The current urology training lacks education in sexual medicine. This was demonstrated by a nationwide cross-sectional survey among urologists in training, conducted in June 2015. Regardless of the residency year, most of the residents had never attended training about sexual dysfunction (58.–6 %), possessed little knowledge (48.3 %) and reported a clear need for more education (69.4 %). The majority felt not competent in treating patients with sexual dysfunctions after prostate cancer treatment (55.2 %). Time constraints in a consultation (67.1 %) and lack of education (35.3 %) were the most frequently cited barriers. More than half of the residents reported that the program does not provide sufficient education about sexual dysfunction (54.8 %). Consequently, the results of this survey support efforts to improve the urology program regarding education about prostate cancer-related sexual dysfunction.

Introductie: Urine-incontinentie wordt gedefinieerd als het ongewild verlies van urine en is geassocieerd met een daling in de kwaliteit van leven (KvL). De schatting is dat tussen de 10 en 40% van de vrouwen van 15-64 jaar te maken zal krijgen met urine-incontinentie en dat dit percentage bij vrouwen ouder dan 65 jaar zelfs nog hoger ligt. Een operatie door middel van een mid-urethrale sling is een geaccepteerde en bewezen effectieve methode om vrouwen van hun stress urine-incontinentie (SUI) te genezen. In navolging van de tension-free vaginal tape (TVT), de tension-free vaginal tape-obturator (TVT-O) en de trans obturator tape (TOT) zijn de ‘mini-slings’ zoals de MiniArc (2007 AMS) ontwikkeld. De TVT, TVT-O en TOT laten alle succespercentages zien tussen de 84 en 100% na een minimale follow-up van 1 jaar. Het doel van deze studie was het evalueren van de effecten van de MiniArc 1 jaar postoperatief, met aandacht voor zowel de continentie als de kwaliteit van leven. Materiaal en methoden: 77 patienten, geopereerd tussen maart 2008 en november 2009, zijn in deze studie geevalueerd. Gepresenteerd worden de data 1 jaar postoperatief. Preoperatief hadden alle vrouwen last van geobjectiveerde SUI. Na 1 jaar was de respons 74%. De patienten zijn geevalueerd met de vragenlijst voor urine-incontinentie van de Nederlandse Vereniging voor Obstetrie en Gynaecologie (NVOG), die is samengesteld uit de EuroQol-5 Dimensies (EQ-5D), de Patient Global Impression of Improvement (PGI-I), de Incontinence Impact Questionnaire (IIQ), de Urinary Distress Inventory (UDI), de Prolapse/Urinary Incontinence Sexual Questionnaire, korte vorm (PIS-Q SF) en de Defecation Distress Inventory (DDI). Resultaten: Een jaar postoperatief gaf 68% van de patienten aan dat de SUI verbeterd was, terwijl slechts 44% beaamde compleet droog te zijn. De UDI (stress) was in alle subcategorieen significant verbeterd na 1 jaar. De IIQ (alledaags functioneren) liet na 1 jaar een significante verbetering zien in 3 van de 5 subcategorieen. Conclusie: De follow-up na 1 jaar suggereert dat de MiniArc minder effectief is bij de behandeling van SUI dan de gang-bare slingprocedures.

Bij 65-75% van de mannen met een gevorderd ziektestadium metastaseert de prostaatkanker naar het bot. Bovendien hebben patienten met prostaatkanker vaak verschillende andere risicofactoren voor botcomplicaties, zoals osteoporose en androgeendeprivatietherapie. Maligne botlaesies kunnen de botstructuur aantasten. Dat kan onder andere resulteren in pathologische fracturen, compressie van het ruggenmerg en ernstige botpijnen. Deze skeletal-related events (SRE’s) hebben een negatieve impact op de kwaliteit van leven van de patient.

Prostate cancer is predominantly associated with osteoblastic bone metastases, but an increase in bone resorption has been demonstrated consistently, both histologically and biochemically. For this reason, bisphosphonates, which effectively suppress bone resorption, have been used in patients with prostate cancer metastatic to the skeleton. We studied clinical and biochemical responses 5 days and 3 months after administration of the new, potent bisphosphonate, olpadronate, in 28 patients with prostate cancer and bone metastases. All patients received 4 mg of olpadronate intravenously daily for 5 days. No additional treatment was given to the first 12 patients, while treatment was continued with oral olpadronate 200 mg daily in the following 16 patients. Serum alkaline phosphatase (ALP) activity was elevated in 93% of the patients and was positively correlated to urinary hydroxyproline excretion (r = 0.81, p < 0.0001), suggesting a coupling between bone formation and resorption. A rapid and significant suppression of bone resorption was observed in all patients after intravenous treatment. This was sustained for 4-6 weeks in all patients, but reversed thereafter in patients not receiving oral maintenance therapy. No significant changes in serum ALP activity were observed in either group during the 3 months of follow-up. At the start of treatment all patients had severe bone pain and 82% and 36% were using NSAIDs and/or opiates, respectively. Although clinical response was not a primary objective of the study, we observed that intravenous therapy was associated with a decrease in bone pain in 76% of patients and a reduction in the use of analgesics. At 3 months this response was generally sustained only in those patients who were maintained on continuous oral therapy (p < 0.05 compared with the group treated with intravenous olpadronate only). The clinical response thus appeared to parallel the biochemical changes in bone resorption.
