This work reports a complete conformational analysis of caffeic acid, an exceptionally versatile pharmacophore, using laser ablation chirped-pulse Fourier transform microwave spectroscopy. The whole conformational space consisting of eight distinct species has been fully deciphered based on the trend of the rotational constants supported by theoretical computations. We show how rotational spectroscopy can be confidently used to distinguish between conformers even when the structural differences are minimal, such as those involved in the conformational panorama of caffeic acid. Additionally, the structural information here provided, such as the planarity observed in all the conformers, could help to elucidate the mechanisms underlying the biological and pharmacological activity of hydroxycinnamic acids.

Background Most patients with heterozygous familial hypercholesterolemia (FH) do not achieve current LDL-C goals proposed by European guidelines with conventional lipid-lowering therapy (LLT). Chronic use of PCSK9 inhibitors (PCSK9i) have shown to reduce LDL-C levels up to 61% on top of statins. Persistence to chronic LLT is important to reduce the burden of atherosclerotic cardiovascular disease (ASCVD). Purpose To analyze persistence and effectiveness of PCSK9i in clinical practice setting in FH patients from the SAFEHEART register with long-term follow-up. Methods SAFEHEART is an open, long-term prospective study of a cohort of subjects with molecular diagnosis of FH. Follow-up is carried out every year through a standardized phone-call to collect clinical conditions, persistence to medications, lipid profile, and cardiovascular events. This study analyses subjects ≥18 years of age on stable LLT who have received PCSK9i. Results 696 individuals (46% females), median age 56.4 years (IQR 49–66) started with PCSK9i (49% alirocumab and 51% evolocumab). Out of them 38% had history of ASCVD, and 89% were on maximum LLT. Median LDL-C at the moment of starting PCSK9i was 145 mg/dL (IQR, 123–177), representing a poor 2016 & 2019 ESC/EAS guidelines achievement (3% and 0.1% respectively). After a median follow-up of 3.7 years (IQR, 2.3–4.8), 669 patients (96%) remained on PCSK9i treatment during entire follow-up. Only 27 patients (4%) discontinued, 5 temporarily (0.7%) and 22 permanently (3.2%). Most common reasons for PCSK9i treatment interruption were medical decision (n=6), adverse event (AE) (n=5), patient decision not related with AE (n=5) and comorbidity (n=5). Median time to permanent discontinuation was 15 months (IQR, 4–33). Median LDL-C levels observed and % of LDL-C reduction obtained after 1 year of treatment and in the last follow-up visit were: 63 mg/dL (IQR, 43–88), 61 mg/dL (IQR, 44–82), 57.6% (IQR, 39.5–69) and 58% (IQR, 44–68), respectively. 2016 ESC/EAS guidelines LDL-C goals was achieved by 70% of patients at year 1 and 77% in the last follow-up visit after the introduction of PCSK9i (p Conclusion Long-term persistence to PCSK9i treatment in FH patients is very high (96%) and reasons for discontinuation are diverse. Effectiveness in LDL-C reduction and LDL-C goal achievement improved significantly with introduction of PCSK9i in clinical practice setting. Introduction: Madrid has been the epicenter of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic in Spain. We analyzed our experience with SARS-CoV-2 infected and cancer patients. Patients and Methods: We included patients from March 1 to April 30 2020 at Hospital Universitario Puerta de Hierro, Majadahonda, Madrid (Spain). The inclusion criteria were diagnosis of SARS-CoV-2 infection made by reverse transcription polymerase chain reaction (RT-PCR) of nasopharyngeal specimens in cancer patients who were admitted to the hospital due to the need for respiratory support. The exclusion criteria were suspected cases not confirmed. The primary objective was to analyze the mortality rates of patients with cancer, especially those with lung cancer and COVID-19. Resumen La fragilidad es considerada como un estado clinico en el que el paciente presenta un mayor riesgo de evento adverso, definido como un mayor riesgo de discapacidad, dependencia, morbilidad, mortalidad, institucionalizacion y hospitalizacion. La fragilidad es un concepto dinamico, de tal forma que a «mayor fragilidad, mayor riesgo de evento adverso», por lo que su deteccion permitira una toma de decisiones mas adecuada a las caracteristicas de la persona mayor. Las pruebas de ejecucion son las herramientas recomendadas tanto en Atencion Primaria como especializada para la deteccion de fragilidad. El seguimiento de actividades instrumentales de la vida diaria adquiere importancia en la deteccion precoz en la clinica asistencial del declive funcional. Y ante la sospecha de fragilidad se recomienda la realizacion de una valoracion clinica de tipo multidimensional, o en forma de valoracion geriatrica integral en los casos establecidos o mas avanzados, para corroborar la fragilidad, valorar necesidades y establecer planes de intervencion adecuados e individualizados.

