kinase in mice was measured in the exposed (80.8 U/L) versus the control (79.6 U/L) group: the difference does not exceed standard deviation.Our results seem to exclude any alteration in the activity of catalase and MB-CK after 12 hours' exposure to the static magnetic field. However some homeostatic mechanisms peculiar to pluricellular organisms might act in vivo to adapt to the effects of the static magnetic field during exposure.

We investigated the clinical usefulness of liver MRI in a large multicenter study because the control of iron concentration (LIC) is crucial in thalassemia major (TM) prognosis and conflicting results have been reported in small and heterogeneous groups of patients using Magnetic Resonance Imaging (MRI), the only very common non-invasive procedure.One hundred and eight consecutive TM patients, selected according to a specific protocol, were included in the study. In 29 of these LIC was measured by atomic absorption spectrophotometry on liver biopsy and expressed as microgram/gr dry weight. MRI was performed with a 0.5 T superconducting unit. The ratio of the average signal intensity (SIR) of the liver to that of paraspinal muscle was calculated and expressed as a percentage.Mean +/- (SD) of SIR was 100 +/- 26% (test for normality: p = 0.02) and of LIC was 3677 +/- 4662 micrograms/g/dry liver (test for normality: p0.00001). The logit transformation of LIC (lLIC) achieved both normality of distribution of lLIC and linear regression of lLIC on SIR (t = 7.36; p0.00001) according to the equation: Y = -0.0136 + SIR* (0.157) = lLIC log10 (K-LIC/LIC) where K = 79.433. Thus the values of LIC are expressed by the equation LIC K/(1 + 10 y) micrograms/g/dry liver. Moreover, a mild correlation was found between SIR and AST (p = 0.01; r = -0.30), ALT (p = 0.02; r = -0.21), gamma GT (p =0.01; r = -0.37; r = -0.25).This study shows that LIC can be calculated as a function of SIR. A validation study is necessary before introducing this prediction rule in clinical practice.

The possible evolution of the usual CT signs in oncology makes the logical premise of our experience. Our goal was to transform instrumental findings in information richer in clinical yield, in compliance with the guidelines of the National Cancer Institute (Bethesda, Md, USA).Our study was developed over a 3-year period and involved 432 selected patients with primary neoplasms. The lesions were in pulmonary, gastroenteric and genitourinary sites and stages differed (T2/T3, N0/N3, M0/M1); the Karnofsky score ranged 60-90. We integrated the tumor staging obtained with CT with gravitational tests by means of simple changes in the position of forced decubitus resulting in procubitus and triclinium-like positions (right and left lateral decubitus). We restudied only the cT class of each lesion, looking for a better depiction of the lesion-adjacent, fat, and compared the results the examinations in the standard versus the modified decubitus. Judgements were relative to the technical quality of the modified examination and to the potential impact of these findings on the following pTNM staging.In our 432 patients and only relative to the cT parameter, CT with standard decubitus correctly staged 312 lesions (mean accuracy: 72.2%) versus 352 for CT with modified decubitus (mean accuracy: 81.5%). Improvements were more marked in gastric (22.9% of 48 cases), vesical (23% of 26 cases) and colic (22.3% of 76 cases) lesions, and less marked in pulmonary (10.6% of 132 cases), ovarian (14% of 71 cases) and endometrial (17.1% of 70 cases) lesions. The routine application of pTNM verifications to CT staging helped show the most frequent causes of cTNM mistaging in solid lesions which, in most of the T2/T3 classes, involve the direct spread to an adjacent organ and the depth of invasion of adjacent structures.The combination and comparison of cTNM with pTNM are useful not only for final tumor staging but also to assess the pretreatment diagnostic accuracy of CT studies. Introducing gravitational tests and reading examinations in pairs improved lesion location, which is the most important technical information for subsequent prognosis and treatment and can help decrease pre-/post-treatment management costs.

