Interprofessional and intraprofessional collaboration in crises situations in the intensive care unit regarding COVID-19 - a qualitative, retrospective analysis Abstract. Background: Interprofessional collaboration is elementary in the intensive care unit, a success factor in difficult patient situations and complex therapies. The COVID-19-pandemic challenged newly composed treatment teams, however, experience does not exist. Aim: We analyzed personal experiences and views on inter- and intraprofessional collaboration in intensive care units during the COVID-19-pandemic and identified influencing factors. Methods: We used a qualitative, retrospective study design, collected data from physicians, nurses in intensive care, anesthesia and surgery and physiotherapists during group discussions using the story / dialogue method. We analyzed the data according to Mayring's Qualitative content analysis. Results: We identified two main categories, each with three sub-categories: 1. Mastering a exceptional situation actively (Recognizing a common goal; Acting in solidarity; Getting to know each other in inter- / intraprofessional collaboration), 2. Having overcome the exceptional situation in retrospect (Maintaining personal contacts; Gaining new knowledge and perspectives; Taking what has been learned into the future). The participants rated the interprofessional and intraprofessional collaboration as good to very good. Conclusions: Factors promoting collaboration and positive experiences are to be incorporated into everyday work. The intraprofessional management team thereby defines common goals and values for the best possible patient care.

Die Zusammenarbeit mit Familien nimmt mittlerweile einen hohen Stellenwert auf Intensivstationen ein. Dennoch stellt sich die Frage: Sind wir wirklich bereit, uns für die Anliegen der Familien zu öffnen? Was benötigen wir konkret, um familienzentrierte Konzepte nachhaltig einzuführen? Und welche Strategien empfiehlt die Literatur?

Zusammenfassung. Hintergrund: Das Schmerzmanagement ist bedeutend im Therapiespektrum der Intensivstation, doch Leitlinienempfehlungen werden nicht konsequent umgesetzt. Strategien der klinischen Argumentation und Entscheidungsfindung in Bezug auf Schmerz sind kaum beschrieben. Fragestellung und Ziel: Geleitet von der Frage „Wann und wie berücksichtigen Intensivpflegende Schmerz im Behandlungsprozess von Patientinnen und Patienten mit Bewusstseins- und kognitiven Einschränkungen?“ wollten wir deren Denk- und Handlungsmuster im Schmerzmanagement explorieren. Methode: Im Rahmen eines Mixed Methods-Forschungsprogramms zum Schmerzassessment bei nicht auskunftsfähigen Intensivpatientinnen und -patienten führten wir eine Sekundäranalyse von Daten aus der qualitativen Teilstudie mittels qualitativer Inhaltsanalyse nach Mayring durch. Ergebnisse: Schmerz spielt im Behandlungsprozess eine wichtige, aber untergeordnete Rolle. Nachdem primär Vitalfunktionen stabilisiert sind, prüfen Intensivpflegende Bewusstsein und Kognition, um von der Patientin bzw. vom Patienten Informationen zu erhalten. Intensivpflegende differenzieren dann zwischen Schmerz, Angst, Stress und Unwohlsein. Sie stützen ihre Entscheidung auf Erfahrung und Intuition. Schlussfolgerung: Fortbildungen fördern Wissen und Bewusstsein der Intensivpflegenden zum Schmerzmanagement. Fremderfassungsinstrumente für Schmerz bieten valide Informationen. Sie unterstützen ein analytisches Vorgehen und damit den Kompetenzaufbau in der klinischen Argumentation und Entscheidungsfindung.

ZusammenfassungEin Intensivaufenthalt kann Spuren hinterlassen und zu physischen, psychischen oder kognitiven Beeinträchtigungen führen. Der Fachbegriff hierfür lautet Post-Intensive Care Syndrome (PICS). Follow-up-Angebote können zur Prävention eines PICS beitragen. Was beinhalten diese Angebote und wie wirksam sind sie?