Background: Madrid has been the epicenter of the SARS-CoV2 pandemic in Spain. We analyzed the experience at our hospital with SARS-CoV2 infection and cancer patients (p). Method(s): We analyzed our experience from March 1 to April 30 at the Puerta de Hierro University Hospital in Madrid. Diagnosis of SARS-CoV2 infection was made by RT-PCR, suspected cases not confirmed were excluded. Result(s): Overall in-hospital mortality cancer p with COVID-19 was 15.2% (95%CI, 6.3;5.2), similar to 12.7% (95%CI,11.1;4.4) with p=0.615 of the global COVID-19 hospitalised population and greater than that of patients admitted without SARS-CoV-2 infection during the same period 4.3% (95%CI;3.6;5.2) p0.001. Among 653 patients receiving active cancer therapy during this period, 24 (3.7%) developed COVID-19 and required admission, 4.2% of were receiving chemotherapy, 9.5% immunotherapy and 2.1% targeted therapies. Lung and breast cancer were the most frequent (26.1%), followed by colorectal (19.6%) and breast cancer. No significant differences due to the cancer treatment received were observed. Mortality in lung cancer patients was the highest (25%). The univariate analysis (between p who developed serious event vs. those who did not), showed that higher Brescia, CURB-65 scale, lactate dehydrogenase (LDH) or C-reactive protein (CRP) levels at admission, the greater risk of developing severe complications (p0.05) [Formula presented]. Conclusion(s): Patients with cancer, especially lung cancer, and SARS-CoV2 infection have a worse overall prognosis than the general population. Objective parameters such as LDH, CRP at admission, Brescia index or CURB-65 should alert us to a more serious evolution and suggest early an early intensive care unit (ICU) admission. e13527 Background: The SARS-CoV-2 (COVID19) pandemic has had a huge impact on health care, leading to the declaration of a “state of alarm” in many countries. This situation has possibly conditioned the care of cancer patients, compromising hospitalization resources due to healthcare restraints. We aim to analyze the impact of the COVID19 pandemic on the hospitalization of cancer patients during the “state of alarm” in an Academic Hospital in Madrid, considered one of the most affected regions in the World during the first wave of the pandemic in Europe. Methods: Quasi-experimental study with similar controls collected in the same period of the previous year from cancer patients admitted to the Medical Oncology (MO) department of the Puerta de Hierro University Hospital in Madrid. Patients were selected from diagnoses coded by the ICD-10 obtained from discharge reports in the Admission Department. Cancer and viral pneumonia codes (excluding influenza) were analyzed. Admission to Internal Medicine (IM) department was also explored to describe hospitalization due to COVID19 pneumonia. We defined two groups: pandemic (03/14/20 - 06/20/20) and control (pre-pandemic) (03/14/19 - 06/20/19). Results: 74 cancer patients were admitted to MO department in 2020, while 162 admissions were recorded for the equivalent period in 2019 (- 54%). No differences were observed related to sex and age. In 2020, emergency and programmed admissions decreased by the same proportion, and no difference in mortality was observed. Admissions for all neoplasms decreased, but with higher magnitude in breast cancer. Viral pneumonia (no influenza) in cancer patients were mostly admitted to IM (96%) rather than MO department. 79 cases were identified in 2020, 22 in 2019 (p < 0.001), with a non-significant increase in mortality in patients with pneumonia: 25,3% in 2020 and 13,6% in 2019. Conclusions: The COVID 19 pandemic seriously impacted cancer hospitalization care, decreasing admissions to the MO department in half compared to the equivalent period. We observed a reduction in admissions through all neoplasias. Admissions due to viral pneumonia in cancer patients increased 3.5 times, mostly admitted to IM changing the dynamics of cancer hospitalization burden and compromising resources. A non-significant increase in mortality from viral pneumonia was observed. The analysis of hospitalizations through administrative data can allow better management of resources and facilitate rapid decision-making in a pandemic situation yet to be resolved.[Table: see text]