The inflammatory involvement of the sacroiliac joint is frequent during seronegative spondylarthritis. The clinical diagnosis of sacroiliitis may be very difficult, especially in the early stage, because joint motion cannot be assessed directly and the clinical picture is very similar to that of lumbar pain. Conventional radiography is negative as long as the structural change in the joint is limited to the synovial membrane and the cartilage (early stage). Computed Tomography (CT) also has many drawbacks, and thus the changes can be shown only when chondritis and enthesitis have already damaged the bone. The disease onset is usually preceded by a long latency; early diagnosis is needed for a proper and timely treatment, which can be made only with a highly sensitive and specific technique. We investigated the diagnostic accuracy of MRI in the early detection of sacroiliitis during seronegative spondylarthritis.Forty patients with suspected sacroiliitis and negative radiographic findings were submitted to MRI; thirty-seven of them were HLA B27 positive. MRI was performed with a .5 T superconducting unit; T1-weighted SE, T2-weighted FSE, T2* GE, and STIR images were acquired on the oblique coronal plane parallel to the anterior sacrum. Ten asymptomatic volunteers were also examined as a control group.An irregular sacral border and marrow changes at the insertion of the sacroiliac ligaments were seen in 3/10 asymptomatic volunteers. MRI was negative in 7/40 patients, while the synovial compartment was replaced by some tissue with low signal intensity of T1 and high signal on T2 in the other 33 patients; this finding was referred to synovial pannus. Persisting low-signal foci were seen in the synovial compartment in 16/33 patients, which were referred to spared cartilage. High-signal regions were depicted at the bone periphery in 9/33 patients, which areas were consistent with bone erosion; the subchondral bone was markedly hypointense in 5 of these patients, indicating sclerosis. Finally, diffuse high signal intensity was found in the bone marrow in 3/33 patients and referred to infectious sacroiliitis.MRI appears the method of choice for the early detection of seronegative sacroiliitis because it can show the early changes in cartilage and subchondral bone, filling the gap between the onset of symptoms and radiographic evidence. Moreover, MRI uses no ionizing radiations and makes therefore a precious tool for the diagnosis and follow-up of young patients, hopefully decreasing the use of CT which however provides better detailing of bone and bone degeneration.

The technologic improvement of surface coils in MRI has allowed better visualization of the skin and thus permitted the clinical use of this technique in dermatology. MRI allows to assess the depth and extent of skin tumors and to detect any malignant transformation. The MR differentiation between benign and malignant skin lesions relies on morphological criteria which however do not have an absolute diagnostic value. We investigated the role of paramagnetic contrast agents in the differentiation between benign and malignant skin lesions.Forty-one patients, 33 with benign and 8 with malignant skin tumors, were submitted to MRI. All the examinations were performed with a 1.5 T superconductive unit, with a 2.5 cm surface coil. Axial T1- and T2-weighted SE images were acquired with 2 mm slice thickness. Paramagnetic contrast material was administered to all patients. The signal intensity of the skin lesions was calculated before and after paramagnetic contrast agent administration positioning a region of interest. A percentage ratio of contrast enhancement was calculated to quantify contrast agent uptake and the relative values were compared between benign and malignant lesions. A qualitative analysis was also performed rating the contrast enhancement of each lesion as high, intermediate, or absent.The quantitative analysis showed a statistically significant difference (p.5) between the contrast enhancement values of benign and malignant lesions. In particular, malignancies had values ranging 117.3 (+/- 28.7) to 125 (+/- 32.4), while benign lesions had -20.6 to 99.8 (+/- 21.1). Conversely, no difference in contrast enhancement was found at qualitative analysis.MRI is a promising tool for characterizing skin tumors. Our preliminary results should be confirmed on larger series of patients with the use of high temporal resolution imaging sequences.