Zusammenfassung. Ein defizitorientiertes Altersbild führt häufig zu Nachteilen für Studierende über vierzig Jahren. Oft sind sie mit Vorurteilen gegenüber ihrer Leistungsfähigkeit konfrontiert und finden nur selten eine Möglichkeit, sich beruflich weiter zu qualifizieren. Die Forschung zeigt jedoch, dass die Lernbiographie und nicht das Alter über das Leistungspotential im Studium entscheidet. Anhand des Nachdiplomstudiums Anästhesiepflege am Universitätsspital Zürich zeigt der Artikel Handlungsbedarf auf und beschreibt Lösungsmöglichkeiten.

The problem connected to the long-term storage of high-level nuclear waste has initiated an extensive search for suitable host rock formations and backfill materials to guarantee an isolation of hazardous nuclear elements from the biosphere for thousands of years. Besides other alternatives, clays and clayey materials have been in the focus of various sorption studies. With respect to long-term predictions of migration processes, the transuranium elements and fission products are of particular interest. The present review gives a detailed summary of sorption studies conducted during the past decades describing the impact of various geochemical parameters on the immobilization of Am(III) by different clays and clay minerals.

The physiological signs are a reliable source to identify stress states, and wearable sensors provide precise identification of physiological signs associated with the stress occurrence. The literature review shows that the use of physiological signs as a source for stress patterns identification is still a critical investigation subject. Few studies evaluate the effect of combining several different signals and the implications of the data acquisition procedures and details. This article’s objective is to investigate the possible integration of data obtained from heart rate variability, electrocardiographic, electrodermal activity, and electromyography to detect stress patterns, considering a new experimental protocol to data acquisition. The data acquisition involved the Trier Social Stress Test, wearable sensor monitoring, and complementary stress perception instruments, resulting in a publicly available dataset. This dataset was evaluated using different machine learning classifiers, considering the obtained annotated data and exploring different physiological features and their combinations.

Monte Carlo Tree Search (MCTS) parallelization is one of the many possible enhancements for MCTS algorithms. However, no work has been done on evaluating these methods in the rather new area of General Video Game Playing (GVGP), an area that challenges the creation of agents that can play any videogame even without prior knowledge about the video game they are going to play. To address this gap, this paper proposes the implementation and evaluation of the three main MCTS parallelization methods as agents of the General Video Game AI framework, a popular framework for GVGP agents evaluation. This paper is not focused on comparing the parallel MCTS agents to other existing GVGP agents, but rather on exploring how the MCTS parallelization methods compare between themselves. This paper also presents a testing methodology for evaluating these agents, which is based on a set of three experiments focused on different aspects of the parallel MCTS algorithms. In these experiments, the overall best results were achieved by the root parallelization method using the sum merging technique and the UCT’s sigma value of \( \sqrt 2 \).

Zusammenfassung„Der Patient zieht am Tubus und extubiert sich beinahe selbst. Ich kann nicht ständig am Bett stehen und nach ihm schauen. Darum musste ich ihm zur Sicherheit die Hände festbinden.“ In einer solchen Situation hat sich wahrscheinlich jede Intensivpflegefachperson schon einmal befunden: Einerseits ist es wichtig, die Patientensicherheit zu gewährleisten. Andererseits gilt es aber auch, die erforderlichen therapeutischen und pflegerischen Aufgaben während der Schicht auf der Intensivstation zu erfüllen.

Zusammenfassung. Hintergrund: Familienangehörige von Sterbenden auf der Intensivstation (IPS) sind vielen Belastungen ausgesetzt und erleben kaum Einbezug in Situationen am Lebensende. Sie kritisieren eine geringe Beteiligung an der Pflege, mangelnde Information sowie fehlende Privatsphäre. Auch Pflegende sehen sich mit Hürden in der Pflege am Lebensende auf der Intensivstation konfrontiert. Ziel: Diese Studie untersuchte, welche Strategien Intensivpflegende in der Deutschschweiz haben, um Familienangehörige in Situationen am Lebensende einzubeziehen. Methode: Mit dem Ansatz von Grounded Theory wurde mit zwölf halbstrukturierten Interviews ein Modell entwickelt, das pflegerische Strategien zum Einbezug von Familienangehörigen in End-of-Life-Situationen (EoL) auf der Intensivstation zeigt. Ergebnisse: Pflegende schaffen persönlichen Raum und Ruhe für Familienangehörige und ermöglichen ein Dasein zu jeder Zeit. Sie sind den Familienangehörigen ein Halt und ermöglichen, sich bewusst vom sterbenden Menschen zu verabschieden. Schlussfolgerungen: Künftige Arbeiten sollten die Wirksamkeit der beschriebenen Strategien besonders im Hinblick auf Belastungsreaktionen Familienangehöriger nach EoL-Situationen untersuchen. In der Praxis müssen Räumlichkeiten geschaffen werden, die Privatsphäre ermöglichen. Die Orientierung des Betreuungsteams an gemeinsamen Werten und Zielen ist indiziert. Ein intradisziplinärer Austausch und Mentoring müssen unterstützt werden. Um Pflegende zukünftig für EoL-Situationen auf der Intensivstation zu befähigen, ist es unumgänglich, ihre Edukationsleistung anzuerkennen und zu fördern.