Esta investigacion se centro, por una parte en la percepcion de apoyo a la autonomia en las clases de Educacion fisica, que tienen tanto el alumnado como el profesorado, y por otra, en la coherencia entre percepcion y la realidad observada en las clases. Se aplicaron dos escalas al profesorado y al alumnado con el objeto de conocer con que frecuencia se producen comportamientos instructivos favorables al desarrollo de la autonomia, y se observaron y grabaron en video clases impartidas por el profesorado. Los resultados muestran que la frecuencia de los comportamientos instructivos de apoyo a la autonomia durante la intervencion de ensenanza es significativamente menor que la percibida por el alumnado y el profesorado. La realidad observada muestra un perfil de profesorado con gran margen de mejora en comportamientos como: calidad de la comunicacion, funcionalidad de las tareas, fomento del pensamiento del alumnado, situaciones en las que el alumnado pueda expresar su opinion sobre las tareas y motivacion intrinseca.

Se ha realizado un estudio cuasi-experimental con grupo de control equivalente, para analizar la validez del juego “Flash Focus. Vision training in minutes a day”, de Nintendo, sobre la posible mejora de la coordinacion oculo manual, la anticipacion de tiempo y movimiento y la atencion selectiva. La muestra estuvo compuesta por 29 estudiantes de Ciencias de la Actividad Fisica y el Deporte de la Universidad Autonoma de Madrid, divididos en un grupo experimental de 21.37 anos de edad (SD= 5.07), y otro control de 22.37 anos (SD= 3.49), que realizaron un total de 15 sesiones de entrenamiento con el juego. Solo uno de los resultados de los contrastes realizados, el referido a la atencion sostenida, muestra diferencias significativas en el grupo de entrenamiento (p=0.01); mientras que el resto de variables atencionales estudiadas muestran resultados no significativos en las comparaciones pre y post. Este resultado justifica el interes por seguir investigando en esta linea y esta en concordancia con los encontrados en el empleo de esta misma consola con aplicaciones diferentes.PALABRAS CLAVE: Atencion, atencion sostenida, Nintendo, videojuegos. ABSTRACTThe aim of the current study consists of checking wether the training with Nintendo’s videogame “Flash Focus. Vision training in minutes a day” improves hand-eye coordination, time anticipation and movement and selective attention. For this purpose, a quasi-experimental pre-post design research with equivalent control group was carried out. The sample was made up of 29 university students from the degree of Sport Sciences. Results show that just only one of the dependent variables, selective attention, presented significant differences between the group which received the training and the control group. Further research with larger samples should be carried out in order to test the principles result of this paper and it is in agreement with the found ones in the use of this same console with different applications.KEY WORDS: Attention, maintained attention, Nintendo, videogames.

Objective Previous studies on monitoring of post-transplant cytomegalovirus (CMV)-specific cell-mediated immunity (CMI) are limited by single-centre designs and disparate risk categories. We aimed to assess the clinical value of a regular monitoring strategy in a large multicentre cohort of intermediate-risk kidney transplant (KT) recipients. Methods We recruited 124 CMV-seropositive KT recipients with no T-cell-depleting induction pre-emptively managed at four Spanish institutions. CMV-specific interferon-γ-producing CD4+ and CD8+ T cells were counted through the first post-transplant year by intracellular cytokine staining after stimulation with pp65 and immediate early-1 peptides (mean of six measurements per patient). The primary outcome was the occurrence of any CMV event (asymptomatic infection and/or disease). Optimal cut-off values for CMV-specific T cells were calculated at baseline and day 15. Results Twelve-month cumulative incidence of CMV infection and/or disease was 47.6%. Patients with pre-transplant CMV-specific CD8+ T-cell count Conclusions Monitoring for CMV-specific CMI in intermediate-risk KT recipients must be regular to reflect dynamic changes in overall immunosuppression and individual susceptibility. The early assessment at post-transplant day 15 remains particularly informative.