Integrated imaging plays a fundamental role in the study of periskeletal soft tissue tumors, for both diagnosis and treatment planning. The steady and progressive technologic progress of color Doppler US equipment now permits the integration of conventional morphostructural parameters with the biofunctional data of lesion flow patterns and relative qualitative features. To assess color Doppler capabilities in differentiating benign from malignant soft tissue tumors, we reviewed the B-mode and color Doppler findings of 43 consecutive patients with a palpable periskeletal soft tissue mass. All patients were examined with a real time unit (Ultramark 9 HDI), with a broadband (5-10 MHz) linear transducer operating at 6.5 MHz for Doppler measurements. The PRF was set at 1500 to 800 Hz with 70% color gain; a 100 Hz wall filter was used. We kept the color box in the area of interest as small as possible to keep the frame rate high; pulsed Doppler studies were performed with a small sample volume and 2000 Hz PRF. The following signs were considered: morphostructural features; the presence/absence of color signals; the (peripheral/internal) site of vascular branches, their caliber and course; the number of afferent vascular poles; resistance index. As a rule, malignant masses tend to differ from benign masses for the presence of multiple vascular afferent branches, especially if they have an irregular pattern and caliber, and for the variability of the resistance index measured in different parts of the same mass. Further examinations, performed with second level imaging (CT and MRI) and microhistologic tests, respectively after biopsy and surgical resection, confirmed the high predictive value of color Doppler US, with only 1 false negative and 2 false positives; color Doppler sensitivity and specificity were 94.7% and 91.6%, respectively, which are higher values than those obtained with US alone (63% and 66.6%). Therefore, we believe that color Doppler US can be systematically applied to the study of periskeletal soft tissue masses, integrating conventional US for the correct selection of the patients to be submitted to second level investigations.

Transient patellar dislocation is a rare finding. It is usually due to direct trauma in mediolateral direction or to indirect trauma related to violent quadriceps muscle contraction. The clinical diagnosis of transient forms is usually rather difficult because the associated signs (hemarthros and pain at the joint and/or at the vastus medialis insertion) are not specific. Conventional radiography and CT are very accurate in bone studies, but exhibit major limitations in the detection of capsular injuries with intramedullary and/or cartilaginous bone involvement. We investigated MR diagnostic reliability in the study of the injuries caused by transient patellar dislocation. 526 MR examinations of the knee were reviewed retrospectively; fifteen of them were positive for transient patellar dislocation. We used a .5 T superconductive MR unit with a circumferential extremity coil. Axial, coronal and sagittal T1-weighted spin echo, T2*-weighted gradient echo and axial STIR images were acquired. The following criteria were considered specific for the diagnosis of former lateral patellar dislocation: retinacular changes; patella site; signal intensity changes in femoral and patellar intramedullary bone; joint effusion; cartilage and subchondral bone changes. All the patients with transient patellar dislocation had hemarthros with inner retinaculum involvement. The femoropatellar joint was incongruous in 115 cases; patellar subluxation was external in 8/11 patients and lateral in the remaining 3 patients. Medullary bruises were found in 8/15 cases and cartilage injuries in 13/15. Finally, other joint components, such as the posterior horn of the medial meniscus or the anterior cruciate ligament were involved in 6/13 cases. Arthroscopy was performed in 6 patients with associated injuries and confirmed all MR findings (100% agreement). To conclude, in our experience MRI was a very reliable tool to study the injuries due to transient patellar dislocation because it showed specific changes and possible associated traumas, which helps choose the most suitable treatment.

We describe an unusual labyrinthine malformation in a case of Goldenhar syndrome studied with CT and steady-state MR imaging. A single posterosuperior semicircular canal was found with no common crus formation. The malformation could not be attributed to any embryologic development. Nosologic considerations are discussed.