Time-resolved laser fluorescence spectroscopy (TRLFS) is used to investigate the complexation of Cm(III) with methacrylate in NaCl solution as a function of the temperature (20–60 °C), ionic strength (0–2.5 mol/kg) and ligand concentration (0–0.2 mol/kg). The aqueous speciation of Cm(III) is obtained by deconvolution of the experimental emission spectra using pure component spectra of all Cm(III) species involved under the given experimental conditions. By applying the specific ion interaction theory (SIT), the temperature-dependent thermodynamic stability constants (log K0n(T), n = 1,2) are determined by extrapolating the conditional stability constants to zero ionic strength. Additionally, the binary ion interaction coefficients (e(i,j)) for the interaction between Cl− and the complex species [Cm(Meth)n]3−n (n = 1,2) are calculated. The temperature dependency of log K0n in combination with the integrated Van't Hoff equation provides the molal standard reaction enthalpies and entropies (ΔrH0m, ΔrS0m) of the respective complexation reactions confirming that both, the formation of [CmMeth]2+ and [Cm(Meth)2]+ are endothermic and entropy driven. At temperatures ≥70 °C, an additional Cm(III) species is formed that is attributed to the thermically induced polymerization of methacrylate.

Zusammenfassung Kompetenzgerechter Personaleinsatz auf Intensivstationen Im Tandem arbeiten, einander ergänzen und Mitarbeitende mit unterschiedlichsten Berufsabschlüssen ins Team integrieren – dies gehört zu den vielfältigen Chancen und Herausforderungen des Skill-Grade-Mix. Als zukunftsorientiertes Modell der Zusammenarbeit hat sich der Skill-Grade-Mix auf den Intensivstationen des Universitätsspitals Zürich bewährt. Er ist eine Antwort auf Personalengpässe, wirkt nachwuchsfördernd und leitet einen Kulturwandel ein: Unterschiedliche pflegerische Berufsgruppen übernehmen gemeinsam Verantwortung für Patienten.

Zusammenfassung. Hintergrund: Auf der Intensivstation leiden 4 – 45 % der Patientinnen und Patienten unter anhaltender Ventilatorabhängigkeit und können aufgrund erschwerter Entwöhnung nicht verbal kommunizieren. Die Trachealkanüle beeinträchtigt unter anderem physiologische Prozesse beim Atmen und Schlucken. Das Passy Muir Sprechventil (PMV) ermöglicht Patientinnen und Patienten, am Beatmungsgerät zu sprechen und zu schlucken. Die Desensibilisierung des Larynx und Pharynx sowie ein unzureichender Schluckreflex lassen sich verhindern. Ziel und Methode: Im Rahmen eines multiprofessionellen Praxis- und Qualitätsentwicklungsprojekts mit theoretischen Schulungen und Praxisbegleitung implementierten wir das PMV, um tracheotomierten, beatmeten Patientinnen und Patienten sicheres Sprechen und Schlucken zu ermöglichen. Ergebnisse: Intensivpflegefachpersonen, Physiotherapeutinnen und Physiotherapeuten sowie Ärztinnen und Ärzte schätzten die Patientensicherheit durch den Einsatz des PMV als hoch ein. Bei neun Patientinnen und Patienten mit insgesamt 51 PMV-Anwendungen traten drei unerwünschte Ereignisse in Form von Atemnot und Sauerstoffsättigungsabfall ein. Ein Ereignis führte zum Abbruch der PMV-Anwendung. Schlussfolgerung: Durch die Kombination aus verbindlicher Schulung für das gesamte Behandlungsteam, Betreuung der Patientinnen und Patienten durch zwei Fachpersonen unterschiedlicher Berufsgruppen während des PMV-Einsatzes und einem neu erstellten handlungsleitenden Dokument liess sich ein hoher Grad an Patientensicherheit erreichen. Beatmete Patientinnen und Patienten auf der Intensivstation können sich mithilfe des PMV verbal mitteilen und besser schlucken. Komplikationen sind früh erkennbar und lassen sich wirksam beheben.