El importante aumento de la prevalencia de la obesidad durante las ultimas decadas en el mundo occidental impulsa la creacion de estrategias terapeuticas que ayuden a su control. La eficacia real de cualquier tratamiento medico dependera de su cumplimiento. En el caso de la obesidad, el cumplimiento dependera de la motivacion del paciente para perder peso y de las caracteristicas intrinsecas del propio plan dietetico y se reforzara con la eficacia del propio tratamiento. Los tratamientos con dietas bajas en hidratos de carbono se han propuesto como alternativa en los ultimos anos a las tradicionales hipocaloricas y hipograsas. Desde 2003, varios estudios aleatorizados de mas de 6 meses de duracion abordan su eficacia comparandolas con otras estrategias nutricionales. Como conclusion, independientemente de otras consideraciones metabolicas, se constata que la disminucion ponderal lograda a los 12 meses no es superior con las dietas bajas en hidratos de carbono respecto a otras alternativas, fundamentalmente por no estimular la adherencia al plan dietetico.

Nowadays, black liquor recovery is important for the economics of the pulp and paper production industry. As a result of the high capital cost of the recovery unit, the corrosive nature of the smelt, and the risk of smeltwater explosions, alternatives to the conventional recovery cycle are under research in order to achieve more efficient and environmentally cleaner processes. These alternatives fall into two categories: high-temperature and low-temperature, according to whether or not the melting point of the black liquor inorganics is reached. The advantage of low-temperature processes is to avoid the formation of smelt. In this work, the feasibility of the thermal processing at low temperature of straw black liquor in two different bench scale reactors has been tested. This study analyzed the effects of pseudoephedrine (PSE) provided at different time of day on neuromuscular performance, side effects, and violation of the current doping cut-off threshold [World Anti-Doping Agency (WADA)]. Nine resistance-trained males carried out bench press and full squat exercises against four incremental loads (25%, 50%, 75%, and 90% one repetition maximum [1RM]), in a randomized, double-blind, cross-over design. Participants ingested either 180 mg of PSE (supra-therapeutic dose) or placebo in the morning (7:00 h; AM(PLAC) and AM(PSE)) and in the afternoon (17:00 h; PM(PLAC) and PM(PSE)). PSE enhanced muscle contraction velocity against 25% and 50% 1RM loads, only when it was ingested in the mornings, and only in the full squat exercise (4.4-8.7%; P 0.05). PSE ingestion raised urine and plasma PSE concentrations (P 0.05) regardless of time of day; however, cathine only increased in the urine samples. PSE ingestion resulted in positive tests occurring in 11% of samples, and it rose some adverse side effects such us tachycardia and heart palpitations. Ingestion of a single dose of 180 mg of PSE results in enhanced lower body muscle contraction velocity against low and moderate loads only in the mornings. These mild performance improvements are accompanied by undesirable side effects and an 11% risk of surpassing the doping threshold.

Biostratigraphic data provided by the sections of the Caravaca–Cehegin region, SE Spain, have contributed decisively to the lively debate that has taken place in recent years concerning the Berriasian/Valanginian boundary. This work presents the results from new integrated stratigraphic analyses of these sections, based on the distribution of ammonites, calpionellids and calcareous nannofossils, and their calibration with the magnetic polarity scale. It is confirmed that the conflicting interpretations in the literature on the distribution of some ammonites and calcareous nannofossil species are mainly due to differences in taxonomic assignment. The event succession observed allows precise characterization of the Berriasian/Valanginian boundary, defined by the first occurrence of Calpionellites darderi. Some of the sections studied fulfil the prerequisites to be considered as potential boundary stratotypes.

This paper presents an analysis of the vertical distribution of ammonites and calcareous nannofossils throughout the uppermost Barremian ‐ lowermost Aptian interval of pelagic sections in the External Zones of the Betic Cordillera (southern Spain) . We were able to identify all the standard ammonite zones in this interval and have established for the first time a direct calibration of calcareous nannofossil events against the ammonite zonation . The results confirm that the Barremian / Aptian boundary , defined by the first appearance of Deshayesites , does not coincide with any other biostratigraphic event . Nonetheless , the sequence of events recorded in the studied sections provides alternative criteria on which to characterize this boundary with reasonable accuracy .