The supraspinatus muscle performs about 60% of the elevation-abduction motion of the arm; therefore, it has a prominent functional role among the extrarotational muscles of the shoulder and is the most injured in subacromial space conditions. Seventy-four patients, aged 21-64 years, were examined to compare ultrasonography (US) results with surgical findings in supraspinatus conditions and to analyze the possible pitfalls in US diagnosis. All the patients underwent conventional X-ray, US and then surgery or arthroscopy. The following criteria were considered: morphology, thickness, echotexture, the convexity of the superior border of supraspinatus tendon, the relationships with the subacromial bursa and the tendon of the biceps long head, the regularity of the bone cortex of the humeral head. US showed: chronic degenerative tendinopathy in 10 patients; perforating focal injuries in 21 patients; deep focal injuries in 10 patients; intramural focal injuries in 6 patients; superficial focal injuries in 8 patients; complete tendon tear with detachment in 19 cases. 62/74 US diagnoses were surgically confirmed, with a specificity of 83.7%. In our experience, US provided very useful information about the pattern, size and site of the injuries and was very helpful in the surgical planning.

We investigated the ultrasonographic (US) features of Stener injury and thus the role of US in the diagnosis of this condition. Stener injuries are characterized by the tear of the cubital ligament system of the first metacarpophalangeal joint during hyperabduction. This condition may be associated with the displacement of the ligament proximal stump above the thumb adductor muscle. We examined 12 patients (9 men and 3 women) with a clinical diagnosis of joint instability due to hyperabduction of the first metacarpophalangeal joint. All the US exams were performed with a linear probe (7.5-10 MHz); a 13-MHz probe was also used in 7 patients. All the patients were submitted to radiography in the standard projections, to detect associated bone injuries. US allowed the identification of a round lesion surrounded by a hypoechoic halo, between the distal edge of the dorsal interosseous muscle of the first finger and the first metacarpal bone (the proper Stener injury) in 4/12 patients. In contrast, no typical sign was shown in the other 8 patients. US results were then compared with surgical findings. The diagnosis of proper Stener injury was surgically confirmed in 4 patients. Only 2 patients with clinical signs of thumb instability, who were negative for Stener injuries at US, were submitted to surgery which demonstrated the ligament tear responsible for joint instability, but ruled out the displacement. To conclude, US must be integrated with clinical signs because this imaging method accurately shows complete tears with displaced proximal stump of the cubital ligament.

We report our experience with MRI of the normal and pathological inner ear with fast spin-echo and modified gradient recalled at steady state sequences. Although earlier studies on temporal bone MRI were discouraging, improvements in MR technology combined with the use of paramagnetic contrast media can make MRI a useful diagnostic tool for the assessment of inner ear pathology. Conventional spin-echo imaging seems not to be the modality of choice because of the relatively thick slices and the long acquisition times.

The extensive use of magnetic resonance imaging in medical diagnosis needs experimental confirmation of the real biological harmlessness of magnetic fields and radiofrequencies in the imaging process. To date, no unquestionable conclusions have been drawn and experimental results differ in various literature reports. We investigated the effects of radiofrequencies and magnetic fields on the amino acid content of plasma samples from 10 healthy volunteers aged 25 to 30 years; the samples were exposed for 60 minutes to MR fields at 0.5 T with the sequences commonly used in clinical practice. After exposure to magnetic fields, the samples were analyzed with chromatography and the results compared with those of plasma samples not exposed to MR fields. Thirty-four different amino acids were investigated and no significant changes were observed in the total concentration of any of them. Our results show that, at least in a cell-free system, exposure to a magnetic field at 0.5 T causes no significant quantitative changes in amino acid composition, at least no changes demonstrable at chromatography. On the other hand, our preliminary observation does not exclude that exposure to nonionizing radiation may modify in vivo enzyme kinetics with transient qualitative, but not quantitative, changes.