The complexation of Cm(III) with malonate is studied by time-resolved laser fluorescence spectroscopy (TRLFS) in the temperature range from 25 to 90 °C. Three complexes ([Cm(Mal)n]3–2n, n = 1, 2, 3) are identified and their molar fractions are determined as a function of the ligand concentration, the ionic strength, and the temperature. A general shift of the chemical equilibrium toward higher complexes with increasing temperature is observed, with the [CmMal3]3– complex forming only at T > 40 °C. The conditional stability constants (log K′n(T)) are calculated and extrapolated to Im = 0 with the specific ion interaction theory (SIT). The log Kn0(T) values increase by 0.25 to 0.5 logarithmic unit in the studied temperature range. The temperature dependency of the log K°n(T) is fitted by the integrated Van’t Hoff equation, yielding the thermodynamic functions ΔrH°m and ΔrS°m. The complexation of Am(III) with formate in aqueous solution is studied as a function of the pH value using a combination of extended X-ray absorption fine structure (EXAFS) spectroscopy, iterative transformation factor analysis (ITFA), and quantum chemical calculations. The Am LIII-edge EXAFS spectra are analyzed to determine the molecular structure (coordination numbers; Am–O and Am–C distances) of the formed Am(III)–formate species and to track the shift of the Am(III) speciation with increasing pH. The experimental data are compared to predictions from density functional calculations. The results indicate that formate binds to Am(III) in a monodentate fashion, in agreement with crystal structures of lanthanide formates. Furthermore, the investigations are complemented by thermodynamic speciation calculations to verify further the results obtained.

The complexation of trivalent lanthanides with a commercial polycarboxylate based concrete superplasticizer (Glenium® 51) is investigated using different spectroscopic techniques. Time-resolved laser fluorescence spectroscopy (TRLFS) in combination with a charge neutralization model is used to determine temperature dependent conditional stability constants (logβ′(T)) for the complexation of Eu(III) with Glenium® 51 in 0.1 mol kg−1 NaCl solution in the temperature range of 20–90 °C. Only one complex species is observed, and logβ′(T) (given in kg per mol eq) shows a very slight increase with temperature from 7.5 to 7.9. The related conditional molar reaction enthalpy (ΔrH′m) and entropy (ΔrS′m) obtained using the Van't Hoff equation show that the complexation reaction is slightly endothermic and entropy driven. The thermodynamic investigations are complemented by structural data for complexes formed with Gd(III) or Tb(III) and Glenium® 51 using extended X-ray absorption fine structure (EXAFS) spectroscopy. The results imply a non-chelate coordination of the trivalent metals through approximately three carboxylic functions of the polycarboxylate comb polymer which are attached predominantly in a bidentate fashion to the lanthanide under the given experimental conditions.

The complexation of the trivalent heavy metals Eu(III) and Cm(III) with polyacrylate as a model ligand is investigated by time-resolved laser fluorescence spectroscopy (TRLFS) and subsequent thermodynamic modelling (charge neutralization model). The aqueous speciation of both metals is investigated as a function of temperature (20–60 °C) and ligand concentration (0–0.4 g/kg) in 0.1 mol/kg NaCl solution ([H+]total ≈ 10−4 mol/kg). Only one complex species is observed at all experimental conditions. With increasing temperature the aqueous Eu(III) and Cm(III) speciation is slightly shifted towards the complexes species. This effect is attributed to the stronger deprotonation of the polymer, which is also reflected by an increase of its loading capacity. The complex stability constants, however, show no visible temperature dependency. Log β’(T) ranges between 5.8 and 5.9 for both Eu(III) and Cm(III) over the whole range of experimental conditions.

Chitosan/bentonite hybrid composite (CBC) was developed and successfully applied to remove anionic (Amaranth Red (AR)) and cationic (Methylene Blue (MB)) dyes from colored effluents. The influence of several factors on the dyes adsorption capacity was investigated. Kinetic, equilibrium and thermodynamic viewpoints were elucidated. Desorption and reuse studies were performed. It was found that CBC presented a heterogeneous structure with good mechanical and swelling properties, which facilitates the phase separation after adsorption. The adsorption was favored at pH 2 for AR and 10 for MB. Elovich model was the more adequate to represent the kinetic data. The equilibrium data followed the Langmuir model, being the maximum adsorption capacities of 362.1 mg g−1 for AR and 496.5 mg g−1 for MB. The adsorption process was spontaneous, favorable and exothermic. Desorption and reuse was possible three times for AR and two times for MB, maintaining the same adsorption capacity. These results demonstrated that chitosan/bentonite hybrid composite is an advanced adsorbent with advantages, such as, easy phase separation, capability to remove anionic and cationic dyes and potential for reuse.

The distribution coefficients (K d) are determined for the sorption of Th(IV), U(VI), Np(V), Pu(III/IV/VI), and Am(III) in the system natural Opalinus Clay (OPA) from Mont Terri, Switzerland, and synthetic OPA pore water at pH7.6 as a function of solid-to-liquid ratio and element concentration. A linear sorption behavior under the chosen experimental conditions was obtained. The K d values strongly depend on the oxidation state of the actinide. The K d values for the tri- and tetravalent actinides are in the range of 29–159 m3/kg. U and Np in the oxidation states six and five, respectively, are weakly sorbed on OPA with K d values of 0.04–0.05 m3/kg. The K d values for the redox-stable Am(III) and Th(IV) are in the same range as for Pu(III) and Pu(IV) which are redox sensitive. The K d value of Pu(VI) (13 ± 3 m3/kg) is about one order of magnitude lower than the K d values of Pu(III) and Pu(IV), but still more than two orders of magnitude higher compared to the values obtained for U(VI) and Np(V). This discrepancy is attributed to the partial reduction of Pu(VI) to Pu(IV) during sorption.

Objectives To determine the concordance of Zurich Observation Pain Assessment (ZOPA) with the behavioural Pain Scale (BPS) and the Critical Care Pain Observation Tool (CPOT) to detect pain in nonverbal ICU patients. Design Prospective observational study [BASEC-Nr. PB_2016-02324]. Setting A total of 49 ICU patients from cardiovascular, visceral and thoracic surgery and neurology and neurosurgery were recruited. Data from 24 patients were analyzed. Main Outcome Measurements Three independent observers assessed pain with the BPS, the CPOT or ZOPA prior, during and after a potential painful nursing intervention. Tools were randomized concerning the pain management after each pain assessment. Frequency of nine additional pain indicating items from a previous qualitative, explorative study was calculated. Results ZOPA was positive in 32 of 33 measuring cycles (97.0%; 95%CI: 84.2-99.9%), followed by the CPOT (28/33 cycles, 84.8%; 95%CI: 68.1–94.9%) and the BPS (23/33 cycles, 67.0%; 95%CI: 51.3–84.4%). In 22/33 cycles all tools were concordant (66.7%; 95%CI: 48.2-82.0%). Analgesics were provided in 29 out of 33 cycles (87.9%; 95%CI: 71.8–96.6%). Additional pain indicating items were inconsistently reported. Conclusion ZOPA is concordant with the BPS and the CPOT to indicate pain but detects pain earlier due to the low threshold value. Inclusion of further items does not improve pain assessment.