The interactions of magnetic fields and radiofrequency with biological systems have been described by several authors. However, no definitive conclusions have been drawn yet as to the safety of the magnetic fields and radiofrequencies used in clinical examinations. The immune system is one of the most complex biological systems, in which a network of intracellular signals regulates the immune response. Interleukins are released by an activation process which involves, at least in part, intra- and/or extracellular calcium mobilization. The latter step can be influenced by the in vitro effect of this type of nonionizing radiations produced by an MR system on peripheral blood mononuclear cells. Our results show that the 2-hour exposure to magnetic fields (0.5 T) and radiofrequency caused an increase in the spontaneous release of IL2, IL4, IL10, TNF alpha and INF gamma, while the amount of the same cytokines induced by PHA stimulation was decreased. No differences were observed in the spontaneous or PHA-induced release of IL6 by exposure to magnetic fields (MRI). Furthermore, the expression of the CD 11b molecule was increased at the same time. These results may be useful for us to understand the interactions between magnetic fields and radiofrequencies and the immune system.

The authors reviewed 458 MR examinations of the knee to assess the potentials of this technique in the study of traumatic injuries of the posterior cruciate ligament (PCL) and to investigate the frequency of their association with injuries of other knee joints. MR images were acquired with an 0.5-T super-conductive unit with an extremity coil. T1-weighted spin-echo (SE) and T2*-weighted gradient-echo (GE) sequences were used on sagittal, coronal and, in some cases, axial planes, with 3-mm slice thickness and 0-1 mm slice gap. The following parameters were studied to diagnose partial or complete PCL tears: PCL thickness and outline, disruption of ligamentous fasci and signal intensity features. The injuries were classified as proximal, intermediate and distal according to their site. Thirteen PCL tears were detected, 5 of them complete and 8 partial. Complete tears were found in 4/5 patients in the middle third and one patient had a distal intersection tear with tibial bone avulsion. Partial tears were found in the distal tract in 5/8 patients, in the proximal tract in one patient and in the middle third in 2 patients. In 12/13 patients capsuloligamentous knee tears were associated. Overall MR rate of PCL tears was 2.8%; of them, 92.3% were associated with other capsuloligamentous injuries. MR diagnosis was confirmed at arthroscopy in 12/13 patients. The sagittal plane was best in optimally demonstrating the whole PCL and its injuries. GE T2* sagittal sequences demonstrated the site of PCL tears better than SE sagittal ones. Relative to other authors, we found MRI an extremely reliable method to distinguish partial from complete PCL tears. This was probably due to the thinner slices (3 mm) we used. MRI can accurately assess the extent of traumatic injuries of the PCL and of other capsuloligamentous parts of the knee, which is relevant from the clinical point of view. Indeed, MRI allows useless diagnostic arthroscopy to be avoided and yields major indications to choose the correct treatment.

Posterior tibial tendon (PTT) tears are the most common cause of unilateral flatfoot in adults. It is a chronic pathologic condition with a degenerative inflammatory character which usually affects middle-aged or elderly women, with no history of trauma. PTT tears occur more rarely in young patients, after an acute traumatic event. Clinically, PTT tears present with pain along the tendon, increased by weight bearing, swelling and functional limitation. PTT tears are surgically classified in three types: with tendon hypertrophy, with tendon atrophy and with complete tendon tear. The three different anatomicosurgical patterns require different therapeutic approaches. This work was aimed at investigating MR capabilities in recognizing the direct and indirect signs of PTT tears, assessing MR sensitivity in different types of tears. Fourteen patients with clinically suspected PTT tears and 10 control patients underwent MRI of the ankle (0.5 T, GE Vectra) following a study protocol including SE T1-weighted and FSE T2-weighted scans acquired on the axial and sagittal planes. In all patients, the transverse diameter, the shape and the section surface of the lesions were calculated on the axial planes at different levels; all these parameters were compared with those of the adjacent tendons, i.e., flexor longus ditae and flexor hallucis. Moreover, the probable presence was investigated of intratendineous signal intensity changes, of peritendineous synovial fluid and of indirect signs of PTT tear, i.e., the lack of longitudinal talonavicular alignment and the bulging of the medial navicular tubercle. According to MR results, the 14 patients were subdivided into 8 patients with type-I tears, 2 with type-II tears and 4 patients with type-III tears. The measures of healthy and injured tendons were in substantial agreement: in fewer cases, agreement was found also between symptomatic and control patients with intratendineous signal changes. Thus, the specificity of the two parameters was reduced. Finally, such indirect signs of PTT tear as a hypertrophic navicular tubercle and the lack of longitudinal talonavicular alignment exhibited good specificity, but sensitivity was barely sufficient. MR diagnoses were confirmed at surgery in 6 patients with type-II and type-III tears. In conclusion, MRI appears to be a suitable modality for recognizing and classifying PTT tears and a useful tool for the treatment and follow-up of these patients.