Background: Family members of people dying in the intensive care unit (ICU) are exposed to many stress factors and they often do not experience involvement in End-of-Life (EoL) situations. For example, they criticize a low degree of participation in patients care, delayed or incomplete information and lack of privacy. Even nursing staff is facing various obstacles in EoL situations in ICUs. Aim: This study investigates strategies used by ICU nursing staff in German-speaking Switzerland to increase family members participation in situations at the end of life. Method: Data was collected by conducting 12 semi-structured interviews using an approach based on Grounded Theory. A model was developed to explain nursing strategies for family involvement in EoL situations in the ICU. Conclusions: Nurses provide personal space and tranquillity for family members and allow them to be present at any time. Against this background, they support family members and enable them to say goodbye consciously to a loved one. Subsequent work should examine the effectiveness of the strategies described, particularly in terms of stress reactions displayed by family members in the aftermath of EoL situations. In practice, family members should be provided space for privacy. The entire healthcare team is recommended to identify and pursue common values and objectives. Moreover, intradisciplinary exchange and mentoring need to be encouraged. In order to prepare future nursing staff for EoL situations in the ICU, recognizing and promoting their educational skills is mandatory.

ZusammenfassungDelirante Patienten in komplexen Situationen pflegen, verunsicherte Angehörige begleiten und das interprofessionelle Behandlungsteam unterstützen – die Advanced Practice Nurse (APN) kann eine Schlüsselrolle im Delirmanagement auf Intensivstationen einnehmen. Ein delirspezifisches APN-Modell bildet ihre Tätigkeiten ab und weist auf innovative Formen der Zusammenarbeit hin.

Background: Between 4 to 45 % of intensive care patients suffer from prolonged ventilator dependence. Due to impeded weaning, verbal communication is impossible. The tracheal cannula affects breathing and swallowing. The Passy Muir speaking valve (PMV) allows patients on ventilators to speak. It counteracts pharyngeal as well as laryngeal desensitization and prevents an insufficient swallowing reflex. Aim and methods: To implement the PMV, we conducted a multiprofessional practice- and quality development project. The objective was to ensure safe swallowing and communicating for ventilated patients with tracheostoma. Results: Intensive care nurses, doctors and physiotherapists assessed patient safety as being high. In nine patients with 51 PMV applications, we observed three undesirable events in terms of dyspnea and oxygen desaturation. On one occasion, it was necessary to remove the PMV. Conclusion: A combination of mandatory training for the entire treatment team, presence of two professionals during PMV application, and a newly developed guiding document resulted in a high level of patient safety. By means of PMV, ventilated intensive care patients are able to communicate verbally and to swallow better. Complications can be identified early and solved effectively.

Pore waters of natural clays, which are investigated as potential host rock formations for high-level nuclear waste, are known to contain large amounts of low-molecular weight organic compounds. These small organic ligands might impact the aqueous geochemistry of the stored radionuclides and, thus, their migration behavior. In the present work, the complexation of Cm(III) with formate in aqueous NaCl solution is investigated by time-resolved laser fluorescence spectroscopy (TRLFS) as a function of the ionic strength (0.5–3.0 mol/kg), the ligand concentration (0–0.2 mol/kg) and the temperature (20–90 °C). The Cm(III) speciation is determined by deconvolution of the emission spectra. The obtained distribution of Cm(III) species is used to calculate the conditional stability constants (log K′(T)) at a given temperature and ionic strength which are extrapolated to zero ionic strength by using the specific ion interaction theory (SIT). Thus, the thermodynamic log K0n(T) values for the formation of [Cm(Form)n](3−n)+ (n = 1, 2) and the ion interaction coefficients (e(i,k)) for [Cm(Form)n](3−n)+ (n = 1, 2) with Cl− are obtained. The log K01(T) (2.11 (20 °C)–2.49 (90 °C)) and log K02(T) values (1.17 (30 °C–2.01 (90 °C)) increase continuously with increasing temperature. The log K0n(T) values are used to derive the standard reaction enthalpies and entropies (ΔrH0m, ΔrS0m) of the respective complexation reactions according to the Van’t Hoff equation. In all cases, positive ΔrH0m and ΔrS0m values are obtained. Thus, both complexation steps are endothermic and entropy-driven.

During recent decades, the search for possible repositories for high-level nuclear waste has yielded large amounts of sorption data for actinides on different minerals. Clays and clay minerals are of special interest as potential host-rock formations and backfill materials, by virtue of their good retardation properties. Neptunium (Np) is one of the actinides which is considered in long-term scenarios due to its long-lived nuclide 237Np (t1/2 = 2.1 × 106 y). Because neptunium sorption is heavily dependent on the experimental conditions, comparison of sorption data from different experiments is challenging. Normalizing reported data with respect to the surface area of the sorbent enables conversion of conventional distribution coefficients (Kd) to normalized (Ka) values, which improves comparability among the results of different experiments. The present review gives a detailed summary of sorption data of Np on clays and clay minerals and examines critically the applicability of the Ka approach.

Chronisch kritisch-kranke Menschen laufen oft Gefahr, durch lange Liegezeiten auf der Intensivstation Muskelmasse zu verlieren, was zu ausgepragten Folgeerkrankungen fuhren kann. Je fruher also mit der Mobilisierung begonnen wird, desto besser. Unser Autorenteam hat in unserer Serie uber CCI-Patienten anhand einer umfangreichen Literaturanalyse die Moglichkeiten und Auswirkungen der Fruhmobilisation unter die Lupe genommen.

Hintergrund: Aus internationalen Studien geht hervor, dass ein Großteil der Patienten Aus Gründen der besseren Lesbarkeit wird im Text ausschließlich die männliche Form verwendet. Dies schließt die weibliche Form mit ein., die auf Intensivstationen behandelt werden, moderate bis starke Schmerzen beschreibt. Erwiesen ist auch, dass nicht erkannte und ungenügend therapierte Schmerzen schwere körperliche und seelische Belastungen hervorrufen können. Eines der wichtigsten Therapieziele im Intensivbereich ist deshalb das frühzeitige und sichere Erkennen von Schmerzen, was bei stark bewusstseinsbeeinträchtigten Patienten eine große Herausforderung darstellt. Ziel: Das Ziel dieser Studie ist es, aufzuzeigen, wie mit einer standardisierten Fremdeinschätzung von Schmerzen mit dem Zurich Observation Pain Assessment (ZOPA©) diese Lücke geschlossen werden kann. Methode: Anhand einer theoriegeleiteten interpretativen Fallstudie wird der Einsatz von ZOPA© in der Praxis und dessen Einfluss auf das Schmerzmanagement beschrieben. Ergebnisse: Insgesamt wurden 126 Einzelerfassungen bei einer Intensivpatientin mit einer schweren Subarachnoidalblutung analysiert, wovon 19 Erfassungen auf Schmerz hinweisende Verhaltensmerkmale zeigten. In drei Vierteln dieser Fälle wurden umgehend schmerzlindernde Maßnahmen eingeleitet. Auf der betreffenden Intensivstation wird ZOPA© seit fünf Jahren angewandt, neben der analgetischen Basismedikation sind zusätzliche Analgetika als Bedarfsmedikation verordnet, was Pflegenden den Freiraum lässt, ihre Verantwortung im Schmerzmanagement wahrzunehmen. Schlussfolgerung: Die Fallstudie zeigt, dass sich ZOPA© als geeignetes Fremdeinschätzungsinstrument erweist, anhand dessen Schmerzen frühzeitig erkannt und behandelt bzw. Schmerzsituationen vermieden werden könnten.

The complexation of acetate with Am(III) is studied as a function of the pH (1–6) by extended X-ray absorption fine-structure (EXAFS) spectroscopy. The molecular structure of the Am(III)–acetate complexes (coordination numbers, oxygen and carbon distances) is determined from the rawk3-weighted AmLIII-edge EXAFS spectra. The results show a continuous shift of Am(III) speciation with increasing pH value towards the complexed species. Furthermore, it is verified that acetate coordinates in a bidentate coordination mode to Am(III) (Am—C distance: 2.82 ± 0.03 Å). The EXAFS data are analyzed by iterative transformation factor analysis to further verify the chemical speciation, which is calculated on the basis of thermodynamic constants, and the used structural model. The experimental results are in very good agreement with the thermodynamic modelling.

The complexation of Cm(III) with human serum transferrin was investigated in a pH range from 3.5 to 11.0 using time-resolved laser fluorescence spectroscopy (TRLFS). At pH ≥ 7.4 Cm(III) is incorporated at the Fe(III) binding site of transferrin whereas at lower pH a partially bound Cm(III) transferrin species is formed. At physiological temperature (310 K) at pH 7.4, about 70% of the partially bound and 30% of the incorporated Cm(III) transferrin species are present in solution. The Cm(III) results obtained by TRLFS are in very good agreement with Am(III) EXAFS results, confirming the incorporation of Am(III) at the Fe(III) binding site at pH 8.5.