Twenty women with breast cancer confirmed at cyto-histology, 4 of them with axillary lymph node metastases, were submitted to neo-adjuvant chemotherapy with the conventional systemic treatment combined with intralesion drug administration and monitored by means of color-Doppler ultrasonography (CDU). Before treatment, CDU demonstrated marked color signal increase in 18 cases. After treatment, tumor vascularity clearly decreased and response was more apparent since treatment had been local rather than systemic. In this paper, the consistent semiology of breast cancer is discussed together with the progressive changes in CDU color signal, the latter a valuable parameter for surgical timing. Histology, performed during treatment and on the surgical specimen, allowed treatment results to be monitored, showing a rich proliferation of connective tissue and the persistence of rare neoplastic nests. In 2/4 patients with lymph node metastases, the latter lesions reduced in size. The postoperative follow-up of these patients, which was the object of our study, currently ranges 4 to 36 months, in which period of time no relapses have been observed. The results obtained so far suggest the effectiveness of combined parenteral and local drug administration in the preoperative treatment of breast cancer. Moreover, this approach makes surgery feasible even in the most advanced forms (T3-T4). CDU is the most accurate method to monitor neo-adjuvant treatment effectiveness.

A group of patients complaining of facial nerve palsy (Bell's palsy) were evaluated with MRI using a 0.5 unit and high resolution T1-weighted images, in order to evaluate the efficacy of Gd-DTPA enhancement in depicting the degree of nerve damage: MRI examination was performed within 24 hours to two weeks after the onset of palsy. The authors describe the results obtained from the analysis of 11 cases of Bell's palsy, discussing the prognostic and clinical significance.

In most cases ankle sprains involve external compartment ligaments. In particular, the anterior talofibular ligament is involved alone in 70% of cases and together with calcaneofibular and posterior talofibular ligaments in the remaining 30% of cases. To investigate the potentials and the possible limitations of high-frequency US (7.5-13 MHz) for the preliminary assessment of the extent of damage of the capsulo-ligamentous lesions of the external ankle compartment, 25 athletes with clinical diagnosis of sprain trauma were examined with conventional radiology. Morphology and structure were studied from the semiologic point of view. US findings were compared with MR results. In all patients, US showed anterior talofibular ligament lesions alone in 13 patients and associated with calcaneofibular ligament lesions in 12 patients. The posterior talofibular and the interosseous talocalcaneal ligaments were never demonstrated by US. In 4/13 patients diagnosed by US as having isolated anterior talofibular lesions, MRI demonstrated a coexisting calcaneofibular lesion (4/13) and, in one of them, a posterior talofibular and interosseous talocalcaneal ligaments lesion. The comparison of US and MR findings in the patients US had diagnosed as having associated anterior talofibular and calcaneofibular ligaments lesions, showed 100% agreement; MRI allowed the demonstration of the lesions in the posterior talofibular and interosseous talocalcaneal ligaments in 2/12 patients. Considering the statistical prevalence of the anterior peroneal-astragalic lesions caused by ankle sprains, the use of high-frequency US as the first diagnostic approach seems justified. Nevertheless, MRI is a fundamental complement for accurately assessing damage extent.