The complexation of trivalent actinides (An(III)) with chloride is studied in the temperature range from 25 to 200 °C by spectroscopic methods. Time-resolved laser fluorescence spectroscopy (TRLFS) is applied to determine the thermodynamic data of Cm(III)-Cl(-) complexes, while extended X-ray absorption fine structure spectroscopy (EXAFS) is used to determine the structural data of the respective Am(III) complexes. The experiments are performed in a custom-built high-temperature cell which is modified for the respective spectroscopic technique. The TRLFS results show that at 25 °C the speciation is dominated mainly by the Cm(3+) aquo ion. Only a minor fraction of the CmCl(2+) complex is present in solution. As the temperature increases, the fraction of this species decreases further. Simultaneously, the fraction of the CmCl2(+) complex increases strongly with the temperature. Also, the CmCl3 complex is formed to a minor extent at T160 °C. The conditional stability constant log β'2 is determined as a function of the temperature and extrapolated to zero ionic strength with the specific ion interaction theory approach. The log β°2(T) values increase by more than 3 orders of magnitude in the studied temperature range. The temperature dependency of log β°2 is fitted by the extended van't Hoff equation to determine ΔrH°m, ΔrS°m, and ΔrC°p,m. The EXAFS results support these findings. The results confirm the absence of americium(III) chloride complexes at T = 25 and 90 °C ([Am(III)] = 10(-3) m, [Cl(-)] = 3.0 m), and the spectra are described by 9-10 oxygen atoms at a distance of 2.44-2.48 Å. At T = 200 °C two chloride ligands are present in the inner coordination sphere of Am(III) at a distance of 2.78 Å.

Chronisch kritisch-kranke Menschen sind fur eine lange Zeit von lebenserhaltenden Technologien abhangig. In der Schweiz beschaftigt sich eine Expertengruppe seit mehreren Jahren ausfuhrlich mit diesen Patienten. Sie hat fur unsere neue Reihe uber CCI-Patienten aktuelles Wissen aus der Fachliteratur zusammengetragen, um Chancen und Moglichkeiten in der pflegerischen Behandlung zu verdeutlichen und ihre Umsetzung zu fordern. Zum Auftakt der Reihe erklaren die Experten den Begriff und stellen die therapeutischen Herausforderungen vor.

Der Schwerpunkt der Bezugspflege am UniversitätsSpital Zürich (USZ) liegt auf der Verantwortung einer Pflegefachperson für den Pflegeprozess der von ihr betreuten Patient(inn)en. Dieser wird im Rahmen einer professionellen Beziehung mit der damit verbundenen Verantwortlichkeit für pflegerische Entscheidungen und Handlungen sowie gestützt auf forschungs- und erfahrungsbasiertes Fachwissen gestaltet – vom Tag der Aufnahme bis zur Entlassung aus dem Spital. Der Artikel beschreibt die Evaluation der Pflegeorganisationsform auf drei Intensivabteilungen am USZ im Kontext der Implementierung der Bezugspflege mit dem Instrument zur Erfassung von Pflegesystemen (IzEP©). In einem Vorher-nachher-Vergleich zeigte IzEP© bei der Umsetzung von Konzeptinhalten im Intensivpflegebereich eine klare Steigerung pflegerischer Kontinuität und Patient(inn)enorientierung von anfänglich 56 – 60 % auf Werte von 68 – 75 % (Score von 0 – 100 %). Zugleich wurde vorhandenes Entwicklungspotenzial für dieses Pflegesystem sichtbar. Es wurde deutlich, dass IzEP© aufgrund seines Aufbaus für einen breiten stationären Anwendungsbereich und in der Formulierung der Fragen auf idealtypische Pflegeorganisationsformen ausgerichtet ist. Gegebene Rahmenbedingungen des intensivpflegerischen Settings und spezifische Konzeptideen werden nur bedingt berücksichtigt. Somit bietet es einen eingeschränkten Wiedererkennungswert des eigenen Konzepts für die Befragungsgruppen.
